Association of visceral adiposity index with insulin resistance in adults with diabetes mellitus

Background and objectives: Visceral adiposity is linked to excess morbidity and mortality and positively correlates with the risk of insulin resistance, type-2 diabetes mellitus, cardiovascular disease and premature death. The study was conducted to find out the relationship between visceral adiposity index (VAI) and homeostatic model assessment insulin resistance (HOMA-IR) in diabetes mellitus (DM). Materials and methods: This cross sectional study was carried out on adult population with and without DM. Waist circumference (WC) and body mass index (BMI) were measured. BMI of 25-29.9 kg/m2 and ≥30 kg/m2 was defined as overweight and obese respectively. HOMA-IR method was used to calculate insulin resistance (IR). Standard formula using BMI, WC, triglyceride (TG) and high density lipoprotein cholesterol (HDL-c) was used to calculate VAI. Blood was analyzed for fasting blood glucose (FBS), TG, HDL-c and insulin level. Results: A total of 439 individuals were included in the study of which 269 had DM and 170 were healthy volunteers and the mean age was 41.47±6.82 and 36.16±7.44 years respectively. Compared to healthy controls, a greater number of diabetics had high VAI (86.5% vs. 98.9%) and high IR (43.5% vs. 85.1%). We found the highest sensitivity and specificity at a cut-off of 2.23 of VAI while at 3.65 had the highest specificity. Insulin resistance was observed significantly higher in those with diabetes compared to control, both in case of normal and high VAI at all cut-offs of VAI. Among anthropometric parameters (WC, BMI and VAI), VAI had positive (r=0.21, p<0.001) correlation with HOMA-IR than WC (r=0.10, p=0.043). Visceral fat was linearly related with insulin resistance (ß=0.18, p<0.001). Area under the curve (AUC) (0.66) showed that VAI can discriminate HOMA-IR. Conclusion: There was a high rate of raised VAI in cases with DM. VAI had positive association with HOMA-IR in diabetes mellitus. Although weak, there was an acceptable discrimination between them. Background. Visceral adiposity is related to insulin resistance (IR), a metabolic state considered as a risk factor for other cardiometabolic diseases. In that matter, mathematical indexes such as the visceral adiposity index (VAI) and the lipid accumulation product (LAP) could indirectly assess IR based on visceral adiposity. Objective. To evaluate the association and diagnostic accuracy of VAI and LAP to diagnose IR in the adult population of Maracaibo city. Methods. This is a cross-sectional descriptive study with multistage sampling. Receiver operating characteristic (ROC) curves were built to determine VAI and LAP cutoff points to predict IR. A set of logistic regression models was constructed according to sociodemographic, psychobiologic, and metabolic variables. Results. 1818 subjects were evaluated (51.4% women). The area under the curve (AUC) values for LAP and VAI were 0.689 (0.665–0.714) and 0.645 (0.619–0.670), respectively. Both indexes showed a higher IR risk in the upper tertile in bivariate analysis. However, in the logistic regression analysis for the IR risk, only the 2nd (OR: 1.91; 95% CI: 1.37–2.65; p < 0.01 ) and 3rd (OR: 5.40; 95% CI: 3.48–8.39; p < 0.01 ) LAP tertiles showed a significant increase. This behaviour was also observed after adjusting for hs-C-reactive protein (hs-CPR). Conclusion. Although both indexes show a low predictive capacity in individuals with IR in the Maracaibo city population, the LAP index was more strongly associated with IR.

We assessed the correlation between the visceral adiposity index (VAI; a useful indicator of adipose distribution and function) and homeostatic model assessment of insulin resistance (HOMA-IR) in participants with normal waist circumference. A cross-sectional study was conducted, which included 1834 Chinese adults. The blood pressure, anthropometric measurements, fasting and postprandial blood glucose, fasting insulin, and lipid profiles were measured. The VAI and HOMA-IR were calculated. Participants were divided into 4 groups according to the HOMA-IR level, and the correlation between the VAI and HOMA-IR was analyzed. The VAI gradually increased across the HOMA-IR quartiles ( P < .05), and a Pearson correlation analysis showed that VAI was positively related to the HOMA-IR ( P < .001) in males and females. After adjusting for the other covariates, VAI was independently correlated with the HOMA-IR. A logistic regression analysis indicated that VAI elevation was the main risk factor for the increased HOMA-IR in both genders. Overall, the VAI was closely correlated with the HOMA-IR in a population without central obesity.

