Objectives: To assess the ossicular changes in cholesteatoma both in children and in adults. Methods: This was a cross sectional comparative study which was carried out in the departments of Otolaryngology and Head-Neck Surgery of Bangabandhu Sheikh Mujib Medical University and Shaheed Suhrawardy Medical College Hospital during the period of January 2014 to December 2014. A total number of 55 patients having cholesteatoma were included in this study. Patients were divided into two groups according to their age. Age belonged to 18 years and more than 18 years were considered as group I (n=29) and group II (n=26) respectively. All patients were undergone Tympanomastoidectomy and all intraoperative findings were recorded. The two groups were compared with regard to extension of disease and status of ossicular chain. Results: In this study majority of patients were within 11-18 years in children group and within 19-30 years in adults group. Most of the children were male and female predominant in adult group. Extension of disease was significantly higher in children group. Ossicular erosion was also more frequent in children group. Conclusion: Extension of cholesteatoma and ossicular erosion was significantly higher in children group. Bangladesh J Otorhinolaryngol; April 2016; 22(1): 9-14

Objective: To assess the rate of graft take and hearing improvement after myringoplasty. Methods: This cross sectional study was carried out among sixty (60) patients those who were admitted into the Department of Otolaryngology & Head-Neck Surgery, Dhaka Medical College Hospital and BSMMU with their descriptive history, clinical finding’s, pre and postoperative hearing assessment by PTA during the period of June’ 2006 to December’2006. Results: Sixty cases were diagnosed as CSOM (tubo-tympanic) disease and underwent myringoplasty. The lowest and highest age of the patient’s at presentation was 15 and 45 years respectively with a mean age of 27 years. The overall success rate was 80% (48 out of 60). The surgical failure was 20% (12 out of 60) and those were graft failure, re-perforation & anterior blunting at the last visit. Graft failure occurred during the first 8 weeks in 3 cases out of the 12 cases. Conclusion: Myringoplasty is a valid treatment modality for tubo-tympanic type of CSOM. Background: Emergency tracheostomy is the commonest surgical intervention performed as lifesaving procedure by ENT surgeons on patients in emergency room of ENT and ICU. Several operative and postoperative complications have been reported after tracheostomy including fatal hemorrhage. Massive subcutaneous neck emphysema occurred because ventilation started at the time when the hemorrhage was not completely managed and the tracheal tube was not fully secured. Objective: This study was conducted to compare haemorrhage and surgical emphysema as complication of emergency tracheostomy among patients admitted in Department of ENT and Head-Neck Surgery in Dhaka Medical College & Hospital. Methods and Materials: This was a Cross Sectional Study conducted in the ENT and Head- Neck Surgery Department, Dhaka Medical College Hospital from July 2015 to June 2016. A total of 90 patients were included who fulfilled the selection criteria. Non probability convenient sampling was used to collect data. Statistical analysis was done by SPSS version 21. Results: Mean± SD of the study subjects was 49.84±17.4 years with most patients belonged to 40-79 years age group. 28.9% patients had habit of smoking and betel nut chewing. (48.9%) patients suffered from Ca larynx. Out of 90 tracheostomy patients, 40(44.4%) patients had had different complications including haemorrhage (13.3%), surgical emphysema (10%), haemorrhage with surgical emphysema (6.7%), tube blockage (3.3%), wound infection (3.3%) and injury to local tissue (2.2%). Haemorrhage occurs from anterior jugular vein, middle thyroid veins and thyroid gland and included peroperative haemorrhage (7.8%), immediate post- operative haemorrhage, <24 hours, (4.43%) and late post-operative haemorrhage (1.2%). Conclusion: In this study, haemorrhage followed by surgical emphysema and haemorrhage with surgical emphysema were found as major complications following emergency tracheostomy, J Dhaka Medical College, Vol. 28, No.2, October, 2019, Page 159-163

