COMPARATIVE ASSESSMENT OF EFFICACY OF TADALAFIL AND TAMSULOSIN ALONE AND IN COMBINATION AMONG MEN WITH BENIGN PROSTATIC HYPERPLASIA

Background: Men in 5th and 6th decade of life experience a fall in the quality of life owing to Lower Urinary tract symptoms arising out of benign prostatic hypertrophy. The medical management protocol involves either monotherapy or combination. The present study was done to comparatively evaluate the efficacy of monotherapy as well as combination therapy in treatment of such patients using two drugs, namely Tadalafil and Tamsulosin. Methodology: The study comprised of 90 subjects divided equally in three groups. Group A and B were subjected to monotherapy using Tadalafil and Tamsulosin respectively, while Group C was provided a combination. Observations: There was statistically significant difference in subjects having Tamsulosin alone or in combination therapy as compared to subjects given Tadalafil alone. Conclusion: Tamsulosin alone or in combination therapy has shown a better outcome in the selected study subjects. Keywords: BPH, LUTS, Tamsulosin, Tadalafil

Introduction: Benign prostatic hyperplasia (BPH) is the most common benign tumour in men. About 50% of men at the age of 50 years develop BPH and about half of them report to the doctors with some form of lower urinary tract symptoms (LUTS). Objectives: To see the therapeutic effect of Tamsulosin on alternate day administration in comparison to daily dose for men who had been suffering from LUTS due to BPH. Materials and Methods: This clinical trial was carried out at Combined Military Hospital, Dhaka in a consecutive fashion for about 18 months. A total of 152 patients were selected from outpatient department and after assessing them through inclusion and exclusion criteria and grouped randomly in three groups Group-A (50 patients), Group-B (50 patients) and Group C (52 patients); all the patients were treated with Tablet Tamsulosin (0.4mg) daily for first three months. Then next 15 months they were treated as; Group-A received Tablet Tamsulosin (0.4mg) daily, Group-B received Tablet Tamsulosin (0.4mg) every alternate day and Group C received placebo Tablet (Folic acid 5 mg) daily. Data were collected at the beginning and end of 3rd month, 4th month, 7th month and 18th month of study, in the form of international prostate symptom score (IPSS), maximum urinary flow per second (Qmax) and post-voidal residue (PVR). Results: This study showed that at the initial three month, IPSS score declined in all three groups, Qmax improved and PVR reduced in all groups. In next phase of study; in Group-A patient there was progressive and significant improvement of all parameters (IPSS, Qmax and PVR) at the end of study. In Group-B patient also showed similar result at the end of study. In Group-C patient showed statistically significant difference in outcome in relation to Group-A and Group-B i.e. more in IPSS, less in Qmax and increase in PVR. Conclusion: With this study it is clearly evident that tamsulosin (0.4mg) therapy significantly improves symptoms of men suffering for LUTS due to BPH. And alternate day therapy having similar therapeutic outcome in relation to every day therapy. Journal of Armed Forces Medical College Bangladesh Vol.15 (1) 2019: 22-26

Abstract Background This study aims to assess the severity of lower urinary tract symptoms, and to assess predictors of impaired quality of life among Egyptian adults complain of lower urinary tract symptoms. Methods An observational cross-sectional research was done using an online anonymous poll survey. The survey was implemented through sharing on different social media applications. The survey was posted from June 1, 2020, to June 10, 2020. The overall communities of the Egyptian adults who satisfied the incorporation rules and consented to take an interest in the research were incorporated using convenience and snowball collecting methods (188 adults). A semi-structured questionnaire on socio-demographic characteristics and Arabic Urogenital Distress Inventory (UDI-6) and Incontinence Impact Questionnaire (IIQ-7) short forms were used. Results Approximately 220 Egyptian adults shared in the research but only 188 fulfilled inclusion and exclusion criteria. Approximately 92% of them had at least one symptom of lower urinary tract manifestations. Irritative symptoms presented in 65 (85.5%) of males and 102 (91.1%) of females. Stress symptoms presented in 44 (57.9%) of males and 63 (56.2%) of females with higher statistically significant mean of stress symptoms. Obstruction/discomfort symptoms presented in 51 (67.1%) of males, and 77 (68.8%) of females. Seeking help and duration of the urological problem were statistically significant independent positive predictors of UDI-6-total. The Irritative score, obstruction/discomfort score, and duration of the urological problem were statistically significant independent positive predictors of IIQ-7-total. Conclusions Urological problems are common and have an impact on the quality of life in various domains of physical activity, social relationships, travel, and emotional health.

Abstract Background Benign prostatic hyperplasia (BPH) is a common problem in aging males which has a potential impact on patients’ health-related quality of life. In the present prospective study, we evaluated the effect of adding solifenacin to tamsulosin, compared to tamsulosin alone on overactive bladder symptoms scores (OABSS) and patients’ quality of life (QoL) in patients with filling lower urinary tract symptoms due to BPH. Methods Patients included in our study were randomly assigned into 2 groups: group 1 included patients with BPH who received tamsulosin alone and group 2 included patients with BPH who received a combination of tamsulosin and sofinacin. Treatment period was 12 weeks in both groups. Quality of life and overactive bladder symptoms score questionnaires were obtained and compared in both groups before and after treatment. Results No significant differences were found between both groups before treatment. At the end of treatment period, The QoL score for Group 1 patients was significantly greater than the other group (mean rank was 138.98 in tamsulosin group versus 62.02 in the combination group, P-value < 0.01). Similarly, OABSS for tamsulosin only group was significantly higher than combined treatment patients (mean rank was 145.03 in tamsulin group versus 55.98 in the combination group, P-value < 0.01). Conclusion Adding solifenacin to tamsulosin was associated with an improvement of QoL and OABSS in patients with irritative urinary symptoms due to BPH when compared with tamsulosin monotherapy.

Benign prostatic hyperplasia (BPH) is arguably the most common benign disease among men. This disease is often associated with lower urinary tract symptoms (LUTS) in men and significantly decreases the quality of life. Polyphenol consumption reportedly plays an important role in the prevention of many diseases, including BPH. In recent years, in addition to disease prevention, many studies have reported the efficacy and safety of polyphenol treatment against various pathological conditions in vivo and in vitro. Furthermore, numerous studies have also revealed the molecular mechanisms of the antioxidant and anti-inflammatory effects of polyphenols. We believe that an improved understanding of the detailed pharmacological roles of polyphenol-induced activities at a molecular level is important for the prevention and treatment of BPH. Polyphenols are composed of many members, and their biological roles differ. In this review, we first provide information regarding the pathological roles of oxidative stress and inflammation in BPH. Next, the antioxidant and anti-inflammatory effects of polyphenols, including those of flavonoids and non-flavonoids, are discussed. Finally, we talk about the results and limitations of previous clinical trials that have used polyphenols in BPH, with particular focus on their molecular mechanisms of action.

Lower urinary tract symptoms, including urgency, urgency incontinence, pollakiuria and nocturia are common in the elderly people and has significant impact on quality of life. Pharmacological treatment is recommended when conservative management, such as fluid intake normalization or bladder training, does not alleviate symptoms. Antimuscarinics were often used as the I line treatment for OAB, as these drugs block the activity of the muscarinic M2/M3 receptors in the bladder and provide a significant clinical benefit for OAB symptoms in the elderly people, but their side effects are common, often leading to treatment discontinuation. Mirabegron, a β3-adrenoceptor agonist, shows similar efficacy to antimuscarinic drugs without the risk of anticholinergic effects.
