Stroke menjadi permasalahan yang saat ini terus mengalami peningkatan di seluruh dunia termasuk Indonesia. Penanganan pasien stroke membutuhkan perawatan jangka panjang yang komprehesif melibatkan tenaga kesehatan dan keluarga dalam perawatan sehari-hari. Tinjauan sistematis untuk menganalisis intervensi perawatan pasien stroke selama dirumah dalam meningkatkan kualitas hidupnya. Pencarian artikel dilakukan pada data base elektronik diantaranya Science Direct, Pro-Quest, dan EbscoHost. Kata Kunci yang digunakan adalah stroke patients AND home care OR home rehabilitation AND quality of life. Kriteria artikel dipilih 1)diterbitkan pada tahun 2012-2020 2)full-text 3)artikel dalam Bahasa inggris 4) penelitian RCT atau experimental study. Artikel diseleksi sesuai kriteria dan didapatkan total 5 dari 705 artikel menggunakan PRISMA. Tinjauan sistematis ini menemukan bahwa ada konsistensi temuan di antara penelitian yang menunjukkan bahwa perawatan pasien stroke di rumah adalah pilihan yang efektif dalam pemulihan pasien dan berlangsung dengan optimal selama dirumah. Dukungan kualitas hidup pasien stroke membutuhkan perawatan lanjutan maka dari itu diperlukan peran dari keluarga yang mampu merawat serta menjaga pasien dengan baik sehingga pemulihan pasien dapat berlangsung dengan optimal selama dirumah.

Context: Stroke is one of the main causes of premature death and disability, imposing significant costs on the healthcare system, especially due to expensive hospital care. Home care service is one of the interventions used in the last two decades to reduce the cost of services provided for stroke patients in different countries. Objectives: The present study aimed to systematically review studies related to the economic evaluation of home care compared to hospital care for stroke patients. Data Sources: A search was conducted between January 1990 and January 2021. PubMed, Scopus, Web of Science, and Embase databases were searched systematically. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines were used to select the studies. Data Extraction: To evaluate the quality of studies included in this systematic review, Drummond’s ten-item checklist was used. Results: Five economic evaluation studies were included in this review. The included studies reported different results regarding the effect of home care on improving different indicators and the cost-effectiveness ratio of home care to hospital care. Most previous studies reported that home care is a more cost-effective option for improving many indicators, such as physical function and quality-adjusted life years (QALY), and for reducing mortality and institutionalization, compared to hospital care. Conclusions: Home care is a more cost-effective option than hospital care for stroke patients with regard to some indicators, such as the Barthel index for Activities of Daily Living, Modified Rankin Scale (mRS), quality of life, mortality, and institutionalization. However, there are some exemptions to this conclusion. Due to limitations, such as heterogeneity of interventions in the existing studies, different levels of patients’ disabilities, different perspectives toward economic evaluation, and differences in the healthcare systems of countries, further research is needed according to the context of each country based on clinical trials.

Background: Allergic rhinitis is a common disease that affect nose causing sneezing, watery nose, nasal itching and redness that affect quality of life, productivity at work or school and may underlies complications (e.g. Asthma) for patients and are often accompanied by itchy eye, redness and lacrimation. Aim of the Work: The objective of this study is to systematically assess the efficacy and safety of immunotherapy treatment for patients with Allergic Rhinitis. Method: Our initial search generated a total of 23330 possible relevant titles. Titles, abstracts were preliminary screening so that 22565 were excluded. 154 articles were retrieved in full text the number of studies excluded after assessment of the full text 145, 9 articles met the eligibility criteria and fulfilled the inclusion and exclusion criteria for the review. Data Sources: Medline databases (PubMed, Medscape, ScienceDirect. EMF-Portal) and all materials available on the Internet upto 2018. Data Extraction: If the studies did not fulfill the inclusion criteria, they were excluded. Study quality assessment included whether ethical approval was gained, eligibility criteria specified, appropriate controls, and adequate information and defined assessment measures. Conclusion: Our systematic review provides evidence that Sublingual Immunotherapy (SLIT) tablets effectively relieve rhinitis symptoms in adults with allergic rhinitis, improve their quality of life and provide data about safety of Sublingual Immunotherapy as there were no serious side effects of using SLIT tablets. Nevertheless, the current evidence may be limited due to sample size and the heterogeneity between studies. Large sample size and multiple center RCTs on the efficacy of different formulations of SLIT drugs are still needed to provide further evidence and more precise recommendations.

