Aim: Non-instrumental endodontic therapy has stood out for being a minimal intervention approach that uses a mixture of antibiotics to disinfect the root canals. The purpose of this case report was to describe the treatment and 36-month follow-up of a deciduous necrotic molar using the modified technique of “Injury Sterilization and Tissue Repair” (LSTR). Methodology: A 5-year-old female patient sought dental care complaining of pain in the region of tooth 85 whose oral clinical examination revealed the presence of caries with pulp involvement. In the preoperative period (T1), the patient presented with fistula, abscess and edema. Endodontic treatment was performed using the LSTR technique with triple antibiotic paste. In the follow-up, due to radiographic aspects, a large interradicular lesion was observed in T1 and at the end of the follow-up (T4 / T5), complete bone regeneration was observed in the region. Conclusion: Due to the high antibacterial efficacy of the paste used, the LSTR technique showed satisfactory results in the pulpectomy of a deciduous necrotic molar. Clinical significance: This technique shows the potential to be used in cases to being performed in a shorter operative time, which can be useful in the treatment of children with negative behavior.

Lesion sterilization and tissue repair (LSTR) treatment is very effective in disinfecting the root canal using triple antibiotic mixed in a suitable vehicle. This report is highlighting a case where LSTR therapy was done using triple antibiotic paste in primary molar showing peri-radicular pathosis and root resorption. On one-year follow-up, the tooth was asymptomatic with reduction in peri-radicular pathosis and root resorption. Thus, LSTR can be successful for nonvital pulp therapy in primary teeth with peri-radicular pathosis and root resorption. And also, LSTR therapy can be used as an alternative option to extraction in treating primary teeth with peri-radicular pathosis.

A 7-year-old boy was referred to the department of conservative dentistry and endodontics, Bangabandhu Sheikh Mujib Medial University (BSMMU) with pain on his left lower jaw. After clinical and radiological examination, it was diagnosed as a case of reversible pulpitis. Biodentine pulpotomy was performed followed by permanent restoration with glass-ionomer. The presented case was done to evaluate the efficacy of Biodentine as pulpotomy medicament in primary molar teeth, on follow-up it was found to be successful.Birdem Med J 2017; 7(3): 238-241

Objective: In vitro comparison of the antibacterial efficacy of Diode and Er:YAG laser irradiation with that of NaOCl irrigation in contaminated primary molar root canals. Study Design: 96 root canals prepared from 32 extracted primary molar teeth were mechanically enlarged and the teeth were randomly divided into 4 subgroups. The roots were inoculated with an overnight culture of Enterococcus faecalis in tryptic soy broth for 24 hours. The root canals irradiated with diode and Er:YAG laser and irrigated with NaOCl(5.25%) were experimental groups and untreated canals served as positive control group. Bacterial growth was analysed by counting viable E.faecalis on tryptic soy agar plates. Results: The number of bacteria was significantly reduced in experimental groups in comparison with the control group. Diode laser was determined to be more effective in reducing the number of bacteria when compared to Er:YAG laser. NaOCl irrigation was found significantly most effective. Conclusions: Diode laser irradiation and 5.25 % NaOCl application provided a significant antibacterial effect in vitro, in contaminated primary molar root canals.

Objective: The aim of this case report is to describe the treatment of a primary molar with a deep carious lesion by pulpotomy and placement of a ceramic endocrown. Clinical case: A 7-year-old male patient with profound caries in tooth number 85 was referred to our clinic and underwent a pulpotomy. As the final treatment in tooth restoration, placement of an endocrown was planned, because little more than half of the tooth structure remained. After an additional request from the patient's parents for an advanced and prompt restoration, a computer-aided design/computer-aided manufacturing (CAD/CAM) polymer-infiltrated ceramic network (PICN) block was chosen. A three-dimensional model of the arch was obtained after scanning the dental cast, and the endocrown was designed digitally according to the model. When the design was complete, the endocrown was fabricated with a milling machine. Finally, the endocrown was cemented with self-adhesive resin cement. Results: Over the 9-month follow-up period, no pulpal or periradicular pathology was observed on radiographs. Regarding the crown, the marginal fit was excellent, the anatomical form was protected, and no discoloration occurred. Conclusion: During follow-up, the CAD/CAM PICN block endocrown proved to be a good material for the short- to long-term treatment of a primary tooth. However, more clinical cases and follow-up are required to investigate the long-term effects of antagonistic tooth wear.

