Introduction: This study aimed to examine the perception of faculty on the relevance, feasibility and comprehensiveness of the Professionalism Mini Evaluation Exercise (P-MEX) in the assessment of medical professionalism in residency programmes in an Asian postgraduate training centre. Methods: Cross-sectional survey data was collected from faculty in 33 residency programmes. Items were deemed to be relevant to assessment of medical professionalism when at least 80% of the faculty gave a rating of ≥8 on a 0-10 numerical rating scale (0 representing not relevant, 10 representing very relevant). Feedback regarding the feasibility and comprehensiveness of the P-MEX assessment was also collected from the faculty through open-ended questions. Results: In total, 555 faculty from 33 residency programmes participated in the survey. Of the 21 items in the P-MEX, 17 items were deemed to be relevant. For the remaining four items ‘maintained appropriate appearance’, ‘extended his/herself to meet patient needs’, ‘solicited feedback’, and ‘advocated on behalf of a patient’, the percentage of faculty who gave a rating of ≥8 was 78%, 75%, 74%, and 69% respectively. Of the 333 respondents to the open-ended question on feasibility, 34% (n=113) felt that there were too many questions in the P-MEX. Faculty also reported that assessments about ‘collegiality’ and ‘communication with empathy’ were missing in the current P-MEX. Conclusion: The P-MEX is relevant and feasible for assessment of medical professionalism. There may be a need for greater emphasis on the assessment of collegiality and empathetic communication in the P-MEX.

Background: Effective control of immediate post operative abdominal pain following laparoscopic sterilization is challenging. The objective of the study was to estimate the incidence of immediate severe postoperative pain following laparoscopic sterilization under general anaesthesia by the proportion of patients with the pain assessed on a numerical rating scale. Methods: A cross sectional study was conducted with the approval of institutional review board and ethics committee. Fifty seven participants with written informed consent underwent the study over a period of six months. Pain was assessed by a trained recovery nurse and data was collected and analyzed. The main outcome measure was immediate severe post operative pain on numerical rating scale (NRS). Results: Among the 57 participants, 14 (24.6%) had immediate severe post operative pain with median score of five in the inter quartile range of 0 to 5.75 and 43 (75.4%) participants had no severe pain. Conclusion: The incidence of immediate severe postoperative abdominal pain after laparoscopic sterilization under general anaesthesia is high. Therefore, the management of pain following laparoscopic sterilization requires individually based multimodal analgesia.

Background: Cooled Radiofrequency ablation is a newer technique for management of chronic knee pain in osteoarthritis. The aim of the study is to evaluate the clinical outcomes in patients with chronic osteoarthritis in terms of pain scores for first six months of cooled radiofrequency ablation using ultrasound guidance. Methods: A cross-sectional study with retrospective review of database was evaluated to analyze the change in the Numerical Rating Scale from baseline scores at 1 day, 1 month and 6 months after the Cooled Radiofrequency ablation of genicular nerves around knee in patients with chronic knee osteoarthritis. Results: Median age was 71 years [ 61-73 years (IQR: 25-75)] with more female preponderance. Numerical Rating Scale (Mean ± S.D.) was significantly less at 1 day (1.87 ± 1.22), 1 month (3.03 ± 0.99) and 6 months (3.37 ± 1.098) from baseline values (6.77 ± 1.00). No soreness and numbness were noted.Conclusions: Cooled Radiofrequency using Ultrasound guidance for management of knee pain in chronic osteoarthritis is promising and reduces Numerical Rating Score significantly from baseline at 1 month and 6 months respectively.Keywords: Cooled radiofrequency ablation; genicular nerve; numeric rating scale

Objectives: We evaluated the relationship between telecommuting environment and low back pain (LBP) among desk-based workers in Japan. Methods: This cross-sectional study included 3,663 desk-based, telecommuting workers. LBP was assessed using a 0 to 10 numerical rating scale. The telecommuting environment was evaluated using subjective questions. Mixed-effects logistic regression analysis was used. Results: Mixed-effects logistic model results revealed that not having a place or room to concentrate on work, desk not well-lit enough for work, not having enough space on the desk to work, not having enough legroom, and not having comfortable temperature and humidity conditions in the workspace were significantly associated with higher odds of LBP. Conclusions: Our findings suggest that telecommuting environment is associated with the prevalence of LBP.

