Các rối loạn nhịp thất trên người không có bệnh tim thực tổn, hay còn được gọi là các rối loạn nhịp thất vô căn (idiopathic ventricular arrhythmias), đa phần đều khởi phát từ đường ra tâm thất. Phân biệt ngoại tâm thu thất từ đường ra thất phải và đường ra thất trái chẩn đoán còn khó khăn, đặc biệt là rối loạn nhịp thất có dạng block nhánh trái với chuyển tiếp tại V3. Mục tiêu của nghiên cứu chúng tôi là: mô tả các đặc điểm điện tâm đồ bề mặt của ngoại tâm thu thất có nguồn gốc từ đường ra thất phải và đường ra thất trái; đồng thời tìm hiểu vai trò của tỷ lệ SV2/RV3 trên điện tâm đồ bề mặt trong chẩn đoán phân biệt ngoại tâm thu thất có nguồn gốc ở đường ra thất trái và đường ra thất phải. Đối tượng và phương pháp nghiên cứu: mô tả cắt ngang 150 bệnh nhân ngoại tâm thu thất không có bệnh tim thực tổn và có chỉ định thăm dò điện sinh lý và điều trị RF. Kết quả nghiên cứu: chúng tôi tiến hành nghiên cứu 150 bệnh nhân có ngoại tâm thu thất dạng block nhánh trái đã được thăm dò điện sinh lý và triệt đốt RF thành công ở đường ra thất phải (ĐRTP; n=110) hoặc đường ra thất trái (ĐRTT; n= 40). Các kích thước biên độ sóng được đo đạc bằng thước cặp điện tử. Tỷ lệ SV2/RV3 chính là biên độ sóng S ở chuyển đạo V2 chia cho biên độ sóng R ở chuyển đạo V3 của nhịp ngoại tâm thu thất. Kết quả chỉ số SV2/RV3 ở đường ra thất trái nhỏ hơn đáng kể so với đường ra thất phải có ý nghĩa thống kê (1,23 ± 0,78 so với 6,07 ± 6,32 và p < 0,001). Diện tích duới đường cong (AUC) cho chỉ số SV2/RV3 là 0,934, với giá trị tới hạn là ≤ 1,6 dự đoán ngoại tâm thu thất đường ra thất trái với độ nhạy 90,9% và độ đặc hiệu là 80%. Khi so sánh chỉ số này với 1 số các chỉ số khác ở cả nhóm bệnh nhân nghiên cứu và nhóm bệnh nhân có chuyển tiếp tại V3 chúng tôi nhận thấy chỉ số của chúng tôi cho kết quả cao nhất về giá trị dưới đường cong ROC và độ nhạy, độ đặc hiệu. Chỉ số này còn rất có giá trị ứng dụng lâm sàng cho các nhà nhịp học do tính toán khá dễ dàng và nhanh chóng chỉ với điện tâm đồ thường quy 12 chuyển đạo. Kết luận: Chỉ số tỷ lệ SV2/RV3 rất có giá trị trong chẩn đoán phân biệt ngoại tâm thu thất đường ra thất trái và đường ra thất phải, hữu dụng trên thực hành lâm sàng cho các bác sỹ nhịp học.

This study aims to evaluate the association of serum growth differentiation factor 15 (GDF-15) with in-hospital mortality and arrhythmic risks in patients with acute myocardial infarction (AMI). A total of 296 consecutive patients with AMI were enrolled in our hospital from Jan. 2018 to Dec. 2020. Serum GDF-15 levels were measured at baseline. The primary endpoint was in-hospital all-cause mortality, and the secondary endpoint was major adverse cardiac events (MACEs) during hospitalization, defined as a composite of cardiovascular death, heart failure, sustained ventricular arrhythmias (ventricular tachycardia or ventricular fibrillation), and bleeding. During hospitalization, patients with a higher GDF-15 level had significantly higher incidences of in-hospital mortality (7.4% vs 1.4%; P = .02) and MACEs (9.5% vs 20.9%, P < .01) than those with a lower GDF-15 level. Multivariate logistic regression analysis showed that a higher GDF-15 level was significantly associated with increased risks of in-hospital mortality (OR = 1.92, 95% CI: 1.44-2.50; P < .01) and MACEs (OR = 2.19, 95% CI: 1.56-2.77; P < .01). In conclusion, GDF-15 was associated with the risks of in-hospital mortality and MACEs, indicating that it should be a prognostic biomarker for patients with AMI.