In teeth with pulpal and periapical infection, various group of microorganisms are present. Inhibition of the growth of these microorganisms and suppression of their antimicrobial activity play a crucial role in the success of the endodontic management. Triple antibiotic paste has been reported to have an excellent antimicrobial efficacy and it can be useful in management of teeth with severe infection. A 12-year-old female child reported with pain and swelling in relation with a lower left first permanent molar. Clinical examination of the tooth revealed deep occlusal caries and dentoalveolar abscess. Radiographically, the carious lesion was seen involving the pulp with periodontal ligament widening and inter-radicular bone loss was observed (endo-perio lesion). Considering the position of the tooth and its strategic importance, a conservative approach to salvage the tooth using Triple Antibiotic Paste (TAP) as a medicament was employed with complete radiographic healing by 12 weeks. Subsequent obturation using lateral condensation with gutta percha followed by composite restoration and preformed stainless steel crown was performed. This case report focuses on the endodontic management of perforated first permanent molar with compromised periodontal support and significant bone loss TAP as medicament and Mineral Trioxide Aggregate (MTA) as perforation repair material. Based on the results it can be concluded that Triple Anitibiotic Paste (TAP) can be effective in the disinfection and sterilization of the root canals and repair of complex lesions.

Objectives: Chronically fixed lateral patellar dislocation is uncommon and mostly associated with disruption of the medial patellofemoral ligament (MPFL) which is a major risk factor for this condition, as it functions as the main patella medial stabilizer. This paper focuses on the reconstruction of the MPFL after an extensive lateral release in a fixed lateral patellar dislocation. We aim to explain the technique that was used to reduce and maintain the position of the patella and prevent it from re-dislocation. Extensive lateral release and modified MPFL reconstruction for a chronically fixed patellar lateral dislocation provide a satisfactory result. We aim to explain a modified technique to treat a chronically fixed patellar lateral dislocation Case presentation: We present a case report of a 49-year-old Caucasian male with a history of falling on his right knee 25 years before. He complained of pain on his knee for 8 months before. Physical examination showed chronically fixed lateral patellar dislocation with a normal range of motion. The diagnosis of patellar dislocation was confirmed with a conventional radiograph. The patient underwent extensive lateral release of the knee followed by a modified technique for MPFL reconstruction. Semitendinosus tendon was used as the graft because of the chronic nature of this condition. The graft was inserted into the patellar tunnel and fixed using endobutton®. The graft was inserted into the femoral tunnel that was made at Schottle point from the medial through the lateral side. After that, the cortical screw with the washer was inserted on the lateral femoral side just above the femoral tunnel as an anchor to make sure the tendon is secured. We follow-up the patient for 5 years. Results and Conclusion: On a 5 year follow-up, there was no complaint of recurrent patellar dislocation. The result of this extensive lateral release followed by modified MPFL reconstruction for a chronically fixed lateral patellar dislocation is satisfactory. The extensive release was needed to reduce the fixed dislocated patella and this modified technique of MPFL reconstruction by using screw and washer as an anchor on lateral sides of the femur could be an alternative to prevent re-dislocation.

ABSTRACT Background : The best treatment for anal fistula should extirpate infection and promote healing of the tract, whilst preserving the anal sphincter complex and full continence. Aim: To analyze the success rate after a modified technique for ligation of the intersphincteric fistula tract (LIFT) for patients with anal fistulas. Methods: A prospective (observational cohort study) Brazilian bi-institutional experience with a modified (ligation of the intersphincteric fistula tract without excision) LIFT technique was undertaken. A clinical database was settled for the following variables: age, gender, BMI, comorbidities, distance between external orifice and the anus, previous fistula surgery, type of fistula, operative time, intra- and postoperative complications, duration of follow-up, and success rate. Results: Between November 2015 and January 2017, 38 patients with transsphincteric fistulas were operated on using the modified LIFT procedure. Seventeen (44.7%) were men. Median age was 41 (18-67) years. Median BMI was 26.4 (22-38) kg/m2. Five (13.2%) had undergone previous surgery. The fistula was transsphincteric in all cases. Median follow-up was 32 (range, 14-56) weeks. Success was observed in 30 (79%) patients. Conclusions: The LIFT technique without excision of the fistula tract proved to be safe and effective for transsphincteric anal fistulas.

This case report presents the clinical use of a resin nanoceramic CAD/CAM restoration of a primary second molar without successor in the form of a permanent second premolar tooth in a patient. Three-year follow-up of the case revealed that resin nanoceramic CAD/CAM restoration of the primary molar without successor achieved both aesthetics and function. Despite the high cost of treatment, this type of restoration should be considered if the retained tooth is expected to maintain functionality over the long term.
