SUMMARY Tooth wear is a multifactorial condition of growing concern. In clinical practice, it is often a challenge for prevention and treatment since many etiological factors may be involved. This case report describes an esthetic rehabilitation of a young patient presenting tooth wear due to erosion. The etiological factor of this case was the patient sucking on lemons, an acidic fruit. The main complaint of the patient was the appearance of short maxillary incisors. The treatment involved orthodontic and restorative dentistry techniques. First of all, an orthodontic device was used to increase the vertical dimension of occlusion and create an adequate space for the direct restorations. The posterior teeth were restored with two direct composite resin techniques; the anterior teeth were then restored using a balanced occlusion. The seven-year follow-up of the case is presented. Replicas of the restorations were made and visualized under scanning electron microscopy up to the 12-month evaluation. In the clinical follow-up at seven years, maintenance of the results and restorations can be clearly seen.

Case Report. An 8-year-old girl with amelogenesis imperfecta (AI) reported unsatisfactory aesthetics, difficulty in mastication, and dental hypersensitivity. The intraoral examination observed mixed dentition, malocclusion in anteroposterior relationships, anterior open bite, and dental asymmetry. A hypoplastic form of AI was diagnosed in the permanent dentition. A multidisciplinary planning was performed and divided into preventive, orthopedic, and rehabilitation stages. Initially, preventive treatment was implemented, with fluoride varnish applications, in order to protect the fragile enamel and reduce the dental sensitivity. In the second stage, the patient received an interceptive orthopedic treatment to improve cross-relationship of the arches during six months. Finally, the rehabilitation treatment was executed to establish the vertical dimension. In the posterior teeth, indirect composite resin crowns were performed with minimally invasive dental preparation. Direct composite resin restorations were used to improve the appearance of anterior teeth.Follow-Up. The follow-up was carried out after 3, 6, 12, and 18 months. After 18 months of follow-up, The restoration of integrity, oral hygiene, and patient satisfaction were observed .Conclusion. Successful reduction of the dental hypersensitivity and improvement of the aesthetic and functional aspects as well as quality of life were observed.

Keeping in mind the final result is the basis of any type of treatment, especially those in which the morphology, size and proportion of the anterior teeth will be changed. This is where a good treatment plan based on a diagnostic wax-up tested with a mock-up and approved by the patient becomes crucial. This case report aims to exemplify how to transfer the diagnostic information to the patient’s mouth and direct it not only to the restorative dentistry, but also to the surgeon when performing the crown length. Diagnostic waxing was performed by the laboratory technician, obeying anterior posterior incisal and gingival criteria and curvatures, which were transported to an aesthetic guide through mock up to mark the surgical points. Then, surgery to increase the clinical crown with bone access and after healing, direct venners in composite resin also guided by waxing. In conclusion, a workflow can be established using the wax-up / mock-up that serves as a guide for the periodontist in the approach to surgical lengthening of the crown and for the rehabilitator who uses it to produce changes in the shape of dental dimensions .

ABSTRACT Introduction Beauty standards in today's modernized world scenario are formed by well-aligned and well-designed bright white teeth. One of the major reasons behind patients reporting to dental clinics is pain. Caries in the anterior primary teeth forms one of the major concerns from a restorative point of view. Very few studies are quoted in literature which stresses on the follow-up of anterior restorations in primary teeth. Hence, we evaluated and compared the efficacy of composite resin and resin-modified glass ionomer cement (RGIC) for class III restorations in primary anterior teeth. Materials and methods The present study was conducted in the pediatric dental wing and included a total of 80 patients aged 3 to 5½ years who reported with the chief complaint of carious lesions in the primary anterior teeth. Patients having minimal of a pair of similar appearing small carious lesions on the same proximal surfaces of the deciduous maxillary incisors were included for the study. All the patients were randomly divided into two groups: One in which RGIC restoration was done and other in which composite restoration was done. Cavity preparation was done and filling of the cavity with the restorative materials was carried out. Assessment of the restorations was done at 4, 8, and 12 months time following criteria given by Ryge et al. All the results were analyzed by Statistical Package for the Social Sciences (SPSS) software. Mann–Whitney test and one-way analysis of variance (ANOVA) were used to evaluate the level of significance; p value less than 0.05 was considered as significant. Results For composite and RGIC restorations, the mean score for anatomic shape was 1.21 and 1.10 respectively. While comparing the clinical parameters, nonsignificant results were obtained between composite and RGIC restorative materials at 4-, 8-, and 12-month interval. On comparing the clinical parameters for individual restorative materials at different time intervals, statistically significant results were obtained only for anatomical shape and form. Conclusion Both RGIC and composite resin restorative materials showed acceptable clinical outcomes after 12 months of follow-up in deciduous anterior teeth. In restoring class III restorations in primary anterior teeth, both the restorative materials showed similar outcome. The teeth of Flaccisagitta hexaptera are capable of penetrating the exoskeleton of copepods. The grasping spines, anterior, and posterior teeth are all capable of piercing the epidermis of larval fish as seen in a series of scanning electron micrographs. A new type of anterior tooth structure is observed. A chemosensory function is suggested for a new set of pores found between the sets of anterior teeth. The secretory nature of the vestibular pit is clearly seen for the first time.

This case report describes the orthodontic treatment used to prepare for restorative dentistry, aimed at the functional rehabilitation of an adult patient with Class I malocclusion, right posterior crossbite, and significant tooth wear on the palatal face of the maxillary canine to canine. The orthodontic treatment was performed with a total fixed appliance mini-expandex-type expander, associated with vertical elastics. Good dental intercuspation was obtained and enabled conditions for the restoration of the abrasions on the palatal surfaces of the maxillary incisors and canines and the tips of the buccal cusps of the maxillary right premolars and first molar. Excellent results were found during a 37-month follow-up.

