In recent years, advances in medical, surgical and nursing care have resulted in a dramatic increase in the life expectancy of children born with Down's syndrome (DS), giving rise to an older population of people with DS not previously seen. It has become clear from the study of the ageing DS population that an association exists between DS and Alzheimer's disease (AD). This paper reviews neuropathological and clinical evidence of an association between DS and AD. The clinical features of AD in people with DS and apparent differences between the signs and symptoms of AD seen in individuals with DS and older people with AD in the general population are discussed. Issues pertaining to the diagnosis of AD in people with DS, such as diagnostic criteria, the process of diagnosis, and specific problems of diagnosing AD in people with DS are also discussed. The therapeutic needs of individuals with DS who are diagnosed as having AD are considered and the necessity to strive to make the diagnostic procedure as valid as possible is expressed. The paper concludes that neuropsychological testing has an important role to play in the diagnostic process.

The origin of paradental cysts is related to inflammatory processes, especially pericoronaritis involving impacted or semi-impacted teeth. The authors present a case of paradental cyst related to lower second molar that did not show clinical evidence of inflammatory process. The main aspects related to its classification, diagnosis and clinical characteristics are also discussed.

Background. It has been recognized that microbiota plays a key role in shaping immune system maturation and activity. Since probiotic administration influences the microbiota composition and acts as a biological response modifier, the efficacy of an adjuvant for boosting vaccine-specific immunity is investigated. Methods. A review of the literature was performed, starting from the mechanisms to laboratory and clinical evidence. Results. The mechanisms, and in vitro and animal models provide biological plausibility for microbiota use. Probiotics have been investigated as adjuvants in farm conditions and as models to understand their potential in human vaccinations with promising results. In human studies, although probiotics were effective in ameliorating seroconversion to vaccines for influenza, rotavirus and other micro-organisms, the results for clinical use are still controversial, especially in particular settings, such as during the last trimester of pregnancy. Conclusion. Although this topic remains controversial, the use of probiotics as adjuvant factors in vaccination represents a strategic key for different applications. The available data are deeply influenced by heterogeneity among studies in terms of strains, timing and duration of administration, and patients. Although these do not allow us to draw definitive conclusions, probiotics as adjuvants in vaccination should be considered in future studies, especially in the elderly and in children, where vaccine effectiveness and duration of immunization really matter.

AbstractMutated MCM9 has been associated with primary ovarian insufficiency. Although MCM9 plays a role in genome maintenance and has been reported as a candidate gene in a few patients with inherited colorectal cancer (CRC), it has not been clearly established as a cancer predisposition gene. We re-evaluated family members with MCM9-associated fertility problems. The heterozygote parents had a few colonic polys. Three siblings had early-onset cancer: one had metastatic cervical cancer and two had early-onset CRC. Moreover, a review of the literature on MCM9 carriers revealed that of nine bi-allelic carriers reported, eight had early-onset cancer. We provide clinical evidence for MCM9 as a cancer germline predisposition gene associated with early-onset cancer and polyposis, mainly in a recessive inheritance pattern. These observations, coupled with the phenotype in knockout mice, suggest that diagnostic testing for polyposis, CRC, and infertility should include MCM9 analysis. Early screening protocols may be beneficial for carriers.

Amphotericin B, a systemic antifungal agent, has not been associated with clinical evidence of cardiac toxicity. We report the case of a 76-year-old patient who developed transient asystole with cardiovascular collapse on two occasions, which coincided with infusion of amphotericin B. The patient was semicomatose and had candida septicemia and renal failure. Serum potassium and digoxin levels were elevated, and serum calcium was low when these episodes occurred. Subsequent postmortem examination revealed no gross evidence of cardiac disease apart from fungal vegetations on the aortic valve. A clear causal relationship between amphotericin B and transient asystole is not demonstrated, but a temporal association, confirmed with rechallenge, is documented in this patient. No similar cases were found in a review of the literature.

A case is described of a clear cell sarcoma with ultrastructural evidence of melanin production. The tumour appeared on the distal portion of the index finger of a thirteen-year-old girl and showed little change in size over seven years of observation by the patient. At age twenty she required hospital admission at which time clinical evidence of metastases to axillary nodes, lungs and bone was found and the patient died within two months. The previous cases reported in the literature are discussed.

Abstract Background: Primary ovarian surface epithelial carcinoma of testis and paratestis is very rare, especially mucinous subtype. We report a case of a 48-year-old man with primary mucinous adenocarcinoma of the testis and discuss its gross, histological and immunohistochemical features based on limited reports.Case presentation: A case of a 48-year-old male patient with mucinous adenocarcinoma of the testis is reported. The tumor was confined to the testis and separated from the epididymis. The tumor has neither interstitial infiltration nor metastasis to other organs, nor ovarian or germ cell tumor components. Immunohistochemical analysis showed that the tumor epithelial cells expressed CK20, CK7, CDX-2 proteins, whereas expression of SATB2 was absent. Ki-67 protein was weakly expressed in epithelial cells. The mucinous adenocarcinoma in the present case had the immunohistochemical characteristics of intestinal mucosa.Conclusions: Primary mucinous adenocarcinoma of the testis is rare and there is no standard treatment. The main treatment is surgical resection, but there is no effective treatment for metastatic tumors. Paclitaxel, platinum-based chemotherapeutics may have some curative effects, but there is still a lack of robust clinical evidence.

Prosthetic liners exist to improve amputee safety and comfort by adding a cushioning layer between the residual limb and the prosthetic socket. Many choices in liner technology are available, and clinicians often rely on personal intuition and experience to choose which liners are appropriate for which patients. The purpose of this study was to examine the literature to find what scientific evidence exists to inform prescription practices. ‘Prosthetic liner’ was used as a search term in the Web of Science and PubMed research databases. Fourteen scientific articles met the eligibility criteria and are discussed in this review. The results of this review suggest that there is little scientific evidence to inform prosthetic liner prescription practices. Liner material properties have been well-studied, but their influence on in vivo performance is not well understood. Understanding liner effect on function would be an area of great usefulness.

We present here a very rare clinical case of a 38-year-old man with Kaposiform hemangioendothelioma (khe) of the tongue who presented to our institution with a growth under the left side of the tongue with no pain or discomfort. There were no enlarged lymph nodes and no significant neurologic findings. Diagnostic histopathology confirmed the lesion to be khe. The tumour was removed surgically, and the surgical specimen confirmed the diagnosis. Follow-up at 3 months shows no clinical evidence of recurrence.
