AbstractEducational attainment among women is a well-recognized predictor for maternal mortality. Data from nationally representative surveys and the United Nations are used in the analysis for estimating maternal mortality due to improved education status up to 2030. Analysis of data from 2001 and 2010 Bangladesh Maternal Mortality Survey shows that MMR varies considerably by education level. The study shows that during 2011–2030, 15% maternal deaths will be averted due to fertility change (i.e. fewer births) and 24% of the maternal deaths can be averted only by improving the female education levels. However, in order to achieve the Ending Preventable Maternal Mortality (EPMM) target of 59 maternal deaths per 100,000 live births by 2030 for Bangladesh, a further 64% reduction will be required. Factors outside the health sector, like female education, will continue to have an impact maternal mortality in Bangladesh. However, reaching the EPMM target for Bangladesh by 2030 will also require significant investments in maternal health programs, in particular those to increase access to and quality of services.

Abstract Hundreds of millions of people live in countries that do not have complete death registration systems, meaning that most deaths are not recorded and that critical quantities, such as life expectancy, cannot be directly measured. The sibling survival method is a leading approach to estimating adult mortality in the absence of death registration. The idea is to ask survey respondents to enumerate their siblings and to report about their survival status. In many countries and periods, sibling survival data are the only nationally representative source of information about adult mortality. Although a vast amount of sibling survival data has been collected, important methodological questions about the method remain unresolved. To help make progress on this issue, we propose reframing the sibling survival method as a network sampling problem. This approach enables a formal derivation of statistical estimators for sibling survival data. Our derivation clarifies the precise conditions that sibling history estimates rely on, leads to internal consistency checks that can help assess data and reporting quality, and reveals important quantities that could potentially be measured to relax assumptions in the future. We introduce the R package siblingsurvival, which implements the methods we describe.

AbstractWe analyze socioeconomic differences in adult mortality in four African countries-the Democratic Republic of Congo, Ethiopia, Rwanda and Sierra Leone-using the adult mortality module in the Demographic and Health Surveys (DHS), calculating mortality based on the sibling mortality reports collected from female respondents aged 15-49. We discuss the advantages and potential issues associated with this data source. While mortality events precipitated by those civil conflicts tend to affect all groups, we conclude that they appear to affect men, and in particular urban and more educated men to a greater extent than the other groups.

IntroductionAs of 26 March 2021, the Africa Centres for Disease Control and Prevention had reported 4 159 055 cases of COVID-19 and 111 357 deaths among the 55 African Union member states; however, no country has published a nationally representative serosurvey as of October 2021. Such data are vital for understanding the pandemic’s progression on the continent, evaluating containment measures, and policy planning.MethodsWe conducted a cross-sectional, nationally representative, age-stratified serosurvey in Sierra Leone in March 2021 by randomly selecting 120 Enumeration Areas throughout the country and 10 randomly selected households in each of these. One to two persons per selected household were interviewed to collect information on sociodemographics, symptoms suggestive of COVID-19, exposure history to laboratory-confirmed COVID-19 cases, and history of COVID-19 illness. Capillary blood was collected by fingerstick, and blood samples were tested using the Hangzhou Biotest Biotech RightSign COVID-19 IgG/IgM Rapid Test Cassette. Total seroprevalence was estimated after applying sampling weights.ResultsThe overall weighted seroprevalence was 2.6% (95% CI 1.9% to 3.4%). This was 43 times higher than the reported number of cases. Rural seropositivity was 1.8% (95% CI 1.0% to 2.5%), and urban seropositivity was 4.2% (95% CI 2.6% to 5.7%).DiscussionOverall seroprevalence was low compared with countries in Europe and the Americas (suggesting relatively successful containment in Sierra Leone). This has ramifications for the country’s third wave (which started in June 2021), during which the average number of daily reported cases was 87 by the end of the month:this could potentially be on the order of 3700 actual infections per day, calling for stronger containment measures in a country with only 0.2% of people fully vaccinated. It may also reflect significant under-reporting of incidence and mortality across the continent.