Aim The purpose of this study was to obtain data on current approaches for the restoration of endodontically treated teeth among general dental practitioners (GDPs) in Manchester, United Kingdom. Method A questionnaire containing 19 single-answer, multiple-choice type questions was mailed to 503 GDPs practising in the Greater Manchester area in January 2002. An explanatory covering letter and a stamped addressed return envelope were enclosed. The data obtained were processed using SPSS statistical software. Results Three hundred and fifty-one (70%) of the practitioners responded to the questionnaire. The restoration of root-filled teeth was normally undertaken within 1–2 weeks of completing root canal therapy by 63% of the practitioners. Only 35% of the GDPs used posts routinely in the restoration of root-filled anterior teeth; the corresponding figure for posterior teeth was 15%. While a cast, precious metal post was the preferred choice in the restoration of anterior teeth, the use of prefabricated posts and related techniques predominated in the restoration of posterior teeth. Composite resin was the most popular choice of material for core build-up procedures in anterior teeth. Amalgam tended to be favoured for core build-ups in posterior teeth. The majority of the practitioners (56%) routinely restored root-filled anterior teeth by means of porcelain-fused-to-metal crowns. Seventy-three per cent of the GDPs preferred to restore root-filled posterior teeth by means of a full veneer crown. Conclusions The results of this study suggest that the practitioners surveyed had a sound understanding of the principles involved in the restoration of endodontically treated teeth, with the possible exception of the need to establish a durable coronal seal as soon as possible after the placement of a root filling.

Permasalahan estetik merupakan salah satu hal penting dalam perawatan kedokteran gigi restoratif dan harus sejalan dengan penampilan yang alami serta harmonis berdasarkan prinsip-prinsip estetik. Untuk menginformasikan perawatan restorasi estetik komplek dari empat gigi anterior maksila. Laki-laki berusia 17 tahun datang dengan fraktur insisal karena kecelakaan. Empat tahun yang lalu, gigi tersebut telah dirawat endodontik dan direstorasi dengan mahkota jaket namun tujuh hari yang lalu, mahkota lepas. Pemeriksaan klinis menunjukkan bahwa gigi 11 fraktur horizontal pada sepertiga mahkota. Gigi 21 ditumpat dengan komposit tetapi memiliki anatomi dan warna yang buruk. Terdapat diastema di antara gigi insisivus kiri atas. Semua gigi normal terhadap perkusi, palpasi dan probing periodontal. Radiograf periapikal menunjukkan adanya resorpsi apikal pada gigi 11 dengan radiolusensi apikal dengan diameter 3-4 mm. Setelah pemeriksaan, perawatan yang dilakukan pada gigi 11 adalah retreatment dan apeksifikasi menggunakan MTA, pemasangan pasak komposit indirek dan mahkota jaket PFM. Gigi 21 dan diastema di antara gigi insisivus maksila kiri direstorasi dengan komposit. Gigi 22 juga direstorasi dengan resin komposit untuk mendapatkan lebar ideal. Kontrol 1 bulan menunjukkan gigi asimptomatik, terlihat adanya regenerasi jaringan periapikal dan gigi berfungsi normal. Perawatan keempat gigi anterior maksila dengan mempertimbangkan kondisi estetik dan konservasi sisa jaringan keras gigi menunjukkan keberhasilan sehingga gigi dapat berfungsi normal. ABSTRACT: Aesthetic Treatment of Four Maxillary Anterior Teeth with External Resorption. Esthetic aspect has become increasingly important in the modern restorative dentistry and synonymous with a natural, harmonious appearance based on esthetic principles. This case report aimed to inform the esthetically complex restorative treatment of four maxillary anterior teeth. A 17-year-old male had a fractured incisor following a traumatic incident. Four years earlier, the tooth was treated endodontically and restored with a crown. Seven days ago, the crown fell off. A clinical examination revealed that tooth 11 was horizontally fractured on the mid-third of the crown. Tooth 21 had been previously restored using composite, but it had a bad contour and color. There was also a diastema between left maxillary incisors. All teeth responded normally to percussion and palpation and had normal periodontal probing. A periapical radiograph showed apical resorption at teeth 11 with an apical rarefaction 3-4 mm in diameter. After complete examination and esthetic analysis, several treatments were undertaken. Tooth 11 was retreated using MTA to facilitate the apexification followed by indirect, customized composite post and porcelain-fused-to-metal crown. Tooth 21 and the diastema between the left maxillary incisors were closed using composite resin. Tooth 22 was also restored using composite resin to get the ideal width of the tooth. After 2 weeks, the treated teeth were asymptomatic, radiographic examination demonstrated apparent regeneration of periapicular tissue, and the teeth functioned normally. The teeth were successfully treated by endodontic therapy and restoration with crown and composite resin to restore esthetic problems.

Amelogenesis imperfecta (AI) is a disorder of genetic origin, linked to the amelogenin gene, which presents developmental abnormalities in the quantity and quality of tooth enamel. The main clinical problems are the extensive loss of dental tissue, occlusal problems, impaired aesthetics, tooth sensitivity and gingival inflammation. This study described a clinical case and the approach in the treatment of a patient with AI, with gingival plastic surgery, porcelain crowns on the posterior teeth, porcelain veneers on the anterior teeth and application of botulinum toxin to correct the gingival smile. AI is an anomaly with complex development and early diagnosis is essential for the conduct and success of the case, to improve aesthetics and function, preserving the characteristic tooth wear.
