Objective To evaluate the systemic immune-inflammation index (SII) in twin pregnancies. Methods The retrospective study included 200 twin (dichorionic/ diamniotic and monochorionic/diamniotic) and 200 singleton pregnancies (control group). All pregnant women were in the first trimester (<14 weeks of gestation). The study and the control groups were compared in terms of demographic characteristics, laboratory test results, and SII values. Results The mean week of gestation was 9.7±2 in dichorionic diamniotic twin pregnancy group, 10.4±2 in monochorionic diamniotic twin pregnancy group and 9.4±2 for the control group (p=0.083). The lymphocyte value was lower in the dichorionic group and monochorionic group than in the control group (p=0.020 and p=0.020, respectively). The platelet-to-lymphocyte ratio (PLR) values were higher in the dichorionic group than in the control group (p=0.002). However, there was no statistical difference between the groups for neutrophil-to-lymphocyte ratio (NLR) and SII values (p>0.05). Conclusion Physiological inflammatory/immune reaction of early pregnancy may affect PLR values in twin pregnancies. We observed that chorionicity is considerable for these inflammatory indices, especially for dichorionic twin pregnancies. However, we could not detect a significant change in SII values in twin pregnancy.

Abstract Purpose To evaluate the usability of first-trimester maternal serum ProBNP levels in the prediction of intrauterine growth restriction (IUGR). Methods In this prospective study, blood samples taken from 500 women who applied to our polyclinic for routine serum aneuploidy screening between the 11–14th gestational weeks were centrifuged. The obtained plasma samples were placed in Eppendorf tubes and stored at −80+°C. For the final analysis, first-trimester maternal serum ProBNP levels of 32 women diagnosed with postpartum IUGR and 32 healthy women randomly selected as the control group were compared. FGR was defined as estimated fetal weight below the 10th percentile for the gestational age. Results The mean ProBNP levels were statistically and significantly higher in the women with intrauterine growth restriction (113.73±94.69 vs. 58.33±47.70 pg/mL, p<0.01). At a cut-off level of 50.93, ProBNP accurately predicted occurrence of IUGR (AUC+= 0.794 (95% confidence interval 0.679–0.910), p+= 0.001) with sensitivity and specificity rates of 78.1 and 69.0%, respectively. Conclusion First-trimester serum ProBNP level was significantly higher in women who developed IUGR compared to healthy controls. First-trimester ProBNP level can be used as a potential marker to predict the development of IUGR in pregnant women.

In twin pregnancies, amnionicity and chorionicity are crucial as they strongly determine prenatal and perinatal management. First trimester ultrasound allows a highly reliable diagnosis of amnionicity and chorionicity, making it an internationally accepted standard in antenatal care. However, in rare cases, amnionicity can change from diamniotic to monoamniotic throughout pregnancy, substantially impacting perinatal management. We report the case of a confirmed monochorionic diamniotic twin pregnancy with a diagnosis of spontaneous septostomy of the dividing membrane (SSDM) at 28 weeks of gestation, resulting in a pseudomonoamniotic pregnancy. Even though SSDM is a rare condition and its sonographic diagnosis might be challenging, it should be considered if, in a known diamniotic pregnancy, there is a sudden failure to visualise the intertwin membrane truly separating both twins.

Aim-Objectives: Vitiligo is an acquired idiopathic disease which progresses with melanocyte destruction and is clinically characterized by depigmented lesions of unknown etiology. Vitiligo may be coexistence with a autoimmune and endocrine disorders. This study examined the sT3, fT4, TSH, Anti-TPO, Anti-Tg, Vitamin B12 and fasting blood glucose (FBG) values, and thrombocyte-to-lymphocyte ratio(TLR), neutrophil-to-lymphocyte ratio(NLR), the mean platelet volume(MPV) the correlation of depigmented lesions with the extent of body involvement sites(IS). Materials and Method: The study enrolled 67 patients aged 0‒65 who were diagnosed with non-segmental generalized vitiligo and in whom an increase in lesions had been observed in the last six months. The IS of the lesions in the patients were evaluated as IS ˂10%(1st group), 10%‒20% (2nd group), 20% - 30%(3rd group), 30% - 40%(4th group), 40% ‒50 %(5th group), and ˃50%(6th group). The control group consisted of patients who had presented to the outpatient clinic having had no history of vitiligo detected in themselves nor in their families. Results: No significant correlation was found between IS and sT3, fT4, TSH, Anti-TPO, Anti-Tg, Vit. B12, PBG or MPV. A moderately positive correlation was found between IS and duration (p <0.05) and a mildly positive correlation between IS and NLR and TLR (p <0.05). Conclusion: This study show that every patient diagnosed with vitiligo, independent of the IS, should undergo examination for autoimmune disease. A mild positive correlation between VTA and NLO-TLO was found to be an indicator of increased inflammation in vitiligo patients as the extent of lesions increased.

