This clinical report describes a direct composite laminate veneer restoration of the maxillary anterior teeth in one chair time to produce a better esthetic appearance in a patient with diastemata and missing laterals.

Anterior spacing is a common esthetic problem of patient during dental consultation. The most common etiology include tooth size and arch length discrepancy. Maxillary lateral incisors vary in form more than any other tooth in the mouth except the third molars. Microdontia is a condition where the teeth are smaller than the normal size. Microdontia of maxillary lateral incisor is called as “peg lateral”, that exhibit converging mesial and distal surfaces of crown forming a cone like shape. A carefully documented diagnosis and treatment plan are essential if the clinician is to apply the most effective approach to address the patient’s needs. A patient sometimes requires a multidisciplinary approach to correct the esthetics and to improve the occlusion. This case report describes the management of an adult female patient with a proclined upper anterior teeth, upper anterior spacing, deep bite and peg shaped upper right lateral incisor tooth through orthodontic and restorative treatment approach.

Abstract Aim The aim of this report is to present five-year follow-ups of two different applications for the use of direct laminate resin-based composite veneers to improve esthetics. Background Defects in the maxillary anterior teeth, such as enamel hypoplasia and peg lateral, can present esthetic challenges. Furthermore, a treatment plan that can be completed in a single appointment is highly desirable. This case report presents two different clinical cases involving the use of direct laminate resin-based composite veneers with five-year follow-ups. Case Report Case 1: A 17-year-old female patient was referred for treatment of her anterior teeth, which were unesthetically altered due to enamel hypoplasia and dental caries. A treatment plan was developed that included restoring the affected teeth with direct resin-based composite laminate veneers to improve the patient's appearance. The six maxillary anterior teeth were prepared for and restored with direct resinbased composite laminate veneers. At the fiveyear follow-up, the patient was satisfied with the restorations both esthetically and functionally. Case 2: A 15-year-old female patient also was referred for treatment to improve the appearance of her maxillary anterior teeth. A treatment plan was developed with two objectives: (1) to restore the undersized supernumerary crown in the area of the maxillary right lateral incisor and (2) to close the anterior diastemas. The facial surfaces were conservatively prepared and resin-based composite was applied with the aid of transparent crown forms. After completion of the treatment, the patient was recalled at six-month intervals. At the five-year follow-up appointment, the restorations were intact, no adverse effects were noted, and the resultant appearance was highly satisfactory for the patient. Summary The use of direct resin-based composite laminate veneers and adhesive bonding systems has been shown to provide an esthetic alternative to metal-ceramic or all-ceramic crowns for the rehabilitation of anterior teeth. This treatment option offers another advantage, namely a lower cost compared to an indirect technique. Other more complex and costly treatment options in the future are not ruled out. Clinical Significance In the present two cases, the initial and five-year follow-up results support the use of direct resin-based composite laminate veneers with minimal altering of healthy tooth structure. Such results should encourage clinicians to seek a cost-effective technique such as direct resin restorations to improve a patient's esthetic appearance in a single appointment. ABSTRACT This clinical report describes the treatment of a patient dissatisfied with the esthetics of the smile due to the presence of narrow teeth, yellow and the presence of diastema between teeth 11 and 21. Clinical examination revealed absence of the disocclusion guides. The treatment plan consisted in obtaining study casts, assembly in a semi-adjustable articulator and diagnostic waxing, to evaluate the possibilities of a restorative treatment. After the approved the viability of the procedure, dental bleaching was started in-office with 38% hydrogen peroxide associated with at-home bleaching with 15% carbamide peroxide, in order to reduce the yellowing of the teeth. Afterwards, the functional evaluation of the restorative procedure was carried out through the prior preparation of the mock-up. After functional approval of the mock-up, lip frenectomy was performed, since the frenulum is the cause of the diastema between the teeth 11 and 21. After 4 weeks was carried out the restorative procedure of teeth 13 to 23, with direct composite resin, improving the aesthetics of the smile and restoring the disocclusion guides.

Amelogenesis imperfecta (AI) has been defined as a group of hereditary enamel defects. It can be characterized by enamel hypoplasia, hypomaturation, or hypocalcification of the teeth. AI may be associated with some other dental and skeletal developmental defects. Restoration for patients with this condition should be oriented toward the functional and esthetic rehabilitation. This clinical report describes the oral rehabilitation of a young patient diagnosed with the hypoplastic type of AI in posterior teeth and hypomatured type of AI in anterior teeth.

