Functional gastrointestinal disorders are considered as a clinical variant of the interaction between psychosocial factors and gastrointestinal tract physiology implemented through the "brain - intestine" axis. The purpose of the study was to investigate psychological peculiarities of children with irritable bowel syndrome. The study included 68 children aged from 14 to 18 years with the confirmed diagnosis of irritable bowel syndrome. To determine the type of character accentuation, the Schmishek -Leonhard test questionnaire (child version) was used. The SCL-90-R test questionnaire (by L. Derogatis method) was applied to determine psycho-emotional disorders. The study has found out the majority of patients have certain psycho-emotional disorders. The group of children with irritable bowel syndrome, who were experiencing constipation, more frequently demonstrated an emotional type of character accentuation and a somatized and depressed type of psychological disorders. The patients with irritable bowel syndrome and diarrhoea had cycloid and demonstrative types of character accentuation and demonstrated an anxious type according to the questionnaire. The patients with mixed bowel rhythms exhibited features of a cycloid, emotional, and pedantic type of character accentuation. Depression, anxiety, somatised types were reported as manifestations of psychological disorders.

Context: Irritable bowel syndrome (IBS) is one of the most common chronic gastrointestinal (GI) disorders. Recently, many studies have found the association between irritable bowel syndrome (IBS) & the psychological factors. Aims: The study aimed to determine the relationship between psychological features and different subtypes and severity of irritable bowel syndrome. Settings and Design: This cross-sectional study was performed among 208 outpatients diagnosed as irritable bowel syndrome in two government medical college hospitals of Dhaka city from January, 2015 to December, 2015. The subtypes and severity of irritable bowel syndrome were assessed by Rome-III and ‘Irritable bowel syndrome severity scoring scale’ respectively. Psychological features were assessed by Brief Symptom Inventory consisting 53 items of psychological problems. Results: Global severity index of Brief Symptom Inventory, which is the best indicator of psychological distress, was found significantly higher in respondents with diarrhea predominant IBS (p< 0.001) and severe IBS patients (p< 0.001). Mean scores of other psychological problems among Brief Symptom Inventory -53 subscales (hostility, interpersonal sensitivity, paranoid ideation, psychoticism, depression, anxiety) were significantly higher in diarrhoea predominant IBS and severe IBS patients compared to the others (p<0.001). Severity score of irritable bowel syndrome was also moderately correlated with Global severity index (r= 0.417, p<0.001). Conclusions: This study found relationship between psychological features and different pattern of irritable bowel syndrome. Patients with irritable bowel syndrome (IBS) usually suffer from non-specific and overlapping signs that hamper the diagnostic process. In line with this, biomarkers specific for IBS could be of great benefit for diagnosing and managing patients. In IBS, the need is for apparent distinguishing features linked to the disease that improve diagnosis, differentiate from other organic diseases, and discriminate between IBS subtypes. Some biomarkers are associated with a possible pathophysiologic mechanism of IBS; others are used for differentiating IBS from non-IBS patients. Implementation of IBS biomarkers in everyday clinical practice is critical for early diagnosis and treatment. However, our knowledge about their efficient use is still scarce. This review discusses the biomarkers implemented for IBS diagnosis and management, such as blood (serum), fecal, immunological, related to the microbiome, microRNAs, and some promising novel biomarkers associated with imaging and psychological features of the disease. We focus on the most common, studied, and validated biomarkers and their biological rationale, diagnostic, and clinical value.
