The article describes a clinical case of the successful management of pregnancy, delivery and post-natal period in the female patient with fibrous cavernous tuberculosis with extensive drug resistance and multiple thoracic surgeries in the past. This clinical case demonstrates that it is possible for a mother with the advanced form of tuberculosis to give birth to a healthy mature newborn.

Представлен клинический пример эффективного лечения пациентки с деструктивным туберкулезом легких, вызванным микобактериями туберкулеза с множественной лекарственной устойчивостью (МЛУ МБТ). Особенности данного случая: заболевание туберкулезом пациентки, ежегодно проходившей флюорографическое обследование, наличие выраженной клинической симптоматики и быстрое развитие множественных деструкций в легких. Назначение противотуберкулезного препарата — линезолида — в комплексной терапии позволило достигнуть абациллирования в кратчайшие сроки, выраженной положительной клинико-рентгенологической динамики специфического процесса, закрытия полостей распада у пациентки с деструктивным туберкулезом легких с МЛУ МБТ.

The article presents a clinical case of pulmonary tuberculosis with destruction and bacterial excretion in the patient with systemic sarcoidosis and cerebral lesions. Tuberculosis was characterized by the infiltrate and cavity in S1+2 of the left lung, tuberculous mycobacteria were detected by microscopy, GeneXpertMBT/Rif, and culture. Systemic sarcoidosis with brain involvement was diagnosed based on intrathoracic lymphadenopathy in 2015, development of dissemination in the lungs and neurological symptoms by 2018, deterioration of changes by 2019, rapid partial resolution of foci in the lungs, and moderate regression of neurological disorders during the treatment with prednisolone. A full course of anti-tuberculosis chemotherapy (316 doses) resulted in persistent sputum conversion, resolution of the infiltrate and cavity healing. Prednisolone was administered simultaneously for 318 days with increased doses (45-35 mg) for the first 2 months, then titrated down to 15 mg and remained so until the end of the treatment with gradual reduction and discontinuation. Changes in the lungs and improvement of clinical and radiological manifestations of neurosarcoidosis were documented.

This article presents a clinical case of a 52-year-old man suffering from oligobacillary border-tuberculoid leprosy in the acute phase associated with an established diagnosis of pulmonary tuberculosis and hepatitis C.Aim: to present a clinical case of leprosy with atypical and complicated manifestations.Materials and methods. A 52-year-old patient had been receiving treatment for allergic dermatitis for a long time.Results. The patient was diagnosed with: A 30.2. Borderline tuberculoid leprosy (Paucibacillary Leprosy, Borderline tuberculoid form. Acute phase). The patient demonstrated atypical complications of the leprosy process in the form of ulcerative necrotic rashes.