Summary: The aim of this article is to present an additional alternative to restore theanterior zone, using crowns of steel chromium with a ceromer aesthetic front by means of Art-glass®.It is presented a clinical report of a masculine patient of 1 year 10 months of age, with earlychildhood dental caries, with coronal destruction of the four deciduous superior incisors. Thetreatment planning consisted of pulpar treatment with pulpectomy and endodontic obturation withVitapex®, two pulpotomies and the restoration of these with steel chromium crowns with Art-glass®.These crowns are an excellent option for an aesthetic restoration, because of its great variety ofcolors, its easy manufacture, and being an indirect technique allows leaving a free space of ceromerto obtain a good coronal adjustment. Permasalahan estetik merupakan salah satu hal penting dalam perawatan kedokteran gigi restoratif dan harus sejalan dengan penampilan yang alami serta harmonis berdasarkan prinsip-prinsip estetik. Untuk menginformasikan perawatan restorasi estetik komplek dari empat gigi anterior maksila. Laki-laki berusia 17 tahun datang dengan fraktur insisal karena kecelakaan. Empat tahun yang lalu, gigi tersebut telah dirawat endodontik dan direstorasi dengan mahkota jaket namun tujuh hari yang lalu, mahkota lepas. Pemeriksaan klinis menunjukkan bahwa gigi 11 fraktur horizontal pada sepertiga mahkota. Gigi 21 ditumpat dengan komposit tetapi memiliki anatomi dan warna yang buruk. Terdapat diastema di antara gigi insisivus kiri atas. Semua gigi normal terhadap perkusi, palpasi dan probing periodontal. Radiograf periapikal menunjukkan adanya resorpsi apikal pada gigi 11 dengan radiolusensi apikal dengan diameter 3-4 mm. Setelah pemeriksaan, perawatan yang dilakukan pada gigi 11 adalah retreatment dan apeksifikasi menggunakan MTA, pemasangan pasak komposit indirek dan mahkota jaket PFM. Gigi 21 dan diastema di antara gigi insisivus maksila kiri direstorasi dengan komposit. Gigi 22 juga direstorasi dengan resin komposit untuk mendapatkan lebar ideal. Kontrol 1 bulan menunjukkan gigi asimptomatik, terlihat adanya regenerasi jaringan periapikal dan gigi berfungsi normal. Perawatan keempat gigi anterior maksila dengan mempertimbangkan kondisi estetik dan konservasi sisa jaringan keras gigi menunjukkan keberhasilan sehingga gigi dapat berfungsi normal. ABSTRACT: Aesthetic Treatment of Four Maxillary Anterior Teeth with External Resorption. Esthetic aspect has become increasingly important in the modern restorative dentistry and synonymous with a natural, harmonious appearance based on esthetic principles. This case report aimed to inform the esthetically complex restorative treatment of four maxillary anterior teeth. A 17-year-old male had a fractured incisor following a traumatic incident. Four years earlier, the tooth was treated endodontically and restored with a crown. Seven days ago, the crown fell off. A clinical examination revealed that tooth 11 was horizontally fractured on the mid-third of the crown. Tooth 21 had been previously restored using composite, but it had a bad contour and color. There was also a diastema between left maxillary incisors. All teeth responded normally to percussion and palpation and had normal periodontal probing. A periapical radiograph showed apical resorption at teeth 11 with an apical rarefaction 3-4 mm in diameter. After complete examination and esthetic analysis, several treatments were undertaken. Tooth 11 was retreated using MTA to facilitate the apexification followed by indirect, customized composite post and porcelain-fused-to-metal crown. Tooth 21 and the diastema between the left maxillary incisors were closed using composite resin. Tooth 22 was also restored using composite resin to get the ideal width of the tooth. After 2 weeks, the treated teeth were asymptomatic, radiographic examination demonstrated apparent regeneration of periapicular tissue, and the teeth functioned normally. The teeth were successfully treated by endodontic therapy and restoration with crown and composite resin to restore esthetic problems.

Dental trauma injuries occur more frequently in upper anterior teeth, especially in enamel and dentin without pulp involvement. This article presents a clinical case report of a patient dissatisfied with the color of the tooth 21 restoration performed after suffering dental trauma. The format was adequate, so a previous molding of the palatal region was performed, and a condensing silicone guide was made. The use of the palatal guide facilitates the execution of the restoration and guides the layering of selected resins. Following the Natural Stratification concept, this case demonstrated a restorative treatment of direct restorations in composite resin, which can be considered an alternative for fractured anterior teeth, presenting clinical agility and excellent esthetic results.

Discrepancy between a large jaw size and small teeth results in a spaced alignment of teeth. Orthodontic treatment to correct such cases will not be sufficient for esthetic results of anterior teeth. Restorative treatment may be the ideal solution to reshape small teeth in well planned proportion to fit the size of the jaw and enhance the appearance of the patient smile. This paper presents the process of restoring spaced anterior teeth with minimally prepared ceramic veneers using the RED proportion.

<p><strong>Case report</strong>: Even if directly veneering discolored anterior teeth with composite is quite widespread, proper reproduction of their natural appearance requires much from the professional. Comprehensive management, from planning to finishing and polishing, of a severely discolored upper central incisor through its directly<strong> </strong>veneering is critically reported. <strong>Relevance</strong>: Direct composite laminate veneers are, indeed, a viable treatment option to correct esthetic abnormalities involving most of the labial surface of anterior teeth. This relative conservative therapy, since accurately performed by the well qualified dentist, will assign the patient esthetic, functional and emotional rehabilitation, culminating in a harmonious and pleasant smile.<strong></strong></p>