Background and objectives: The visceral adiposity index (VAI), estimating visceral adiposity dysfunction through a simple formula, could serve as a useful tool for identifying individuals at higher cardiometabolic risk. Its relationship with insulin resistance (IR), assessed using the homeostasis model assessment of IR (HOMA-IR), and metabolic syndrome (MetS) components remains unclear. The study aimed to investigate the association of VAI with both HOMA-IR and MetS. Materials and Methods: After undergoing anthropometric and biochemical studies, 783 individuals were divided into three groups according to a number of present MetS components. The VAI cut-offs signaling MetS and HOMA-IR were determined by maximizing the sum of the sensitivity and specificity. Correlation analysis was performed to explore the associations between VAI and other tested parameters. A logistic stepwise regression analysis was applied to identify statistically significant determinants of HOMA-IR. Given the variability of reference values, two thresholds of HOMA-IR were applied, namely 2.0 and 3.8. Results: VAI increased significantly between the groups with a rising number of MetS components. The VAI cut-off for MetS was 2.37, with a sensitivity of 0.86 and a specificity of 0.78. The same cut-off point identified subjects with HOMA-IR = 3.8, with a sensitivity of 0.79 and a specificity of 0.66. The VAI cut-off for HOMA-IR = 2.0 was 1.89, with a sensitivity of 0.74 and a specificity of 0.68. The strongest correlations of VAI were noted with HOMA-IR (r = 0.51) and insulin (r = 0.49), respectively, while the strongest correlation of HOMA-IR was with waist circumference (r = 0.54). Not one of the routine parameters was a significant predictor in the regression analysis. Conclusions: The obtained results show an existing association of VAI with HOMA-IR. The high sensitivity and specificity of the cut-offs may allow the application of VAI in common clinical practice.

Background: Diabetes mellitus is a metabolic disease known by chronic hyperglycemia which results from defective insulin action and secretion. Metabolic Syndrome consists of a constellation of metabolic abnormalities that confer increased risk of diabetes mellitus. The aim of our study is to find out whether non-invasive, clinically measurable surrogates could be useful in identifying body fat distribution and help predict metabolic syndrome and diabetes risk and to compare the performance of anthropometric indices with lipid indices in identifying metabolic syndrome and diabetes. Methods: 50 individuals with metabolic syndrome ,50 individuals with type 2 diabetes mellitus and 50 controls were selected by purposive sampling technique. For cases and controls history was taken, physical examination was done .Fasting blood sugar, Serum High density lipoprotein and Serum Triglyceride levels were estimated. Body mass index, a body shape index, visceral adiposity index, lipid accumulation factor was calculated. Results: The mean values visceral adiposity index, lipid accumulation factor were significantly increased (p<0.001) in cases compared to controls. Conclusion: Our study concluded that lipid indices visceral adiposity index, lipid accumulation factor is better than anthropometric indices like body mass index, a body shape index in predicting metabolic syndrome and type 2 diabetes mellitus. Anthropometric indices when used should be correlated with metabolic variables and clinical symptoms.

