Knowledge and Use of Services by the Elderly: A Critical Review of the Literature

This article critically reviews the existing literature on the perceptions, knowledge, and use of services by the elderly. Definitive statements concerning these topics are difficult to make because of the contradictory research findings and inadequacies of existing studies. However, the following observations are supported by this review: many elderly do not hold positive attitudes towards services, a bare majority appear aware of services, utilization rates are extremely low, and the correlates of service use are not well understood. In addition, this article identifies a number of weaknesses with the existing research and discusses the following specific recommendations for improvement: conceptualization and operationalization of knowledge and use as continuums, not as discrete phenomena, examination of the processes by which individuals come to know of and use services, and consideration of the roles played by informal networks as facilitators or deterrents to knowledge and use of services.

Partnership with startups offers large firms knowledge about, and access to new technologies. Incumbents’ emphasis on corporate-startup collaboration has therefore reached a new level and various models for corporate-startup collaboration can now be found among large enterprises. “Co-creation” between large firms and technology startups, is one of these models that increases in traction. The model is, however, under-researched and research on frameworks and metrics for evaluating the business effects from corporate-startup co-creation is scarce. The purpose of this paper is therefore to extend the existing body of knowledge by investigating frameworks and metrics for evaluating corporate-startup “co-creation” and to suggest a framework for evaluation of corporate-startup co-creation programs. A literature review on identified frameworks and metrics is presented, covering research findings on evaluation models for corporate-startup collaboration. The main finding in this paper is a “multi-stakeholder framework” for evaluating the collaboration’s results in corporate-startup co-creation models.

ABSTRACTBackground:Empirical data on the use of services due to mental health problems in older adults in Europe is lacking. The objective of this study is to identify factors associated with service utilization in the elderly.Methods:As part of the MentDis_ICF65+ study, N = 3,142 people aged 65–84 living in the community in six European and associated countries were interviewed. Based on Andersen's behavioral model predisposing, enabling, and need factors were analyzed with logistic regression analyses.Results:Overall, 7% of elderly and 11% of those with a mental disorder had used a service due to mental health problems in the last 12 months. Factors significantly associated with underuse were male sex, lower education, living in the London catchment area, higher functional impairment and more comorbid mental disorders. The most frequently reported barrier to service use was personal beliefs, e.g. “I can deal with my problem on my own” (90%).Conclusion:Underutilization of mental health services among older people in the European community is common and interventions are needed to achieve an adequate use of services.

O transtorno afetivo bipolar (TAB) é uma condição potencialmente grave, caracterizada por mudanças de humor e disfunção cognitiva e funcional. O presente artigo revisa as evidências atualizadas das intervenções farmacológicas e não medicamentosas direcionadas ao TAB em idosos. Diretrizes baseadas em evidências, embora sejam úteis na promoção de uma terapia racional e eficaz, ainda são relativamente reduzidas no TAB em idosos. As recomendações atuais para mania aguda incluem o uso de antipsicóticos atípicos (por exemplo, risperidona, quetiapina, olanzapina), lítio (de maneira criteriosa) e a eleição de valproato como terapia padrão ouro. Na depressão aguda do TAB, os agentes de primeira linha em monoterapia podem incluir lítio, lamotrigina, quetiapina e quetiapina de liberação prolongada. No que se refere à escolha do estabilizador de humor ideal, o maior potencial de efeitos colaterais relacionados ao lítio vem desestimulando sua prescrição em indivíduos idosos. A eletroconvulsoterapia, embora limitada pela evidência, pode ser uma opção para casos graves/refratários.
