EXPERIENCE OF USING LASERS FOR VESTIBULOPLASTY IN PERY-IMPANT AREA

In this article authors raised the problem of the formation of an attached gum zone in the implant area. The important factors in the long-term and stable outcome of dental implantation are the availability of sufficient bone volume, adequate oral hygiene and correct positioning of the implant. Current studies point at the role of periodontal soft tissues in achieving an aesthetic result as a result of implant treatment. Review of the literature about the statistics of mucositis and peri-implantitis, problems of the dental implantation planning, alternative points of view on the recipient tooth socket, trauma of the donor site, etc. point at the immediacy of the problem. The problem of the advantages and disadvantages of methods and materials for gingival plastics continue to be relevant. The article presents the experience of the formation of a protective soft tissue buffer zone around the implant as a result of laser vestibuloplasty. Dental lasers are successfully used for the reconstruction of soft tissues in preparation for orthopedic treatment, plastic surgery of the oral cavity vestibule and frenulum. Laser application as a surgical instrument allows to make an incision more precisely and to overview the surgical area. Operative technique does not differ from the vestibuloplasty for the periodontal diseases. As a result of using an erbium or diode laser, the necessary vestibule of the oral cavity is formed. Features of the epithelialization of the oral mucosa and fixation of the movable edge of the wound make it possible to increase the area of the immobile gum. The authors presented a photo protocol describing the clinical case. According to indications, the use of the laser radiation allows to achieve the necessary clinical result for the long-term clinical and aesthetic success of dental implantation.

Relevance. A long-term inflammation in the oral cavity, unreasonable treatment of periodontal patients with antibiotics cause imbalance between certain types of fungal-bacterial associations accompanied by oral dysbiosis and change of local immune status. So, development and application of new comprehensive diagnosis and treatment techniques in periodontal patients, use of products for correction of microbiota and local immunity are a current and a long-term task. Purpose is to optimize the comprehensive treatment protocol of inflammatory periodontal diseases by introducing probiotics and evaluation of oral microbiome and cytokine profile.Materials and methods. 140 patients were examined. Of these 60 patients had chronic generalized plaque-induced gingivitis and 60 patients suffered from moderate chronic generalized periodontitis. Bacterial and fungal microbiome was assessed and the host immune response was evaluated in all patients before and after the treatment. Half of the patients were treated conventionally and the other half were treated according to a modified scheme: probiotic “Bifidumbacterin Forte” was added.Results. A large number of yeast-like fungi Candida and commensal bacteria were detected in periodontal pockets of patients with chronic generalized plaque-induced gingivitis and moderate chronic generalized periodontitis. That correlates with a significant increase of pro-inflammatory cytokines (TNF-α, IL—8), decrease of concentration of INF-γ and increase of antiinflammatory cytokine IL-4.Conclusion. Changes in clinical, microbiological and immunological values during a modified combination therapy with a probiotic and during a conventional treatment demonstrated that effectiveness of treatment of chronic gingivitis and chronic periodontitis increased by 40% and 37% respectively.

Peripheral Ameloblastoma (PA) is the rarest variant of ameloblastoma. It differs from the other subtypes of ameloblastoma in its localization: it arises in the soft tissues of the oral cavity coating the tooth bearing bones. Generally, it manifests nonaggressive behavior and it can be treated with complete removal by local conservative excision. In this study we report a case of PA of the maxilla in a 78-year-old female patient and we describe the four different histopathological patterns revealed by histological examination. After local excision and diagnosis, we planned a long term follow-up: in one year no recurrence had been reported. The choice of treatment is illustrated in Discussion.

Aim. Evaluation of dental status in patients with inflammatory periodontal diseases. Methods. The study of dental status was performed with the use of clinical and historical data from 269 patients with inflammatory periodontal diseases at the age of 18-65 years. Among examined patients there were 75 (27.9%) people with gingivitis, 54 (20.1%) with mild periodontitis, 66 (24.5%) with moderate and 74 (27.5%) with severe periodontitis. Control group consisted of practically healthy donors at the age of 18-52 years (40 people) with sanitized oral cavity. Results. From history and interviewing it was found that 72% of the surveyed patients brush their teeth twice a day, 24% - once a day and 4% of patients do not brush their teeth. Family history of periodontal diseases among parents was recorded by 86% of participants. Dental status was characterized by the increase of the values of all dental indices with increasing disease severity regardless of gender. Need for correction of the depth of the vestibule, lips, cords, and bridles was identified in 51.7% of cases in patients with gingivitis and in 96.6% of cases of periodontal diseases. With increasing severity of periodontitis index (sum) of teeth with decay and fillings and removed teeth was increased by 1.5 times in gingivitis, by 2.2 times in mild periodontitis, by 2.6 times in moderate and by 2.9 times in severe periodontitis compared to control group. Partial adentia in gingivitis is 3 times more prevalent in males than in females and in severe periodontitis is 1.6 times more prevalent in females than in males. Conclusion. Dental status of the patients with inflammatory periodontal diseases was characterized by increased values of all dental indices compared to control group that demonstrates typical course of inflammatory periodontal diseases and decrease of local immunologic resistance of oral cavity and the organism in total; timely correction of local factors (local trauma), anatomical and topographical features of dentofacial system and status of oral soft tissues allows prevention of pathologic processes in periodontal tissues.

