Introduction: Patient is a 14 years old Chinese boy presented with skeletal Class II with incompetent lips, severe dental Class II malocclusion and severe crowding complicated with both sides mesio-angular impacted of mandibular 2nd molars. Treatment Plan: Growth modification using functional appliance for skeletal Class II correction with maxillary arch expansion and correction of mandibular plane angle by intrusion of maxillary molars. Extraction of lower 1st premolars and 2nd molars on both sides was carried out in conjunction with fixed orthodontic alignment and to facilitate of spontaneous eruption of lower third molar. Midline correction was followed by space closure and finishing and detailing. Results: Alignment of upper arch in the regained space with maxillary expansion without any extraction was done. Anterior maxillary dentitions were retracted using Temporary Skeletal Anchorage Devices (TSADs). In lower arch 1st molars were mesially protracted by using class II elastics. 3rd molars spontaneously erupted in the extraction space of 2nd molar. Midline corrected with good facial profile and competent lips. Conclusions: Timely execution of expert treatment plan can bring cost effective and time saving results with minimum loss. By proper counseling, patient compliance and confidence can be boosted which could have a positive effect on the treatment outcome. Bangladesh Journal of Medical Science Vol.20(3) 2021 p.682-690

Orthodontic treatment of Class II, division 1 malocclusion in one or two phases is still controversial in contemporary Orthodontics. The present clinical case presents the orthodontic treatment of a 6-year-old patient with aesthetic complaint regarding the smile and bullying, presenting skeletal Class II, Class II malocclusion, Angle division 1, overjet with exaggerated protrusion of the maxillary incisors, exaggerated overbite with the lower incisors touching the palatal mucosa, absence of lip sealing and concave inferior face profile. In Phase I, rapid maxillary expansion was performed with Hyrax-type expander together with the Herbst fixed functional orthopedic device for 11 months. In Phase II, the fixed orthodontic appliance associated with intermaxillary elastics was used for 13 months. The two-phase treatment of Class II malocclusion, division 1 showed to be effective after 24 months, establishing adequate occlusal and functional results and improving the aesthetics of the lower third of the face.

A 7-year 10-month-old boy was evaluated for mouth breathing and snoring habits. Examination revealed soft convex tissues, maxillary protrusion, mandibular retrusion, and a class II sagittal osteofascial pattern. The patient failed a water holding test. He was clinically diagnosed with skeletal class II malocclusion caused by mouth breathing. Under interceptive guidance of occlusion (iGo), the malocclusion improved with fixed maxillary expansion using functional appliances and interventional treatment of mouth breathing by lip closure exercises. These treatments enabled the patient to gradually return to nasal breathing and guided him to develop physiological occlusion for a coordinated jaw-to-jaw relation. At the 5-year 2-month post-correction follow-up visit (at the age of 13 years), the patient had stable occlusion, a coordinated osteofascial pattern, and normal dentition, periodontium, and temporomandibular joints.

ABSTRACT Introduction: Class II malocclusion, which has a significant incidence in the population, may compromise facial esthetics and the smile, as well as the masticatory and respiratory functions. Often associated with skeletal abnormalities, it severely affects and compromises quality of life. An accurate diagnosis is fundamental to prepare a treatment plan to correct dental and skeletal anomalies. Objectives: This study discusses treatment alternatives to the correction of Class II division 1 and 2 malocclusion in growing patients, using a Bionator and an extraoral appliance.

Abstract Objectives Class II malocclusion is largely due to a retrognathic mandible. Mandibular rotation is closely related to changes in the occlusal plane during growth. The problems in the occlusal plane could cause disadvantages in the soft tissue profile in Class II malocclusion, presenting treatment challenges for an orthodontist. This study aimed to investigate the importance of the occlusal plane for a better soft tissue profile in Class II malocclusion for Javanese patients. Materials and Methods The total number of cephalogram softcopies of patients with skeletal Class II malocclusion were selected based on ANB values (> 4 degrees), no agenesis teeth except the third molar, and all permanent teeth. The cephalograms were calculated using digital tracing by Morpheus 3D imaging. The points and areas to be analyzed on the cephalogram were predetermined. The examination was performed in a span of 1 month and performed via a statistical test using Pearson’s test and multiple regression analysis (p < 0.05). Results There were significant correlation values between the angles produced by the occlusal plane to sella national, Frankfurt horizontal, mandibular plane, and Z-angle (p < 0.05). Conclusion Patients with skeletal Class II malocclusion have a significant correlation between the occlusal plane and the vertical plane, thereby affecting the shape of the soft tissue profile, which causes a facial imbalance. By improving mandibular movement, the soft tissue profile can also be corrected.

