Ovarian cancer is the fourth cancer with most incidence in Indonesian female with 10.238 cases in 20141. Tumor marker CA-125 is assosciated with ovarian cancer, importantly epithelial ovarian cancer. This study aims to find out diagnostic value (sensitivity, specificity, positive predictive value, negative predictive value) of CA-125 among patients with epithelial ovarian cancer in Dr. Soetomo General Hospital Surabaya in 2016. This study used analytic cross sectional method and was performed by evaluating medical records of patients suspected for ovarian malignancy in Dr. Soetomo General Hospital Surabaya in 2016. There were total 97 patients found fit for criteria of inclusion in this study. Tissue histopathological examination confirmed 66 patients have epithelial ovarian malignancy and 31 patients do not. Samples distributed using 35 U/ml as CA-125 upper limit, TP: 54.64%, FP: 19.59%, FN: 13.40%, dan TN: 12.37%. Diagnostic value obtained as follows: sensitivity 80.30%, spesificity 38.71%, positive predictive value 73.61%, negative predictive value 48%, and accuracy 67.01%. Tumor marker associated with ovarian cancer CA-125 has found high in sensitivity but low in specificity among patients with epithelial ovarian cancer in Dr. Soetomo General Hospital Surabaya in 2016.

Background and Aim: Ovarian cancer is among the most common cancers in women worldwide. CA125 is the most frequent biomarker used in the screening for ovarian cancer. CA125 has no high sensitivity and specificity as a screening test in the medical community; however, because of being simple and noninvasive, it is almost always requested for evaluation and ruling out cancer. It plays an important role in the treatment and post-treatment process, the prediction of prognosis, and the relapse of the disease. The present study aimed to determine the relationship between a high level of CA125 tumor marker and ovarian cancer by detecting spesivity, sensivity, positive and negative predictive values. Methods & Materials: In this cross-sectional study, all cases undergoing CA125 test in Velayat Hospital in 2017-1028 were evaluated for having ovarian cancer. In addition, the CA125 level was compared between healthy individuals and patients with ovarian cancer. Finally, the obtained data were analyzed using SPSS. Ethical Considerations: The present study was approved by the Qazvin University of Medical Sciences (Ethics Code: IR.QUMS.REC.1396.316). Results: In this study, 35.3% of the study participants received a definite diagnosis of ovarian cancer. Generally, CA125 values were negative in 41.8% and positive in.58.2% of the study subjects. The sensitivity of the test was measured as 80.1%, the specivity as 53.6%, the positive predictive value equaled 48.4%, and the negative predictive value was measured as 83%. There was a significant relationship between age and the presence of ovarian cancer, and serum CA125 levels. Conclusion: The present study suggested that age and the serum level of CA125 were statistically significant. Finally, CA125 levels were significantly related to ovarian cancer. It provided moderate specivity and specivity as well as low positive predictive value and high negative predictive value as a tumor marker; it is valuable for ruling out of tumor but not appropriate as a screening test.

Object: To assess the diagnostic value of Risk of Ovarian Malignancy Algorithm (ROMA) in predicting ovarian malignancy. Methods: Diagnostic test was performed at dr. Mohammad Hoesin Hospital Palembang during June 2016 to November 2016. Data were analized with SPSS version 21.0 and Med-calc statistic. Results: A total of 57 subjects were recruited in this study. Subjects were divided into two groups: the premenopausal and postmenopausal group. Analysis with ROC curve was performed, the ROMA optimal cut-off of ROMA was 23.7% and 48.15% in the premenopausal and the post-menopausal group, respectively. With the optimal cut-off, the sensitivity was 79.41% and specivicity was 75%, positive predictive value wa 73.07% and negative predictive value 83.77% with accuracy 76.92% in diagnosing ovarian malignancy. Compared to RMI-3, the sensitivity was 65.5% and specivicity was 85.7% with accuracy 75.44%. Conclusion: ROMA is not a reliable diagnostic tools of ovarian malignancy. Objective: This research aimed to analyze sensitivity and specificity values of platelets, RMI and both combined as ovarian cancer diagnostic modality in Haji Adam Malik Hospital in 2016-2018. Methods: This is analytic research with a diagnostic test design on 204 patients who had been diagnosed with ovarian cancer and patients with ovary benign tumors which have been examined the value of full blood and the malignancy ratio index and ovarian mass that has been proven by the results of anatomic pathology at General Hospital Haji Adam Malik Medan in 2016-2018. The data is tabulated into 2x2 table and then calculated for each sensitivity, specificity, positive predictive value and negative predictive value. Result: By using platelet cut-off value>450000 per mm3 in diagnosing ovarian cancer with sensitivity 55.44% and specificity 83.65% were obtained. IRK has a sensitivity 83.16% and specificity 76.92%. Platelet and IRK values ​​provided the highest diagnostic value (specificity) compared to when they were each single which was 97.11% while the combination of platelet and IRK values ​​had a sensitivity of 49.50%. Conclusion: Platelet and IRK values ​​gives the highest diagnostic value (specificity) compared to when both are used, namely 97.11%.

