Background: This study evaluated the efficacy of a multisonic technology for the debridement of vital and necrotic pulp tissues in freshly extracted human mandibular molar teeth. Methods: Twelve teeth with a diagnosis of symptomatic irreversible pulpitis (SIP) and twelve teeth with a diagnosis of pulp necrosis with symptomatic apical periodontitis (SAP) were extracted. The GentleWave® procedure was performed on 10 teeth from each group. Four non-treated teeth served as histologic controls. Histological consecutive 5 µm sections were obtained from the apical, middle, and coronal portion of the canals. The canals were evaluated for the presence of pulpal debris and bacteria. Results: In nine out of the ten specimens with SIP, no pulpal debris was detected in any portion of the canals. In the necrotic pulp group, eight out of the ten specimens had no detectable pulpal debris in any portion of the canal spaces. No bacteria were detected in the main canals, isthmuses, or lateral canals, but were detected deep within the dentinal tubules in 10 specimens. Conclusions: This study demonstrated that the multisonic technology was effective at removing vital and necrotic pulp tissue as well as bacteria from the root canal system, including inaccessible areas.

ABSTRACT Aim C-shaped canal system is a seldom-found root canal anatomy which displays a challenge in all stages of endodontic treatment. According to the literature, this type of canal morphology is not a common finding in the mandibular first molar teeth. Background This case report presents endodontic management of a mandibular first molar with a C-shaped canal system. Case report Root canal system was cleaned and shaped by nickel-titanium (NiTi) rotary instruments combined with selfadjusting file (SAF). Obturation was performed using warm, vertical condensation combined with the injection of warm gutta-percha. Follow-up examination 12 months later showed that the tooth was asymptomatic. The radiological findings presented no signs of periapical pathology. Clinical significance The clinician must be aware of the occurence and complexity of C-shaped canals in mandibular first molar teeth to perform a successful root canal treatment. The supplementary use of SAF after application of rotary instruments in C-shaped root canals might be a promising approach in endodontic treatment of this type of canal morphology. During endodontic diagnosis multirooted teeth morphologically, shows variable root canal system . This case report explains the anatomical variations of root and root canals. Here it is shown that a single canal is present in mandibular second molar. Therefore we should be aware that a single canal can also be present in mandibular molar.

Apicoectomy failure is generally related to inappropriate marginal sealing of the retrocavity, which allows percolation of microorganisms and their products from root canal system to periapex. This study evaluated tubular penetration of canal sealers and marginal permeability after retrocavity irradiation with Er;Cr:YSGG laser and retrofilling with MTA or cyanoacrylate. Twenty-two single-rooted teeth were decoronated and endodontically treated, their apical 3 mm were resected and the root ends were retroprepared with a low-speed bur. Twenty roots were randomly assigned to 4 groups (n=5): GI and GII - retrofilling with MTA and cyanoacrylate, respectively; GIII and GIV - retrocavity irradiation with Er;Cr:YSGG laser (2.78 µm, 4 W, 20 Hz, 70.8 J/cm²) and retrofilling with MTA and cyanoacrylate, respectively. The remaining 2 roots served as positive and negative controls. The analysis of rhodamine B dye infiltration (p=0.05) demonstrated that laser irradiation and MTA retrofilling presented significantly higher permeability rates (p<0.05). Retrofilling with cyanoacrylate showed significantly lower permeability, either when laser was used or not on retrocavity. SEM analysis depicted more cyanoacrylate penetration through dentinal tubules when compared to MTA, suggesting a more efficient marginal sealing. Based on these results, it may be concluded that cyanoacrylate provided a less permeable retrofilling regardless of the retropreparation method, suggesting a more favorable condition to the establishment of the periapical healing.

