Results of the study related to polymorphism of ACE gene (rs1799752)‎, integrin αIIbβ3, and CSK gene (rs1378942) influencing development of arterial hypertension in young patients with metabolic syndrome are presented. Hypertension as a component of the metabolic syndrome was detected in 15.0% of young patients. Prevalence of mutant alleles of the studied genes among the examined patients was quite high, so homozygous DD genotype was found in 21.6%, and mutant D allele of the ACE gene in 47.4%. A high risk of hypertension in patients with MS was detected in carriers of the T allele of the CSK (rs1378942) gene – 54.8%, which was most often observed in a combination of polymorphic ACE and CSK gene loci (p = 0.0053).

Background: The impact of metabolic syndrome on female sexual dysfunction received modest consideration in clinical practice. The aim of the research was to analyze the international literature to determine the relationship between the metabolic syndrome, its components and female sexual disorders. Methods: We identified relevant full-length papers by electronic databases as Index Medicus/Medline, Scopus, Life Science Journals, from 2005 to the present. Studies were searched using the following as search query: metabolic syndrome, female sexual dysfunction, obesity, systemic arterial hypertension, diabetes mellitus, dyslipidemia. Results: Women with metabolic syndrome showed higher prevalence of sexual inactivity and low sexual desire, orgasm and satisfaction respect to women without metabolic syndrome. Particularly metabolic components as diabetes mellitus, dy-slipidemia, systemic arterial hypertension were strongly associated with lower sexual desire, activity and Female Sexual Function Index total score. In contrast, other studies showed no relationship. Conclusion: Our study showed that in the clinical evaluation of women with metabolic syndrome routine inquiring about female sexual dysfunction should be recommended to ameliorate sexual function and quality of life. However more prospective and longitudinal studies on the sexual effects of metabolic syndrome should also be suggested to know the factors related to women's sexuality better.

Comorbid pathology is important in the survival of patients with breast cancer. The main aim of the present study was to investigate prognostic criteria of the 5-year survival menopausal women with breast cancer, including both the classic criteria (age, tumor size, lymph node status, hormonal status) and the presence of comorbid pathology (arterial hypertension, visceral obesity, diabetes mellitus, metabolic syndrome, immunodeficiency). A prospective cohort study included 98 patients with breast cancer (aged 63 ± 9 years, BMI 31.6 ± 4.8 kg/m2). Criteria favorable short-term (5 years) of the cancer prognosis of breast cancer in menopausal women are the size of the tumor less than 5 cm, the absence of metastases in regional lymph nodes, and the absence of the metabolic syndrome, dyslipidemia, high activity of suppressor cells. Age, anthropometric indices (BMI, waist circumference etc.), presence of arterial hypertension do not give information about the 5-year survival rate in breast cancer.

Non-alcoholic fatty liver disease (NAFLD) is a problem that is getting increasingly important and is often considered in the literature as a sign of liver damage in the metabolic syndrome. Currently, there is a tendency to rapid growth and rejuvenation of patients with this pathology. Metabolic syndrome, which is a polyetiological disease and is characterized by metabolic disorders and the launch of various pathological cascade reactions, can cause cardiovascular diseases, diabetes mellitus and their complications. Problem of comorbidity leads to deterioration in the quality of life of the patient and in severe cases can cause death. The article describes a clinical case of newly diagnosed NAFLD in a young patient with obesity in comorbidity with hyperandrogenism, postcholecystectomy syndrome, and reactive pancreatitis. The article presents a system analysis, bibliosemantics and case analysis of a particular patient with NAFLD. The search for sources was carried out using the scientific and statistical database of medical information. The absence of pathognomonic signs and complaints of this disease draws the attention of scientists and clinicians to conduct a comprehensive study of this problem, as well as improving the detection system of NAFLD in the early stages. This will help to avoid the development of the disease and various complications associated with the metabolic syndrome, as well as save and improve the patient’s life. Fighting against the “diseases of the 21st century” should begin long before the manifestation of obvious signs of the disease. This clinical case emphasizes the relevance of this problem in medical practice. Active identification and prevention of the initial manifestations of this disease in young people will help reduce the risk of developing comorbid diseases. Patients with NAFLD and obesity have an increased risk of various complications, therefore, they need more medical attention and frequent monitoring of liver indices and functional status.

Arterial hypertension is probably one of the most widespread diseases. According to statistics, the prevalence of AH at the age of 45–54 years among women is 58%, and at the age of 55–64 years it increases to 74.5%. Given the fact that the antihypertensive drugs are provided in the age group of 55–64 years, the order of 80% provides that the effectiveness of the use of drug therapy is only 30%. It is at this age that the postmenopause period occurs. On average, the duration of postmenopausal varies from 5 to 6 years and can last up to 65–69 years. Compared with men of the same age group, cardiovascular diseases are significantly less frequently affected. The presence of cardinal changes in the mechanism of occurrence and course of the pathology of the cardiovascular system in men and women, primarily due to the general differences and cardioprotective effects of ovarian hormones. It has been established that in the period of postmenopause a significant increase in the frequency of arterial hypertension occurs in women. Hypoestrogenism plays a significant role in the formation of arterial hypertension in this cohort of patients, and this affects the endothelium leading to its dysfunction, and its implementation is carried out through the components of the metabolic syndrome. From an analysis of the literature, it can be seen that the effect of hormone replacement therapy should be positive. Nevertheless, it should be noted that the development of arterial hypertension in postmenopause is a complex and multifaceted process.
