The current doses of recombinant growth hormone (rGH) are two to three times those used in the pituitary growth hormone era. These rGH doses (0.025 to 0.043 mg/kg/d) are similar to or moderately greater than the physiologic requirements. Growth velocity and height gains have been shown to be greater with 0.05 mg/kg/d of rGH than with 0.025 mg/kg/d. Larger doses of GH and early initiation of treatment result in greater heights at the onset of puberty and greater adult heights. Earlier onset of puberty and more rapid maturation, as indicated by bone age, were not observed in children who were given 0.18 to 0.3 mg/kg/wk of rGH. The frequency of adverse events is very low, but diligent surveillance of all children who are treated with rGH is essential.

The objective of the present study was to evaluate the efficacy and safety of the application of the new soluble pharmaceutical form of Rastan for subcutaneous injections at a dose of 15 IU/ml and compare its action with that of Rastan lyophilisate, 4 IU, designed to prepare solutions for subcutaneous administration. The two dosage forms are used to treat children suffering from growth hormone deficiency. The study included patients at the age from 4 to 12 years presenting with idiopathic growth hormone deficiency; they were randomized into two groups. During the first three months, the patients of both groups were treated with different pharmaceutical forms of recombinant growth hormone (rGH). The children in group 1 were given Rastan for subcutaneous injections and those in group 2 received Rastan lyophilysate for the preparation of solutions for the subcutaneous administration. Either form of GH was used at an equal daily dose of 0.033 mg/kg b.w. The patients of both groups showed marked improvement of the parameters of linear growth within the first three months. The difference in the growth rates was not significantly different between the two groups which suggests the identical effect of the two forms of rGH. During the next 9 months when the patients of both groups were treated only with the rGH for subcutaneous injections, the absolute growth response, height SDS, and the level of insulin-like growth factor 1 (IGF-1) continued to increase. It points out to the stable growth-promoting effect of Rastan for subcutaneous injections. No clinically significant abnormal changes in the results of complete blood cell count and biochemical analysis of blood were apparent during 12 months of therapy with this form of rGH. The same was true of the levels of free T4, cortisol, and prolactin in the blood. No adverse effects attributable to the therapy with rGH were documented.

Background: Despite the developments of recombinant growth hormone (rhGH) treatment and the benefits in long-term clinical health outcomes, evidence has shown that many children with growth hormone deficiency (GHD) still fail to achieve their target adult height. Suboptimal outcomes have been largely attributed to treatment non-adherence. Methods: A search of 11 electronic databases was undertaken to identify relevant articles, published in English, between 1985 and 2018. Additional search strategies included hand-searching topic review articles to identify eligible studies. Articles were screened against the inclusion eligibility criteria and data on sample characteristics, study design, outcomes, and key findings was extracted. The results were narratively synthesised and categorised using the COM-B theoretical framework. Results: Twenty-one full-text articles were assessed for eligibility, of which 6 articles met the inclusion criteria. The prevalence of non-adherence in the included studies varied from 7 to 71%. Potentially modifiable factors associated with rhGH non-adherence were categorised within the COM-B framework; key factors included: a lack of knowledge and understanding of the condition and treatment, discomfort and pain associated with injections, and the quality of the healthcare professional-patient relationship. Conclusion: This review highlights the scope of the adherence problem evident amongst the paediatric GHD population and in addition presents the wide range of potentially modifiable factors that explain this health-related behaviour.

Six patients (21–50 years) with growth hormone deficiency and panhypopituitarism were given recombinant growth hormone, somatropin, 0.04–0.1 U·kg·body wt−1·day−1, for 12 months. All patients reported improved well-being with increased working capacity. Bone mineral density, as measured by single photon absorptiometry at two sites on the forearm, showed increased values in 5/6 patients after 12 months when measured at the most distal site (predominantly trabecular bone) and in 4/6 at the more proximal site (predominantly cortical bone). Five patients continued therapy for an additional year and after 18 months a significant increase in bone mineral density was seen at both the distal and proximal sites. The mean annual increase in bone mineral density was 12.0±0.6 (sem)% and 3.8±1.3% at the distal and proximal sites, respectively. In a growth hormone deficient control group without growth hormone therapy, the corresponding values were −2.4±0.6% and −1.9±0.4%, respectively. Lean body mass, estimated anthropometrically, increased significantly after 12 months and total body potassium, measured by whole body counting technique, increased in 4/6 patients. During growth hormone treatment, the IGF-1 values were above the mean values for age and 50% of the values were above the mean + 2 SD. B-glucose, P-insulin, serum IGF-2, procollagen-III peptide and phosphate increased and urea, creatinine and IGF-binding protein-1 decreased during treatment. The beneficial effects of growth hormone substitution, especially on bone mineral density, indicate that growth hormone substitution should be considered in all patients with hypopituitarism and growth hormone deficiency, irrespective of age.
