Purpose. To present the course of a complete macular rupture, depending on the management tactics. Material and methods. On the example of two clinical cases, the outcome of a complete macular rupture is presented: without surgical treatment and as a result of surgery. Results. Surgical methods of treatment made it possible to obtain high visual functions in the patient - visual acuity up to 0.9. Conclusions. Modern methods of surgical treatment of MR provide restoration of the structure of the outer layers of the retina and high visual functions, provided that the optical media of the eye are transparent. Subject: Salivary stone disease is a disease that occurs as a result of inflammatory processes occurring in the salivary glands. More recently, salivary stone disease was considered a rare disease. Thanks to the available modern methods of diagnosis, the detection of salivary stone disease is significantly progressing. Purpose: In our work we aimed to figure out the most effective methods of diagnosis of salivary stone disease . Among the private methods of radiation diagnosis of salivary stone disease, we have identified ultrasound and CT examination of the salivary glands and ducts. Our task was also to determine how conservative treatment is competitive with the surgical method of treatment of this disease. Results: As a result of our studies , we concluded that the ultrasound method in the diagnosis of inflammation of the salivary glands and the presence of concretions in the glands and ducts is highly informative , safe and accessible to patients . Conservative methods of treatment used in patients who were diagnosed with the presence of concretions at the outlet of the salivary gland ducts are no less effective than surgical methods. Surgical treatment of salivary stone disease is advisable only in cases of block of the salivary gland and ducts concretions more than 0.5 cm in diameter. Conclusion: Salivary Stone disease in our medical institution is diagnosed quite often. The method of ultrasound diagnosis is the main method of examination of patients who came to us with symptoms of salivary stone disease. CT method is advisable to carry out patients in need of surgical treatment of the disease. The method of CT diagnosis allows to establish the exact coordinates and sizes of concretions located in the salivary glands and deep in the ducts.

Damage to the posterior capsule of the lens after laser vitreolysis causes the formation of traumatic cataracts and a decrease in visual acuity. The search for surgical methods of treating such complications is relevant. Purpose. To develop and improve the technique of posterior capsulorexis during phacoemulsification and implantation of IOL combined with vitrectomy in the presence of an initial injury of the posterior lens capsule. Material and methods. Clinical case - a patient came to the clinic with complaints about a decrease in visual acuity and quality after laser vitreolysis performed in another clinic. Observed the damage of the posterior capsule of the lens. The operation was performed according to the developed technique. Results. A method of primary posterior capsulorexis during phacoemulsification and implantation of IOL after vitreolysis, combined with vitrectomy, is proposed. Achieved high visual acuity after the operation, OD=1.0. Conclusions. 1) The developed technique of primary posterior capsulorexis is safe and allows partially preserving the posterior capsule of the lens, while forming a «window» in the area of damage, and implanting the IOL into a capsule bag. 2) The installation of scleral ports during primary posterior capsulorexis makes it possible to successfully combine this operation with vitrectomy and prevent the displacement of lens fragments to the fundus. Purpose. Optimization and identification of key conditions for successful surgical treatment of PDR in patients with permanent hemodialysis. Material and methods. The results of surgical treatment of 8 patients (10 eyes) with PDR who received permanent hemodialysis were analyzed. All patients underwent surgery 18-20 hours after the hemodialysis session. 25G vitrectomy with phacoemulsification 1-5 days after intravitreal injection inhibitor of anti-VEGF. Results. In 9 cases, an improvement in visual functions was achieved. Visual acuity of 0.08-0.1 in 5 cases, 0.1-0.2 in 3 cases and 0.3 in one case on a single eye. Conclusions. The described stereotype is the optimal approach for the surgical treatment of PDR in patients with permanent hemodialysis. Key words: proliferative diabetic retinopathy, hemodialysis, vitrectomy.

Keratoconus is a progressive degenerative disease in which, due to the weakening of the biomechanical strength of the cornea, its cone-shaped deformation occurs, leading to the development of induced myopia and astigmatism, as well as to a decrease in corrected visual acuity. Treatment of this disease consists in achieving stabilization of the process by strengthening the structure of the cornea, as well as in correcting refractive disorders caused by corneal deformation. Purpose. The purpose is to evaluate the effectiveness of correction of refractive disorders by implanting a closed MyoRing ring in the corneal stroma of patients with keratoconus. Material and methods. The data of 98 MyoRing implantation operations using CISIS technology using the PoketMaker ultraceratom for stage 3-4 keratoconus were analyzed. The observation period is up to 9 years. The initial myopia averaged -9.23 D ±3.82, astigmatism-6.13 ±2.51. Results. Stabilization of the refractive effect occurred usually within 3-6 months after surgery. On average, the following correction of refractive disorders was achieved: myopia-8.14 D ± 2.67 and astigmatism-4.28 D ± 2.12. Keratometry indicators also in most cases stabilized within 3-6 months after surgery and remained unchanged throughout the entire follow-up period. Conclusions. Implantation of a closed MyoRing ring in keratoconus makes it possible to significantly correct refractive disorders and prevents further progression of the disease. AbstractStoma is an intestinal fistula created in emergency or by elective indications, and it is done to drain out the digestive tract content. In some patients there is a disturbance passage of gastric contents through the stoma, which may take the form of chronic constipation or even periodic subileus that will sooner or later require surgical treatment.The aim of the study was the assessment of the causes and method of treatment of constipation in patients with intestinal stoma.Material and methods. A total of 331 patients with stoma followed by Ostomy and Proctology Outpatient Clinic were included in the study in the years 2011-2014. The study included 146 women and 185 men and the average age was 61.3 ± 12.7 years. Within the entire froup, 273 patients had the end stoma performed whereas in 58 patients the loop stoma was created. The highest percentage of patients were the ones with diverticulosis and colorectal cancer, i.e. 132 and 114 patients respectively. A stoma was created in 35 patients due to inflammatory bowel disease (IBD), in 23 patients because of cancer, in 14 as a result of injuries and in 13 due to rectovaginal fistula.Results. Out of the entire group subject to study (331 patients) 93 patients (28.1%) suffered from constipation. 50 patients with constipation required surgical intervention. The most common indication for surgical treatment was the parastomal hernia (36 patients, 72%), other indications were the narrowing of the stoma (5 patients, 10%), its collapse (6 patients, 12%) or prolapse (3 patients, 6%). Parastomal hernia was responsible for 84% of constipation within the stoma and 86.1% were treated with laparotomy (31 out of 36 patients). Other causes of constipation were the stomal stenoses (5 patients), collapse of the stoma (6 patients) and stomal prolapse (3 patients). All patients were treated surgically with a good final result.Conclusions. Constipation associated with dysfunction of the stoma in most cases should be treated surgically. Parastomal hernia is the most common cause of constipation in the stoma. Treatment should be performed in due time because of the possibility of developing complications, especially dangerous one is a strangulated parastomal hernia and ischemia of stoma.

