Abstract An optimist says the glass is half-full, a pessimist half-empty, whereas a good engineer says that the glass is twice as big as it needs to be. There has been much debate over the years about the relative functionality, application and even necessity of proppant in delivering effective hydraulic fractures. Often these debates have been directly linked to major changes in core frac applications, more recently in the dominant North American onshore unconventional market. However, the debates have all too often used broad or unclear brush strokes to describe shifting fracture requirements. Meanwhile, the developing oilfield in the rest of the world resides in more permeable areas of the resource triangle, great care must be taken to ensure that conventional lessons hard learned are not lost, but also that unconventional understanding develops. Over recent years there have been many debates and publications on the relative value of the use of proppant (and associated conductivity), although the true question was about appropriate fracture design in different rock/matrix qualities and environments. Certainly, the vast majority of fracturing engineers appreciate the difference between continuous proppant-pack conductivity and other techniques, such as infinite conductivity, pillar fracturing or duning designs. However, there is increasing evidence that conventional fracturing is suffering from populist attitudes, leading to ineffective fracturing. Additionally, and just as impactful, that unconventional fracturing continues to rely on the lessons learned and physics derived directly from our conventional experience but applying this in an entirely different environment. Primarily, the main concern is with the transfer of recent lessons learned and techniques utilised in one rock quality and environment, to an entirely different scenario, resulting in the misapplication, reduced IP30, poorer NPV or reduced long term EUR and IRR. Examples will be referenced where appropriate proppant selection and frac design can be the difference between success and failure. Fundamentally, we have not sufficiently developed our understanding of the role of proppant and conductivity, for application in unconventionals and thereby rely far too much on our previous conventional thinking. While at the same time we are exporting often inappropriate unconventional populist practice into very conventional environments, thereby potentially achieving the abhorrence of the worst of both worlds. This paper will describe and address scenarios where appropriate engineering selection, rather than popularity-based decision making, has resulted in a successful outcome. It will also attempt to ensure that we show the importance of studying your rock, in anticipation of engineering design, and that this should be a key consideration. The paper will also suggest that as an industry we urgently need to address our approach to consideration of conductivity, placement and importance and ensure that unconventional knowledge and learning progresses with a beneficial outcome for all.

Background: Total shoulder arthroplasty (TSA) has been demonstrated to be an effective treatment for multiple shoulder pathologies. The purpose of our study was to compare the relative value units (RVUs) per minute of surgical time for primary and revision TSA.Methods: The American College of Surgeons National Surgical Quality Improvement Program database was queried to identify patients that underwent primary TSA, one-component revision TSA, and two-component revision TSA between January 1, 2015 and December 31, 2017 using current procedure terminology codes. RVUs were divided by mean operative time for each procedure to determine the amount of revenue generated per minute. Rates were compared between the groups using a one-way analysis of variance with post-hoc Tukey test. Statistical significance was set at p<0.05.Results: When dividing compensation by surgical time, we found that two-component revision generated more compensation per minute compared to primary TSA (0.284±0.114 vs. 0.239±0.278 RVU per minute or $10.25±$4.11 vs. $8.64±$10.05 per minute, respectively; p=0.001). Conclusions: The relative value of revision TSA procedures is weighted to account for the increased technical challenges and time associated with these procedures. This study confirms that reimbursement is higher for revision TSA compared to primary TSA.

