BACKGROUND The scientific and medical communities require advanced study of maternal health and well-being to develop improved outcomes for mothers and their babies but can be challenged measuring mental health during pregnancy and into postpartum due to accessibility and cooperation issues. OBJECTIVE This work employs digital methods to describe mental health and maternal experience from the first trimester of pregnancy through 3 months postpartum among a cohort similar to the general population of pregnant women in the US. The objective was to collect a data set of longitudinal measures for epidemiologists to develop predictive modeling to determine early in pregnancy which women will develop perinatal depression with a high degree of accuracy. METHODS The authors recruited 1,179 pregnant women from BabyCenter.com and followed them with repeated measures using custom, mobile optimized, questionnaires administered online. The authors employed a combination of typical and novel digital strategies to minimize attrition and build trust among participants. In all stages of recruitment, data collection, and participant engagement, the authors adapted their strategies to meet the target audience and leveraged digital means to achieve the final goal. RESULTS Active participants completed 7,757 assessments. These assessments contribute to a robust data set that is enabling predictive models that can identify with a high degree of accuracy women at risk of developing mood disorders before symptoms onset. CONCLUSIONS The study’s success at retaining pregnant women through the postpartum period was based on leveraging a platform with an already established reach and trust among the target audience (BabyCenter), and adapting engagement methodologies tailored to the needs of this target audience.

Abstract Background: Pregnancy and early motherhood are sensitive times where epidemic disease outbreaks can affect mental health negatively. Countries and health care systems handled the pandemic and lockdowns differently and knowledge about how the COVID-19 pandemic affected the mental well-being of pregnant women and new mothers is limited and points in different directions. Aim. to investigate symptoms of anxiety and depression in a population of pregnant women and new mothers in various stages of infection pressure and lock down during the first 15 months of the COVID-19 pandemic in Denmark. Methods: The study population was nested an inception cohort of women recruited in their first trimester of pregnancy. Data about mental health of the woman was obtained in relation to pregnancy and child development (first trimester, 8 weeks postpartum and 5 months postpartum), and data were analysed cross-sectionally according to calendar time (periods defined by infection rate and lock-down during the COVID-19 epidemic).Results: No differences in reported levels of depressive symptoms between the six examined time periods of the pandemic were observed. Specifically, symptoms remained unchanged after the first lock-down. No major changes in anxiety symptoms were observed in relation to increased infection pressure or lockdowns, but a small increase was observed during the second lockdown in women 8 weeks postpartum. Conclusion: No clear change in mood among pregnant women was seen between during the stages of Covid-19 pandemic in Denmark.

Post-secondary institutions across North America have adopted animal-assisted activities as a way to promote better mental health in their students. The current research study of 242 Canadian college and university students sought to contribute to our collective understanding of the aspects of the programs and characteristics of students that are related to promotion of better mental health in post-secondary students including decreased stress, and increased happiness and well-being. Results of a repeated measures design showed that students demonstrated greater positive effects on stress, happiness, and well-being when they touched dogs as compared to when they observed them. Furthermore, positive mental health outcomes were correlated with greater durations of contact as well as with higher levels of animal affiliation in students. Implications for post-secondary institutions are discussed.

Abstract Background: Prenatal anxiety and depressive symptoms have significantly increased since the onset of the coronavirus (COVID-19) pandemic. Furthermore, home confinement regulations have caused a drastic increase in time spent sedentary. Online group fitness classes may be an effective strategy that can increase maternal physical activity and improve mental health outcomes by providing an opportunity for social connectedness. The present study explores the experiences of pregnant women who participated in an online group exercise program during the pandemic and identifies relationships with maternal mental health and well-being. In addition, we present person-informed recommendations on how to improve the delivery of future online prenatal exercise programs.Methods: Semi-structured interviews were conducted with pregnant women (8-39 weeks of pregnancy) who participated in an online group exercise program, from March to October 2020 in Spain. A phenomenological approach was taken, and open-ended questions were asked to understand women’s experiences throughout the pandemic and the role the online exercise classes may have had on their physical activity levels, mental health, and other health behaviours such as diet. A thematic analysis was performed to evaluate data. In addition, women completed the State-Trait Anxiety Inventory (STAI) and these data supplemented qualitative findings. Results: Twenty-four women were interviewed, and the average STAI score was 32.23 ± 9.31, ranging from low to moderate anxiety. Thematic analysis revealed that women felt safe exercising from home, an increased availability of time to schedule a structured exercise class, and consequently an improvement in their adherence to the program and other behaviours (i.e., healthier diet). Women emphasized feeling connected to other pregnant women when they exercised online together, and overall, this had a positive effect on their mental well-being. Women suggested that future online exercise programs should include flexible options, detailed instructions and facilitation by a qualified exercise professional.Conclusion: Pregnant women are receptive to online group exercise classes and expressed that these are an accessible option to accommodating physical activity during the pandemic. In addition, the online group environment provides an important sense of connectivity among pregnant women exercising together and this may mitigate the detrimental effect of COVID-19 on maternal mental health.

