The aim: to provide an overview of domestic and international studies examining various aspects of issue of depressive delusional ideas in endogenous delusional depression disease patternMaterials and method: in order to compile a literature review for the keywords depressive delusions and delusional depressions, data from scientific articles posted in MedLine and PubMed databases as well as other bibliographic sources have been searched and analyzed during the formation of scientific psychiatry to the present.Discussion: based on the analysis of scientific publications, this paper presents data on the psychopathological description of depressive delusions and its relatedness to the depressive affect. The researchers’ viewpoints on primary and secondary characteristics of depressive delusions are being analyzed. We have identified pathogenetic characteristics of the latter that affect the prognostic assessment, such as features of the depressive triad in endogenous depression, severity and type of depressive affect, risk of suicidal behavior, characteristics of premorbid personality traits, genetic background, as well as therapeutic efficacy of treatment methods for depressive delusions in patients with delusional depression. This research reveals the controversial nature of some provisions of scientific publications that gradually divert from clinical diagnostic approaches, which tend to be replaced by psychological and psychoanalytic ones when carrying out prognostic assessment in cases of depressive delirium, which is characteristic of current psychiatric science.Conclusions: scientific publications data analysis testifies to the priority of the clinical and psychopathological method in studying the issues of depressive delusions structure in delusional depression disease pattern as well as in clinical differentiation of depressive delusions, justifies its clinical and prognostic value and enables to choose the treatment effectively.

This was a retrospective study in which 50 hospitalized depressed women were rated on 25 items of symptomatology and these items were then subjected to factor analysis. Five factors were extracted and attention was focused on the first factor. Items which loaded heavily on this factor included insomnia, especially in the middle of the night and early morning, severity of depressed mood and global severity of illness, retardation, guilt and self-reproach, weight loss, delusional symptoms, visceral symptoms, agitation, and loss of interest. The symptoms and signs which loaded heavily thus fit the classical pattern of endogenous depression. The patients were scored on this factor and these factor scores were then correlated with the patients' ratings on premorbid personality, personal history, and evidence of apparent precipitant, items which were independent of the factor analysis. The factor scores were found to correlate negatively with presence of apparent precipitants and negatively with hysterical character. The distribution curve of factor scores suggested a distinct group of patients in a hump at the end of the curve. Corresponding factors in other studies were shown to approximately reproduce the items loading on this factor. When applied to our patients these factors produced factor scores which correlated highly with our factor scores and selected the patients in substantially the same distribution. We are currently engaged in investigating the characteristics of our endogenous group of patients to determine how they differ from the remainder of our population. We expect the current studies to lead to a prospective study in which we will take up problems of rater reliability and study a larger patient sample.

