A retrospective, quasi-experimental cohort study compared antibiotic use before and after implementation of a procalcitonin assay at a community acute care hospital. This study demonstrated that the implementation of the procalcitonin assay was associated with a decrease in antibiotic days of therapy in adult patients with pneumonia.

Background: Preliminary studies conducted by researchers on December 2015 at Public Health Center Mijen, Semarang get a yield of 7 people in the category of obesity, be found 2 people have a varicose veins in the legs and 5 other people not have varicose veins in the legs. Aim: Analyzing correlation between the level of obesity with events varicose vein on adult patients aged 26-45 years old in Public Health Center Mijen, Semarang. Methods: The research design was cross sectional. Samples were taken from adult obese patients who come to the Public Health Center Mijen Semarang as much as 42 respondents, selected by accidental sampling technique. Data were gathered using observation sheets, then analyzed using Spearman rank test. Results: The p-value of the Spearman rank test was 0.003, with the correlation coefficient of 0.443. Conclusion: There is a correlation between the level of obesity with the incidence of varicose veins on adult patients in Public Health Center Mijen Semarang. Suggestion: Recommended for respondent to control weight and attention intake foods consumed. Keywords: Obesity; varicose vein, adult

Background: Contaminated healthcare facility plumbing is increasingly recognized as a source of carbapenemase-producing organisms (CPOs). In August 2019, the Tennessee State Public Health Laboratory identified Tennessee’s twelfth VIM-producing carbapenem-resistant Pseudomonas aeruginosa (VIM-CRPA), from a patient in a long-term acute-care hospital. To determine a potential reservoir, the Tennessee Department of Health (TDH) reviewed healthcare exposures for all cases. Four cases (33%), including the most recent case and earliest from March 2018, had a history of admission to intensive care unit (ICU) room X at acute-care hospital A (ACH A), but the specimens were collected at other facilities. The Public Health Laboratory collaborated with ACH A to assess exposures, perform environmental sampling, and implement control measures. Methods: TDH conducted in-person infection prevention assessments with ACH A, including a review of the water management program. Initial recommendations included placing all patients admitted to room X on contact precautions, screening for CPO on room discharge, daily sink basin and counter cleaning, and other sink hygiene measures. TDH collected environmental and water samples from 5 ICU sinks (ie, the handwashing and bathroom sinks in room X and neighboring room Y [control] and 1 hallway sink) and assessed the presence of VIM-CRPA. Moreover, 5 patients and 4 environmental VIM-CRPA underwent whole-genome sequencing (WGS). Results: From February to June 2020, of 21 patients admitted to room X, 9 (43%) underwent discharge screening and 4 (44%) were colonized with VIM-CRPA. Average room X length of stay was longer for colonized patients (11.3 vs 4.8 days). Drain swabs from room X’s bathroom and handwashing sinks grew VIM-CRPA; VIM-CRPA was not detected in tap water or other swab samples. VIM-CRPA from the environment and patients were sequence type 253 and varied by 0–13 single-nucleotide variants. ACH A replaced room X’s sinks and external plumbing in July. Discharge screening and contact precautions for all patients were discontinued in November, 5 months following the last case and 12 consecutive negative patient discharge screens. Improved sink hygiene and mechanism testing for CRPA from clinical cultures continued, with no new cases identified. Conclusions: An ICU room with a persistently contaminated sink drain was a persistent reservoir of VIM-CRPA. The room X attack rate was high, with VIM-CRPA acquisition occurring in >40% of patients screened. The use of contaminated plumbing fixtures in ACH have the potential to facilitate transmission to patients but may be challenging to identify and remediate. All healthcare facilities should follow sink hygiene best practices.Funding: NoDisclosures: None

<p><strong>Abstrak</strong></p><p><strong>Latar Belakang : </strong>Prevalensi Hipertensi di Indonesia semakin meningkat mencapai 25,8% pada penduduk berusia 18 tahun keatas.  Amlodipin merupakan salah satu pilihan obat untuk terapi Hipertensi yang termasuk golongan Calsium Channel Blocker (CCB). Penggunaan obat antihipertensi sendiri merupakan salah satu faktor risiko yang menyebabkan sindroma mata kering.</p><p><strong>Tujuan : </strong>Penelitian ini bertujuan untuk mengetahui pengaruh penggunaan obat antihipertensi terhadap sindroma mata kering pada penderita Hipertensi di wilayah kerja Puskesmas Jagir kota Surabaya.</p><p><strong>Metode penelitian : </strong>Penelitian ini adalah penelitian observasional analitik dengan metode cross-sectional. Subyek pada penelitian ini adalah 48 responden yang didapatkan dibagi kedalam 2 kelompok yakni, kelompok minum obat antihipertensi &gt;6 bulan dan tidak minum obat. Dilakukan pengisian kuesioner DEQ-5 dan pemeriksaan schirmer I.</p><p><strong>Hasil penelitian : </strong>Analisa deskriptif dari tes schirmer I frekuensi sindroma mata kering lebih tinggi pada responden yang mengkonsumsi obat antihipertensi dari pada responden yang tidak minum obat antihipertensi. Hasil analisa statistik menggunakan uji chi-square signifikansinya yaitu sebesar &lt;0,001 atau dapat dikatakan bahwa p&lt;α (0,05), menunjukkan ada pengaruh pemberian obat antihipertensi terhadap sindroma mata kering yang berdasarkan tes Schirmer pada pasien Hipertensi di wilayah kerja puskesmas Jagir  kota Surabaya.</p><p><strong>Simpulan : </strong>Penggunaan obat antihipertensi berpengaruh terhadap sindroma mata kering pada penderita hipertensi di wilayah kerja Puskesmas Jagir kota Surabaya</p><p><strong>Kata Kunci : </strong>Sindroma mata kering, obat antihipertensi, hipertensi</p><p><strong> </strong></p><p><strong>Abstract</strong></p><p><strong>Background: </strong>Hypertension prevalence in Indonesia is increasing to reach 25.8% in the population aged 18 years and over. The use of antihypertensive medication itself is one of the risk factors that cause dry eye syndrome.</p><p><strong>Purpose</strong>: This study aims to determine the effect of the use of antihypertensive drugs on dry eye syndrome in patients with hypertension at Jagir Public Health Center in Surabaya working area.</p><p><strong>Methods:</strong> This study was an observational analytic study with a cross-sectional method. The subjects in this study were 48 respondents found divided into 2 groups namely, the group taking antihypertensive drugs&gt; 6 months and not taking medication. The DEQ-5 questionnaire was completed and Schirmer I was examined.</p><p><strong>Results:</strong> Descriptive analysis of the Schirmer I test for the frequency of dry eye syndrome was higher in respondents who took antihypertensive drugs than those who did not take antihypertensive drugs. The results of statistical analysis using the chi-square test of significance that is equal to &lt;0.001 or it can be said that p &lt;α (0.05), indicates there is an effect of antihypertensive drug administration on dry eye syndrome based on the Schirmer test in hypertension patients at Jagir Public Health Center in Surabaya..</p><p><strong>Conclusion:</strong> The use of antihypertensive drugs has an effect on dry eye syndrome in patients with hypertension at the Jagir Public Health Center in Surabaya</p><strong>Keywords:</strong> Dry eye syndrome, antihypertensive drugs, hypertension
