Background/Aim. Doppler ultrasonography is now a reliable diagnostic tool for noninvasive examination of the morphology and hemodynamic parameters of extracranial segments of blood vessels that participate in the brain vascularisation. This diagnostic modality in recent years become the only diagnostic tool prior to surgery. The aim of the study was to determine hemodynamic status in symptomatic and asymtomatic patients with severe carotid stenosis prior to and after carotid endarterectomy (CEA). Methods. A total of 124 symptomatic and 94 asymptomatic patients who had underwent CEA at the Clinic for Cardiovasculare Disease ?Dedinje? in Belgrade were included in this study. Doppler ultrasonography examinations were performed one day before CEA and seven days after it. The peak systolic velocity (PSV), end-dyastolic velocity (EDV), time-averaged maximum blood flow velocity (MV), resistance index (RI) and the blood flow volume (BFV) of the ipsilateral and the contralateral internal carotid artery (ICA) were measured. Results. Diabetes was the only risk factor found significantly more frequent in symptomatic patients. There were significantly more occluded contralateral ICAs in the group of symptomatic patients. There was a significant increase in PSV, EDV, MV and BFV of the ipsilateral ICA after CEA and a significant decrease in PSV, EDV, MV and BFV of the contralateral ICA after CEA. RI is the only hemodynamic parameter without significant changes after CEA in both groups of patients. Comparing the values of hemodynamic parameters after CEA between the group of symptomatic and the group of asymptomatic patients no significant differences were found. Conclusion. The occlusion of the contralateral ICA is an important factor differentiating between symptomatic and asymptomatic patients with severe carotid stenosis. Successful surgery provides good recovery of cerebral hemodynamics in both symtomatic and asymptomatic patients.

We describe the vasculature of the camelid testis using plastic casting. We also use color pulsed-wave Doppler ultrasonography to measure testicular blood flow and compare the differences between testicular blood flow in fertile and infertile camelids. The testicular artery originates from the ventral surface of the aorta, gives rise to an epididymal branch, and becomes very tortuous as it approaches the testis. Within the supratesticular arteries, peak systolic velocity (PSV) was higher in fertile males compared to infertile males (P=0.0004). In addition, end diastolic velocity (EDV) within the supratesticular arteries was higher for fertile males when compared to infertile males (P=0.0325). Within the marginal arteries, PSV was also higher in fertile males compared to infertile males (P=0.0104). However, EDV within the marginal arteries was not significantly different between fertile and infertile males (P=0.121). In addition, the resistance index was not significantly different between fertile and infertile males within the supratesticular (P=0.486) and marginal arteries (P=0.144). The significance of this research is that in addition to information obtained from a complete reproductive evaluation, a male camelid's fertility can be determined using testicular blood flow measured by Doppler ultrasonography.

Progresson of extracranial carotid disease is considered to be significant independent predictor in the evaluation of individual cerebrovascular prognosis. Doppler ultrasonography is a useful screening method in the diagnosis and evaluation of extracranial carotid disease. The aim of this study was to establish the most sensitive hemodynamic parameter of Doppler-ultrasonographic spectral analysis in clinical evaluation of extracranial carotid disease. Investigation included 90 patients (of both sexes) in hospital and outpatient clinic care. Spectral analysis (M-scanning technique) was used for the evaluation of hemodynamic status of carotid sinus, including the following parameters: Pourcelot (A-D/A), Gosling (A/B) and Mol (A/D) resistance parameters. After statistical processing the results of the research affirmed hemodynamic parameters' values and correlation between Pourcelot hemodynamic parameter and the degree of carotid stenosis ?=0,59 for all patients, and ?=0,58 for patients with pathological values. Correlation between Mol parameter and the degree of carotid stenosis was ?=0,50 for all patients, and ?=0,57 for patients with pathological values. Gosling parameter was not significantly different in mean value and was present in all three groups. Functional relationship between the degree of carotid stenosis and each of hemodynamic parameters was established. It was concluded that Pourcelot and Mol parameters were significant indicators of asimptomatic carotid disease, but Pourcelot parameter was considered as a more sensitive indicator in the evaluation of extracranial carotid disease.