Surgical methods of treatment of Peyronie’s disease by plication corporoplasty proved to be effective in the near and distant postoperative periods in the examined group of patients. These methods made it possible to quickly achieve the normalization of the shape of the penis without affecting the erectile function which enabled for patients to restore copulative activity and improve the quality of life.

The trauma of the anterior cruciate ligament is one of the most prevalent kinds of trauma among sportsmen. It is necessary to determine the degree of trauma of the ligament as accurately as possible which will allow appointing the optimal tactic of treatment. But nowadays there are a lot of different kinds of conservative and surgical treatment and also various complexes of rehabilitation. In this work authors introduce analysis and systematization of modern methods of treatment and rehabilitation of patients with injuries of anterior cruciate ligament with the goal to study evolution and modern condition of tactical and technological ways of rehabilitation of patients with injuries of the anterior cruciate ligament.

Purpose. To analyze changes in binocular vision functions in patients with acutely acquired esotropia after preoperative correction by prisms. Material and methods. The study included 10 patients with acutely acquired esotropia with diplopia. Depending on the tactics of preoperative case management, two groups were distinguished: 1 - 5 patients, this group underwent preoperative prismatic correction of diplopia and the deviation angle, 2 - 5 patients, in this group underwent surgery without prior prismatic correction. Fresnel prisms were used to select and assign prismatic correction. The patients were monitored dynamically. Results. In the course of treatment 1 group according to the indicator of Worth's four-dot test color test, revealed a statistically significant increase in the frequency of value of the binocular single vision (p=0.041), the frequency of the presence of stereovision according to the Fly-test (p=0.013). Conclusion. Prismatic correction in combination with surgical treatment had better curative benefits than surgery itself in improving binocular and stereoscopic visual functions. Malignant glaucoma is a difficult task for an ophthalmologist in the treatment process. This is a multifactorial condition that occurs in anatomically predisposed eyes. Malignant glaucoma remains one of the most severe clinical forms of glaucoma requiring timely diagnosis and immediate treatment. Diagnostic issues, pathophysiological mechanisms of development and methods of treatment are still controversial. The diagnosis of malignant glaucoma is often late due to similarity with some other pathological conditions. In many cases malignant glaucoma is resistant to medication and laser treatment. Surgical treatment of malignant glaucoma is complex and not always effective. The review discusses the possible pathophysiological mechanisms of development, diagnostic methods of research, as well as the applied conservative, laser and surgical methods of treatment of malignant glaucoma.

Introduction. Emphysematous pyelonephritis (EPN) is a rare form of acute suppurative pyelonephritis, which is characterized by a rapid and severe course due to the pathogenic effect of the gas-forming bacterial flora. Currently, only a few hundred clinical cases of EPN are described in the literature. Currently, there are no standards for the surgical treatment of patients with EPN, but there is a clear tendency towards a wider use of minimally invasive methods of treatment and limitation of indications for nephrectomy. A risk-based approach based on the proposed radiological classifications of EPN is considered preferable. Purpose. Тo study all literature sources available for the current period on the Internet, describing clinical cases and experience in treating patients with a diagnosis of emphysematous pyelonephritis. Materials and methods. Тhe results of a search in the scientific databases PubMed, MEDLINE, Embase, elibrary were analyzed for the queries «emphysematous pyelonephritis», «classification», «gassing bacterial flora», «diabetes mellitus», «conservative treatment», «nephrectomy», «percutaneous drainage». 754 scientific publications were found, 25 were selected for detailed analysis. Results and discussion. Today, there are no standards for surgical treatment of patients with EP, but there is a clear trend towards wider use of minimally invasive methods of treatment and limiting the indications for nephrectomy. Minimally invasive methods of treating EN allows organ-preserving treatment and reduces the risk of renal failure. The lack of clear indications for the choice of the scope of surgical treatment and the low orientation of specialists towards radiological diagnostic criteria are the main reasons for excessive organ-carrying treatment of patients with PF. A risk-mediated approach based on the proposed clinical and radiological classifications of EP is considered preferable. In controversial cases, it is possible to recommend an attempt at minimally invasive treatment in combination with adequate conservative therapy, and only if it is ineffective, resort to performing nephrectomy. Conclusion. Indications for nephrectomy should be strictly limited and revised taking into account modern approaches. Nephrectomy should be an option of choice only if conservative treatment is impossible or ineffective.