Ovarian cancer is the sixth most common cancer found among women. Incidence of ovarian cancer in Dr. M. Djamil Central Hospital had increased from 103 cases to 156 cases in 2011-2012. The Risk Of Ovarian Malignancy Algorithm (ROMA) consists of qualitative serum test where several variables including HE4 serum, CA125 serum, and menopausal status are turned into numerical score (Hye Yon Cho et al 2015). The latest diagnostic modalities developed by International Ovarian Tumour Analysis (IOTA) classifies tumour into four sub groups, based on ultrasound characteristics: unilocular, multilocular, adnexal mass with solid component without papilloma projection, adnexal mass with one or more papilloma projection. Based on this knowledge, author would like to investigate accuracy of ROMA and IOTA score in predicting malignancy status of early ovarian tumour.Objective: Determining ROMA and IOTA so thus can be utilised to predict wether the ovarian tumour is benign or malignant.Method: This was analytical study, with cross sectional design and took place in Obstetrical and Gynaecological Outpatient Clinic of Dr. M. Djamil Central Hospital from August 2016 to January 2019. Sample was recruited using consecutive sampling method. Sixty one participants were recruited with CI 99%. Patients who met inclusion criteria would undergo Ca125 serum test and IOTA score. HE4 level was determined after surgery  and the sample of a tumour was sent to pathology anatomy laboratory of Medical Faculty of Andalas University to investigate the malignancy status. Statistical analysis involved univariate and bivariate test. The variables included frequency distribution, ROMA, and IOTA score. Bivariate analysis included specificity, sensitivity, positive predictive value, negative predictive value, positive likelihood ratio, negative likelihood ratio.Result: Thirty one patients (50,8%) from peri menopausal group had HE4 level ≤70 pmol/L and 30 patients (49,2%) from post menopause group had HE4 level ≤140 pmol/L. Eleven patients (18%) had CA125 level <35 and 50 patients (82%) had CA125 level ≥35. ROMA index showed low risk in 17 patients (27,9%) and high risk 44 patients (72,1%). IOTA with M Rules was 51 patients (83,6%) and B rules was 10 patients (16,4%). Diagnostic value for ROMA score had sensitivity of 71,9%; RMI of 84,2%; and IOTA 82,5%. ROMA and IOTA accuracy was 68,9% and 92,2%, respectively.Conclusion: IOTA has better sensitivity in predicting ovarian tumour compared with ROMA. ROMA has higher positive predictive value compared with IOTA positive predictive value. Abstract Background There are many causes of pleural disease including variable benign and malignant etiologies. DWI is a non-enhanced functional MRI technique that allows qualitative and quantitative characterization of tissues based on their water molecules diffusivity. The aim of this study was to evaluate the diagnostic value of DWI-MRI in detection and characterization of pleural diseases and its capability in differentiating benign from malignant pleural lesions. Results Conventional MRI was able to discriminate benign from malignant lesions by using morphological features (contour and thickness) with sensitivity 89.29%, specificity 76%, positive predictive value 89%, negative predictive value 76.92%, and accuracy 85.37%. ADC value as a quantitative parameter of DWI found that ADC values of malignant pleural diseases were significantly lower than that of benign lesions (P < 0.001). Hence, we discovered that using ADC mean value of 1.68 × 10-3 mm2/s as a cutoff value can differentiate malignant from benign pleural diseases with sensitivity 89.3%, specificity 100%, positive predictive value 100%, negative predictive value 81.2%, and accuracy 92.68% (P < 0.001). Conclusion Although DWI-MRI is unable to differentiate between malignant and benign pleural effusion, its combined morphological and functional information provide valid non-invasive method to accurately characterize pleural soft tissue diseases differentiating benign from malignant lesions with higher specificity and accuracy than conventional MRI.

