The objective of this paper is to provide a context for strategic use of fluid sampling while drilling (FSWD) in the deep-water environment. Our work is based on data collected from Gulf of Mexico wells over the last 7 years and we incorporate both operator and service company experience. In this paper we review the current FSWD technology and the quality of the fluid samples. We provide practical guidelines for executing the FSWD operation and review types of wells where FSWD has been most effective. We also discuss the role FSWD plays in the business of efficient well construction (drilling, evaluating, and completing). Strategic use of FSWD can provide time savings and operational risk mitigation. FSWD has proven to provide high quality data and fluid samples, however, an awareness of the differences between conventional fluid sampling (wireline) and sampling while-drilling is important for maximizing benefits. Additionally, long term strategic commitment to FSWD is likely to provide the largest benefits to operators. FSWD has been around for about 10 years, but how, and where, to apply the technology has not been clear to many operators. The broader industry can benefit by learning from experiences accumulated through consistent and extensive FSWD use in deep-water wells showing how the technology has progressed, and how it is used to achieve business benefits.

Global AgeWatch Index and Insights by HelpAge International aim to contribute to achievement of long-term transformative change in respect to ageing and the lives of older people by advocating for better production of timely and good quality data to inform policy and program response.The Global AgeWatch Index is a composite index that measures quality of life of older people, and ranks countries based on four domains – income security, health status, enabling environment and capability. The index was developed in partnership with Professor Asghar Zaidi. The index was published during 2013-2015.The Global AgeWatch Insights is a research-based advocacy tool that examines situation of older people in low- and middle-income countries, assesses availability of relevant data and evidence to support the analysis, and identification of policy actions. The Insights are produced in partnership with AARP. The reports were launched in 2018 are planned to be released every three years with a different thematic focus. The first report focuses on the inequities of the health systems in twelve low- and middle-income countries.

Abstract. The hydrological and biogeochemical response of rivers carries information about solute sources, pathways, and transformations in the catchment. We investigate long-term water quality data of eleven Swiss catchments with the objective to discern the influence of catchment characteristics and anthropogenic activities on delivery of solutes in stream water. Magnitude, trends and seasonality of water quality samplings of different solutes are evaluated and compared across catchments. Subsequently, the empirical dependence between concentration and discharge is used to classify different solute behaviors. Although the influence of catchment geology, morphology and size is sometime visible on in-stream solute concentrations, anthropogenic impacts are much more evident. Solute variability is generally smaller than discharge variability. The majority of solutes shows dilution with increasing discharge, especially geogenic species, while sediment-related solutes (e.g. Total Phosphorous and Organic Carbon species) show higher concentrations with increasing discharge. Both natural and anthropogenic factors impact the biogeochemical response of streams and, while the majority of solutes show identifiable behaviors in individual catchments, only a minority of behaviors can be generalized across catchments that exhibit different natural, climatic and anthropogenic features.

We investigate long-term water quality data of 11 Swiss catchments with the objective to discern the influence of major catchment characteristics and anthropic activities on delivery of solutes in stream water. Magnitude, trends, and seasonality of water quality samplings of different solutes are evaluated and compared across catchments. Subsequently, the empirical dependence between concentration and discharge is used to classify the solute behaviors. While the anthropogenic impacts are clearly detectable in the concentration of certain solutes (i.e., Na+, Cl−, NO3, DRP), the influence of single catchment characteristics such as geology (e.g., on Ca2+ and H4SiO4), topography (e.g., on DOC, TOC, and TP), and size (e.g., on DOC and TOC) is only sometimes visible, which is also because of the limited sample size and the spatial heterogeneity within catchments. Solute variability in time is generally smaller than discharge variability and the most significant trends in time are due to temporal variations of anthropogenic rather than natural forcing. The majority of solutes show dilution with increasing discharge, especially geogenic species, while sediment-bonded solutes (e.g., total phosphorous and organic carbon species) show higher concentrations with increasing discharge. Both natural and anthropogenic factors affect the biogeochemical response of streams, and, while the majority of solutes show identifiable behaviors in individual catchments, only a minority of behaviors can be generalized across the 11 catchments that exhibit different natural, climatic, and anthropogenic features.

Many animals use sound for communication, navigation, and foraging, particularly in deep water or at night when light is limited, so describing the soundscape is essential for understanding, protecting, and managing these species and their environments. The nearshore deep-water acoustic environment off the coast of Kona, Hawai’i, is not well documented but is expected to be strongly influenced by anthropogenic activities such as fishing, tourism, and other vessel activity. To characterize the deep-water soundscape in this area we used High-frequency Acoustic Recording Packages (HARPs) to record acoustic data year-round at a 200 or 320 kHz sampling rate. We analyzed data spanning more than 10 years (2007-2018) by producing measurements of frequency-specific energy and using a suite of detectors and classifiers for general and specific sound sources. This provided a time series for sounds coming from biological, anthropogenic and physical sources. The soundscape in this location is dominated by signals generated by humans and odontocete cetaceans (mostly delphinids), generally alternating on a diel cycle. During daylight hours the dominant sound sources are vessels and echosounders, with strong signals ranging from 10 Hz to 80 kHz and above, while during the night the clicks from odontocetes dominate the soundscape in mid-to-high frequencies, generally between 10 and 90 kHz. Winter-resident humpback whales are present seasonally and produce calls in lower frequencies (200-2,000 Hz). Overall, seasonal variability is relatively subtle, which is unsurprising given the tropical latitude and deep-water environment. These results, and particularly the inclusion of sounds from frequencies above 2 kHz, represent the first long-term analysis of a marine soundscape in the North Pacific, and the first assessment of the intense, daily presence of manmade noise at this site. The decadal time series allows us to characterize the dynamic nature of this location, and to begin to identify changes in the soundscape over time. This type of analysis facilitates protection of natural resources and effective management of human activities in an ecologically important area.

