Laparoscopy has been used for decades now in the treatment of various abdominal pathologies in general surgery, urology, gynecology and pediatric surgery. The use of laparoscopy in establishing the diagnosis when other modalities have failed is also well known. The aim of this study was to assess the uses of laparoscopy as a diagnostic tool in various abdominal problems in Mosul City hospitals and its benefits in achieving the accurate diagnosis. Between the year 2009 and 2019, Laparoscopy was used as a diagnostic tool in Mosul City hospitals for 200 patients with various abdominal problems The patients were classified into five categories including abdominal trauma, acute and chronic abdominal and pelvic pain, cirrhosis and hepatitis, malignancies and intra-abdominal testes.

Before 1946, when I completed my training in general surgery, I knew very little about the field that eventually became known as pediatric surgery. I knew that children did not get a fair shake in surgery; that was amply proved during my internship and residency. Surgical patients came from the adult world, and children had a difficult time competing with them. Surgeons in general were frightened of children, and they distrusted the ability of anesthetists to wake children up after putting them to sleep, a position not far from that of many anesthetists. The younger and smaller the patient, the more significant the hazard. I knew, also, that in the United States and in Europe, where some surgery of children was more successfully carried out, it fell usually into one of the specialties, especially orthopedics. In those days there was a need for such specialization in the treatment of diseases that are no longer problems: tuberculosis of the bone, osteomyelitis, and polio. I wish I could say that my knowledge of the sad state of child surgery as I saw it in Philadelphia made me determined to bring about changes for the better. Actually, during the last year of my general surgery training at the Hospital of the University of Pennsylvania, I was invited to become surgeon in chief of the Children's Hospital of Philadelphia. Pediatric surgery was thrust upon me. Nevertheless, I was excited about the chance to make surgery safer for children, and I entered my career with that goal.

Echocardiography is a noninvasive diagnostic tool which has the capacity to provide reliable information on cardiac anatomy and function without risk or discomfort to the patient. The quality and quantity of information obtained in any given study is highly dependent upon the skill of the examiner. The anatomic and physiologic data obtained through its use in the severely ill infant allows immediate separation of infants with serious anatomic cardiovascular defects from those with conditions simulating heart disease. In some cardiac defects the echographic findings are specific enough to allow an accurate diagnosis.

Adding fellows to surgical departments with residency programs can affect resident education. Our specific aim was to evaluate the effect of adding a pediatric surgery (PS) fellow on the number of index PS cases logged by the general surgery (GS) residents. At a single institution with both PS and GS programs, we examined the number of logged cases for the fellows and residents over 10 years [5 years before (Time 1) and 5 years after (Time 2) the addition of a PS fellow]. Additionally, the procedure related relative value units (RVUs) recorded by the faculty were evaluated. The fellows averaged 752 and 703 cases during Times 1 and 2, respectively, decreasing by 49 ( P = 0.2303). The residents averaged 172 and 161 cases annually during Time 1 and Time 2, respectively, decreasing by 11 ( P = 0.7340). The total number of procedure related RVUs was 4627 and 6000 during Times 1 and 2, respectively. The number of cases logged by the PS fellows and GS residents decreased after the addition of a PS fellow; however, the decrease was not significant. Programs can reasonably add an additional PS fellow, but care should be taken especially in programs that are otherwise static in size.

In 2015, the United States Navy hospital ship (USNS) COMFORT, deployed to 11 Caribbean and Latin American countries over a six-month period to provide humanitarian civic assistance. Personnel from the United States Navy and multiple nongovernmental organizations collaborated to offer surgical and medical care. Data from past deployments aid in planning for future missions by prioritizing finite resources and maximizing care. The data analyzed included all patients evaluated and treated by the Directorate of Surgical Services of the USNS COMFORT between April and September 2015. Comparative and descriptive statistics were performed to analyze patient demographics, surgical subspecialty performing the procedures, types of general and pediatric surgical procedures performed, operative times, and complication rates. Of the 1256 surgical cases performed aboard USNS COMFORT during CP15, 24.8 per cent were general surgery cases, followed by 16 per cent ophthalmology, 10.6 per cent pediatric surgery, 10 per cent plastic surgery, and eight additional specialties with <10 per cent of the cases each. Total operative time was 1253 hours with a total room time of 1896.5 hours. The identified complication rate was 1.99 per cent across all specialties. The USNS COMFORT platform offers the unique capability to provide humanitarian surgical assistance. Reporting these data demonstrate that there is a need for humanitarian assistance and this can be provided safely through the Continuing Promise mission. Future deployments may target resources toward the surgical services with higher volumes, which were general surgery, ophthalmology, pediatric surgery, and plastic surgery.

Chronic abdominal and pelvic pain are common debilitating clinical conditions experienced by millions of patients around the globe. The origin of such pain commonly arises from the intestine and bladder, which share common primary roles (the collection, storage, and expulsion of waste). These visceral organs are located in close proximity to one another and also share common innervation from spinal afferent pathways. Chronic abdominal pain, constipation, or diarrhea are primary symptoms for patients with irritable bowel syndrome or inflammatory bowel disease. Chronic pelvic pain and urinary urgency and frequency are primary symptoms experienced by patients with lower urinary tract disorders such as interstitial cystitis/painful bladder syndrome. It is becoming clear that these symptoms and clinical entities do not occur in isolation, with considerable overlap in symptom profiles across patient cohorts. Here we review recent clinical and experimental evidence documenting the existence of “cross-organ sensitization” between the colon and bladder. In such circumstances, colonic inflammation may result in profound changes to the sensory pathways innervating the bladder, resulting in severe bladder dysfunction.
