Abstract Objective: To analyze, through a systematic review with meta-analysis, the effects of aerobic training with and without progression on systolic blood pressure (SBP) and diastolic blood pressure (DBP) in hypertensive adults.Method: The search for the studies was carried out in the PubMed, Cochrane Central, SPORTDiscus and LILACS databases. Clinical trials that analyzed the effect of aerobic training, lasting at least six weeks, on blood pressure in hypertensive individuals comparing with a control group without intervention were selected. The selection of studies and data extraction were carried out independently by two pairs of researchers. Results: Of the 13028 studies found, 24 were selected and included in this review. There was a reduction in SBP after aerobic training with progression (-10.67 mmHg; 95% CI -15.421, -5.926; p <0.001) and without progression (-10.17 mmHg; CI -12.213, -8.120; p <0.001). DBP also decreased after aerobic training with progression (-5.49 mmHg; 95% CI -8.663, -2.310; p <0.001) and without progression (-6.51 mmHg; 95% CI -9.147, -3.868; p < 0.001). Conclusion: Aerobic training promotes a reduction in the SBP and DBP levels of adults with hypertension, regardless of whether or not the training variables progression.

Abstract The objective of the present study was to determine the effectiveness of the regular practice of traditional resistance training (RT) on systolic (SBP) and diastolic blood pressure (DBP) in normotensive elderly persons. A systematic review of randomized clinical trials and meta-analyses was performed. Searches were performed without language restrictions in different databases. Randomized clinical trials published from 1966 to 2010 that assessed the effects of traditional RT on resting blood pressure (BP) and/or for the treatment of high BP were included. Only studies that assessed the effects of traditional RT on elderly adults, regardless of the number of exercises, with the presence of a control group and comparisons between groups, were included. Twenty-nine studies were found, but only six met the inclusion criteria. The mean difference was used for meta-analysis, using a 95% confidence interval and a random effect model. Traditional RT induced a significant decrease in SBP (-6.63 mmHg; p=0.02) but not in DBP (-3.34 mmHg; p=0.11). These results suggest that traditional RT may be a non-pharmacological strategy for the control of BP in the elderly.

Eligibility criteria: Measures in the intervention group: aerobic training (taijiquan, Baduanjin, qigong, yoga, swimming, cycling, jogging, brisk walking, etc.) and resistance training were used simultaneously, including studies conducted on the same day and on different days. There are no restrictions on training time, frequency and intensity.Control group measures: no intervention or conventional treatment.

Background: Lack of exercise is a prevalent problem in patients receiving dialysis. Although guidelines recommend these patients to undertake suitable exercise, no exercise type or intensity has been suggested, and the effect of exercise on muscle fitness in dialysis patients is not clear. This study investigated the effect of exercise on muscle fitness, including muscle mass, muscle strength, and physical performance, in patients on dialysis. Methods: A systematic review and a meta-analysis of randomized controlled trials (RCTs) were conducted. Five English and 4 Chinese databases were searched from their inception to July 2018. Two independent reviewers searched the different databases, selected trials, conducted bias assessment, and extracted the data. Results: A total of 21 RCTs meeting the inclusion criteria were included in this review. Pooled results demonstrated that resistance training significantly improved leg mass (standard mean difference [SMD] 0.34, 95% CI [0.06–0.62], p= 0.02) whereas aerobic training did not (SMD 0.87, 95% CI [–0.11 to 1.86], p = 0.08). Resistance training increased both grip strength (weighted mean difference [WMD] 4.71 kg, 95% CI [2.42–6.99], p < 0.00001) and knee extension strength (WMD 3.93 kg, 95% CI [0.59–7.28], p = 0.02) significantly. Aerobic training improved grip strength (WMD 7.70 kg, 95% CI [3.35–12.05], p= 0.005) and the time of finishing short version of the sit-to-stand test (STS; WMD –4.69 s, 95% CI [–9.01 to –0.38], p = 0.03) but with insufficient evidence. In the dimension of physical performance, both aerobic training and resistance training have some beneficial effect on improving the score of 6-min walking test (WMD 85.76 m, 95% CI [63.43–108.09], p < 0.00001; WMD 41.92 m, 95% CI [8.06–75.75], p = 0.02, respectively) and median version of STS test (WMD 4.30 repetitions, 95% CI [1.22–7.39], p = 0.006; WMD 2.60 repetitions, 95% CI [0.64–4.56], p = 0.006, respectively). Conclusions: Regular resistance training with a moderate to high intensity may lead to improvement in muscle mass and muscle strength of patients undergoing dialysis, especially for the trained muscles. Both aerobic training and resistance training may help dialysis patients improve physical performance.

Context: Cardiovascular diseases cause 17 million deaths annually worldwide, of which hypertension is responsible for 9.4 million and a 7% burden of disease. High blood pressure is responsible for 45% of deaths from heart disease and 51% of deaths from stroke. Objective: The aim of this systematic review and meta-analysis was to quantify the effect of isometric resistance training on systolic, diastolic, and mean arterial blood pressure (SBP, DBP, and MAP, respectively) values in normotensive adult participants. Data Sources: This study was registered with the PROSPERO database. Eligible studies were identified after performing a systematic search within the following databases: PubMed, Scielo, BioMed Central, Clinical Trials, EMBASE, Cochrane Central Register of Controlled Trials, and EBSCO. Study Selection: Randomized controlled trials that categorized participants as normotensive according to the guidelines of the American Heart Association and the American College of Cardiology were included. Study Design: Systematic review with meta-analysis. Level of Evidence: Level 1. Data Extraction: Data related to participant characteristics, exercise programs, level of evidence, risk of bias, Consensus on Exercise Reporting Template, and outcomes of interest were systematically reviewed independently by 2 authors. Results: A total of 6 randomized controlled trials were included. The following reductions in blood pressure (compared with the control group) were generated by isometric resistance training: SBP (mean difference [MD], −2.83 mm Hg; 95% CI, −3.95 to −1.72; P < 0.00001), DBP (MD, −2.73; 95% CI, −4.23 to −1.24; P = 0.0003), and MAP (MD, −3.07; 95% CI, −5.24 to −0.90; P = 0.005). Conclusion: It appears that isometric resistance training reduces SBP, DBP, and MAP in normotensive young adults in a statistically significant and clinically relevant manner. This type of exercise could be considered effective in preventing arterial hypertension.
