Compound odontoma associated with dentigerous cyst in the anterior mandible – case report

Introduction: The dentigerous cyst, also called a follicular cyst is an odontogenic cyst that develops in association with crown of an impacted tooth, predominantly in mandibular third molars of young patients. The Odontoma is a ectomesenquimal tumor of unknown origin that are more considered developmental malformations (harmatomas) than benign neoplasms. Occasionally, the dentigerous cyst is associated with odontoma. Objective: The article aims to report a case of surgical treatment of dentigerous cyst associated with compound odontoma and unerupted tooth in anterior region of the mandible. Case report: A male patient, 17 years of age, without systemic changes, was sent to the Center for Dental Specialties of Horizonte, Ceará, Brazil, specialty of Oral and Maxillofacial Surgery, for diagnosis and treatment of oral lesions visualized after periapical radiographic examination to determine failed eruption of element 42. Observing the periapical radiograph it was possible to visualize radiopaque lesion suggestive of a compound odontoma and a cystic capsule in association with the element 42. A panoramic radiographwas requested and it was planned a surgical removal of odontoma with curettage of bone cavity and removing the cystic capsule and element 42. There were no significant postoperative complications, the suture removal was performed 7 days after surgery where it was possible to observe a good healing, no swelling and no paresthesia. Conclusion: The treatment of choice is surgical excision of the lesions with the tooth associated to the cyst, it should be performed with proper planning, avoiding injury to vital structures and should not be delayed in order to avoid possible occlusion sequelae.

The extraction of impacted third molars is among the most common surgical procedures carried out in the field of Oral and Maxillofacial Surgery. Both the patient and dentist must therefore have scientific evidence-based information concerning the estimated level of surgical difficulty of every case to consider in referring cases of impacted third molars for specialists’ handling. We have undertaken a study in which demographic and radiological variables were considered together to evaluate the risk factors for surgical difficulty in a cohort of 100 impacted mandibular third molars. There were 13 variables evaluated for surgical difficulty. Total surgical time intervention was noted at the end of each surgery. Each variable was analysed with total surgical time intervention with univariate and multiple linear regression. Out of 13 variables, 9 were found statistically significant. The most significant predictors for surgical difficulty were Body Mass Index, Depth of impacted tooth and Retromolar space. No postoperative complications were reported.

The term odontoma refers to hamartomatous lesions (malformations) rather than true neoplasms. They are the most common odontogenic tumour of jaws. They are slow growing in nature. Miss­ing permanent tooth or retained deciduous tooth brings the patient to the clinician usually dur­ing second decade of life. Two types of odontomas has been described: complex and compound. The compound type is more common and frequently found in the anterior maxilla. A 15-year-old male patient reported to department Oral and Maxillofacial Surgery, with missing permanent right central and lateral incisor with retained deciduous in place. Intraoral radiographs were done and multiple small denticles were seen. A total of 39 denticles of various sizes were enucleated from the lesion making this case very unusual.

The Aberdeen Royal Infirmary oral and maxillofacial surgery department is involved in the joint planning of cases with the orthodontic and restorative departments of the Aberdeen Dental Hospital to agree an optimal treatment plan for patients, with input from all three specialties. A 7-year-old girl was referred to the orthodontic department by her GDP due to non-eruption of the upper left central incisor. This was related to an associated dentigerous cyst. This presentation illustrates the phases of treatment involving marsupialization of the dentigerous cyst; surgical extraction and orthodontic treatment in order to provide the patient with the optimal outcome for her dentition. A retrospective assessment of the case is presented through photographs and radiological imaging outlining the chronology of the treatment and the outcome of marsupialization of the dentigerous cyst. It highlights this more conservative surgical approach as giving the best chances of preserving unerupted teeth in a younger patient. Marsupialization of a dentigerous cyst associated with UL1 and conservative management of cystic pathology led to preservation and natural mesial eruption of UL3 into the position of UL1. The unerupted UL1 associated with cystic pathology was extracted due to its ectopic position and root dilaceration. Seven years after diagnosis of the dentigerous cyst associated with the unerupted UL1, surgical and orthodontic management has facilitated the for patients tooth to erupt into the UL1 position. Restorative treatment is being planned following orthodontic treatment to restore for patients tooth to simulate the missing UL1. CPD/Clinical Relevance: This case encourages the appropriate referral of young patients with dentigerous cysts to achieve a satisfactory outcome.

INTRODUCTION: The dentigerous cyst is a developmental odontogenic cyst, whose therapeutic approach depends on the size of the lesion. This paper describes the treatment performed on a 14-year-old male patient who presented with a large dentigerous cyst on the permanent mandibular left second molar. CASE REPORT: The permanent mandibular left first molar was extracted, and the lesion was decompressed and marsupialized by excision of the overlying mucosa, opening a window into the cystic cavity and suturing an acrylic resin drain to the oral mucosa. The specimen removed during surgery confirmed the diagnosis of a dentigerous cyst. Within ninety days of surgery, there was a significant reduction in lesion size. Orthodontic treatment was then started, and included traction of the impacted tooth and opening of adequate space with a fixed appliance, providing favorable tooth alignment and leveling. CONCLUSION: Marsupialization and orthodontic traction should be considered when planning the treatment of a dentigerous cyst, mainly in young patients with large lesions, to avoid damage to the surrounding structures.

