Aim: This qualitative study was conducted to determine the problems experienced by patients with chronic diseases and their caregivers during the home care process from nursing students' perspectives. Methods: Phenomenological method of qualitative research was used in the study. Data was gathered via in-depth interviews with seven nursing students who had a family member with chronic disease. Students were asked about the problems they and their family members experienced during the home care process with a semi-structured questionnaire. Data obtained from the interviews were analyzed using content analysis. Results: As a result of thematic analysis, problems experienced by the patients with chronic diseases and their caregivers during the home care process are grouped under five main themes. These problems were physical problems, social problems, financial problems, psychological problems, and treatment-related problems. Conclusion: This research revealed that the home care process of patients with chronic diseases causes various problems for both patients and their caregivers. Therefore, it is necessary to be aware of these problems, increase the quality of home care services and ensure the continuity of home care. In this way, physiological, psychological, financial, social, and treatment-related problems can be prevented. Keywords: caregivers, chronic disease, family health, home care, nursing students

This study aims to evaluate the impact of urinary incontinence on the quality of life in those receiving home care services and the factors that influence this. This cross-descriptive study was performed in 180 patients who were served from Home Care Services between 01 and 28 February 2018. An International Consultation on Incontinence Questionnaire–Short Form (ICIQ-SF) was implemented in person to those who gave their informed consent with orientation and cooperation. The mean age of a 180 patients was 74.40 ± 7.80 years (min = 18, max = 104), of which 55.2% were women. The mean ICIQ-SF score was found to be 12.42 ± 4.83 (min = 0, max = 21). The lowest points were given as responses to the question, “In your opinion, what amount of leakage do you experience?” The highest points, however, were given to the question of “How much does urinary leakage interfere with your everyday life?” It was seen that when quality of life is assessed with a visual analog scale within the ICIQ-SF, the quality of life for 66.6% of elderly individuals was affected moderately or significantly. The mean ICIQ-SF scores were higher in those with chronic diseases ( p < .005). Urinary Incontinence negatively affects quality of life to a moderate and significant degree in those receiving home care. The quality of life for those who have chronic diseases was worse. In this respect, there is a need to support patients in those receiving home care services.

RESUMOObjetivo: compreender as concepções de pessoas idosas acerca das repercussões do adoecimento crônico na sua saúde mental. Método: trata-se de estudo qualitativo, descritivo, realizado com 13 pessoas idosas com diagnóstico de doenças crônicas cadastradas em uma Unidade de Saúde da Família. Coletaram-se as informações a partir da aplicação de um formulário de um roteiro de entrevista semiestruturada. Analisaram-se as entrevistas conforme a técnica de análise de conteúdo. Resultados: a partir da análise do conteúdo das descrições originárias das entrevistas, emergiram quatro categorias temáticas: << Sentimentos relacionados à doença crônica >>; << Mudanças nos hábitos de vida após a descoberta da doença crônica >>; << Dificuldade para aceitação da doença crônica >>; e << A busca pela espiritualidade para aceitação da doença crônica >>. Conclusão: percebeu-se, com o alcance do objetivo do estudo, a necessidade de qualificação dos profissionais atuantes da Atenção Básica para assistir o ser idoso, não apenas para a resolutividade de suas demandas no momento, mas também para saber ouvir e identificar as possíveis repercussões que esse adoecimento traz para a vida desses indivíduos e sua saúde mental. Conclusion: in order to reach the objective of the study, it was noticed the need to qualify professionals working in Primary Care to assist the elderly people, not only for the resolution of their demands at the moment, but also to know how to listen and identify possible repercussions that this disease brings to the lives of these individuals and to their mental health. The determination of nursing students’ attitudes toward elderly discrimination and their opinions about home care is important in that it will affect the quality of care provided to elderly individuals in their homes by the students in the future. For this reason, the aim of this study was to determine relationship between nursing students’ attitudes toward elderly discrimination and their opinions about home care services. This descriptive study was conducted with a total of 318 students from a university nursing faculty during the fall semester of the 2016-2017 academic year. Data for the study were collected using the Home Care Services Evaluation Questionnaire and the Ageism Attitude Scale. It was found that there was a positively significant but weak relationship between the students’ age discrimination scale total score and the positive discrimination subdimension score, and their opinions about home care services ( p < .001). Students’ attitudes toward elderly discrimination were found to make a statistically significant contribution to their opinions about home care services. It was ascertained that students’ attitudes toward elderly discrimination affected their opinions about home care services. It is recommended that geriatric nursing and home care nursing lectures be included in the nursing education curriculum, awareness of the importance of the concept of old age and elderly care be increased by establishing internship fields for students in institutions that provide home care for the elderly, and positive attitudes toward the elderly be promoted.

With increasing chronic diseases, the use of home care is rising in the world. Home care in Iran has many challenges and to improve that, we should identify the challenges and barriers of home care. The aim of this study was to identify and explore the barriers of home care in Iran. This is a qualitative study with content analysis approach that was conducted in Iran in 2015. Fourteen key informants comprising health policymakers, faculty members, nurses, and physicians as well as patients and families engaged in home care purposefully participated in this study. Data was obtained using face-to-face semistructured interviews. A focus group discussion was also used to complete the findings. Graneheim and Lundman’s approach was used for analysis of data and Lincoln and Guba’s criteria were used to confirm the trustworthiness of study’s findings. The data were divided into three main categories and eight subcategories. Main categories included treatment-based approach in the healthcare system, cultural dimensions, and the lack of adequate infrastructure. A position for home care in the healthcare system, considering cultural dimensions in Iranian society and providing an appropriate infrastructure, can be beneficial to improve the situation of home care services in Iran.

Aim: The aim of the study is to determine the factors that may affect the demands of patients who come with the request to have a blood test without any chronic disease or a planned examination check. Methods: The data of this descriptive, cross-sectional study, were collected with a questionnaire that was prepared to examine the opinions of the patients who claim to have a blood test by coming to the family health center without any complaints. Patients over 18 years of age, who did not have any chronic disease and had no scheduled examination appointments were included in the study. Results: A total of 278 patients who wanted to have a blood test within the 6-months period were included in the study. Female patients who wanted to have a blood test were significantly more than male patients. When we look at the causes of patients who wanted to have a blood test; 61.2% (n=170) patients stated that they are concerned about their health and 6.1% (n=17) stated that they were affected by media warnings. There was no significant relationship between the frequency of blood test requests of patients and their age, gender, education, and general health status. Conclusion: Patients with high expectations and anxiety may be more willing to perform blood tests at inappropriate intervals. Family physicians, whose primary role is preventive medicine, have consultancy and information duties in order to protect their patients from the risk of over-examination and diagnosis. ABSTRACT The 2015 – 2020 Dietary Guidelines for Americans provides guidance for choosing a healthy diet. There is a focus on preventing and alleviating the effects of diet-related chronic diseases. These include obesity, diabetes, cardiovascular disease, and stroke, among others. This article briefly reviews the primary guideline items that can be used to teach patients with respect to improving their diet. Clinical exercise physiologists who work with patients with chronic disease can use these guidelines for general discussions regarding a heart-healthy diet.