Background: Blood pressure is influenced by psychosocial (stress), genetic, age, gender, nutritional status, and lifestyle (diet, lack of fiber consumption, smoking, lack of physical activity). 30 ml of young coconut water contains 61 mg of potassium, 5.45 mg of sodium, and 1.3 mg of sugar, affecting blood pressure changes. Coconut water contains K minerals and is useful for lowering blood pressure. Whereaswatermelon contains anti-hypertensive content, namely sodium, beta carotene, and potassium. Watermelon is rich in water, amino acids, L-arginine, which can maintain healthy blood pressure. Aim: This study aims to determine the effect of young coconut water and watermelon juice on blood pressure in the 2013 nursing students of Respati University Yogyakarta. Methods: This type of research is a quasi-experimental design with a nonequivalent control group pre and post-test. The sample was taken by simple random sampling, namely the control group (18 respondents), the young coconut water group (18 respondents), and papaya juice (18 respondents). Data pre-posttest of each group was tested using Paired T-test. The comparison control-treatment group was tested using an independent-test. Results: The mean differences of systole and diastole pre-posttest blood pressure in the control group were -1.8 mmHg and -1.0 mmHg. The mean difference in systole blood pressure and the pre-posttest diastole of young coconut water groups were -3.1mmHg and -2.4 mmHg. The mean systole and diastole blood pressure in the pre-posttest watermelon juice group were -2.9 mmHg and -1.5 mmHg. The pre-posttest results of systole and diastole blood pressure analysis in the p-value control group were 0.100 and 0.450. The pre-post test results of systole and diastole blood pressure analysis of young coconut water groups gained p-value of 0.030 and 0.194. The pre-post test results of the juice watermelon group's systole and diastole blood pressure analysis gained p-value of 0.032 and 0.181. The posttest results of systole and diastole blood pressure analysis in the control group and young coconut water gained p-value of 0.014 and 0.157. The post-test results of the systole and diastole blood pressure analysis control group and juice watermelon gained p-value of 0.013 and 0.420. Conclusion: Consumption of young coconut water and watermelon juice affects systole blood pressure, but it does not affect diastole blood pressure in nursing students in 2013 in Respati University Yogyakarta. Energy and protein intake as measured by 24-hour urinary nitrogen values are similar in twin and singleton pregnancies. The relationship between urinary nitrogen and nitrogen intake is equally significant in twin and singleton pregnancies. Dietary zinc, copper, and iron are not different in women with twins, nor are the levels of these elements in plasma. These observations are surprising in view of the extra fetal demands on the mother and the different adaptation of twin pregnancies.

A follow-up of 78 twin pregnancies diagnosed in the first trimester indicated that early ovular resorption occurred in 63% of spontaneous and 64% of Clomiphene-induced gestations, so that singletons were born. No failure was observed in the 12 twin conceptions following administration of gonadotropins or Cyclofenil.

Purpose. To evaluate the neutrophil/lymphocyte ratio (NLR) and the platelet/lymphocyte ratio(PLR) value in the development of branch retinal vein occlusion (BRVO)patients. Methods. 81 patients with BRVO and 81 age and sex-matched subjects were recruited as the control group. The BRVO diagnosis was confirmed under comprehensive ophthalmologic examinations. NLR and PLR parameters obtained from peripheral blood were recorded. Results. Both the mean NLR and PLR was significantly higher in the BRVO group compared with the control group (p < 0.001). In ROC analysis, the AUC for NLR was 0.82, and NLR of >2.48 predicted BRVO with a sensitivity of 58% and specificity of 98%. The AUC for PLR was 0.78, and PLR of >110.2 predicted BRVO with a sensitivity of 72% and specificity of 72%. Conclusion. The current study demonstrated that BRVO patients had increased NLR and PLR levels compared with control subjects. The NLR and PLR may be used as independent predictors for identifying risk for the development of BRVO.

ABSTRACT Dilation of the uterine cervix for 16 hours as a preliminary to the induction of legal abortion during the first trimester of human pregnancy resulted in a marked increase in the plasma level of both progesterone and human chorionic gonadotrophin (HCG). In a control group (with the dilating instrument taped on the medial side of the upper thigh) no increase in progesterone occurred. The increase in the plasma level of HCG was more pronounced than that of progesterone, the mean increase being 80 and 50 per cent respectively. Following the termination of the pregnancy, the half life (t½) of endogenous HCG in peripheral plasma could be calculated, and was found to be about 7 hours.

AbstractObjective:To determine the relationship between severe tinnitus and inflammation using the neutrophil-to-lymphocyte ratio as a marker of stress.Methods:A total of 107 patients who had been suffering with severe tinnitus (tinnitus handicap inventory scale grades of 3–5) for at least 2 weeks were recruited. Patients underwent detailed ENT examinations and audiometric tests to exclude a relevant pathological cause of the tinnitus. Patients with systemic diseases, malignancy or inflammatory diseases that could alter neutrophil-to-lymphocyte ratio were excluded. A total of 107 age- and sex-matched healthy control participants were also recruited. Routine laboratory test results and neutrophil-to-lymphocyte ratio were compared between the patients and controls.Results:Lipid profile, liver function, white blood cell count, haemoglobin level, mean corpuscular volume, and vitamin B12 and folate levels were similar among the patients and controls. However, mean neutrophil-to-lymphocyte ratio was significantly higher among the patients than the controls (p < 0.05).Conclusion:The findings of this novel study suggest that neutrophil-to-lymphocyte ratio should be considered during the evaluation of tinnitus patients as a potential clinical marker of tinnitus. Further studies are required to verify the findings.