Objective The aim of this study was to evaluate the objective and subjective morbidity of symphyseal chin bone harvesting used for reconstruction of alveolar defects in young cleft patients. Design All patients who had undergone chin bone harvesting for alveolar cleft reconstruction in the period from 1992 through 2000 at the Department of Oral and Maxillofacial Surgery of the University Hospital Groningen, Groningen, The Netherlands, were invited to participate in this retrospective study. Patients’ acceptance, perioperative and postoperative morbidity were evaluated. A survey of the medical records was performed. In addition, the patients completed a questionnaire for their appreciation of the procedure. They were also subjected to a clinical and radiographic examination. Patients Thirty patients (21 males and 9 females; mean age 11.8 ± 3.6 years) participated in this study. Results Neither the medical records nor the experiences of the patients showed significant morbidity. The procedure was appreciated with 6.8 ± 3.5 (scale 0 to 10). Postoperative pain was scored as 1.2 ± 2.5 (scale 0 to 10). Three patients reported transient sensory disturbances at the donor site. Two patients showed a slight sensibility disorder in the symphyseal region. In three patients, an endodontic problem had developed in a lower incisor. Conclusion This study showed that chin bone harvesting for reconstructing alveolar cleft in young patients is a well-accepted procedure with low objective and subjective morbidity. Notwithstanding this low morbidity, the patients (and their parents) have to be informed about the risk of objective and subjective disturbances of the sensibility in the donor region and the risk of dental pulp necrosis.

Introduction: Professional skills, training and experience is mandatory for removing impacted 3rd molars with aid of local/general anesthesia, sedation. As for symptomatic 3rd molars decision for removing is not difficult usually, but for removing asymptomatic 3rd molars decision is less clear & requiring good clinical experience. Objective: The objective this study was to determine frequency of lingual nerve damage during surgical removal of impacted 3rd molar Design: Descriptive cases series Study Setting: This study was conducted at Department of Oral and Maxillofacial Surgery, Sandeman Provincial Hospital Quetta over 6 months (April 10, 2018 till Oct 10, 2018). Materials and Methods: All 149 cases after approval of hospital ethical committee fulfilling the inclusion criteria were included in this study. After clinical and radiographic diagnosis of affected tooth like depth, position or any other difficulty, the surgical procedure was selected. An impacted tooth was labeled when it was failed to explode into the dental arch within the expected developmental window. After operation surgical sites of all patients were reviewed by the same consultant after 7 days. It was instructed to all patients that report any problem in lingual sensation; by clinical examination sensory deficits were recognized bilaterally. Results: The mean age of all cases was 29.82 ± 6.81 years with minimum and maximum age of 18 and 40 years. There were 83(55.7%) male and 66(44.3%) were females. According to types of impaction 22(14.8%) cases had Mesioangular, 10(6.7%) had Horizontal, 12(8.1%) had Vertical, 17(11.4%) had Distoangular, 14(9.4%) had Class I, 15(10.1%) had Class II, 11(7.4%) had Class III, 14(9.4%) had Class A, 16(10.7%) had Class B and 18(12.1%) cases had Class C. A total of 16(10.7%) cases had inguinal injury in this study. Conclusion: This study concludes that 10.7% cases had inguinal injury in this study. So, this procedure of impacted mandibular third molar is associated with minor but expected complications like the lingual nerve damage. Hence, preoperative and intraoperative consideration must be taken up to reduce this preventable complication. Keywords: Impacted teat, 3rd molar, surgery, complications, lingual nerve damage

Complex odontoma, associated with an impacted tooth and a dentigerous cyst is a rare condition, especially in children. The management of such young patients could be difficult, and many aspects must be taken into consideration, like the patient’s collaboration during the procedures and the possibility of nerve damage. In this case report, we describe how to manage these kinds of patients, in order to reduce the risks and improve cooperation, taking advantage of the great benefits of conscious sedation.

Dentigerous cysts are cysts of the epithelial lining of the jaws derived from the dental follicle of unerupted teeth. These cysts are often found during routine radiographic examination along with unerupted teeth. Dentigerous cysts are usually asymptomatic unless the size becomes large, thus causing swelling. This case report describes the management of dentigerous cyst in the upper left canine region with canine impaction to the orbital floor. Dentigerous cyst was treated with cyst enucleation but the impacted canine was not taken because of the magnitude of risk that can occur which was a disruption to the eye muscle. Conclusion. Treatment of dentigerous cysts can be performed by enucleation technique and management related the impacted tooth at the orbital floor needs a further evaluation consideration.

Introduction. Odontomas are the most common type of odontogenic tumors and generally they are asymptomatic. These tumors are formed from enamel and dentin, and can have variable amounts of cement and pulp tissues. According to radiographic, microscopic, and clinical features, two types of odontomas are recognized: Complex and compound odontomas. Complex odontomas occur mostly in the posterior part of the mandible and compound odontomas in the anterior maxilla. Case Report. A young girl patient, 9 years old came to Department of Oral and Maxillofacial Surgery with a slow growing and asymptomatic swelling in her left posterior mandible for 5 years in his history taking. The panoramic radiograph show  a radioopacity and radiolucent lesion at the lower second molar region, with well-corticated limits. An insisional biopsi   confirmed  as compound odontoma. The surgery  performed with simple enucleation and curettage under general anaesthesia. Discussion. Compound odontomas are usually located in the anterior maxilla, over the crowns of unerupted teeth, or between the roots of erupted teeth. In this case report, Compound odontomas are found in the posterior mandible. Conclusion. Compound odontomas in the posterior mandible is a rare. The treatment of odontomas depends on the size of the lesion. The early diagnosis, the treatment of choice is conservative surgical enucleation and curettage and prognosis is excellent.