The objective of this study was to evaluate whether intratumoral blood flow as assessed by transvaginal color Doppler ultrasonography (TVCD) correlates with some tumor features in cervical cancer. Clinical, sonographic, and histologic data on 49 women (mean age: 50.3 years, range: 25–85 years) diagnosed as having a carcinoma from the uterine cervix were reviewed. Intratumoral blood flow was assessed by TVCD in all cases. Subjective impression of the amount of flow (scanty, moderate, or abundant) as well as the lowest resistance index (RI) and highest peak systolic velocity (PSV, cm/s) were used for analysis. These data were correlated with some tumoral features such as histologic type, histologic grade, tumor volume, and tumor stage. Intratumoral blood flow was found in all cases. Abundant blood flow was found more frequently in squamous carcinoma, moderately or poorly differentiated tumors, tumors with larger volume, and advanced stage tumors (P < 0.0001). Significantly lower RI was found in moderately or poorly differentiated tumors and advanced stage tumors (P < 0.01) and significantly higher PSV was found in moderately or poorly differentiated tumors, tumors with larger volume, and advanced stage tumors (P < 0.01). No correlation was found between RI and PSV and histologic type. Our data indicate that intratumoral blood flow as assessed by TVCD correlates well with some tumor features in cervical cancer.

Background During labor, ephedrine is widely used to prevent or to treat maternal arterial hypotension and restore uterine perfusion pressure to avoid intrapartum fetal asphyxia. However, the effects of ephedrine on uterine blood flow have not been studied during uterine contractions. The purpose of the study was to assess the effects of ephedrine on uterine artery velocities and resistance index using the Doppler technique during the active phase of labor. Methods Ten normotensive, healthy parturients with uncomplicated pregnancies at term received intravenous ephedrine during labor to increase mean arterial pressure up to a maximum of 20% above their baseline pressure. Peak systolic and end-diastolic Doppler flow velocities and resistance indices were measured in the uterine artery before and immediately after administration of bolus intravenous ephedrine and after ephedrine washout. Umbilical and fetal middle cerebral arterial resistance indices and fetal heart rate were also calculated. Results After ephedrine administration, mean arterial pressure increased by 17 +/- 4%. End-diastolic flow velocity in the uterine artery at peak amplitude of uterine contraction was restored to 74% of the value observed in the absence of contraction. The systolic velocity was totally restored, and the uterine resistance index was significantly decreased, compared with the values in the absence of contraction. Between uterine contractions, ephedrine induced similar but less marked effects. Fetal hemodynamic parameters were not altered by ephedrine administration. Conclusions Bolus administration of intravenous ephedrine reversed the dramatic decrease in diastolic uteroplacental blood flow velocity and the increase in resistance index during uterine contraction, without altering fetal hemodynamic parameters. This suggests that the increase in uterine perfusion pressure during labor could in part restore uterine blood flow to the placenta during uterine contraction.

Background We measured Doppler parameter of common femoral artery (CFA) blood flow in infants and children and investigated how far these parameters change with age, height, weight, body mass index (BMI), and body surface area (BSA). Material and Methods CFA was investigated in 97 infants and children (43 females, 54 males, ages ranged from 0.13 to 220 months) on both sides in a supine position after 5 min rest using a 7.5-MHz linear transducer (Type Elegra, Siemens, Germany) 1 cm proximal to the femoral bifurcation. Peak systolic velocity, end-diastolic velocity, blood flow volume, mean average velocity, resistance index, and pulsatility index were measured. Results Theoretically, Doppler parameters could change with age, height, weight, BMI, and BSA. Because age, weight, height, and BSA were highly correlated (Spearman correlation coefficient > 0.90) only one of these variables could be included in a regression model. Only diameter and flow velocity (FV) correlated with age and height. Peak systolic velocity, mean average velocity, pulsatility index, and resistance index did not. Correlation between BMI and age and BMI and height was small. A regression model for the natural logarithm of FV was estimated: ln(FV) = −5.853 + 0.021 height + 0.049 BMI (R2 = 0.73, p < 0.0001 of height, p value of BMI: 0.028; p value of intercept: < 0.0001). The 95% confidence intervals were: −6.470 to −5.237 for the intercept, 0.006 to 0.093 for BMI, and 0.017 to 0.025 for height. Predicted FV and their 95% confidence intervals were calculated. The upper confidence limits were 8–10 times that of the lower ones. Conclusion Only FV showed height- and BMI-dependent changes in growing children. Prediction of FV in individual arteries was possible, but with a large range.

