Comparative analysis of early results of reconstructive procedures on coronary arteries with and without endarterectomy in diffuse coronary atherosclerosis

Steady growth of patients with diffuse coronary atherosclerosis is a challenge for cardiac surgeons and intervention cardiologists for achieving a full revascularization of myocardium. Considering results of an endarterectomy, more and more surgeons perform reconstructive procedures on coronary arteries without removal the atheromatous plaques. In the literature review various versions of the surgical equipment, perioperative techniques and results of extended segmental plastic surgeries are discussed regarding the patients with diffuse coronary atherosclerosis. The purpose of the present work consists in highlighting changes in reconstructive procedures technique.

The review presents a comparative analysis of data from patients with myocardial infarction with non-obstructive coronary arteries. Data were obtained from the total register of acute coronary syndrome in the Krasnodar Krai and were compared with data of foreign registers.

Coronary angiography was performed in 216 men of whom 78 also had a cine-angiogram of one femoral artery. Stenoses of the coronary arteries were measured with a pair of calipers and the femoral angiograms were computer-analysed. The patients were divided into 2 groups according to whether the bulk of the coronary stenoses affected the central or the peripheral part of the coronary arteries. The groups were investigated regarding differences in lipid and metabolic risk factors. Coronary atherosclerosis was most often found in the proximal part of the right coronary arteries. No significant correlation between coronary and femoral atherosclerosis was found. Previous myocardial infarction was associated with more coronary artery stenosis (p < 0.003). No pattern of risk factors was found to discriminate between central and peripheral coronary atherosclerosis.

The aim – to conduct a comparative analysis of the presence, frequency and duration of episodes of myocardial ischemia and arrhythmias based on the results of Holter monitoring in patients with coronary heart disease depending on the condition of the coronary arteries. Materials and methods. We examined 53 patients (group I) with stable coronary heart disease (CHD) and slightly altered coronary arteries (INOCA), who were hospitalized in the period from October 2018 to February 2021 at the “City Clinical Hospital № 8” of Kharkiv City Council. Group II included 52 patients with a diagnosis of stable coronary heart disease, and according to coronary angiography (CAG) had stenosis of coronary arteries (CA) more than 50 %. Results. According to the results of comparative analysis, it was found that in group I there were signs of myocardial ischemia – depression of the ST segment in 62.3 % (n=33) and elevation of the ST segment in 11.3 % (n=6), compared with group II -73 % (n=38) and 5.66 % (n=3), respectively. Ventricular arrhythmias (VA) have been reported in 52 patients of group I, and in 44 patients of group II. VA 4 and 5 type according to Laun, was significantly higher in group II compared with group I (p=0.0324). The occurrence of ventricular tachycardia was recorded in 5.7 % (n=3) of patients in group I and 9.3 % (n=5) patients of group II (p=0.347). In group II, there was a tendency to more episodes of ischemia compared with group I (p=0.072). The duration of ischemia was significantly longer in group I, compared with group II (p=0.042). Conclusions. The results of the study did not show significant differences in the development of the number of episodes of myocardial ischemia according to Holter monitoring depending on the condition of the coronary arteries. The duration of episodes of ischemia in patients with INOCA is significantly longer than in patients with obstructive atherosclerosis. Background.Cardiovascular disease, including coronary artery disease (CAD), remains a leading cause of mortality. The results of recent studies demonstrate that microRNA engages in pathogenesis of different pathological states, including coronary atherosclerosis and its clinical manifestation — CAD.Objective.To evaluate microRNA-27а, microRNA-133а and microRNA-203 expression in cardiomyocytes and blood serum of CAD patients with different degrees of coronary artery lesion.Design and methods. 100 patients were included in the study, they formed 3 groups: 40 patients with CAD and significant atherosclerosis of 1-2 coronary arteries; 40 patients with multi arterial coronary disease and 20 persons without CAD and significant comorbidity. The microRNA-27a, microRNA-133а and microRNA-203 expression was determined in cardiomyocytes and blood serum of these patients by real-time polymerase chain reaction.Results.Level of microRNA-27a, microRNA-133а and microRNA-203 expression in left atrium cardiomyocytes of CAD patients with significant stenosis of 3 or more coronary arteries was higher, then their expression in the patients with 1-2-vessel and in group without coronary atherosclerosis. Examination of the microRNA expression level in blood serum and in left atrium cardiomyocytes revealed a significant difference. The expression of microRNA-27a and microRNA-133а in cardiomyocytes is above than in blood serum, while the expression of microRNA-203 in the myocardium is lower, than in the blood serum. Level of microRNA-27a, microRNA-133а and microRNA-203 expression in blood serum in CAD patients with significant stenosis of 3 or more coronary arteries was significantly higher, than in patients with 1-2 stenosis of coronary arteries. Furthermore, classification tree method established that risk of atherosclerotic heart disease increases five-fold if microRNA-203 expression in blood serum is more than 100 REU.Conclusion.MicroRNA-27а and microRNA-133а expression in cardiomyocytes of left atrium is higher than in blood serum in CAD patients. MicroRNA-203 expression level in blood serum may be a new predictive marker of coronary artery degree atherosclerotic stenosis in CAD patients.

Relevance. Through dermal coronary intravascular revascularization by means of stenting is an effective method of treatment CHD patients with the type 2 diabets. At the same time frequency of stents restenosing for this special cohort of patients is fluctuating from 12 by 40 %.Objective. To study prognostic significance of the new biomarker of intravascular inflammation of resisin in blood of CHD patients with DM 2 who suffered from stenting. Material and methods. In the study 60 patients (48 men and 12 women, in the middle age 60.9 years) with CHD and DM2 are included. The patients were divided into two groups: Patients with positive progress of comorbide pathology belonged to the first group (n = 30); in the second group (n =3 0) patients with unfavorable progress of cardiac vessel pathology were included. The further observation was carried during 12 months. Content of all the patients in blood of resistin with help of enzyme immunoassay analysis was determined, as well as lipid blood serum mixture and additional metabolic risk factors. By the indications control a coronary angiography was conducted. Restenosis of coronary arteries was counted as the narrowing of a coronal artery lumen of ≥70% in the place of an intervention.The result. Unfavorable flow, including restenosis of coronary arteries was revealed in 30 cases (50%). ROC-analysis showed great predictive significance of resistin – (area under a curve >5/35, Sensitivity 86.2, Specificit 70.0) in development of constrictive coronary atherosclerosis restenosis of coronary arteries after their stenosis.The conclusion. The study of resistin level in DM patients blood for the valuation of the disease prognosis and optimization of the tactics for choice of coronary pathology treatment seems expedient and informative.
