Abstract The aim of this research was to evaluate the microbiological indoor air contamination level in chosen facilities of the primary health-care for adults and children. The total numbers of mesophilic bacteria, staphylococci, coli-group bacteria and moulds in both surgery rooms and patients’ waiting rooms were determined. Air samples were collected with a MAS 100 impactor and the concentration of microorganisms was estimated by a culture method. The microbiological air contamination level was diverse: the number of mesophilic bacteria ranged from 320 to 560 CFU/m3, number of staphylococci - 10-305 CFU/m3, coli group bacteria - 0-15 CFU/m3 and moulds - 15-35 CFU/m3. The bacteriological contamination level of the air in examined community health centers was higher than described in the literature for hospitals and exceeded the acceptable values proposed for the surgery objects.

Background and objective: The Primary Health Care Corporation in Qatar was established in 2012 and is comprised of 23 Health Centers. One of its goals is to create excellence in its workforce. A preceptorship program needs to be initiated at the Primary Health Care Corporation to ensure a high level of training for its nurses. The purpose of these preceptorship programs is to ensure nurses are equipped to carry out Qatar’s National Health Strategy and in doing so the Primary Health Care Corporation has this as its goal. This study amis to assess the effectiveness of preceptorship program models that can eventually be used for adoption as training programs for nurses in Health Centers in Qatar.Methods: A literature review of twenty articles published between 2006 and 2017 that focused on different models of preceptorship programs was conducted. The Mixed Methods Appraisal Tool was used to assess the quality of these studies. The data was analyzed by categorizing the included articles in a matrix sheet based on study design.Results and conclusions: Preceptorship programs are effective in four key areas: increasing nursing knowledge, supporting effective and safe care delivery by newly graduated nurses, increasing organizational support, and decreasing turnover rate and cost.

This study aimed to identify programmatic vulnerability to STDs/HIV/AIDS in primary health centers (PHCs). This is a descrip - tive and quantitative study carried out in the city of São Paulo. An online survey was applied (FormSUS platform), involving administrators from 442 PHCs in the city, with responses received from 328 of them (74.2%), of which 53.6% were nurses. At - tention was raised in relation to program - matic vulnerability in the PHCs regarding certain items of infrastructure, prevention, treatment, prenatal care and integration among services on STDs/HIV/AIDS care. It was concluded that in order to reach comprehensiveness of actions for HIV/ AIDS in primary health care, it is necessary to consider programmatic vulnerability, in addition to more investment and reor - ganization of services in a dialogue with the stakeholders (users, multidisciplinary teams, and managers, among others).

Studies related to the work environment in primary health care are scarce in the literature. The present study aimed to validate the psychometric properties of the Practice Environment Scale of Nursing Work Index (PES-NWI) in primary health care (PHC) and to evaluate its construct validity through exploratory and confirmatory factor analyses in a sample of Portuguese nurses. A quantitative, cross-sectional, and validation study design was implemented. Methods: The sample consisted of 1059 nurses from the PHC units of all 55 health center groups (HCGs) in mainland Portugal, 15 health centers in the Autonomous Region of Madeira, and 6 health centers in the Autonomous Region of the Azores. The study tested different structural models using exploratory and confirmatory factor analysis techniques. The reliability of the scale was tested by determining Cronbach’s alpha coefficient. Results: The internal consistency of the PES-NWI was 0.91. Exploratory and confirmatory factor analyses were performed on the PES-NWI model in PHC with five factors: NPOA, NFQC, NMALSN, SRA, and CNPR. The results show that the scale presents acceptable fit quality indexes in the final factorial solution and adequate convergent validity. Conclusion: The PES-NWI in PHC has an adequate, robust, and reliable five-factor structure. The scale is valid and can be used in clinical practice, nursing management, and PHC research.

Abstract Background Studying and monitoring the efficiency of primary health care centers has a special place in the health system. Although studies have been conducted in the field of efficiency in Iran, few have focused on rural primary health care centers. In addition, previous studies have not used the child mortality rate and Behvarzes as input and output. Objective The present study was conducted aimed to estimate the technical efficiency of rural primary health care centers and determinant factors in Hamadan using data envelopment analysis and Tobit regression. Methods This is a Longitudinal study of rural primary health care centers in Hamadan province (2002–2016). Data Envelopment Analysis was employed to estimate technical efficiency of sampled health facilities while Panel Tobit Analysis was applied to predict factors associated with efficiency levels. The outputs were child mortality rate under 1 year of age and child mortality rate 1 year to 5 years of age. The input was Behvarzes (rural health workers). Results The results of efficiency analysis showed that the average efficiency scores of the centers had a fluctuating trend during the period of the study, but the average performance scores generally decreased in 2016, as compared with 2002. The highest and lowest average performance scores were observed in 2003 (0.78) and 2013 (0.56), respectively. Number of physicians and rural primary healthcare centers per population had a positive statistically significant and the number of midwives and the total fertility per population had a negative statistically significant effect on efficiency. Conclusions The findings suggest some level of wastage of health resources in primary health centers. Findings indicate a level of waste of health resources in primary health centers. Behvarz functions in providing primary care services can be considered in the reallocation and optimal use of available resources at the level of rural health centers.

Background: In response to the COVID-19 global pandemic the Primary Health Care Corporation (PHCC) in Qatar recognized the need to limit face-to-face consultations within health centers in order to prevent unnecessary spread of the virus by offering an alternative solution for patients to access primary health care. PHCC responded by establishing teleconsultation services (telephone and video) within health centers and establishing a community call center offering teleconsultations for family medicine, ophthalmology, and dentistry services, with a nursing tele-triage service operating 24 hours a day, 7 days a week. Methods: All previously booked appointments within health centers were converted to outbound telephone consultations. The community call center was widely advertised and received incoming patient inquiries. This led to an analysis of the teleconsultation service utilization on a daily, weekly, and monthly basis. Results: The 27 health centers successfully provided 703,845 teleconsultations (via video and telephone calls) between 1st April and 31st October 2020 compared to 964,285 in-person consultations. A significant percentage of patients continue to opt for a telephone consultation rather than a face-to-face visit. Between 29th March and 28th November 2020, the community call center received a total of 159,137 calls with 61% of calls managed and resolved by a nurse, without the need for a consultation by a physician. Conclusion: The rapid response by PHCC to find alternative means to offer primary health care services during a pandemic was activated early and the public responded positively. Teleconsultation services within Qatar have been widely accepted by patients who have more choices to access primary care services. Since April 2020, when teleconsultations were fully implemented in PHCC, to October 2020, 42% of all consultations have been via teleconsultation . Teleconsultation services both within health centers and the community call center, offer an alternative means of care delivery, empowering patients.