Abstract. Introduction: Following the onset of the COVID-19 pandemic, most countries imposed strict governmental lockdowns. Research investigating the psychological impact of pandemic-induced lockdowns is accumulating, though to date no study has examined the psychological health and associated parameters of well-being in countries that underwent additional lockdowns as the pandemic continued into resurgence “waves.” Aim: The present study provides an overview of the psychological impact of COVID-19 across the two lockdowns in the Cypriot population. Methods: In total, 957 participants completed an online survey during the first lockdown, 134 of whom completed a similar survey again during the second lockdown. The outcomes assessed included stress, positive and negative affect, and well-being. Results: The results indicated no population-wide severe reactions in the participants. Repeated measures analyses showed similar mental health levels during both the first and the second lockdowns. Further inspection of participants’ scores indicated that, for all mental health variables, approximately half of the participants improved, while the other half deteriorated. Discussion: Perceived social support and psychological flexibility predicted most psychological outcomes during both lockdowns. Further research is necessary to understand the continuing effects of the pandemic and associated lockdowns on mental health.

Objective:This study reassesses the generational disparity in immigrant offending. Patterns and predictors of offending are compared using traditional peer-based models and an alternative within-family (parent–child dyad) model.Method:The National Longitudinal Survey of Youth (1979; NLSY79) and NLSY-Child and Young Adult (NLSY_CYA) data are merged to create an intergenerational data set to compare generational disparities in immigrant offending across peers and within families. Differences in self-reported offending (prevalence and variety) by immigrant generation are assessed using a combination of descriptive analyses (χ2and analysis of variance) and regression models.Results:While NLSY_CYA children generally are at a greater risk of offending compared with the NLSY79 mothers, the difference in offending is greatest between first-generation mom and second-generation child dyads. Disparities in offending are driven in large part by exceedingly low levels of offending among first-generation immigrants.Conclusion:Although the factors driving an increase in offending between parent–child generations are not unique to immigrants, they are amplified in immigrant families. Whereas the second generation is remarkably similar to their U.S.-born counterparts in terms of their involvement in crime, suggesting a high degree of swift integration, the greater involvement in crime among the children of immigrants compared to their foreign-born mothers suggests a decline in well-being across successive generations.

IntroductionRates of mental health illnesses and burnout are increasing internationally. Therapeutic yoga is increasingly used to improve and maintain physical, mental and emotional well-being and general health. This protocol describes a study to evaluate the effectiveness of an existing primary care group-based therapeutic yoga programme, the Yoga of Stress Resilience programme, which combines yoga and psychotherapeutic techniques, in improving mental health and decreasing burnout. Implementation factors will also be evaluated for potential scale-up.Methods and analysisA pragmatic before–after interventional trial design will be used to study changes in occupational participation and mental health outcomes, including anxiety, depression, burnout, functional impairment, insomnia, perceived stress, loneliness, self-compassion and readiness for change in adults experiencing anxiety and burnout. Repeated measures analysis of variance will be used to determine changes in outcome measures over time. Regression and multivariate analyses will be conducted to examine relationships between participant characteristics and outcomes and among various outcomes. The Reach, Effectiveness, Adoption, Implementation, and Maintenance framework will be used to guide the analyses.Ethics and disseminationApproval from the Hamilton Integrated Research Ethics Board has been waived: project number 7082 (full review waived). Informed consent will be obtained prior to enrolling any participant into the study. All data will be kept confidential. Peer-reviewed publications and presentations will target researchers and health professionals.Trial registration numberThe ClinicalTrials.gov registry (NCT03973216).

Purpose: Within the millennial population cohort, identify groups reporting increased risk of nonspecific psychological distress. As the largest living population cohort, taking stock of health and well-being early is necessary as substantial national resources may be needed as this cohort ages. Design: The 2017 National Health Interview Survey data, an annual multipurpose survey of the US population, was used. Sample: A sample of 7303 respondents were created by limiting data set to birth years 1980 to 1998. Measures: Outcomes were feeling like everything is an effort, worthlessness, hopelessness, restlessness, nervousness, and sadness. Combined these statements of feeling make up a measure of nonspecific psychological distress, past 30 days. Analysis: A logistic regression was performed on each outcome. All models controlled for demographic variables known to be associated with psychological distress. Results: Females are 1.4 times more likely than males to report nonspecific psychological distress ( P < .001), whereas Hispanics and Blacks are less likely to report nonspecific psychological distress (odds ratio [OR] = 0.49, OR = 0.57, P < .001). American Indians were less likely to report worthlessness (OR = 0.30, P < .05). However, multiple race individuals increasingly reported hopelessness (OR = 1.55, P < .05). Young adults are less likely than emerging adults to report sadness (OR = 0.85, P < .05). Conclusion: In this sample, racial/ethnic groups fared better than referent groups. Health programs need to integrate intersectional identities into promotion of mental health.

