Introduction. The laryngeal hemangioma in children is a benign vascular neoplasm but it could become malignant after localization. Case outline. After two weeks of corticosteroid treatment in a 15-month-old boy, there were no desired results. This case report is a small contribution to the research of targeted Propranolol treatment in infantile laryngeal hemangioma and the role of Epinephrine in the initial treatment in life-threatening conditions. Conclusion. When laryngeal hemangioma has ?bad? localization and any surgical treatment is denied in spite of the vital risk, the choices of treatment are epinephrine (as the first choice) and propranolol.

Background Transorbital intracranial penetrating injury is rare. Damage caused by a huge metallic foreign body is very critical and life-threatening. Method We report an extremely rare case of transorbital intracranial penetrating metal strip (a car windshield wiper), which has not previously been reported in the literature. Results Emergency craniotomy was performed; the object was removed successfully, and the patient's life was saved. Conclusion With the life-threatening penetrating brain injury caused by a huge foreign body, prompt surgical treatment and comprehensive postoperative treatment are important to save patients' lives.

<p><strong>Objective</strong>. The aim of this report was to describe the occurrence of a bacterial brain abscess in a healthy individual, without any predisposing condition. <strong>Case report</strong>. A thirteen-year old boy was admitted to the Department of Neurosurgery after the onset of vomiting, headache and dizziness. A neurological deficit was detected during the physical examination so urgent magnetic resonance imaging of the brain was performed, revealing an intrahemispheric, right positioned solitary expansive mass with ring enhancement. Purulent material was obtained during osteoplastic craniotomy with total extirpation of the brain abscess. Aggregatibacter aphrophilus and Bacteroides uniformis were isolated. The patient’s general condition improved and the neurological deficit subsided as a result of the prompt recognition and treatment of this life threatening condition. <strong>Conclusion</strong>. To achieve a favourable clinical outcome, prompt recognition and surgical treatment of a brain abscess are of primary importance,followed by administration of appropriate antimicrobial therapy. To our best knowledge, this is the first report of this combination of microorganisms as the cause of a brain abscess.</p>

AbstractSinus pericranii (SP) are abnormal vascular connections between extracranial scalp venous channels and intracranial dural sinuses. This vascular abnormality rarely results in significant sequelae, but in select cases, it can be symptomatic. We describe the case of a 7-year-old girl with an SP who experienced intermittent visual, motor, and sensory symptoms not previously described in the literature. Her symptoms resolved after surgical treatment of the SP. We propose a mechanism for her symptoms and the rationale for the role of neurosurgical intervention along with a review of the literature.

Ectopic pregnancy is a life-threatening condition occurring in 1-2% of all pregnancies. The most common site of implantation for an ectopic pregnancy is the fallopian tube. Authors report a case of recurrent ipsilateral ectopic pregnancy following right partial salpingectomy of a 29-Year-Old woman that led to tubal rupture. The pregnancy was conceived spontaneously. Diagnostic of ruptured ectopic pregnancy was done after clinical and ultrasonography examination. The presence of a massive hemoperitoneum with a positive pregnancy urinary test that lead us to the diagnosis of ectopic pregnancy. She underwent a laparotomy for a suspicion of ruptured ectopic pregnancy. The ectopic pregnancy was identified in the left remnant fallopian tube. Partial salpingectomy, removal of tubal stump, and resection of the uterine cornua, was performed. The postoperative recovery was uneventful. She has stayed for five days at the Hospital. All patients, even though they have already received a definitive contraception by tubal section and ligature or unilateral or by bilateral salpingectomy for any reason, must seek an ectopic pregnancy in case of pelvic pain, vaginal bleeding and/ or amenorrhea. Authors propose to carry a total salpingectomy after a chosen surgical treatment.

Embryonal tumor with abundant neuropil and true rosettes (ETANTR) is rarely seen in the brainstem, and there are few case reports of brainstem ETANTR in the literature. Accordingly, the characteristics and the role of surgical treatment of this rare entity remain unclear. The authors present a case of brainstem ETANTR involving a 33-month-old boy along with a review of the literature and discuss the role of surgical removal in the treatment of this entity. In the authors’ case, the tumor was surgically treated with subtotal resection, which resulted in improvement of the patient’s preoperative symptoms. Chemotherapy was initiated but did not appear to be effective, radiotherapy was declined, and the boy died 6 months after the operation. Based on their analysis of 10 previously reported cases and their own case, the authors conclude that, with respect to survival, surgery may be beneficial even in cases of ETANTR in the brainstem. They note, however, that further studies with a large number of cases are needed to validate the role of surgical treatment in brainstem ETANTR.

Electrical storm is a life-threatening condition and requires immediate treatment. In most cases ventricular arrhythmia originates from previously formed lesions in the cardiac muscle. Such patients, following the necessary initial treatment, should be forwarded to catheter ablation, which is proven to reduce arrhythmia recurrence and to improve overall morbidity. Along with the technological progress related to electroanatomical mapping the growing role of meticulous substrate mapping and modification for successful ablation is being recognized.

Surgery has been considered the first choice of treatment in planocellular skin cancers. However, adjuvant radiotherapy is often required in R1 resection or in lymph node positivity. Inoperable cases are also treated with ionizing radiation with palliative purpose. The authors present a case report of a successful treatment of an 87-year-old diabetic patient with a T4N1M0 stage periauricular destructive tumour treated with 3D conformal adaptive radiotherapy. Complete remission occurred although the initial treatment aim was only palliation. Orv. Hetil., 2015, 156(40), 1625–1627.

Abstract Background: Pneumatosis cystoides intestinalis (PCI) is a rare disease that occurs frequently in the large and small intestine and has no obvious clinical symptoms. It is difficult to preoperatively diagnose and is easily undiagnosed or misdiagnosed. The main pathological features are air-bearing cysts in the intestinal submucosa, intermuscular wall and subserous membrane. Case presentation: PCI is a rare disease, which often occurs in the large intestine and small intestine,without obvious clinical symptoms. It is difficult to diagnose before operation and is easy to be missed and misdiagnosed. Duvernoi first described the disease in 1730. The main pathological features were air-bearing cysts in intestinal submucosa, intermuscular wall and subserous membrane. Conservative treatment is the first choice when there are no serious complications, and timely surgical treatment is needed for serious and life-threatening complications. This report presents the clinical and pathological analysis of PCI in a 92-year-old man. The patient was admitted to hospital because of acute peritonitis,and was diagnosed as PCI with schistosomiasis after emergency operation. Postoperative follow-up is generally in good condition.Conclusions: The incidence of PCI is low. Because of a lack of specificity in clinical manifestations and endoscopic findings, PCI is difficult to diagnose preoperatively and is easily missed and misdiagnosed. Conservative treatment is the first choice when there are no serious complications; however, timely surgical treatment is required for serious and life-threatening complications.PCI treatment is effective andconvenient, and the prognosis is optimistic. Therefore, clinicians should increase their awareness of PCI to avoid unnecessary misdiagnosis.
