The diagnosis and classification of salivary gland tumours is complicated by the wide variety of histological types that exist. Many authors attribute this complexity to the myoepithelial component of these tumours. The objective of this study is to evaluate the histological and immunohistochemical properties of a parotid gland myoepithelioma, in order to further our understanding of the differential diagnosis of salivary gland tumours which contain myoepitheliocytes. Histological specimens were analyzed using haematoxylin and eosin (H&E), periodic acid Schiff (PAS), Cason, Alcian blue, toluidine blue, a-SMA, p63 and ki67. The tumour examined was completely encapsulated, with solid cellular regions delimitated by a stroma. The stroma consisted of wide acidophilic and PAS-positive hyaline septae with areas of metachromasia. The tumour cells contained clear cytoplasm and round nuclei with lax chromatin, although some had more elongated nuclei and occasional dense chromatin. Neither cellular atypia nor mitotic figures were observed. Immunostaining was positive for a-SMA and p63, while it was negative for ki67. The histological characteristics of the tumour analyzed were consistent with a benign myoepithelioma, a rare tumour which represents less than 1% of salivary gland neoplasias. Immunostaining confirmed the morphological diagnosis of myoepithelioma. The absence of cytological changes and mitosis and its encapsulation differentiate it from its malignant counterpart. In comparison to pleomorphic adenoma, the myoepithelioma does not demonstrate ductal differentiation or chondromyxoid stroma. Importantly, the epithelial-myoepithelial carcinoma does develop tubular structures not seen in myoepithelioma. p63, which may act as an oncogene, is expressed within the nuclei of myoepitheliocytes of normal salivary glands. Its expression is retained in tumour myoepitheliocytes and thus it may play a role in oncogenesis.

Parotid glands were obtained from five adult (four male and one female) armadillos (Dasypus novemcinctus) which were perfusion-fixed. The glands were located in a position similar to that of most mammals. They extended interiorly to the anterior portion of the submandibular gland.In the light microscope, it was noted that the acini were relatively small and stained strongly positive with the periodic acid-Schiff (PAS) and alcian blue techniques, confirming the earlier results of Shackleford (1). Based on these qualities and other structural criteria, these cells have been classified as seromucous (2). The duct system was well developed. There were numerous intercalated ducts and intralobular striated ducts. The striated duct cells contained large amounts of PAS-positive substance.Thin sections revealed that the acinar cells were pyramidal in shape and contained a basally placed, slightly flattened nucleus (Fig. 1). The rough endoplasmic reticulum was also at the base of the cell.

Salivary gland tumours are uncommon and account for just 6% of all head and neck neoplasms. Worldwide incidence varies, from 0.4 to 13.5 cases per 100 000 population. The parotid gland is by far the most commonly affected site, accounting for 80% of cases. The vast majority of these tumours are benign; only approximately 20–25% being malignant. This article considers the relevant clinical anatomy of the parotid gland, key aspects of assessment with history and examination, and when to refer to secondary care for further investigation. It will touch on the common benign and malignant parotid neoplasms and give an overview of secondary care management.

AbstractThe ultrastructural, X-ray microanalytical, histochemical and immunocytochemi-cal features of intraluminal crystalloids found in adenocarcinomas of the parotid gland have been studied. The crystalloids, putatively derived from an abnormal crystalization of salivary duct proteins, are considerably different from the crystalloids found in normal parotid glands, pleomorphic adenomas, and sialocysts.

Thirty tumors including 27 distinctive cutaneous neoplasms and 3 metastatic tumors from 26 dogs were collected from diagnostic submissions to 3 laboratories. Characteristic histopathologic features included location in the subcutis or dermis (or both); lobular, nodular, and nest-like architecture; and a component of epithelioid cells with clear cytoplasm. Additional features present in most cases included follicular dermal papilla-like structures, low mitotic index, nuclear pleomorphism, necrosis, and mineralization. Cytoplasmic periodic acid Schiff—positivity, which was abolished by pretreatment with diastase, indicated the presence of glycogen in all cases. The oil red O stain did not demonstrate cytoplasmic lipid. Melanin granules, accentuated by the Fontana-Masson method, were observed infrequently. A sparsely cellular mucinous stroma and stromal cartilaginous differentiation were uncommon. By immunohistochemistry, neoplastic cells stained positively for cytokeratin (29 of 29), vimentin (28 of 28), S-100 protein (24 of 29), and melan A (8 of 12); results were negative for smooth muscle actin and calponin in all cases. Clinical follow-up information was obtained on all 26 dogs. One tumor recurred, 1 metastasized to a regional lymph node, and 1 metastasized to regional lymph nodes twice. In another case, possible pulmonary metastasis was noted radiographically. The findings are consistent with a poorly differentiated, low-grade, adnexal carcinoma of the skin. Similar canine cutaneous neoplasms have been reported as “clear-cell hidradenocarcinoma” and “follicular stem cell carcinoma.” The authors propose the designation “cutaneous clear cell adnexal carcinoma.”