Recognizing that students benefit when they receive autonomy-supportive teaching, the current study tested the parallel hypothesis that teachers themselves would benefit from giving autonomy support. Twenty-seven elementary, middle, and high school physical education teachers (20 males, 7 females) were randomly assigned either to participate in an autonomy-supportive intervention program (experimental group) or to teach their physical education course with their existing style (control group) within a three-wave longitudinal research design. Manipulation checks showed that the intervention was successful, as students perceived and raters scored teachers in the experimental group as displaying a more autonomy-supportive and less controlling motivating style. In the main analyses, ANCOVA-based repeated-measures analyses showed large and consistent benefits for teachers in the experimental group, including greater teaching motivation (psychological need satisfaction, autonomous motivation, and intrinsic goals), teaching skill (teaching efficacy), and teaching well-being (vitality, job satisfaction, and lesser emotional and physical exhaustion). These findings show that giving autonomy support benefits teachers in much the same way that receiving it benefits their students.

Transition Space at the Museum is a community arts-based group program aiming to foster the psychosocial rehabilitation of adolescents and young adults with mental health problems. In this pilot evaluation, we assessed the preliminary effectiveness of the program at improving participants’ well-being and social functioning. Following a mixed-methods, single-group, repeated-measures design, we collected data before, during, and after program from participants, clinicians, and close relatives using standardized questionnaires and semi-structured interviews. We found converging quantitative and qualitative results supporting the safety and potential of the program to improve the way participants feel and function socially in the short term.

Background The growing incidence of mental ill-health in health professionals, including doctors, is a global concern. Although a large body of literature exists on interventions that offer support, advice and/or treatment to sick doctors, it has not yet been synthesised in a way that takes account of the complexity and heterogeneity of the interventions, and the many dimensions (e.g. individual, organisational, sociocultural) of the problem. Objectives Our aim was to improve understanding of how, why and in what contexts mental health services and support interventions can be designed to minimise the incidence of doctors’ mental ill-health. The objectives were to review interventions to tackle doctors’ mental ill-health and its impact on the clinical workforce and patient care, drawing on diverse literature sources and engaging iteratively with diverse stakeholder perspectives to produce actionable theory; and recommendations that support the tailoring, implementation, monitoring and evaluation of contextually sensitive strategies to tackle mental ill-health and its impacts. Design Realist literature review consistent with the Realist And Meta-narrative Evidence Syntheses: Evolving Standards quality and reporting standards. Data sources Bibliographic database searches were developed and conducted using MEDLINE (1946 to November week 4 2017), MEDLINE In-Process and Other Non-indexed Citations (1946 to 6 December 2017) and PsycINFO (1806 to November week 2 2017) (all via Ovid) and Applied Social Sciences Index and Abstracts (1987 to 6 December 2017) (via ProQuest) on 6 December 2017. Further UK-based studies were identified by forwards and author citation searches, manual backwards citation searching and hand-searching relevant journal websites. Review methods We included all studies that focused on mental ill-health; all study designs; all health-care settings; all studies that included medical doctors/medical students; descriptions of interventions or resources that focus on improving mental ill-health and minimising its impacts; all mental health outcome measures, including absenteeism (doctors taking short-/long-term sick leave); presenteeism (doctors working despite being unwell); and workforce retention (doctors leaving the profession temporarily/permanently). Data were extracted from included articles and the data set was subjected to realist analysis to identify context–mechanism–outcome configurations. Results A total of 179 out of 3069 records were included. Most were from the USA (45%) and had been published since 2009 (74%). More included articles focused on structural-level interventions (33%) than individual-level interventions (21%), but most articles (46%) considered both levels. Most interventions focused on prevention, rather than treatment/screening, and most studies referred to doctors/physicians in general, rather than to specific specialties or career stages. Nineteen per cent of the included sources provided cost information and none reported a health economic analysis. The 19 context–mechanism–outcome configurations demonstrated that doctors were more likely to experience mental ill-health when they felt isolated or unable to do their job, and when they feared repercussions of help-seeking. Healthy staff were necessary for excellent patient care. Interventions emphasising relationships and belonging were more likely to promote well-being. Interventions creating a people-focused working culture, balancing positive/negative performance and acknowledging positive/negative aspects of a medical career helped doctors to thrive. The way that interventions were implemented seemed critically important. Doctors needed to have confidence in an intervention for the intervention to be effective. Limitations Variable quality of included literature; limited UK-based studies. Future work Use this evidence synthesis to refine, implement and evaluate interventions.