Background: This study aims to investigate whether compensation is equitable among the most commonly performed orthopedic hand surgeries and when compared with general orthopedic procedures. Methods: The National Surgical Quality Improvement Program database was queried for all orthopedic procedures, from 2016 to 2018, performed more than 150 times using Current Procedural Terminology (CPT) codes. Physician work relative value unit (wRVU) data were obtained from the 2020 US Centers for Medicare and Medicaid Services fee schedule. Linear regressions were used to determine whether there was an association among wRVU, operative time, and wRVU per hour (wRVU/h). Reimbursement for hand surgery CPT codes was compared with that of nonhand orthopedic CPT codes. The CPT codes were stratified into quartile cohorts based on mean operative time, major complication rate, mortality rate, American Society of Anesthesiologists class, reoperation rate, and readmission rate. Student t tests were used to compare wRVU/h between cohorts. Results: Forty-two hand CPT codes were identified from 214 orthopedic CPT codes, accounting for 32 333 hand procedures. The median wRVU/h was significantly lower for procedures in the longest operative time quartile compared with the shortest operative time quartile ( P < .001). Compared with hand procedures, nonhand procedures were found to have significantly higher mean operative time ( P < .001), mean complication rate ( P < .001), mean wRVU ( P = .001), and mean wRVU/h ( P = .007). Conclusions: The 2020 Physician wRVU scale does not allocate proportional wRVUs to orthopedic hand procedures with longer mean operative times. There is a decrease in mean reimbursement rate for hand procedures with longer mean operative time. When compared with general orthopedic procedures, hand procedures have a lower mean wRVU/h and complication rate.

The article presents the results of a study of the integration of the agricultural holding Rusagro, based on the use of a matrix of dependence of the company's profitability indicators on its relative value in the market. Approbation of this method allows to reasonably make management decisions regarding the development of an agricultural holding.

Fixed closure (FC) is a standard fisheries management tool for protecting sensitive species or species requiring conservation. However, an FC might not effectively manage migratory species because of the large uncertainties of their migration. Adaptive real-time closure (ARTC) is a tool that updates closure areas according to the latest information. This study aimed to evaluate the effectiveness of ARTC to conserve sensitive species for data-limited fisheries by a series of simulations using migration models with hotspots. In the single species simulation, the conservation ratio for the sensitive species in FC varies widely at greater migration uncertainty. In ARTC, a longer duration of a hotspot resulted in a higher conservation ratio. When the mean duration of hotspots was medium or long, the conservation ratio for the sensitive species was more than 50 % in more than 99 % of the simulation trials. In multispecies fisheries, a clear trade-off was observed between the conservation ratio of sensitive species and other species. ARTC was more effective than whole closure when the proportion of sensitive species was high or without closure when the proportion was low. Conditions in which ARTC was most appropriate were described for hotspot duration, increased numbers of individuals in a hotspot, and the relative value of conservation, representing the ratio of the value of conserving sensitive species to one of catching other species.

The article is devoted to the problem of implementation of intelligent control systems in transport. An important task is to assess the information parameters of the control systems. In the existing works the question of definition of one of the basic parameters of functioning of locomotive control systems - information value of separate signs of a train situation is not considered. This does not make it possible to determine the order of signal processing at the input and assess their contribution to the adoption of a control decision. Moreover, informativeness is a relative value, which is expressed in the different information value of a particular feature for the classification of different train situations. Also, the informativeness of the feature may depend on the type of decisive rules in the classification procedure. The quality of recognition of a train situation in which the locomotive crew is, depends on the quality of the features used by the classification system. The decisive criterion for the informativeness of the features in the problem of pattern recognition is the magnitude of losses from errors. To determine the range of the most informative features of train situations, the method of random search with adaptation was used. The results of the work make it possible to optimize the operation of automated and intelligent train control systems by reducing the amount of calculations and simplifying their algorithm.

