The time dependence of visual functions recovering of the patients after diabetic vitreopapillary traction syndrome surgical treatment

The aim of the work was to analyze the long-term results of using a temporary “liquid” implant in the surgery of congenital glaucoma in children. The basis of the developed method of filtrative antiglaucomatous surgery (Patent of Ukraine No. 45099 of 2009) – viscosinusotrabeculotomy – has been set the task of reducing the risk of developing intra- and postoperative complications, reducing the scarring rate and maintaining the newly created ways of the intraocular fluid outflow, which in general will increase the effectiveness of surgical treatment of congenital glaucoma in children. 54 children (91 eyes) with simple congenital glaucoma at the age of 1 to 36 months were operated on average (8.7 ± 8.2) months. The persistent and long-lasting hypotensive effect achieved due to viscosinusotrabeculotomy in children with developed and far-advanced stages of congenital glaucoma stops the processes of stretching of the membranes of the eye and stabilizes their size, which in general allows preserving and visual functions improving, in infancy – creating conditions for their formation.

The paper describes a clinical example of the intraocular correction of high-grade hyperopic anisometropia. Surgical treatment made it possible to restore binocular vision, obtain high visual functions without additional correction, preserve the accommodative ability of the eye, and achieve rapid visual rehabilitation.

Purpose. To analyze changes in binocular vision functions in patients with acutely acquired esotropia after preoperative correction by prisms. Material and methods. The study included 10 patients with acutely acquired esotropia with diplopia. Depending on the tactics of preoperative case management, two groups were distinguished: 1 - 5 patients, this group underwent preoperative prismatic correction of diplopia and the deviation angle, 2 - 5 patients, in this group underwent surgery without prior prismatic correction. Fresnel prisms were used to select and assign prismatic correction. The patients were monitored dynamically. Results. In the course of treatment 1 group according to the indicator of Worth's four-dot test color test, revealed a statistically significant increase in the frequency of value of the binocular single vision (p=0.041), the frequency of the presence of stereovision according to the Fly-test (p=0.013). Conclusion. Prismatic correction in combination with surgical treatment had better curative benefits than surgery itself in improving binocular and stereoscopic visual functions. Purpose. To present the course of a complete macular rupture, depending on the management tactics. Material and methods. On the example of two clinical cases, the outcome of a complete macular rupture is presented: without surgical treatment and as a result of surgery. Results. Surgical methods of treatment made it possible to obtain high visual functions in the patient - visual acuity up to 0.9. Conclusions. Modern methods of surgical treatment of MR provide restoration of the structure of the outer layers of the retina and high visual functions, provided that the optical media of the eye are transparent. Key words: macular rupture, surgical treatment of macular rupture, course of macular rupture without treatment.

Purpose. Optimization and identification of key conditions for successful surgical treatment of PDR in patients with permanent hemodialysis. Material and methods. The results of surgical treatment of 8 patients (10 eyes) with PDR who received permanent hemodialysis were analyzed. All patients underwent surgery 18-20 hours after the hemodialysis session. 25G vitrectomy with phacoemulsification 1-5 days after intravitreal injection inhibitor of anti-VEGF. Results. In 9 cases, an improvement in visual functions was achieved. Visual acuity of 0.08-0.1 in 5 cases, 0.1-0.2 in 3 cases and 0.3 in one case on a single eye. Conclusions. The described stereotype is the optimal approach for the surgical treatment of PDR in patients with permanent hemodialysis. Key words: proliferative diabetic retinopathy, hemodialysis, vitrectomy.

The success of surgical treatment of glaucoma, stabilization and preservation of visual functions are fundamentally dependent on the steady normalization of the eye hydrodynamics. The problem of refractory, resistant to glaucoma treatment in children and adults, including congenital glaucoma, remains extremely urgent. The article presents the results of glaucoma treatment, including refractory and buffalma glaucoma, using polymer-salt material "LitAr". The proposed composite proved to be useful both for planned application and in case of buftalma to provide fast remission and improvement of vision. An important fact is that the polymer-salt material "LitAr" has been successfully used both in primary operations and corrective interventions in patients with resistant to treatment glaucoma.

