Basic health research in 2007 showed the existence of the gap of IPKM (community health development index) among provinces, including Aceh province. Maternal and child health issue were important indicators in IPKM. This research aimed to determine knowledge of Posyandu’s cadres and mothers as well as the perspective of health offi cer towards Posyandu activities. This was a qualitative study with participatory observation design. There were two areas (Puskesmas) chosen as the research location, and at each location was conducted focus group discussion (FGD) to the mothers and Posyandu’s cadres and in-depth interview towards the informant (health offi cer). Generally, Posyandu’s cadres have good knowledge about maternal and child health, while mothers have less knowledge. The results showed that Posyandu has not been optimal in health service. This condition associated with low visits of mothers as well as the involvement of Posyandu’s cadres themselves. External factors (support of husband, family, social environment) could interfere the belief of mothers towards Posyandu services. Knowledge becomes an important aspect to infl uence awareness and willingness of mothers to utilize the service of Posyandu. Abstrak Hasil Riskesdas 2007 menunjukkan adanya kesenjangan IPKM (Indeks Pembangunan Kesehatan Masyarakat) antar propinsi maupun pada tingkat propinsi itu sendiri seperti di Provinsi Aceh. Kesehatan ibu hamil dan balita merupakan salah satu indikator penting dalam IPKM. Penelitian ini bertujuan untuk mengetahui pengetahuan kader Posyandu maupun ibu balita serta perspektif tenaga kesehatan terkait kegiatan Posyandu. Penelitian ini merupakan penelitian kualitatif dengan disain observasi partisipatif. Terdapat dua wilayah terpilih sebagai lokasi penelitian, dan pada setiap lokasi dilakukan diskusi kelompok terarah terhadap kelompok ibu balita dan kader Posyandu wawancara mendalam terhadap kader Posyandu dan ibu balita, serta tenaga kesehatan. Secara umum pengetahuan kader Posyandu telah cukup baik, namun untuk ibu balita belum sepenuhnya baik. Hasil penelitian menunjukkan belum optimalnya pelayanan Posyandu. Ketidakoptimalan tersebut berhubungan dengan rendahnya kunjungan para ibu balita, maupun keterlibatan kader Posyandu itu sendiri. Faktor eksternal (pandangan suami, keluarga & lingkungan sosial) dapat menjadi unsur yang mempengaruhi keyakinan ibu balita terhadap pelayanan Posyandu. Pengetahuan menjadi aspek penting terhadap kesadaran dan kemauan ibu balita untuk datang ke Posyandu.

Background: Government needs community empowerment especially in Posyandu activity to improve health degree in Indonesia. Posyandu is the spearhead in maternal and child health services. Therefore, the cadre is a driver in Posyandu activities so that innovation is needed for an application to assist the task of cadres in reporting and recording of maternal and child health data.Objective: This study aims to determine the linkage of government policy in the use of mobile application apps by cadres on Anjungan Mandiri Posyandu (AMP).Method: The research method used is qualitative with case study approach. This research was conducted on active cadre in Posyandu of Pasawahan Village Kidul Subdistrict of Pasawahan Purwakarta Regency on May to July 2017. The data was collected by Focus Group Discussion (FGD) on 15 cadres. Sampling technique using purposive sampling. Results: The results of this study found that the recording and reporting of data in the form of Posyandu Information System (SIP) by cadres using AMP.Conclusion: It can be concluded in this research that with the application of mobile apps health on AMP can assist duty cadre in recording and reporting data to health center. Increasing interest in the utilization of human milk for the feeding of premature infants has been evident in the United States in the past few years and in the Scandinavian countries and several other parts of Europe for a longer time. Because premature infants exhibit even greater limitations of gastrointestinal and digestive functions and of immunologic status than do full-term infants, human milk may offer particular advantages. Such advantages include ready digestibility and absence of foreign proteins; in addition, it is possible that feeding of human milk may protect against enteric infections. In view of current interest in human milk for feeding premature infants, it seemed desirable to consider the possible associated benefits and risks. Such a consideration was undertaken on December 2 and 3, 1975, in Chevy Chase, Maryland, under the sponsorship of the Office for Maternal and Child Health, Bureau of Community Health Services, Health Service Administration, Department of Health, Education and Welfare. The meeting was attended by individuals believed knowledgeable about one or another area related to possible benefits and risks of feeding fresh or processed human milk by bottle or gavage to premature infants in hospitals. Participants in the workshop included representatives of two com- mittees of the American Academy of Pediatrics, two committees of the National Academy of Sciences-National Research Council, and representatives of the American College of Obstetricians and Gynecologists, the American Nursing Association, the Center for Disease Control, and the National Institute of Child Health and Human Development. Several representatives of the Office for Maternal and Child Health and of the Division of Clinical Services, Bureau of Community Health Service were also present.

Abstract Performance-based financing (PBF) has been promoted and increasingly implemented across low- and middle-income countries to increase the utilization and quality of primary health care. However, the evidence of the impact of PBF is mixed and varies substantially across settings. Thus, further rigorous investigation is needed to be able to draw broader conclusions about the effects of this health financing reform. We examined the effects of the implementation and subsequent withdrawal of the PBF pilot programme in the Koulikoro region of Mali on a range of relevant maternal and child health indicators targeted by the programme. We relied on a control interrupted time series design to examine the trend in maternal and child health service utilization rates prior to the PBF intervention, during its implementation and after its withdrawal in 26 intervention health centres. The results for these 26 intervention centres were compared with those for 95 control health centres, with an observation window that covered 27 quarters. Using a mixed-effects negative binomial model combined with a linear spline regression model and covariates adjustment, we found that neither the introduction nor the withdrawal of the pilot PBF programme bore a significant impact in the trend of maternal and child health service use indicators in the Koulikoro region of Mali. The absence of significant effects in the health facilities could be explained by the context, by the weaknesses in the intervention design and by the causal hypothesis and implementation. Further inquiry is required in order to provide policymakers and practitioners with vital information about the lack of effects detected by our quantitative analysis.

Abstract Background Various factors influence health service utilization at the community level. Research on the barriers to uptake of local health services is essential to reduce maternal and child mortality and morbidity. The Amazon region of Peru has some of the poorest health indicators in the country. The current study set out to better understand the health-seeking behavior and perspectives of mothers in Amazonian communities, exploring individual- and contextual-level barriers for seeking care at local health facilities for common maternal and child health issues. Methods The study employed a mixed-methods design by conducting 50 structured interviews with mothers of children under the age of 4. The study took place in 5 communities in Loreto, Peru. The quantitative data was analyzed with descriptive statistics to identify participants’ socio-demographic characteristics and reported utilization of health services. The qualitative data was analyzed in three rounds: inductive codebook development, application of the codebook, and thematic synthesis to contextualize the quantitative results and better understand the perspectives of the mothers regarding maternal and child health issues and the local health services. Results Overall, reported health service utilization among study participants was relatively high. However, the mothers identified several individual- and contextual-level factors that may affect their experiences and the health-seeking behaviors of other mothers in their communities: (i) embarrassment, fear, and trust, (ii) insufficient number and poor attitudes of health personnel, (iii) limited supply of basic medicines and materials in the health facility, and (iv) low demand for family planning services and limited awareness of adolescent-specific services. Conclusion Several findings in the current study reflect the reduced conditions of health services, while others display that many mothers maintain a positive outlook on the health services available to them and are proactive in the care of their child. The study provides valuable insight into the use of local health services and the common perspectives that are hindering further uptake at the community level in the Amazon of Peru, with important implications for health policy.