20 % of ischemic stroke appear to originate from carotid artery atherosclerotic disease. Serum biomarkers reflecting the activity of atherosclerotic process and may help for estimate risk of acute cerebrovascular events. Several serum inflammatory markers have been proposed for risk assessment, but their prognostic role less known. The aim of this study is to clarify the prognostic value of biomarkers of atherosclerosis lipoprotein-associated phospholipase A2 (Lp-PLA2) and E-selectin in patients with symptomatic and asymptomatic carotid stenosis. Materials and methods. The study involved 106 patients with atherosclerotic carotid stenosis >50 % (74 men and 32 women, mean age 62.6±0.9) from which 76 symptomatic (35 with acute ipsilateral atherothrombotic stroke and 41 after carotid endarterectomy) and 30 asymptomatic patients. The control group consisted of age- and sex-matched 20 healthy subjects. The level of serum Lp-PLA2 and E-selectin was determined using a commercially available enzyme-linked immunosorbent assay kit. Results. The level of Lp-PLA 2 was in general significantly higher (p<0.05) in patients groups than in the control group and most high Lp-PLA2 concentration was in groups of symptomatic patients who underwent carotid endarterectomy. The level of E-selectin in the study patients was significantly higher than in the control group (p<0.05). The correlation of Lp-PLA 2 with E-selectin was significant for total patients (R=0.365664, p=0.00085) and group after carotid endarterectomy (R=0.429143, p=0.01796), but not for asymptomatic group (p>0.05). Receiver Operating Characteristics curves of logistic regression models which takes into joint both indicators was specificity and sensitive for predicting the occurrence of ischemic stroke. Conclusion. Conducted study show that the levels of Lp-PLA 2 and E-selectin have a significant impact on the development of stroke in patients with atherosclerotic carotid stenosis and can be used to predict it. A multidimensional model of the dependence of the probability of stroke on a linear combination of Lp-PLA 2 and E-selectin allows to obtaining significantly higher characteristics of the accuracy of stroke prediction than models with each factor alone.

The article presents the results of color Doppler ultrasonography of the left renal vein and gonadal vein with the determination of the peak systolic velocity of blood flow and abnormal blood reflux in patients with left-sided varicocele. The objective of the research was to compare preoperative ultrasound characteristics of the left renal vein, left gonadal vein, peak systolic velocity of blood flow and the presence of abnormal blood reflux, the formation of ultrasound criteria for the selection of patients for surgical correction of phlebohypertension. Results. Ultrasound signs of aorta mesenteric compression were absent in 24 (24.5%) patients; the signs of aorta mesenteric compression without critical left renal vein stenosis were observed in 64 (65.3%) patients; critical stenosis of the left renal vein was diagnosed in 10 (10.2 %) patients. The patients with critical stenosis of the left renal vein underwent left renal vein transposition. Conclusions. Ivanissevich surgery with prognostically low risk of relapse is recommended for the patients with left-sided varicocele without any signs of aorta mesenteric compression. Patients with the signs of aorta mesenteric compression require clear determination of the degree of left renal vein stenosis. We consider the transposition of the left renal vein to be indicated in case of critical stenosis when the correlation of the diameters of the distal and proximal segments of the left renal vein is ≥ 3 and the ratio of peak systolic velocities in the proximal and distal segments is ≥ 6. Thus, the diagnosis of phleborenohypertension in the patients with varicocele by means of color Doppler ultrasonography with the determination of blood flow velocity indicators in the left renal vein circulation and the determination of the critical stenosis of the left renal vein is decisive in choosing the surgical method of treatment. This provides an opportunity to evaluate the cause of varicocele occurrence and choose the optimal method of surgical treatment.

<p class="MsoNormal" style="text-align: justify; line-height: normal; margin: 0cm 0cm 0pt;">Objective: To evaluate if the vascularization patterns in the thyroid gland parenchyma by the conventional ultrasound mode B, and color Doppler ultrasonography correlated with the peak systolic velocity (PSV) of the inferior thyroid artery using pulsed Doppler in patients with Hashimoto’s thyroiditis (HT) in various stages. Methods: Patients with diagnosis of HT were enrolled in this prospective study in the period two years. Thyroid glands of all patients were evaluated with conventional ultrasound mode B, color-flow Doppler ultrasonography, and peak systolic velocity (PSV) of the inferior thyroid artery. Data were analyzed applying variance (ANOVA) and Pearson’s or Spearman’s correlation. Results: A hundred twenty patients (10 men and 110 women) were included in the study. Highly elevated PSV were associated with very lower thyroid echogenicity and heterogeneous pattern thyroid gland (p= 0.01) and intrathyroidal blood flow (p= 0.004). Conclusions: We conclude that evaluation the vascularization patterns of the thyroid gland parenchyma in patients with HT when compared to conventional ultrasound mode B, and with the PSV of the inferior thyroid artery by pulsed Doppler showed a high correlation. Probably this method could be recommend as a measure of thyroid blood flow as an essential part of evaluating ultrasonography in the HT.</p>
