Background: Opioid maintenance treatment (OMT) secures a low mortality in a vulnerable population and is often a lifelong treatment. One would therefore expect an increasing prevalence of OMT among the elderly. This article describes a study with three elements; a literature review, an investigation of occurrence and an exploration of elderly patients’ needs and experiences. Material and methods: PsycINFO, Ovid Medline and Embase were searched with reference words for opioid maintenance and age. Norway has a national cohort study of patients in OMT describing recruitment, retention in treatment, age and mortality. A prognosis of numbers in treatment older than 50 and 60 years were extrapolated based on probabilistic premises. Finally, a qualitative interview study of OMT patients in two municipalities near Oslo was performed. Five OMT patients aged between 58 and 64 years were interviewed with open questions on thoughts, needs and experiences. Findings: The review points to an increasing population with psychiatric and somatic comorbidities and diverse needs. Their opinions and experiences seem largely unexplored. The population estimates indicate that Norway by 2018 will have between 3084 and 3567 individuals older than 50 years and between 201 and 302 individuals older than 60 years in OMT. The qualitative study demonstrated that the elderly OMT patient typically attributes their being alive to OMT. It is a life with a high prevalence of ailments and diseases. Their somatic diseases are mostly adequately treated by their GP, but they feel left alone with a high level of mental problems and loneliness. They miss follow-up from the OMT and social services and are not met on existential thoughts and sorrow for their life. A wish to end OMT during their lifetime is noteworthy. This would necessitate a close collaboration with the OMT services and support from a social network. Conclusions: The study points to insufficient knowledge of and strategies to meet an increasing number of individuals with composite problems in OMT. The numbers will remain too small to warrant specially designed local projects, but large enough to necessitate awareness and competencies in the ordinary municipal health and social services.

A description of a pilot project in daytime care for the elderly and its results will provide the background for the presentation of the theory named “The Four Wall Syndrome”. This phrase was coined by the author to assist in the evaluation of the Day Care candidates and in the projection of the number of interviews as well as the type of interview techniques used when assessing the candidates for admission to a Seniors' Day Care program. The project began in May 1976, under the auspices of the New Brunswick Provincial Department of Social Services. Statistical proof of the project's success ensured its continuance for a second year. When the grant funds for the second year had been used, the Social Service Department decided to perpetuate Seniors' Day Care in the new package deal of community based services for the elderly in three key areas of New Brunswick. The ability of the program co-ordinator to apply “The Four Wall Syndrome” theory to interviews of candidates would assist in the screening process for admission to the program. It is believed that such a theory could be applicable to other programs for the elderly.

ABSTRACTIt is often argued that there are significant differences in the costs of providing public and non-public services. However, these arguments have almost invariably been based on crude comparisons of bald expenditure figures of rather dubious validity. In this paper we describe and apply a conceptual framework which attempts to place such inter-sectoral comparisons on a more reliable basis. Our application is to day care services for elderly people provided by local authority social services departments, area health authorities and voluntary organizations, although the framework has much wider relevance. Our results provide clear evidence to refute the oft-made assumption that voluntary services are universally cheaper than their statutory counterparts. Standardizing costs for the dependency characteristics of users and the activities of day units, we find that voluntary-statutory cost differences are dependent upon the scale of operation. Small voluntary units certainly enjoy a cost advantage, but larger voluntary units are unlikely to be cheaper, and are probably more expensive, than local authority units of a similar scale.

Abstract There has been considerable interest in the concept of consumer-directed care (CDC) for ageing populations. One type of CDC employs vouchers that older people can use to procure a panel of social services that best fits their needs. The voucher-based strategy has been institutionalised in Hong Kong as the Community Care Service Voucher for the Elderly (CCSV). Using a qualitative, semi-structured interview method, researchers probed the experiences of two groups (twenty-six CCSV users and twenty-seven CCSV non-users) with the voucher programme between March 2014 and September 2015 during the first phase of CCSV. The interviews revealed a set of social and institutional supports needed to empower clients to effectively utilise the voucher. These supports (or forms of capital) were consistently present amongst active CCSV users and deficient amongst non-users. The research demonstrates that instituting a voucher does not, by itself, lead to optimised choices but, rather, that a network of institutional and social resources must be assembled in support of the voucher programme. In some instances, a case management approach may be needed, as older people (and family) need to combine different forms of institutional and social support to comprehend the voucher programme and utilise it for their particular needs.