This article is a review of the work of domestic and foreign authors on the effect of smoking on periodontal tissues, the mechanism of action of nicotine on the microvasculature, hygiene and soft tissues of the oral cavity. A modern diagnostic method using the Florida Probe computer system is considered. A treatment plan is proposed for patients with inflammatory periodontal diseases, the etiological factor of which is exposure to nicotine.This article is a review of the work of domestic and foreign authors on the effect of smoking on periodontal tissues, the mechanism of action of nicotine on the microvasculature, hygiene and soft tissues of the oral cavity. Background: To study the endoscopic trans-lateral molar (ETLM) approach to infratemporal fossa (ITF) lesions and analyze the advantages and disadvantages of this method. Methods: Four cases of ITF lesions were analyzed retrospectively. The clinical features, diagnosis and treatments, the operative process, and clinical applications of this surgical approach were discussed. Results: Postoperative pathologies were 2 pleomorphic adenomas, 1 schwannoma, and 1 inflammatory lesion. All patients had self-resolving cheek swelling and pharyngalgia in the short term, but 2 patients had numbness in the long term. There was no infection or bleeding in the postoperative period, and no difficulty in chewing after disease recovery. There was no tumor recurrence during the follow-up period. Conclusion: The ETLM approach is convenient, minimally invasive, and allows complete excision of benign ITF lesions, posterior to the lateral pterygoid muscle and mainly below the level of the hard palate. It is a simple and direct access to the ITF, but it is a narrow access because of the limitations of bones and soft tissues. Appropriate patient selection is mandatory for successful surgery.

<p class="abstract"><strong>Background:</strong> For anterior mandibular defects, vascularized bone flap reconstructions are the best for providing a solid arch necessary to restore form and function, especially in patients receiving tumour excision surgery in head and neck region. Our aim was to study aesthetic and functional outcome of free fibula osteocutaneous flap (FFOCF) in anterior mandibular defects in case of anterior floor of the mouth malignancies with mandibular infiltration.</p><p class="abstract"><strong>Methods:</strong> The clinical data of 15 oral cavity carcinoma patients with involvement of anterior floor of mouth with mandiblular infiltration that underwent FFOCF reconstruction at our institute, government medical college, Jammu, was prospectively collected from January 2018 till January 2020. In this study, the aesthetic and functional outcome of these patients was evaluated at 6 months and 1 year post surgery.</p><p class="abstract"><strong>Results:</strong> The aesthetic result of reconstruction was excellent in most patients. Also, there was no long-term donor-site morbidity. There was no peri-operative death. Flaps survived in all patients. Functional outcomes including the ability to tolerate oral diet and speech outcome were good in all cases.</p><p class="abstract"><strong>Conclusions:</strong> The FFOCF, in reconstruction surgery of advanced oral cavity carcinoma permits more radical resection of tumour with good functional as well aesthetic outcome.</p>

To date, there has been an increase in the scientific interest in the state of soft tissues surrounding dental implants and their influence on the long-term prognosis of implant treatment. It is known, that the risk factors for the development of periimplantitis include a deficiency or complete absence of an attached keratinized gingiva in the area of implants. The article provides a comparative analysis of various methods of mucogingival surgery in the field of dental implants using free gingival autografts and xenogenic dermal matrices.

In the present article the general and specific cases of hemostasis, in relation to reconstructive surgery of oral cavity, development of which helps to expand the recommendation of dental implants treatment, were systemized and presented for discussion. Secondary healing is the integral condition of successful recovery of organotypical bones and soft tissues volumes in the process of reconstructive surgery. In case of unsatisfactory intraoperative hemostasis the emergence of hematoma of pathological hemorrhagic substratum is possible. It will negatively influence the wound process as secondary healing and, as a result of, will lead to compromising or negative outcome of reconstruction. By this, the relevance of the development of controlled hemostasis methods at reconstructive surgery and dental implantation aiming at increasing clinical and predictive efficiency of the current treatment is determined.

Background This retrospective study was performed to verify the advantages and disadvantages of the free lateral arm flap for defect reconstruction of the forearm and hand. Patients and Methods Between 2001 and 2010, 21 patients underwent defect coverage of the forearm and hand with the free lateral arm flap. The mean patient age was 48 years (17–78). The results concerning defect origin, flap size, pedicle length, operative time, revisions of the anastomoses or other complications, donor site morbidity, and length of hospital stay were evaluated. Results The majority of defects were caused by infections or chronic wounds. The defects were localized at the forearm in 6 cases and at the hand in 15 cases. The flap width ranged from 3 to 8 cm, and the length was from 5 to 20 cm. All flaps survived. Only in one case, a revision of the anastomosis was necessary. Primary closure of the donor site was possible in all patients. No complications occurred during the healing procedure. The majority of the patients were satisfied with the aesthetic result at the recipient site as well as at the donor site. Conclusion The free lateral arm flap is a very reliable option for defect coverage at the forearm and hand for small and medium size defects. A satisfactory aesthetic appearance, an excellent tissue quality, and frequent primary donor site closure are great advantages for selecting this flap.