Management of skeletal class II relation in growing patient require careful evaluation of growth status, proper diagnosis and treatment plan to address the patient’s need. Twin block is the appliance of choice among functional appliance to correct the skeletal class II malocclusion due to it’s acceptability by the patient and simplified design. Maxillary canine impaction is the second most prevalent impaction after the third molars. Treatment of maxillary canine impaction is usually a challenge to orthodontic approach as the defect lies in the esthetic region of the jaw. This case report describes the orthodontic management of a 9 years male patient having skeletal Class II malocclusion with horizontally impacted maxillary canine. Some modifications in the treatment mechanics are deemed essential to address the patient’s need for achieving the optimal esthetic and to improve the occlusion. Twin block is beneficial for the treatment of skeletal malocclusion in a growing patient to achieve the maximum benefit. The orientation of impacted teeth may change from horizontal to favorable one, hence it should be considered a viable treatment alternative. In such case, early diagnosis and interception at the right time along with radiographic follow-ups are necessary to achieve successful results. Class II malocclusion has a high incidence in the population, which may compromise smile aesthetics, occlusion function and stability. Skeletal Class II may affect facial aesthetics and upper airway volume. Class II malocclusion is routinely associated with skeletal Class II condition, having as treatment alternatives the use of Extra Buccal Appliance (EBA) or removable or fixed propulsor appliance. This article describes a case of a patient who did not accept the use of EBA and so it was fitted a Flex Developer propulsor for Class II correction and discussed the advantages and disadvantages of therapeutic alternatives for Class II correction.

Analysis of sagittal jaw relationship is important in orthodontic diagnosis and treatment planning, and can be investigated by angular or linear parameters. In the present study an analysis was performed using profile cephalograms of 40 children with skeletal Class II malocclusion. A correlation analysis is presented, involving five different methods of expressing sagittal jaw relationships. Whilst some degree of agreement could be noted, it was not possible to suggest a pair of measurements, which in combination, could give a more accurate picture of sagittal jaw relationships.

Background: A unique clinical challenge presents when dealing with a compromised first permanent molars with bilateral posterior crossbite, severe crowding and impacted maxillary canines with skeletal class II base malocclusion patient. Case presentation: 14-year-old female patient had dental Class II skeletally, complicated with increase overjet, badly destructed permanent mandibular 1st molars constricted maxillary arch. Extraction of 1st molars followed by expansion have been planned to relieve crowding. Extraction of 1st molars in this time (furcation of 3rd molars start to develop) help in replacement by 2nd molars. In the progression of treatment, Conclusion: A well-balanced and esthetic occlusion by edge wise orthodontic treatment has been archived in this case. Adequate planning and early treatment of Angle Class II malocclusion with maxillary atresia and anterior open bite provides harmonization of maxillomandibular bone bases in the three planes of space. Orthodontic aligners have emerged as an alternative treatment having the following advantages: being more aesthetic and more comfortable for the patient, less treatment time when they are correctly indicated, less chairside time, less complications, possibility of remote monitoring, easier feeding, and dental hygiene. On the other hand, they offer difficulties to treat adults with severe skeletal Class II malocclusions, posterior crossbite and anterior open bite. Therefore, the objective of this clinical case report is to early correct skeletal Class II malocclusion with maxillary atresia through the use of mechanical orthopedics and devices that help eliminate habits and close the anterior open bite, and later the use of orthodontic aligners to finish the treatment. It was concluded that the early interceptive treatment of malocclusion was efficient to harmonize the bone bases in the anteroposterior, vertical, and transversal directions, changing this malocclusion from high to low complexity and, consequently, highly predictable and with an excellent prognosis for treatment with orthodontic aligners. The treatment with ClearCorrect aligners corrected the occlusion in a shorter period of time when compared to corrective orthodontics and with a high predictability in relation to the virtual setup.