Background: Thyroid carcinoma generally has a good prognosis. The main focus of current research on thyroid carcinoma is to increase the accuracy of preoperative diagnosis of thyroid nodules. When the result of fine needle aspiration biopsy (FNAB) is indeterminate, clinicians often have doubts in determining the surgical management. Objective: Protein BRAF expression analysis can help improve the accuracy of FNAB and optimize the management of differentiated thyroid carcinoma. Methods: This study is a diagnostic test performed from October 2016 at Sanglah General Hospital with 38 patients as subjects who fulfilled the inclusion criteria. Data is being presented in descriptive form before diagnostic test is done to determine sensitivity, specificity, positive predictive value, negative predictive value and the accuracy of immunocytochemistry test for BRAF on indeterminate thyroid nodule. Results: Thirty-eight samples met the inclusion criteria during the study period. Three samples were male (7.9%) and 35 samples (92.1%) were female. The mean age of the sample was 45.21 years (SD ±10.910 years) with ages ranging from 23 to 66 years. Of the 12 samples undergoing isthmolobectomy, 7 samples (58.4%) were determined to be malignant from histopathological results. The sensitivity value of BRAF immunocytochemistry test is 45.45% with a specificity value of 81.25%, a positive predictive value of 76.92%, a negative predictive value of 52% and an accuracy of 60.50%. Analysis of the receiver operator (ROC) curve shows the area under the curve (AUC) of 63.4% with a confidence interval of 45.5–81.2%. Conclusion: Immunocytochemistry BRAF test have a reliable diagnostic value and can be taken into consideration in the preoperative diagnosis of thyroid malignancies.

AbstractThis study aims to assess the diagnostic value of high-sensitivity C-reactive protein (hs-CRP), procalcitonin (PCT), and interleukin (IL)-6 in the diagnosis of pneumonia caused by Rhinovirus alone or with bacterial coinfection in Vietnamese children under 5 years of age. A cross-sectional study was conducted on 26 children under 5 years of age with severe pneumonia due to Rhinovirus at the National Pediatric Hospital. IL-6, hs-CRP, and PCT tests were performed. The diagnostic values of PCT, IL-6, and hs-CRP in classifying those with viral alone and those with bacterial coinfection were determined. Of 26 children, 10 children were diagnosed to have bacterial coinfections (38.5%). The optimal cutoff point for PCT was > 2.30 ng/mL (sensitivity 50%, specificity 94%, positive predictive value 83%, and negative predictive value 75%). The optimal cutoff point for hs-CRP was > 1.53 mg/dl (sensitivity 90%, specificity 56%, positive predictive value 56%, and negative predictive value 90%). Finally, the optimal cut-off point for IL-6 was > 441.5 pg/mL (sensitivity 20%, specificity 100%, positive predictive value 100%, and negative predictive value 60%). The accuracy rate of PCT was the highest with 69.2%, followed by hs-CRP with 65.4%. Inflammatory biomarkers such as PCT and hs-CRP were able to distinguish children with severe pneumonia caused by Rhinovirus alone and those with bacterial coinfection.

6014 Background: There are no data to support CA-125 as a surrogate biomarker for ovarian cancer PD in patients on maintenance therapy with a PARPi. We aimed to assess the concordance of PD by CA-125 with RECIST PD in patients treated with maintenance PARPi. Methods: We extracted data on PD as defined by GCIG CA-125 and investigator-assessed RECIST from the SOLO2/ENGOT-Ov21 (NCT01874353) trial. Patients were categorized into: (i) CA-125 and RECIST non-PD concordant; (ii) CA-125 and RECIST PD concordant; and (iii) CA-125 and RECIST discordant. We excluded those with PD other than by RECIST, PD on date of randomization, and no repeat CA-125 beyond baseline. To assess the concordance of CA-125 PD with RECIST PD and CA-125 non-PD with RECIST non-PD, we computed the positive predictive value (PPV), i.e. the probability that patients with CA-125 PD also had RECIST PD, and negative predictive value (NPV), i.e. probability that patients with no CA-125 PD also did not have RECIST PD, respectively. Results: Of 295 randomised patients, 275 (184 olaparib, 91 placebo) were included in the primary analysis. 80 (29%) had CA-125 PD and 77 had concordant RECIST PD, resulting in a PPV of 96% (95% CI 90%-99%). Of 195 patients without CA-125 PD, 101 also did not have RECIST PD, resulting in a NPV of 52% (95% CI 45%-59%; Table). Among those with RECIST PD (n = 171), a greater proportion of patients with RECIST-only PD had a normal baseline CA-125 than those with both CA-125 and RECIST PD (94% vs 69%; p< 0.001). Of 94 patients without CA-125 PD but had RECIST PD, 65 (69%) had CA-125 that remained within normal range, while 27 (29%) had rising and elevated CA-125 that did not meet the criteria for GCIG CA125-PD. Discordance between RECIST PD and CA-125 non-PD was similar in early (≤12 weeks) and late ( > 12 weeks) PD (56% vs 55%, respectively; p= 0.96). Conclusions: Almost half the patients with RECIST PD did not have CA-125 PD and most had CA-125 still within the normal range. Regular imaging should be considered as part of surveillance in patients on maintenance olaparib rather than relying on CA-125 alone. [Table: see text]