BACKGROUND Pediatric medical conditions have the potential to result in challenging psychological symptoms (eg, anxiety, depression, and posttraumatic stress symptoms [PTSS]) and impaired health-related quality of life in youth. Thus, effective and accessible interventions are needed to prevent and treat psychological sequelae associated with pediatric medical conditions. Electronic health (eHealth) interventions may help to meet this need, with the capacity to reach more children and families than in-person interventions. Many of these interventions are in their infancy, and we do not yet know what key components contribute to successful eHealth interventions. OBJECTIVE The primary objective of this study was to conduct a systematic review to summarize current evidence on the efficacy of eHealth interventions designed to prevent or treat psychological sequelae in youth with medical conditions. METHODS MEDLINE (PubMed) and PsycINFO databases were searched for studies published between January 1, 1998, and March 1, 2019, using predefined search terms. A total of 2 authors independently reviewed titles and abstracts of search results to determine which studies were eligible for full-text review. Reference lists of studies meeting eligibility criteria were reviewed. If the title of a reference suggested that it might be relevant for this review, the full manuscript was reviewed for inclusion. Inclusion criteria required that eligible studies (1) had conducted empirical research on the efficacy of a Web-based intervention for youth with a medical condition, (2) had included a randomized trial as part of the study method, (3) had assessed the outcomes of psychological sequelae (ie, PTSS, anxiety, depression, internalizing symptoms, or quality of life) in youth (aged 0-18 years), their caregivers, or both, (4) had included assessments at 2 or more time points, and (5) were available in English language. RESULTS A total of 1512 studies were reviewed for inclusion based on their title and abstracts; 39 articles qualified for full-text review. Moreover, 22 studies met inclusion criteria for the systematic review. Of the 22 included studies, 13 reported results indicating that eHealth interventions significantly improved at least one component of psychological sequelae in participants. Common characteristics among interventions that showed an effect included content on problem solving, education, communication, and behavior management. Studies most commonly reported on child and caregiver depression, followed by child PTSS and caregiver anxiety. CONCLUSIONS Previous research is mixed but suggests that eHealth interventions may be helpful in alleviating or preventing problematic psychological sequelae in youth with medical conditions and their caregivers. Additional research is needed to advance understanding of the most powerful intervention components and to determine when and how to best disseminate eHealth interventions, with the goal of extending the current reach of psychological interventions.

Introduction Approximately 26 million people in the United Kingdom are living with one long-term condition and 10 million are living with two or more; these figures are projected to continue increasing (NHS England 2018). People with long-term conditions are two to three times more likely to have poor psychological wellbeing and utilise 50% of GP appointments, 64% of outpatient appointments and over 70 of inpatient bed days. Research in this population could help with increasing constraints on healthcare budgets and resources. Technology-enabled healthcare in the community might help improve quality of life and reduce healthcare costs of managing chronic disease but the overall impact is unclear, we therefore conducted a systematic review. Methods Keywords and MeSH terms were used to search MEDLINE and CINAHL. We included qualitative and quantitative studies that reported on adult home-care patients diagnosed with at least one long-term condition, comparing telehealth to usual home care. Meta-analyses and sensitivity analyses were conducted using RevMan 5. Qualitative findings were thematically synthesised and reported narratively. Results In total, 2568 studies were identified and nine (2611 participants) were included. Telehealth was not statistically significantly different versus standard home care for quality of life, psychological wellbeing, physical function, anxiety, depression, disease specific outcomes or bed days of care at 3, 6, 9 and 12 months. Qualitative findings showed patients found telehealth was beneficial for providing peace of mind and legitimising access to healthcare. Conclusion Telehealth may offer reassurance to those living in the community with long-term conditions; however, a lack of high-quality studies and heterogeneity between interventions makes conclusions difficult.

Abstract Objective Motor impairments after stroke can lead to limitations in functional performance and mental health. Previous systematic reviews and meta-analyses have mainly assessed randomized clinical trials of mental practice interventions (MPI) among stroke patients, focusing on physical outcomes. However, mentally rehearsing a motor movement might increase a patient’s self-confidence within the recovery process and thus also lead to positive effects on psychological outcomes. Therefore, we aim to investigate the effectiveness of MPI on psychological health outcomes. Methods We performed a systematic review of RCTs according to PRISMA guidelines. 16.125 records were identified through PubMed, Web of Science, Science Direct, and Cochrane resulting in 8.573 documents after duplicates were removed. After excluding 8.507 documents during title and abstract screening, 64 could be full-text screened. All clinical and randomized controlled trials designed to investigate the effects of mental practice on psychological health in adult patients after stroke were considered eligible. Results Six studies met the eligibility criteria and were included in the qualitative analysis. The six included studies partly showed positive effects of MPI on psychological health such as quality of life, stroke-specific quality of life, fall-related self-efficacy, positive and negative affect but not on anxiety and depression. Overall, the methodological power is lacking. The included studies differ substantially regarding study design, sample size, intervention protocol and psychological health operationalization. Conclusions The results show inconsistent effects of MPI on psychological health. More systematic research is needed to investigate the effects of MPI on psychological health and give several recommendations for future research.

BACKGROUND Stroke is one of the problems that can lead to either disability or death and this will increase the social and economic burden. OBJECTIVE To analyze the effects of motor relearning program (MRP) in comparison with other treatment technique on quality of life (QoL) among stroke patients DATA SOURCES This systematic review includes Randomized Controlled Trials (RCT) for patients suffering from stroke. The articles were retrieved from Google Scholar, research gate, HEC digital library, ProQuest, Iingenta and PubMed. Articles were also accessed from Journals. STUDY SELECTION Data belonged from 2000 to 2015 were included. RCTs that focus on motor relearning program or its task-oriented activity as rehabilitation program of stroke patients were included in this review. RESULTS Total 12 studies were included in this review with 378 patients. Among them, 191 had received MRP, whereas, 187 had received any other treatment technique for stroke rehabilitation. Analysis of Berg Balance Scale (BBS) and Barthel Index shows that studies favor MRP, while result is slightly insignificant (0.008) with BBS and not significant (0.67) for Barthel Index.