LATAR BELAKANG : Selama masa pandemi, pemerintah menetapkan Pembatasan Sosial Berskala Besar (PSBB) untuk menurunkan kasus infeksi COVID-19 sehingga intensitas kejadian LBP lebih meningkat daripada sebelum karantina yang mencerminkan efek negatif dari peraturan tersebut. Hal tersebut disebabkan karena adanya perubahan gaya hidup yang signifikan seperti waktu yang dihabiskan untuk duduk meningkat, waktu berolahraga yang berkurang, dan persentase individu melaporkan lebih banyak mengalami stress selama karantina daripada sebelum ditetapkan PSBB. Studi saat ini melaporkan dampak nyeri punggung bawah yang mengakibatkan terganggunya status fungsional penderita disebabkan oleh kebutuhan biaya pengobatan setiap tahun dan keterbatasan kunjungan penderita ke layanan kesehatan, sehingga hal tersebut mengindikasikan penderita nyeri punggung bawah perlu dilakukan evaluasi seberapa besar ketidakmampuan fungsional yang terjadi dan faktor yang mempengaruhinya.   TUJUAN :  Untuk mengetahui hubungan intensitas nyeri dengan status fungsional penderita LBP di Poliklinik Saraf RSUP Dr. Wahidin Sudirohosudo. METODE :  Observasi analitik dengan pendekatan cross sectional  dan teknik pengambilan sampel adalah simple random sampling.  Data menggunakan kuesioner Numerical Rating Scale dan Indeks Barthel. Penelitian dilakukan di RSUP Dr. Wahidin Sudirohusodo selama dua bulan yang dimulai dari Maret 2021 hingga April 2021. HASIL :   Pada hasil analisis data sampel menggunakan uji korelasi kendall's tau-b , didapatkan nilai korelasi (-0,644) dengan nilai kemaknaan hubungan ( p.sig  0,000) menunjukkan korelasi antara intensitas nyeri dengan status fungsional. KESIMPULAN : Intensitas nyeri memiliki hubungan dengan status fungsional penderita Low Back Pain  (LBP) di RSUP Dr. Wahidin Sudirohusodo. KATA KUNCI  : Nyeri Punggung Bawah , NRS, Indeks Barthel

Abstract Background: The present study investigated the effects of different timings of BFR during HIT on muscle hypertrophy, muscle strength, and pain during exercise.Methods: The study included 14 limbs from seven heathy males. Participants were divided into three groups: BFR during exercise (EX); BFR during rest (RE), and training only (CON). Participants performed elbow flexion exercises by BIODEX, two days / week for eight weeks. BFR was set at 120 mmHg pressure. Elbow flexor peak torque, and muscle cross-sectional area were measured, before and after the training period. The pain during exercise were measured during training.Results: The peak torque was significantly increased the RE and CON (p < 0.05), but no increase was observed in the EX. Exercise repetitions the RE (set 1, 26.5 ± 8.7 reps; set 2, 15.2 ± 8.0 reps; set 3, 13.1 ± 4.7 reps; set 4, 13.8 ± 6.3 reps) decreased two sets earlier than the CON (set 1, 28.0 ± 8.6 reps; set 2, 25.6 ± 7.5 reps; set 3, 24.4 ± 8.2 reps; and set 4, 23.8 ± 7.2 reps) (p < 0.01). The numerical rating scale was higher in the EX during exercise and in the RE during rest compared with the CON. However, the RE did not show exacerbated pain during exercise (p < 0.01).Conclusions: This present study showed that BFR during rest can lead to muscle hypertrophy and muscle strength with fewer exercise repetitions than training only. BFR during rest showed the greatest decreases in peak torque during exercise and did not exacerbate pain during exercise.

The repercussions of the COVID-19 pandemic on children’s lives deserve attention. This study aimed to assess the prevalence of anxiety among Brazilian children and its associated factors during social distancing during COVID-19. We used a cross-sectional design with an online survey from April to May 2020 in Brazil. We included children aged 6–12 years and their guardians. The Children’s Anxiety Questionnaire (CAQ; scores 4–12) and the Numerical Rating Scale (NRS; scores 0–10) were used to measure anxiety. We enrolled 157 girls and 132 boys, with a mean age of 8.84 (±2.05) years; 88.9% of respondents were mothers. Based on CAQ ≥ 9, the prevalence of anxiety was 19.4% (n = 56), and higher among children with parents with essential jobs and those who were social distancing without parents. In logistic regression, the following variables were associated with higher CAQ scores: social distancing without parents; more persons living together in home; and education level of guardians. Based on NRS > 7, the prevalence of anxiety was 21.8% (n = 63); however, no associations with NRS scores were found with the investigated variables. These findings suggest the necessity of implementing public health actions targeting these parents and their children at the population level.

