Background: Intramural hematoma of the duodenum is a rare cause of acquired duodenal obstruction. It is often a diagnostic dilemma and a high degree of suspicion is required to make an early and accurate diagnosis in children. Case Report: A 2-year-old child presented with bilious vomiting and abdominal distension. X-ray abdomen showed high intestinal obstruction. Ultrasound and CT scan abdomen gave suspicion of pancreatic pseudo-cyst near head of pancreas. At surgery, an intramural hematoma of the duodenum was found and drained. Conclusion: Intramural duodenal hematoma is a rare entity especially in children and should be considered in differential diagnosis of acquired duodenal obstruction.

Background. Retroperitoneal lymphangioma is a rare, benign mesodermal tumor arising from the retroperitoneal lymphatics which usually presents in infancy and it is worthy to report a case when it has presented in an adult. After a thorough literature search in English we concluded that less than 200 cases of adult retroperitoneal lymphangioma have been reported so far. Apart from being a rare entity it also presents as a diagnostic dilemma and final diagnosis is often made on surgical exploration.Case Presentation. We report a case of retroperitoneal lymphangioma in a 55-year-old male who presented with abdominal distension and dull aching abdominal pain.Conclusion. Retroperitoneal lymphangiomas are rare tumors of infancy but may also present in adults where they are a diagnostic challenge. Differentiating cystic lymphangiomas from other cystic growths by imaging studies alone are often inconclusive and surgery is frequently required for definitive diagnosis and to ameliorate the symptoms.

Ancient schwannomas are a rare entity of benign cutaneous nerve sheath tumor in patients without von Recklinghausen disease. Head and neck, mediastinal, retroperitoneal ancient schwannomas have been reported and has high degree for malignant transformation. Ancient schwannoma of the peripheries have been reported in only few cases and in hand only two cases have been reported. We present this case for its rarity and the diagnostic dilemma occurring with FNAC. Thus these tumors if present in periphery and in non-neurofibromatosis patient can masquerade as malignant tumors but still can safely are treated with excision biopsy alone and with almost nil recurrence.

Fetus in fetu is an extremely rare developmental anomaly, due to disorganised embryogenesis in a diamniotic monochorionic pregnancy. Approximately 87 reports have been documented in the literature to date. We describe such an entity in two months old boy, who presented with abdominal distension and features of obstruction. Plain X ray abdomen and CT scan helped in correct pre operative diagnosis. The literature on this rare entity is reviewed, and the diagnosis and pathogenesis of the disease are discussed. The recent discovery of anionic redox as a means to increase the energy density of transition metal oxide positive electrodes is now a well established approach in the Li-ion battery field. However, the science behind this new phenomenon pertaining to various Li-rich materials is still debated. Thus, it is of paramount importance to develop a robust set of analytical techniques to address this issue. Herein, we use a suite of synchrotron-based X-ray spectroscopies as well as diffraction techniques to thoroughly characterize the different redox processes taking place in a model Li-rich compound, the tridimentional hyperhoneycomb β-Li2IrO3. We clearly establish that the reversible removal of Li+ from this compound is associated to a previously described reductive coupling mechanism and the formation of the M-(O-O) and M-(O-O)* states. We further show that the respective contributions to these states determine the spectroscopic response for both Ir L3-edge X-ray absorption spectroscopy (XAS) and X-ray photoemissions spectroscopy (XPS). Although the high covalency and the robust tridimentional structure of this compound enable a high degree of reversibile delithiation, we found that pushing the limits of this charge compensation mechanism has significant effects on the local as well as average structure, leading to electrochemical instability over cycling and voltage decay. Overall, this work highlights the practical limits to which anionic redox can be exploited and sheds some light on the nature of the oxidized species formed in certain lithium-rich compounds.<br>