Pheochromocytoma is a catecholamine-producing tumor that although being a rare disease, it poses diagnostic problems because its clinical presentation often mimics certain diseases, including cardiovascular disorders. The effects of excessive catecholamine secretion cause a variety of cardiovascular presentations ranging from hypertension to life-threatening cases such as hypertensive emergency, shock, supraventricular or ventricular arrhythmias, pulmonary edema, and acute coronary syndromes. The principal medical treatment for pheochromocytoma is a blockade of adrenergic receptors. However, surgical or tumor resection often provides complete resolution of abnormal myocardial dysfunction or arrhythmias, so this approach remains the mainstay of treatment that should be performed as soon as the diagnosis of pheochromocytoma is established. As clinicians, we must be aware of the characteristics of the cardiovascular manifestations of pheochromocytoma to make an earlier diagnosis and more appropriate management.

Abstract Background: Abnormal myocardial expression and function of Nav1.5 causes lethal ventricular arrhythmias during myocardial ischemia-reperfusion (I/R). PIASy mediated Caveolin-3 (Cav-3) SUMO modification affects Cav-3 binding to ligand Nav1.5. PIASy activity is increased after myocardial I/R, whether or not this may be attributable to plasma membrane Nav1.5 downregulation and ventricular arrhythmias remains unclear. Methods: Using recombinant adeno-associated virus subtype 9 (AAV9), rat cardiac PIASy was silenced by intraventricular injection of PIASy shRNA. Two weeks later, the hearts were subjected to I/R, and electrocardiography was performed to assess malignant arrhythmias. Tissues from peri-infarct areas of the left ventricle were collected for molecular biological measurement. Results: We found that PIASy was upregulated by I/R, with increased SUMO2/3 modification of Cav-3, reduced membrane Nav1.5 density, and increased ventricular arrhythmia frequency. These effects were significantly reversed by PIASy silencing. In addition, PIASy silencing enhanced Cav-3 binding to Nav1.5 and prevented I/R-induced Nav1.5 re-localization. Using in vitro models of HEK293T cells and isolated adult rat cardiomyocytes exposed to hypoxia/reoxygenation (H/R), this reserch further confirmed that PIASy promoted Cav-3 modification by SUMO2/3 and Nav1.5/Cav-3 dissociation after H/R. Mutation of the SUMO Consensus Sites Lysine in Cav-3 (K38R or K144R) alters the membrane expression levels of Nav1.5 and Cav-3 before and after H/R in HEK293T cells. Conclusions: I/R-induced cardiac PIASy activation contributes to Cav-3 SUMOylation by SUMO2/3 and dysregulated Nav1.5- related ventricular arrhythmias. Cardiac-targeted PIASy gene silencing mediates deSUMOylation of Cav-3 and prevents I/R-induced Nav1.5 down-regulation and ventricular arrhythmias in rats, identifying PIASy as a potential therapeutic target for relevant life-threatening arrhythmias in patients with ischemic heart diseases.

Cardiac radioablation is emerging as an alternative option for refractory ventricular arrhythmias. However, the immediate acute effect of high-dose irradiation on human cardiomyocytes remains poorly known. We measured the electrical activities of human induced pluripotent stem cell-derived cardiomyocytes (iPSC-CMs) upon irradiation with 0, 20, 25, 30, 40, and 50 Gy using a multi-electrode array, and cardiomyocyte function gene levels were evaluated. iPSC-CMs showed to recover their electrophysiological activities (total active electrode, spike amplitude and slope, and corrected field potential duration) within 3–6 h from the acute effects of high-dose irradiation. The beat rate immediately increased until 3 h after irradiation, but it steadily decreased afterward. Conduction velocity slowed in cells irradiated with ≥25 Gy until 6–12 h and recovered within 24 h; notably, 20 and 25 Gy-treated groups showed subsequent continuous increase. At day 7 post-irradiation, except for cTnT, cardiomyocyte function gene levels increased with increasing irradiation dose, but uniquely peaked at 25–30 Gy. Altogether, high-dose irradiation immediately and reversibly modifies the electrical conduction of cardiomyocytes. Thus, compensatory mechanisms at the cellular level may be activated after the high-dose irradiation acute effects, thereby, contributing to the immediate antiarrhythmic outcome of cardiac radioablation for refractory ventricular arrhythmias.