Biased pain evaluation due to automated heuristics driven by symptom uncertainty may undermine pain treatment; medical evidence moderators are thought to play a role in such circumstances. We explored, in this cross-sectional survey, the effect of such moderators (e.g., nurse awareness of patients’ pain experience and treatment) on the agreement betweenn=862inpatients’ self-reported pain andn=115nurses’ pain ratings using a numerical rating scale. We assessed the mean of absolute difference, agreement (κ-statistics), and correlation (Spearman rank) of inpatients and nurses’ pain ratings and analyzed congruence categories’ (CCs: underestimation, congruence, and overestimation) proportions and dependence upon pain categories for each medical evidence moderator (χ2analysis). Pain ratings agreement and correlation were limited; the CCs proportions were further modulated by the studied moderators. Medical evidence promoted in nurses overestimation of low and underestimation of high inpatients’ self-reported pain. Knowledge of the negative influence of automated heuristics driven by symptoms uncertainty and medical-evidence moderators on pain evaluation may render pain assessment more accurate.

Gastritis is a progressive disease that evolves from a non-atrophic to atrophic state and progresses through intestinal metaplasia, with some cases leading eventually to gastric cancer. Since gastritis by definition is an inflammatory process of the mucosal lining of the stomach and is usually associated with pain, we aimed to identify any association between the severity of gastritis and pain and a simple inflammatory marker derived from a complete blood count (CBC). This was a prospective cross-sectional study which enrolled 155 consecutive adult patients who underwent an upper endoscopy. Prior to the endoscopy, all patients were given a questionnaire, numerical rating scale (NRS) and complete blood count evaluation. The biopsy was obtained from the gastric mucosa according to the modified Sydney classification and scored with the Operative Link for Gastritis Assessment (OLGA) scoring system. The results showed a significant correlation between NRS and intestinal metaplasia (p < 0.01); moreover, a correlation was also found between the NRS and OLGA stage (r = 0.469, p < 0.001). A nonlinear curve was constructed for OLGA stage estimation according to NRS scores (r2 was found to be 0.442, with p < 0.001). The results also showed a correlation between the neutrophil to the lymphocyte ratio (NLR) and OLGA stage (p < 0.01). No correlation was found between the other gastric parameters and NLR (p > 0.05). Helicobacter pylori positivity did not correlate with NRS and NLR. As a conclusion, pain measured by NRS and NLR, which are simply calculated from the CBC prior to endoscopy, may be used to predict OLGA stages and estimate the severity of gastritis in endoscopy patients.

Mayoritas mahasiswi Fakultas Kedokteran memiliki kualitas tidur yang buruk. Hal tersebut dihubungkan dengan sistem pendidikan di Fakultas Kedokteran yang sangat ketat dan waktu belajar yang tidak sebentar diduga menyebabkan kualitas tidur buruk pada mahasiswi. Dari beberapa penelitian menyatakan kualitas tidur dapat dihubungkan dengan terjadinya patogenesis nyeri, termasuk dismenore primer. Kejadian dismenore primer pada perempuan juga dapat terjadi pada mahasiswi Fakultas Kedokteran Universitas Islam Bandung. Penelitian ini bertujuan mengetahui hubungan antara kualitas tidur dengan angka kejadian dismenore primer pada mahasiswi Fakultas Kedokteran Universitas Islam Bandung. Jenis penelitian adalah analitik observasional dengan pendekatan cross-sectional. Subjek dipilih secara systematic random sampling dan didapatkan 106 sampel. Masing-masing responden mengisi informed concent, kuesioner Pittsburgh Sleep Quality Index (PSQI), dan kuesioner Numerical Rating Scale (NRS). Data dianalisis menggunakan program Epi Info 7. Hasil penelitian 62,3% memiliki kualitas tidur buruk, 49,1% dismenore primer sedang, 10,4% dismenore primer berat. Angka kejadian dismenore sedang pada kelompok subjek kualitas tidur buruk lebih besar dari kualitas tidur baik, secara statistik perbedaan ini sangat bermakna (p=0,008). Angka kejadian dismenore berat pada kelompok subjek kualitas tidur buruk lebih besar dari kualitas tidur baik, secara statistik perbedaan ini bermakna (p=0,04). Terdapat hubungan yang secara statistik bermakna antara kualitas tidur dengan kejadian dismenore primer pada mahasiswi Fakultas Kedokteran Universitas Islam Bandung.