SummaryA medical, psychiatric and social assessment was conducted on 272 residents admitted consecutively to local authority residential care for the elderly in Nottingham in the year ending 31st January 1978. A high level of medical and psychiatric pathology was discovered, in spite of frequent general practitioner contact in the community and recent hospital admissions. Few of the staff in the old people's homes were sufficiently qualified to deal with the medical and psychiatric conditions of the residents only a third of whom had been examined by a general practitioner during the month after admission. The social service provision in the community showed an uneven pattern and did not appear to have a direct relationship with the residents' requirements, 12 per cent of whom could have remained in the community had there been adequate social assessment and support. Only just over half were appropriately placed, and a further third should have been in the care of the hospital services.Recommendations for change are directed towards the provision of routine medical, psychiatric and social assessment of all potential residents by geriatricians and psychogeriatricians in close collaboration with social services in special local authority assessment homes.

Introduction: Day care centres (DCC) aim to support older adults living in their own homes by providing a variety of activities to promote the independence of those people and reduce the caregiver’s burden. In Poland, there are no standards for providing this form of care. The provided care is delivered by different organisations, and there is a lack of quality control in the majority of places. Regrettably, in Poland, there is a paucity of research on the holistic needs of the elderly attending DCC. Aim of this study: This is the first study which has aimed to identify the Polish day care centres attendees’ needs to ensure that the increasing number of older people receive the best possible care, and as a part of the quality improvement process for recommendation development of the new day care services model in Poland within the ‘Homely Marina’ project. Methods and Materials: A representative sample (n = 269) was randomly selected from day care centres attendees (estimated as 10,688) in Poland. An anonymous survey for the assessment of needs was developed by the authors. Results: The respondents assessed the level of provided services as very good or good. Best rated services included meals, supportive and welfare services (occupational therapy, music therapy, art therapy, cognitive training). Almost half of the participants indicated the need for company as the main reason to attend a DCC. This research reveals a lack of support with regard to additional staff: e.g., a nurse. Conclusions: In Poland, the services offered in DCC should focus on social isolation and sense of loneliness prevention, and maintenance of social activity of the elderly. The presented analysis of needs in Polish day care centres suggests a need for changes which may improve the quality of services. There is a great need to find a balance between home-based care and in-patient care, using better integration of available services and strengthening support for informal caregivers. Robust research with a collection of meaningful outcomes is required to ensure that in Poland, the increasing number of older people is enabled to access high-quality day care service provision.

Abstract. The elderly, people with disabilities, people with HIV/AIDS, and people with mental disorders in vulnerable groups during the Covid-19 pandemic. This group must be given special attention because they are susceptible to Corona virus. The church as a social humanitarian institution is very concerned to pay attention to this group by doing charitable social services. This charitable social service aims to restore the physical and psychological state of this vulnerable group from the bad situation caused by the emergence of this Corona outbreak. These charitable social service activities are carried out using a combination of social work and social analysis methods. The impact of this activity is the recovery of physiological and psychological conditions of vulnerable groups. They were finally allowed to undergo this pandemic and new normal without any worries. Charitable social services have helped to make this group aware of their hygiene awareness in maintaining the condition of their bodies during this pandemic and in the future. The presence of the OiC Command Post in charitable social service activities in the form of providing social assistance in the form of food, vitamins and masks is enough to help vulnerable groups in meeting their nutritional needs during this pandemic. Adequate nutritional intake will greatly help maintain immunity and physical stamina, sustain economic needs, and maintain a healthy body. This charitable social service activity also had an impact on recognizing the existence of the Catholic Church as a religious social institution that not only offered heavenly (eschatological) salvation, but was also able to proclaim the real worldly salvation, now and here.