The article analyzes the state of patients visual acuity after successful surgical treatment of retinal detachment. On the basis of gathered data, it was concluded that in case of detachment of the macula only in 50% of cases it is possible to increase visual acuity to 0.4 and higher. Restoration of visual functions continues for at least 6 months after the operation and is determined by the restoration of the structure of the outer segments of the photoreceptor cells. During this time, it is advisable to conduct drug therapy aimed at normalizing blood flow and functional activity of the retina. Visual functions recovery continues for at least 6 months after the operation and is connected with the restored structure of the outer segments of the photoreceptor cells. Important prognostic factors of central vision restoration in the postoperative period are visual acuity before surgery, duration of existence and height of macular detachment. Data on which of the methods of surgical treatment of retinal detachment allows to achieve higher visual acuity are contradictory. There is practically no data on the comparison of the effect on visual acuity of scleral buckling and vitrectomy in the long-term period, in patients with phakic eyes and with artiphakia. On visual acuity after fitting detachment of the macula may affect macular edema, epiretinal membrane formation and retinal folds, and edema of the peripapillary optic nerve head, progressive deterioration of blood flow in the basin of the central retinal artery, short posterior ciliary arteries and ophthalmic artery. It is believed that these factors are significantly more pronounced after scleral buckling than after vitrectomy. Some indicators of optical coherence tomography correlate with visual acuity after surgical treatment of retinal detachment: the state of the articulation line of the external and internal segments of the photoreceptors, as well as the state of the external limiting membrane.

Aim. To estimate development terms, visual functions upon hospital admission and discharge, medical and surgical treatment results of different nature endophthalmitis. Materials and methods. Data of 40 patients were studied, which received treatment for postoperative, endogenous, posttraumatic endophthalmitis. Main age of patients was 61 year. Results and discussion. Patients with postoperative endophthalmitis have higher baseline visual acuity, and an emergency vitrectomy is a method of choice. Patients with endogenous severe endophthalmitis more often need enucleation. Intravitreal antibiotic injection in endophthalmitis by no means always brings an improvement, but could be used as an adjunct to systemic therapy or as a measure for a patient in anticipation of vitrectomy.

Essentially mesodermal iris dystrophy (EMID) is a type of iridocorneal endothelial syndrome and has a chronic progressing character. It is observable after a local fibrosis is formed in the outer border of the iris, which subsequently causes the peripheral iris to pull up and leads to pupil deformation.Purpose: to evaluate the outcome of modern high-tech surgical treatment, which improves visual functions, prevents the decompensation of intraocular pressure and ensures a cosmetic effect.Materials and methods. Patient R., 48 yrs, was diagnosed with OU EMID, low hyperopia, astigmatism, and presbyopia. In order to prevent the development of secondary glaucoma and correct refractive anomalies, the patient was subjected to phacoemulsification with implantation of a multifocal toric IOL, synechiotomy, and iris plastic surgery. MTIOL was calculated using the VERION™ Image Guided System navigation computing complex (Alcon, USA) and the IOLMaster® 700 optical biometer (Carl ZEISS, Germany).Results. In early post-surgery period, the patient noted an increase in visual acuity, stabilized by the 6th month, a significant decrease of undesirable optical phenomena (glare, halo) and was satisfied with the postoperative cosmetic result. The level of intraocular pressure also stabilized by the 6th month after surgery and remained within the reference range by the 9th month. No intraoperative complications were observed. Control UBM scans showed transparent OU cornea, anterior chamber being shallower than the average depth, exfoliated stroma of the iris, moderate anterior chamber angle, intact ciliary body, centrally located pupil, IOL in capsule bag.Conclusion. The case describes a successful outcome of surgical treatment of a rare variety of iridocorneal endothelial syndrome, increase in visual functions, achievement of the desired cosmetic effect, and improvement in the quality of life of the working age patient.