Hypocalcaemic tetany is one the commonest complication after total thyroidectomy. It may cause significant morbidity. Early detection and treatment have better out come. The main objective of the study is to find the incidence of hypcalcaemic tetany in post operative period after total thyroidectomy and average interval period of hypocalcaemia following surgery. This was an observational study conducted in the department of Otolaryngology & head-Neck Surgery Sylhet M.A.G. Osmani Medical College Hospital during 1st January 2006 to 31st December 2007. Pre-operative routine investigation, Thyroid Function test, Ultrasonography thyroid gland and cytological evaluation by FNAC were done in all patients. Ten patient developed hypocalcaemia after surgery. Among them only one suffered from permanent hypocalcaemia. Most of the patient developed symptoms about 48 hours after surgery. The Incidence and time interval of development of hypocalcaemic tetany after total thyroidectomy found in the series fully coincides with the results of other researchers globally.Faridpur Med. Coll. Introduction: This study aimed to compare the clinico-pathological features of the tubotympanic and atticoantral variety of chronic suppurative otitis media (CSOM).Methods: This was a cross sectional observational study conducted in the department of Otolaryngology – Head & Neck Surgery, Bangabandhu Sheikh Mujib Medical University (BSMMU) and Dhaka Medical College Hospital (DMCH), Dhaka, Bangladesh. The study was done over a period of six months and a total of 60 cases were selected. In group A 30 cases of CSOM tubotympanic variety were included while in group B 30 cases of atticoantral variety of CSOM were selected. The two groups were compared with regard to their clinical presentations, the type and degree of hearing loss and their associated complications by taking a detailed history followed by clinical examination and doing the relevant investigations.Results: In group A, the patients presented with a profuse non smelly discharge. All had a central perforation and majority had mild conductive hearing loss. There were no associated complications. In group B, the aural discharge were foul smelling and scanty. The perforations were 66% in the attic while 33.3% had marginal perforations. The hearing loss was mainly conductive in nature but in group B it’s more severe in degree and also there were more associated severe to profound sensori-neural hearing loss. There were also associated extracranial and intracranial complications in group B patients.Conclusion: The atticoantral variety of CSOM is associated with a foul smelling scanty discharge with severe hearing loss and complications than the tubotympanic variety of CSOM. Objectives: To determine the frequency of thyroid malignancy in clinically or zoographically multinodular goiter in patients undergoing thyroidectomy in Cumilla Medical College Hospital, Bangladesh. Methods: A cross sectional study was carried out at the department of Otolaryngology and Head Neck Surgery of Cumilla Medical College Hospital during the period from January 2016 to January 2019. This study includes all patients admitted and undergoing surgery with multinodular goiter. Results: Out of 146 patients 118 patients were female and 28 patients were male with female male ratio is 4.2:1. Out of 146 patients 13 patients were found histologically thyroid malignancy. Among them 8 patients were male and 5 patients were female. Papillary carcinoma was found in all cases.

Objectives: To determine the frequency of thyroid carcinoma in clinically or sonogragraphically multinodular goitre in patients undergoing thyroidectomy Setting: This is a cross sectional study was carried out at the Department of Otolaryngology and Head-Neck Surgery, Bangabandhu Sheikh Mujib Medical University during the period from January 2012 to June 2012. Methods: This study includes all the patients admitted in Otolaryngology and Head-Neck surgery department of Bangabandhu Sheikh Mujib Medical University having diagnosis of multinodular goitre & underwent thyroidectomy. All patients with non-toxic multinodular goitre (clinically or sonographically) irrespective of age , sex was be included . Results: Out of 50 patients, 41 patients (82%) were female, and 9 patients (18%) were male with a female to male ratio 4.5: 1. This shows female preponderance of multinodular goitre. Out of 50 patients, 5 patients had histologicaly proven thyroid malignancy (4 females and 1 male). In this study overall incidence of carcinoma in multinodular goitre was 10% and female to male ratio 4:1 with the incidence in female was 9.75% and in male 11.11. The incidence of malignancy under 21 years was 50% and above 21 years was 8.33%. Regarding the types of malignancy in this series, papillary carcinoma was the most common variety (80%) followed by follicular carcinoma (20%). Setting: Study was conducted in the Department of Otolaryngology and Head-Neck surgery of Bangabandhu Sheikh Mujib Medical University (BSMMU), Bangladesh from 2006 to 