Objectives:Determine the relationship between quality of an accountable care organization (ACO) and its long-term reduction in healthcare costs.Methods:We conducted a cost minimization analysis. Using Centers for Medicare and Medicaid cost and quality data, we calculated weighted composite quality scores for each ACO and organization-level cost savings. We used Markov modeling to compute the probability that an ACO transitioned between different quality levels in successive years. Considering a health-systems perspective with costs discounted at 3 percent, we conducted 10,000 Monte Carlo simulations to project long-term cost savings by quality level over a 10-year period. We compared the change in per-member expenditures of Pioneer (early-adopters) ACOs versus Medicare Shared Savings Program (MSSP) ACOs to assess the impact of coordination of care, the main mechanism for cost savings.Results:Overall, Pioneer ACOs saved USD 641.24 per beneficiary and MSSP ACOs saved USD 535.59 per beneficiary. By quality level: (a) high quality organizations saved the most money (Pioneer: USD 459; MSSP: USD 816); (b) medium quality saved some money (Pioneer: USD 222; MSSP: USD 105); and (c) low quality suffered financial losses (Pioneer: USD -40; MSSP: USD -386).Conclusions:Within the existing fee-for-service healthcare model, ACOs are a mechanism for decreasing costs by improving quality of care. Higher quality organizations incorporate greater levels of coordination of care, which is associated with greater cost savings. Pioneer ACOs have the highest level of integration of services; hence, they save the most money.

Abstract. The water quality of rainfall and runoff is described for two catchments of two tributaries of the River Thames, the Pang and Lambourn. Rainfall chemistry is variable and concentrations of most determinands decrease with increasing volume of catch probably due to "wash out" processes. Two rainfall sites have been monitored, one for each catchment. The rainfall site on the Lambourn shows higher chemical concentrations than the one for the Pang which probably reflects higher amounts of local inputs from agricultural activity. Rainfall quality data at a long-term rainfall site on the Pang (UK National Air Quality Archive) shows chemistries similar to that for the Lambourn site, but with some clear differences. Rainfall chemistries show considerable variation on an event-to-event basis. Average water quality concentrations and flow-weighted concentrations as well as fluxes vary across the sites, typically by about 30%. Stream chemistry is much less variable due to the main source of water coming from aquifer sources of high storage. The relationship between rainfall and runoff chemistry at the catchment outlet is described in terms of the relative proportions of atmospheric and within-catchment sources. Remarkably, in view of the quantity of agricultural and sewage inputs to the streams, the catchments appear to be retaining both P and N. With little data available on the quality of stormwater discharging from urban catchments to the Great Barrier Reef (GBR), the driver for the Smart Catchments: Saltwater Creek project was “If we don’t monitor, we can’t manage”. To do this, near real-time high-frequency discharge and water quality data are needed. A second driver was to make available, for the first time in a GBR catchment, near real-time water quality data for Council’s management actions but also for the community from an education perspective to engage them in water quality issues. Working collaboratively with industry, community, educators and scientists, the project, led by Cairns Regional Council, fulfils a commitment to the Reef 2050 Long-Term Sustainability Plan. This paper presents a more holistic and effective approach to monitor and manage water quality runoff from urban catchments.

Abstract Purpose: This article addresses several intervention approaches that aim to improve life for individuals with severe aphasia. Because severe aphasia significantly compromises language, often for the long term, recommended approaches focus on additional domains that affect quality of life. Treatments are discussed that involve increasing participation in personally relevant life situations, enhancing environmental support for communication and participation, and improving communicative confidence. Methods: Interventions that have been suggested in the aphasia literature as particularly appropriate for people with severe aphasia include training in total communication, training of communication partners, and activity specific training. Conclusion: Several intervention approaches can be implemented to enhance life with severe aphasia.

The organizational structure of long-term care (LTC) facilities often removes the rehab department from the interdisciplinary work culture, inhibiting the speech-language pathologist's (SLP's) communication with the facility administration and limiting the SLP's influence when implementing clinical programs. The SLP then is unable to change policy or monitor the actions of the care staff. When the SLP asks staff members to follow protocols not yet accepted by facility policy, staff may be unable to respond due to confusing or conflicting protocol. The SLP needs to involve members of the facility administration in the policy-making process in order to create successful clinical programs. The SLP must overcome communication barriers by understanding the needs of the administration to explain how staff compliance with clinical goals improves quality of care, regulatory compliance, and patient-family satisfaction, and has the potential to enhance revenue for the facility. By taking this approach, the SLP has a greater opportunity to increase safety, independence, and quality of life for patients who otherwise may not receive access to the appropriate services.