Aim. To improve the effectiveness of tumors and risk factors detection in reducing time costs at the pre-medical stage using a robotic complex. Methods. To achieve this goal, the previously created robotic complex with a program for self-questioning, taking into account gender, has been improved. A portable USB microscope was built into the complexs housing to transmit images of pathological changes of visible localizations to the touch screen. The survey and examinations were conducted by clinical residents and students of the case-control type in 1638 patients at the Novgorod Regional Clinical Oncological Dispensary (in 20192021) as part of the Doors Open Days. The average age of patients was 54.68.9 years, including 703 men and 935 women. Descriptive statistics methods of Statistica 6.0 software were used. Qualitative characteristics showed as absolute number and the relative value in percentage (%). The differences were considered statistically significant at p 0.05. Results. The use of the complex allowed us to suspect cancer risk factors in 9.031.3% patients, of which 31.081.8%, or 2.82.1% (p=0.017) of the total patients with the further examination was able to confirm oncology-related findings. Patients complained of discomfort in the stomach (59 people) underwent a breath test built into the program of the complex, according to the results of which helicobacteriosis was detected in 29 patients. In 17 patients with fibrogastroscopy, intestinal metaplasia was subsequently established, in 4 gastric ulcers, 3 of which had malignancy. Conclusion. The use of a robotic complex-2 saves the time of a doctor of a lean polyclinic and improves the effectiveness of identifying cancer risk factors and early-stage cancer.

The paper is concerned with the issues of the voluntary land consolidation using the exchange method at land relations reforming. The objective of the article is the substantiation of the exchange method aiming at the large and small land owners (land users) land tenure optimization and the formation of approaches with the help of which the mutual alignment of their interests can be achieved. Land plot ownership and tenure rights exchange by the relative value has been substantiated. It is suggested to define the relative value based on the principle of juxtaposition of land plots by their qualitative, spatial and technological characteristics. The land plots exchange modelling has been improved according to the relative value application and due to the existing land plots boundaries retaining at the exchange. The effectiveness of the suggested land plots exchange method has been demonstrated at the land tenure optimization of an agricultural enterprise and some households in Kyiv Region. Alternative land consolidation projects based on the land plots exchange have been developed, the results of which demonstrate the advantages of the voluntary land consolidation based on the suggested methodology.

This article develops the idea that clinical depression can be seen as a typical human response, largely rooted in human culture, to events of loss or times of adversity. Various biological, psychological, and social factors may cause some individuals to have a depressive reaction that is ineffectually limited in time and/or severity. Recovery occurs mainly based on natural resilience mechanisms, which come into play spontaneously, but which are sometimes inhibited or blocked by specific pathological biopsychosocial mechanisms. One of the mechanisms for this could be the influence of the circuits that regulate pleasure and happiness, along the dorsal diencephalic connection (DDC) pathway from the forebrain to the midbrain via the habenula. Therapy works by undermining the biopsychosocial factors that prevent the natural recovery mechanism from working. Treatment should, therefore, be seen as facilitating rather than causing natural recovery. This approach is in line with the high recovery rate after placebo treatments and the positive influence of pharmacological treatments with completely different sites of action. Acceptance of this model means that when studying new treatments for depression, a new paradigm must be applied in which the relative value of antidepressant treatment is specifically weighted in terms of enabling the natural resilience process.

AbstractVoice pitch carries linguistic as well as non-linguistic information. Previous studies have described cortical tracking of voice pitch in clean speech, with responses reflecting both pitch strength and pitch value. However, pitch is also a powerful cue for auditory stream segregation, especially when competing streams have pitch differing in fundamental frequency, as is the case when multiple speakers talk simultaneously. We therefore investigated how cortical speech pitch tracking is affected in the presence of a second, task-irrelevant speaker. We analyzed human magnetoencephalography (MEG) responses to continuous narrative speech, presented either as a single talker in a quiet background, or as a two-talker mixture of a male and a female speaker. In clean speech, voice pitch was associated with a right-dominant response, peaking at a latency of around 100 ms, consistent with previous EEG and ECoG results. The response tracked both the presence of pitch as well as the relative value of the speaker’s fundamental frequency. In the two-talker mixture, pitch of the attended speaker was tracked bilaterally, regardless of whether or not there was simultaneously present pitch in the speech of the irrelevant speaker. Pitch tracking for the irrelevant speaker was reduced: only the right hemisphere still significantly tracked pitch of the unattended speaker, and only during intervals in which no pitch was present in the attended talker’s speech. Taken together, these results suggest that pitch-based segregation of multiple speakers, at least as measured by macroscopic cortical tracking, is not entirely automatic but strongly dependent on selective attention.