Összefoglaló. Bevezetés és célkitűzés: A peritonsillaris tályog a leggyakoribb mély nyaki infekció. Olyan fül-orr-gégészeti kórkép, amely megfelelő kezelés nélkül életveszélyes szövődményekkel járhat. Döntő jelentőségű az empirikus antibiotikumválasztás, melyhez ismerni kell a leggyakoribb kórokozókat és a várható rezisztenciát. Módszerek: A 2012 és 2017 között peritonsillaris tályog miatt kezelt esetek retrospektív feldolgozását végeztük. Összesítettük a sebészi beavatkozás során vett minták aerob és anaerob irányú tenyésztési eredményeit, valamint az empirikusan választott antibiotikumokat. A rutinszerű mikrobiológiai tenyésztés alapján meghatároztuk a leggyakoribb kórokozókat. Az adatokat nemzetközi felmérések eredményeivel hasonlítottuk össze. Eredmények: A vizsgált 6 év során 217 esetben kezeltünk peritonsillaris tályogos beteget. A tenyésztési eredményeket csak 146 esetben tudtuk elemezni. Ebből 47 esetben került sor Fusobacterium species (ebből 25 esetben Fusobacterium necrophorum), 31 esetben Actinomyces species és 29 esetben Streptococcus pyogenes izolálására. Az esetek kétharmadában vegyes aerob/anaerob baktériumflórát izolált a laboratórium. Következtetés: A tályogok kezelésében önmagában a sebészi beavatkozás – az anaerob környezet megszüntetésével – jelentős klinikai javulást eredményez. A jól választott antibiotikum meggyorsíthatja a lefolyást, és csökkentheti az esetleges szövődményeket. Nagy jelentősége van a megfelelő mikrobiológiai mintavételnek, nem vagy nehezen gyógyuló esetekben ez teremtheti meg a célzott antibiotikumterápiára történő váltás lehetőségét. Felmérésünk alapján a peritonsillaris tályogok jelentős részét vegyes baktériumflóra okozza, így a szájüregi anaerob baktériumokra is ható amoxicillin–klavulánsav vagy antibiotikum kombinációjának (2. vagy 3. generációs cefalosporinok kombinálva klindamicinnel vagy metronidazollal) alkalmazása javasolt mint empirikus antibiotikumterápia. Orv Hetil. 2020; 161(44): 1877–1883. Summary. Introduction and objective: Peritonsillar abscess is the most common deep neck infection. Without adequate treatment, this otolaryngological disease pattern can cause life-threatening complications. The empirical choice of antibiotics is crucial which requires knowledge of the most common pathogens and the potential resistance. Methods: A retrospective analysis of cases treated for peritonsillar abscess was performed between 2012 and 2017. We summarized the aerobic and anaerobic culture results of the surgical samples and the empirically selected antibiotics. The most common pathogens were determined via routine microbiological culture tests. We compared our data with the results of international studies. Results: During the 6-year study at our Clinic, 217 patients with peritonsillar abscess were treated. The microbiological tests were available for analysis in only 146 cases. In 47 cases, Fusobacterium species (including 25 cases with Fusobacterium necrophorum), in 31 cases Actinomyces species and in 29 cases Streptococcus pyogenes were isolated. In 2/3 of the patients, polymicrobial infection was detected. Conclusion: In the treatment of peritonsillar abscesses, surgical intervention can result in clinical improvement because of the elimination of the anaerobic milieu. A well-chosen antibiotic can accelerate the healing process and reduce the complication rate. Proper microbiological sampling is of great importance, and in cases of non-recovery or poor recovery, this may create the opportunity to switch for targeted antibiotic therapy. The results of this study show that polymicrobial flora is very important for the development of the peritonsillar abscess, thus the recommended antibiotic therapy is amoxicillin–clavulanic acid or 2nd/3rd generation cefalosporin combined with metronidazol or clindamycin. ABSTRACT The fossil record preserved throughout the National Park Service spans more than a billion years and is documented in at least 267 park units. The discovery, collection, study, and resource management of fossils from localities which are currently within parks sometimes predate the establishment of the National Park Service and many of the parks. Public education and interpretation at parks such as Agate Fossil Beds and Tule Springs Fossil Beds national monuments and many other designated areas include information on the rich history of paleontological field work by notable paleontologists undertaken prior to the areas being preserved as national park areas. Another important historical aspect for several dozen parks involves the conservation efforts undertaken by the public and interest groups to preserve and protect these important fossil localities. The evolution of the science and methodologies in paleontology is reflected in the resource management undertaken by the National Park Service and documented in park resource management records and archives, scientific publications, and agency policy. Today the National Park Service celebrates fossils by coordinating the National Fossil Day partnership which helps to promote the scientific and educational value of fossils.

Under the conditions of EU association, one of the important for Georgia is to create such tax policy that shall be agreeable and settled with the economic systems of developed countries. The efficiency of the tax system depends on the optimal tax policy, according to which highly available and modern standard-oriented business environment is created. In 2017, the Parliament of Georgia passed the reform of profit tax that is aimed to free the business entities from profit tax during the reinvestment process. All the above mentioned maintains the topicality of ongoing reforms in Georgia. The goal of the research is to identify the impact of the new tax system on the Gross Domestic Product, the stream of investments, stimulation of business, and the period, after which the results of the reform will be favourable for the economic. There are various researches and scientific publications on the given issue. According to the study, the reform will produce a long-term macroeconomic effect that will mainly aim to favour small and middle scale businesses. Liberal tax approaches are advancing Georgia’s investment attractiveness, reflecting an increase of investments. This work is based on Estonian experience presented in statistics and international studies. The information processed by experts and researchers helps us to evaluate the impact of this reform on Georgia.

One hundred and fifty consecutive patients with unipolar psychotic (“endogenous”) depression were investigated to illuminate any relationships existing between premorbid personality traits and the development of depressive illness. In some cases personality and psychological stress appeared to combine to produce depression, while in others physical stresses appeared to be the principal etiological agents, and to operate in the absence of personality and psychological factors. The contribution of these different factors to causation are discussed and explored.

We review research literature on psychotic (delusional) depression, including demographic, illness pattern, clinical, biological marker and treatment issues. Secondly, we report a study of a consecutive sample of 137 patients meeting criteria for DSM-III melancholia, RDC definite endogenous depression and our “clinical” criteria for endogenous depression, of whom there were 35 “psychotic depressives” (PDs). The PDs were contrasted with the remaining 76 depressives (EDs) and with an age and sex-matched subset (MEDs). The PDs were distinctly older than the EDs at assessment and at initial onset of any affective disorder. Compared to the MEDs, they tended to have longer illnesses, were more likely to be hospitalised (and to have longer stays), to receive (in the past and for the current episode) combination antipsychotic/antidepressant medication and/or ECT, and to have a poorer course over the following year. They were no more likely to have a bipolar pattern, a family history of depressive disorder, schizophrenia or alcoholism, or vegetative depressive features. Developmental psychosocial stressors and antecedent life event stressors were not over-represented. Most of the PDs had delusions, one-fifth reported hallucinations and psychomotor disturbance was marked. Other differential clinical findings were sustained mood disturbance, constipation, and the absence of a diurnal variation in mood and energy.
