Exploring Perception of Chronic Hepatitis C: An Idiographic Case Study

The disease Hepatitis C has devastating impact on the individual and their family members. This study demonstrates the case of a 45 year old woman suffering from chronic Hepatitis C. The methods of data collection employed were Mental Status Examination and semi-structured interview. Furthermore, the participant‟s caregiver (sister) was also interviewed. For this purpose, two separate interview schedules were prepared. Data was interpreted by using Interpretative Phenomenological Approach. The most dominant themes extracted within this study were overshadowing physical health; psychological and emotional distress; coping with disease; and social support from family members. In addition, the study showed that the perception of social support and coping have an influence on chronic Hepatitis C individuals. The present study‟s findings would be fruitful for Pakistani people in enhancing their understanding about chronic Hepatitis C.

The data on the polymorphism of cytokine genes associated with individual reactivity on the effects of hepatitis C virus, predict the rate of progression of liver fibrosis. The purpose of this work is study the association of the polymorphic marker G308A of the TNFα gene with its quantitative content and degree of liver fibrosis in patients with chronic hepatitis C. A total of 100 patients with CSF were examined. The polymorphism of G308A gene’s TNFα was studied by amplification of the corresponding genome zones by PCR. The assessment of the degree of fibrosis was performed using the non-invasive Fibrotest method. The study of the quantity of TNFα cytokine in serum of patients was performed by ELISA. The distribution of genotypes on the investigated polymorphic loci was verified using Pearson's χ2 criterion. The frequencies of alleles and genotypes in the groups were compared using Pearson's χ2 criterion with Yates correction for continuity with the number of degrees of freedom 1. In order to detect the correlation dependencies between the individual parameters, the Spearman correlation coefficient was applied. It was found that a smaller degree of fibrosis was observed in carriers of the GG TNFα genotype, and a greater degree of fibrosis in the carriers of the genotype AA TNFα (moderate feedback between the degree of fibrosis and the genotypes of TNFα). The higher content of TNFα is noted in the carriers of the AA genotype TNFα, the lower content of TNFα - in the carriers of the GG TNFα genotype (moderate feedback between the TNFα genotypes and the TNFα content). It has been established that a higher TNFα content is observed in patients with F1-F0 fibrosis, a lower TNFα content in patients with F2-F3 fibrosis (a strong correlation between the degree of fibrosis and the amount of TNFα cytokine). It is assumed that the production of the cytokine is determined at the genetic level, and the severity of changes in the cytokine profile in chronic hepatitis C affects the course of the pathological process. An increase in the TNFα content in chronic hepatitis C may be a marker for significant morphological changes in the hepatic tissue and high activity of the inflammatory process.

Hepatitis C virus (HCV) infections can cause permanent liver-related diseases, including hepatocellular carcinoma (HCC). Low mortality and incidence of HCC have been observed in patients with chronic hepatitis C undergoing direct-acting antiviral therapy. Tumor suppressive let-7 family members are down-regulated in HCC. The present study, therefore, aimed to investigate whether expression levels for the full spectrum of let-7 family members (let-7a, 7b, 7c, 7d, 7e, 7f, 7g, 7i, and miR-98) in the circulatory system are useful as surveillance biomarkers for liver-related diseases to monitor treatment efficacy during HCV infection. To this end, we measured the levels of mature circulating let-7 family members using quantitative reverse transcription-PCR in 236 patients with HCV infection, and 147 age- and sex-matched controls. Using hierarchical cluster analysis and principal component analysis, three clusters were obtained after measuring expression levels of let-7 family members in the patients and controls. Cluster 1 included let-7a/d/e/g, Cluster 2 comprised let-7b and let-7i, and Cluster 3 comprised let-7c/f/miR-98. Let-7b/c/g represented the three clusters and showed the best survival response to liver cancer when analyzed with respect to patient data. Therefore, considering the circulating levels of let7 b/c/g as representatives of the let-7 family may facilitate effective monitoring of liver-related disease.

This study seeks to contribute to our understanding of the lived experiences of family members and friends of individuals with traumatic brain injury (TBI). The study employed an interpretative phenomenological approach, enabling family members and friends to identify and reflect upon the factors they themselves considered most significant about the impact of TBI on someone close to them, and how this experience has affected their own lives and their relationship with the TBI survivor. Eleven participants, two males and nine females aged 22–79, were from the United Kingdom. Several strong and interconnected themes emerged from the interviews, clustering into four superordinate themes: “continuity and discontinuity”; “damage, loss, and grief”; “roles and responsibility”; and “coping and not coping.” The study findings identify the importance of support and information provision for family members and friends of those with TBI to help them manage the long-term impact of TBI.

Introduction: Individuals living with hepatitis C experience a series of changes in their lives, caused by either the disease itself or its treatment or both. In this sense, living with a chronic disease such as hepatitis C implies that patients must change their lifestyles, which requires that they rethink their habits in light of the new reality. To comprehend chronic hepatitis C patients’ perceptions, meanings, and quality-of-life effects. Methodology: Qualitative research; interviews with 12 patients at a specialized outpatient clinic from February to July 2012. The cohort size was determined by theoretical saturation and the interviews were analyzed by means of content analysis. Results: Data were organized into four categories: on medication, referenced fears, impact on sexuality, and coping with the new reality. The categorized data were discussed around the following themes: “Becoming sick” due to treatment and perceived aspects and Undergoing treatment and how to cope with it. Conclusion: Patients’ everyday lives and quality of life in general are affected especially by adverse side effects of hepatitis C medication and an assortment of feelings and uncertainties about the disease prognosis. Notwithstanding, most patients manage to find ways to cope with the treatment and complete it expecting a satisfactory outcome.