2017. Objectives: To find out the frequency, risk factors, and its effect on the management of pharyngocutaneous fistula (PCF). Methods: Observational, Cross Sectional study. Number of laryngectomized patients 249. Results: Total PCF developed 48 (19.27%). Irradiated patients with multiple co-morbidities had more chance to develop PCF. Objective: To observe the incidence, clinical presentation and to perform comparative studyof different sinonasal masses. Study design: Prospective study. Setting: Department ofotolaryngology and Head-Neck Surgery & ENT OPD of Chittagong Medical College Hospital.Patients & methods: 50 patients are included in this study (39 male & 11 female) between theages of 3 years and 80 years who were treated between August 2006 to January 2007. Studybased on history, clinical, radiological, laboratory and histopathological examination. Results:Mean age for male was 35.12 years and for female was 22.63 years. Male to female ratio was3.5:1. Highest frequency was noted in second decade. Most of patient (78%) were from poorclass. Frequency of inflammatory nasal masses were more in second decade, benign tumourin fourth and fifth decade, malignant tumour in second decade (OAN & NHL) and fifth andsecond decades (others). Rhinosporidiosis were most frequent inflammatory nasal masses.Nasal obstruction was the commonest and orbitus symptoms were less frequent symptoms.But orbital symptoms were more prevalent in malignant lesion. Conclusion: sinonasal massesare found in all age group. Rhinosporidiosis are appearing to be the commonest nasal masses.The prevalence of nasal polyp is also high. Otosclerosis is the most frequent cause of conductive hearing loss in patients with intact tympanic membrane. It can be treated by surgery with various techniques. Because of limited manipulation and comparably similar results, stapedotomy at present is preferred over other surgical techniques by most surgeons. Here we present the operative results of 35 otosclerotic patients who underwent stapedotomy between January 2009 - October 2010 in the department of Otolaryngology- Head & Neck Surgery of Bangabandhu Sheikh Mujib Medical University, Dhaka Medical College Hospital and Shahid Suhrawardhy medical college & hospital, Dhaka. The aim of this study was to evaluate the hearing results following stapedotomy in otosclerosis surgery. The average air-bone gap of patients improved significantly to 21.74 dB after operation. Other hearing parameters of patients (including air conduction, bone conduction, speech discrimination score, and tinnitus) also improved. No patient experienced persistent vertigo after the operation and pre operative tinnitus resolved in 12 out of 22 patients. Stapedotomy is a safe and effective modality for improving conductive hearing loss in otosclerotic patients. Bangladesh J Otorhinolaryngol; October 2019; 25(2): 85-93

Background: Foetal Binocular Distance in Second & Third Trimester of Pregnancy and correlation with Gestation Age is important. Objective: The purpose of the study was to sonographically measure the foetal binocular distance, determine the foetal gestational age & to find out relationship between them in 2nd & 3rd trimester of gestation. Methodology: This cross sectional study was carried out in the department of radiology and imaging Mymensingh Medical College Hospital, Mymensingh and Bangabandhu Sheikh Mujib Medical University, Dhaka, Bangladesh during the period of July 2012 to June 2013. In all the cases BPD, FL, AC, Binocular distance (BD) in cm and gestational age in weeks were measured by USG. Each patient was voluntary enrolled into the study without any specific indication. The measurement was performed only once for each patient. The foetal outer binocular distance was identified in the occipito-transverse or occipito-posterior-foetal positions. With the head in the occipito-posterior position, the transducer was placed in a plane that transected the occiput, orbits, and nasal processes. Measurements were obtained only when the fetal face was directly perpendicular to the uterine wall, since measurements in an oblique plane were considered to be unreliable. Result: The correlation between binocular distance (BD) in cm and gestational age (GA) in wks was calculated. This correlation was highly significant (r = 0. 973; p<0.001).Conclusion:Excellent correlation was found to exist between binocular diameter and gestational age. In the absence of known date of last menstrual period or where fundal height does not agree with dates, these parameters are valuable in estimating the gestational age of the foetus.Journal of Current and Advance Medical Research, 2016;3(1):10-15