Salivary gland tumours make an important part of oral & maxillofacial pathology. Only few studies have been done in Pakistani population. The aim of this study was to describe morphological types of salivary gland tumours diagnosed at King Edward Medical College/ Mayo Hospital, Lahore during the years 1999-2001 and to compare their demographic data with those previously published. Material & Methods: This descriptive cross-sectional study was carried out at King Edward Medical College/ Mayo Hospital, Lahore. It reports 117 cases of salivary gland tumours diagnosed at Pathology Department during 1999-2001. Results: Of the 128 specimens of salivary glands, 117(91.4%) were confirmed as salivary neoplasms. Out of them, 62.7% were benign and 37.6% malignant and a slight female predominance (58.1%) was found. The most common location was the parotid gland (65.8%) followed by minor salivary glands (19.6%). Majority oft he t tumours was diagnosed during 3rd to 5 decades of life. Median age for benign tumours was 33 years (range 1-78) and a female predominance (58.9%) was seen again. Median age for malignant neoplasms was 45 years (range 9-70) with a female predilection ( 56.8%). However, 4 out o f 5 patients with Warthin`s tumour were men. Pleomorphic adenoma was the most frequent tumour (51.3%), followed by mucoepidermoid carcinoma (25.6%), adenoid cystic carcinoma (7.7)), Warthin`s tumour (4.3%) and monomorphic adenoma (2.6%). Two cases each of oncytoma & adenocarcinoma were recorded. Rare categories (single case each) of salivary tumours included lipoma, acinic cell carcinoma, basal cell adenoma, capillary haemangioma, metastatic carcinoma and non Hodgkin`s lymphoma. Conclusion: The principal site of salivary tumours was the parotid gland and females were most affected. Pleomorphic adenoma was the most frequent finding. The results of this study are comparable with other studies.

AbstractThe synchronous or metachronous occurrence of two tumours of the salivary glands in one patient is rare. These are mainly benign and of the same histological type. Here we report a 56-year-old man who developed a mucoepidermoid tumour of the left parotid gland four years after diagnosis of adenoid cystic carcinoma of the right submandibular gland. This combination of neoplasms has not to our knowledge been reported before.

We report a rare case of bilateral basal cell adenocarcinoma (BcAC) of the parotid gland in a male patient 30 years after kidney transplantation and continuous administration of immunosuppressive therapy. BcAC is a salivary gland malignancy first recognized as a distinct neoplastic entity in WHO classification of salivary gland tumours in 1991. Over 90% of BcACs are detected in the parotid gland. The most important differential diagnosis is basal cell adenoma. Infiltrative growth is the distinguishing feature of BcAC. Administration of immunosuppressive medication to this patient for three decades may have contributed to development of this rare neoplasia. To our knowledge, similar cases of BcAC have not been reported previously.

AbstractEpithelial salivary gland tumours presenting in the first month of life (perinatal tumours) are exclusively of major salivary gland origin and predominantly of the parotid gland. Approximately 25 per cent of these rare tumors have been histologically and/or biologically malignant. Except for an occasional pleomorphic and monomorphic adenoma, the majority of the tumors are proliferations suggesting a tumorous arrest of maturation of the salivary gland anlage, hence their designation as embryomas. A case report of an embryoma of the parotid is presented, accompanied by a review of the literature.

ABSTRACT Mucoepidermoid carcinoma ex pleomorphic adenoma (MCxPA) is a rare salivary gland tumor predominantly found in major salivary glands. A case of MCxPA involving the soft tissue and bone of the retromolar region of a 26-year-old man is presented. The histopathological features revealed a neoplasm with predominance of pleomorphic adenoma (PA) elements, and presence of mucoepidermoid carcinoma malignant epithelial cells in several areas. Histochemical and immunohistochemical studies were positive for periodic acid Schiff, alcian blue, cytokeratins 7, 13, 14, and 19, Bcl-2, c-erbB-2, FGF-2 and maspin in the malignant areas. The patient underwent a partial resection of the left side of the mandible with neck dissection and MCxPA diagnosis was confirmed.