Bilious vomiting in a neonate may be a sign of intestinal obstruction often resulting in transfer requests to surgical centres. The aim of this study was to assess the use of clinical findings at referral in predicting outcomes and to determine how often such patients have a time-critical surgical condition (eg, volvulus, where a delay in treatment is likely to compromise gut viability).Methods4-year data and outcomes of all term newborns aged ≤7 days with bilious vomiting transferred by a regional transfer service were analysed. Specificity, sensitivity, likelihood ratios, correlations, prior and posterior probability of clinical findings in predicting newborns with surgical diagnosis were calculated.ResultsOf 163 neonates with bilious vomiting, 75 (46%) had a surgical diagnosis and 23 (14.1%) had a time-critical surgical condition. The diagnosis of a surgical condition in neonates with bilious vomiting was significantly associated with abdominal distension (χ2=5.17, p=0.023), abdominal tenderness (χ2=5.90, p=0.015) and abnormal abdominal X-ray findings (χ2=5.68, p=0.017) but not with palpation findings of a soft as compared with a tense abdomen (χ2=3.21, p=0.073). Abnormal abdominal X-ray, abdominal distension and tenderness had 97%, 74% and 62% sensitivity, respectively, with regard to association with an underlying surgical diagnosis. Normal abdominal X-ray reduced the posterior probability of surgical diagnosis from 50% to 16%. Overall, clinical findings at referral did not differentiate between infants with or without surgical or time-critical condition.ConclusionsWe recommend that term neonates with bilious vomiting referred for transfer are prioritised as time critical.

Nanoscale hollow spheres of amorphous phosphorus nitride (P3N5) were synthesized by reacting PCl3 with NaN3 at 150–250 °C. Transmission electron microscope images show that the hollow spheres have a diameter of 150–350 nm, and the thickness of the shell is 20 nm. A very small amount of curly films were also found in the sample prepared at 150 °C. The infrared spectrum indicates a high degree of purity. X-ray photoelectron spectroscopy indicates the presence of P and N, with a molar ratio of 1:1.62 for P:N. Ultraviolet-visible absorption spectroscopy shows an absorption band at 265–315 nm. Under photoluminescent excitation at 230 nm, the P3N5 emits ultraviolet light at 305 nm. With a band gap of 4.28 eV, the products may be a wide gap semiconductor. A possible mechanism and the influence of temperature on the formation of the hollow spheres are also discussed.

Ectopic gall bladder is a rare entity. It can often be misdiagnosed causing diagnostic dilemma leading to various complications. Ultrasonography is the most common investigation for evaluating gall bladder pathologies. However, the confirmation of ectopic gallbladder is not easily possible by this method. On the other hand, hepatobiliary scan (HIDA scan) plays an important role in evaluating the presence and position of ectopic gallbladder. Here we present a case of sonographically suspected ectopic gallbladder confirmed by hepatobiliary scan highlighting the importance of HIDA scan in ectopic gallbladder.Bangladesh J. ABSTRACT Combined autoradiography and X-ray photography has demonstrated that the tissues of the caudal part of the lower jaw and the pronephric region have a considerably larger uptake of radioiodine than other tissues in the immature crucian carp, Carassius carassius L. It is known from previous studies that thyroid follicles occur in these regions. The pronephric thyroid differs from the mandibular thyroid with regard to radioiodine turnover. On an average, the pronephric thyroid has a larger uptake than the lower jaw thyroid. The relative uptakes varied considerably in the population investigated. The ratio mandibular uptake/total thyroid uptake has been determined at several sampling intervals. The means of these ratios show no significant trend with time, a finding which strongly supports the view that the two regions are variably sized parts of a physiologically homogeneous thyroid gland. The analysis is based on a mathematical model of iodine turnover. The method of comparing complex regression functions (sums of exponential functions) should be widely applicable. The correlation between the radioiodine content of either region and total thyroid content is low. Thus, it is necessary to study simultaneously all follicle-carrying tissues in fish with more than one aggregation of thyroid elements. The net radioiodine turnover in the crucian carp indicates a comparatively high degree of thyroid activity and considerably higher than that reported for C. auratus L. This phenomenon is discussed.