Objective To compare the effects of resistance and aerobic training (RT and AT) on spexin (SPX), appetite, lipid accumulation product (LAP), visceral adiposity index (VAI), and body composition in type 2 diabetes mellitus (T2DM) patients. Materials and Methods: Thirty-six T2DM men were randomized to receive RT ( n = 12), AT ( n = 12), or to act as a non-exercise control (CON, n = 12) 3 days a week for 12 weeks. Results: SPX was increased after both RT and AT (66.2% and 46.5%, respectively). VAI, LAP, and homeostasis model assessment-insulin resistance (HOMA-IR) were reduced in both groups, while quantitative insulin sensitivity check index (Quicki) and McAuley’s indexes were increased following both interventions. However, the increases of both hunger and PFC in the RT group were greater than those of the AT. Moreover, the improvement of upper-body strength (41% vs. 10.3%) and lower-body strength (42.2% vs. 20.5%) in the RT group was greater than those of the AT. Conclusion: Our investigation shows that regardless of the modes of the regimen, a 12-week exercise intervention with RT and AT can effectively induce a significant improvement in SPX levels, appetite, LAP, VAI, and body composition in adults with T2DM.

AbstractObjectiveAlthough many studies worldwide have focused on the relationship between vitamin D and insulin resistance, results remain controversial. Furthermore, concentrations of serum 25-hydroxyvitamin D (25(OH)D) in the Chinese population are unclear. We aimed to investigate vitamin D status and its correlation with insulin resistance among a Chinese adult population.DesignSerum 25(OH)D, fasting blood glucose, fasting insulin, glycated Hb (HbA1c) and other metabolic parameters were assessed. Neck circumference, waist circumference, hip circumference, weight and height were also measured. Lifestyle factors including smoking and drinking status were obtained. Diabetes mellitus was diagnosed by HbA1c according to the 2010 American Diabetes Association criteria.SettingEastern China.SubjectsOf 7200 residents included, 6597 individuals were ultimately analysed.ResultsWe enrolled 2813 males (mean age 52·7 (sd 13·5) years) and 3784 females (52·3 (sd 13·5) years); mean serum 25(OH)D concentration was 43·1 (sd 11·6) and 39·6 (sd 9·8) nmol/l, respectively. Additionally, 83·3 % of participants were 25(OH)D deficient. A significant difference in 25(OH)D was observed between males and females in winter and spring (P<0·001). Furthermore, 25(OH)D concentrations were inversely associated with the homeostasis model assessment of insulin resistance (HOMA-IR) in the overweight and pre-diabetic populations. After adjusting for several variables, 25(OH)D was significantly associated with HOMA-IR in winter. When 25(OH)D values were categorized into quartiles, HOMA-IR was significantly associated with decreasing 25(OH)D.ConclusionsThe majority of the Chinese population was vitamin D deficient and this deficiency was negatively associated with insulin resistance, particularly in the overweight and pre-diabetic populations. Moreover, these associations might be more evident in the winter.

Abstract BackgroundThis study aimed to assess the predictive performance of diabetes (DM) by using adiposity indices compared to body mass index and waist circumference. MethodsAmong 8249 consecutive subjects who attended the Nationwide Health Check Up System for Senior Citizens between 2008 and 2018 (≥ 65 years), we examined the associations of several adiposity indices with DM risk, and further explored gender differences. ResultsAmong all adiposity indicators, Chinese visceral adiposity index (CVAI) alone demonstrated the highest discriminatory ability for diabetes mellitus by area under receiver operating characteristic curves (AUC) (0.65, 0.68, and 0.66 for men, women, and all participants, respectively), regardless of gender, with optimal cut-offs set as 126.09 in men and 117.77 in women, respectively. Compared with body shape index (ABSI), CVAI was strongly associated with baseline DM (adjusted OR: 4.16 [3.35–5.17] for 4th vs 1st quartile groups, P < 0.001), which was more pronounced in elderly women (P interaction < 0.05). Over a median of 5.25 years (IQR: 3.07–6.44 years) follow-up, Cox regression models showed higher predictive ability of CVAI rather than the ABSI. Further, this independently predicted new-onset DM (adjusted HR: 1.26, 95% CI: 1.18–1.34) and composite endpoint of new DM and death (adjusted HR: 1.17, 95% CI: 1.10–1.25, both P < 0.001) among those without baseline DM. ConclusionsOur population-based data demonstrated that Chinese visceral adiposity index may serve as a superior clinical indicator of diabetes when compared with conventional anthropometric indices among elderly Chinese, especially in women.
