The interrelation of the severity of the epidemiological situation in a particular country and its institutional characteristics (including the level of health care, quality of management, the level of public trust, cultural characteristics, etc.) is considered. As a result, using the developed index of the severity of the epidemiological situation, the institutional characteristics that most affect the effectiveness of the measures applied were determined. It was figured out that of the 16 characteristics considered, only two (the level of employment and trust in the government) have a medium statistical correlation with the severity of the pandemic. Three more characteristics (prevalence of secular-rational values, degree of urbanization, GDP PPP per capita) have a correlation close to the medium. The remaining characteristics (including health care costs, government efficiency, etc.) have either a weak correlation wiыth severity, or actually do not have it. The results obtained indicate insufficient use and reassessment of existing institutional capacities at the initial stage of the pandemic, as well as insufficient reliability of morbidity and mortality statistics in a number of countries. In conclusion, based on the analysis of statistical indicators, recommendations are given to improve the effectiveness of the use of institutional capacity to counter epidemiological threats, improve this potential, increase the effectiveness of protective and restrictive measures that reduce the severity of the epidemiological situation.

Health care is being capitated throughout the United States, and much of the spread of capitation is attributable to the efforts of insurers to contain costs. The present lack of comprehensive studies evaluating the impact of capitation on overall health care quality leaves vast room for speculation.Capitation does, however, carry a very fundamental certainty with broad implications. Whether the arrangement calls for a fixed sum for treating a particular ailment, a set fee for meeting all of an individual patient’s health care needs, or a standard charge for supplying all the medication for a specific condition, capitation sets limits. Because the health care costs for any given patient or condition are in reality zero-sum rather than fixed, capitation is about pooling patients and rationing. It involves denying services to some patients despite a general contractual commitment to coverage, presumably so that more patients can be covered or receive better care. The implications of capitation are even more significant when set fully in the context of the overall health care reform now underway. Simultaneously and comprehensively, health care is being managed, made for-profit, and consolidated.

When absenteeism threatened to raise health care costs and affect the quality of care at Tillsonburg District Memorial Hospital, a multidisciplinary team came together to develop a strategic response. The hospital seized the opportunity to initiate the “Partners in Health” program, an innovative and collaborative disability management program designed to improve its response to the health issues of all its staff. This paper describes how the program was developed and outlines its comprehensive approach. Partners in Health has proven to be extremely successful at increasing attendance awareness, improving work accommodation and/or rehabilitation after illness or injury, and reducing costs associated with health and lost-time claims. Partners in Health was recognized with the 1995 Health Care Quality Team Award sponsored by the Canadian College of Health Service Executives and 3M.

Contrary to critics' contentions that pharmaceutical promotion and advertising hurts medical care and raises health care costs, the opposite is true. Advertising and promotion improves health care quality by keeping health care providers up to date about the best medicines for preventing, treating and curing diseases. This knowledge reduces the need for more expensive medical care and helps restore good health, which lowers overall health care costs. The article cites a number of instances in which advertising and promotional efforts by pharmaceutical companies have increased awareness of health problems amenable to pharmaceutical intervention, with positive results. Pharmaceutical company support of medical journals and continuing medical education has been an important resource for the dissemination of new medical knowledge. Government regulation of pharmaceutical advertising and promotion is strict. The research-based pharmaceutical industry has adopted guidelines that prohibit many of the activities critics have referred to as abuses. Further government regulation would be unnecessary and unwise.