Abstract Aim The aim of this report is to present a case of Type III dens invaginatus in a maxillary lateral incisor with a periapical lesion and its successful treatment using a combination of surgical and non-surgical therapy. Background The morphology of the root canal system is unique for individual teeth. A precise understanding of the morphology is very important for a successful root canal treatment. Dens invaginatus is a rare dental anomaly that can give rise to many complex anatomical forms. The complete removal of the diseased pulp tissue can be a challenge for clinicians due to the complexity of internal canal anatomy and may require either non-surgical endodontic therapy alone or that therapy may have to be combined with surgical endodontics to achieve an adequate sealing of the root canal system. Case Report A 22-year-old male presented for evaluation of a recurrent painless sinus tract in the attached gingiva adjacent to the maxillary right lateral incisor. The tooth responded negatively to vitality tests while the right central incisor responded normally. Clinically an anatomical malformation in the crown was detected in the form of an abnormal bulbous contour of the tooth in the palatal region above the cingulum. Radiograph evaluation confirmed the presence of dens invaginatus and a periapical radiolucent lesion. The patient received combined nonsurgical and surgical endodontic therapy to achieve a favorable outcome. Summary Complications presented by Type III dens invaginatus and acute periapical abscess necessitated the use of combined non-surgical and surgical endodontic therapy to attain a favorable seal of the root canal. The prognosis at a six-month post-treatment evaluation appeared to be good. Clinical Significance Dens invaginatus requires an early diagnosis and treatment. The root canal anatomy of mandibular second molar teeth is known to be highly variable. Whilst the most frequently seen configuration is two mesial canals and one distal canal, other variations such as four canals, two canals, and C-shaped canal system do also exist. This case report describes the diagnosis and management of unusual root canal configuration of a mandibular second molar, with one canal in a single conical root, using the contemporary advancements in endodontics. Following clinical and radiographic examinations of the case, a diagnosis of symptomatic irreversible pulpitis with symptomatic apical periodontitis of tooth #47 was established, and root canal treatment followed by composite buildup and crown were planned. Clinicians should be aware of the different anatomic variants each tooth may exhibit. Furthermore, clinicians need to possess the proper knowledge and skills that allow them to utilize the diagnostic and therapeutic tools available at their disposal in order to optimize the quality of care provided to their patients.

Aim. Substantiate the use of light exposure as physiotherapeutic methods of root canal treatment.Materials and methods. For the study, 93 human teeth were taken, all teeth were divided into 3 groups of 31 samples. Optical microscopy was used as the main research method.Results. The effect of laser radiation on biological structures depends on the wavelength of the energy emitted by the laser, the energy density of the beam, and the temporal characteristics of the beam energy. The processes that can occur in this case are reflection, absorption, scattering and transmission. The interaction of laser light and tissues occurs with optical proximity between them. This interaction is specific and selective, based on absorption and diffusion. The smaller the approach, the more light will be reflected or transmitted.Conclusions. Currently, in endodontics, diode lasers are the best systems for disinfecting the root canal system due to their ability to penetrate the dentinal tubules. The optical proximity of their wavelengths to bacteria leads to the destruction of the latter due to the photo-thermal effect.

One of the objectives of successful endodontic treatment is the hermetic obturation of the root canal system. To achieve this, the root canal filling must seal the canal space both apically and coronally to prevent the ingress of microorganisms or tissue fluids into the canal space. Apical leakage is reported a common reason for the clinical failure of endodontic therapy. Leakage through an obturated root canal is expected to take place at interfaces between sealer and dentin or sealer and gutta-percha, or through voids within the sealer. Hence, the sealing quality of root canal filling depends much on the sealing ability of the sealer. Therefore, anything that may influence the adaptation of the root filling to the canal wall is can determine the degree and the extent of leakage, and ultimately the prognosis of the endodontic therapy. In endodontic therapy, the smear layer formation results from root canal preparation and may influence the effective seal of the root canal system. The smear layer formation is mainly composed of inorganic components (dentin debris) and organic materials, such as pulp tissue remnant, bacteria, and blood cells. Removal of the smear layer from the root canal walls before the obturation can reduce the leakage of root canal sealer. To remove the smear layer use 10 ml 17% EDTA followed by 10 ml of 5.25% NaOCl as irrigating solution. This paper discribe about how the effectivity of EDTA as irigating solution to remove the smear layer especially in the apical root canal with many lateral canal to reduce the apical leakage.

Aim. To report a case of maxillary second premolar with four roots and four separate foramina and the use of CBCT to help identify the root canal system configuration. Summary. A 45-year-old female patient with a no significant medical history was referred for treatment tooth #15. The tooth was diagnosed with symptomatic irreversible pulpitis and symptomatic apical periodontitis. After taking periapical radiographs and expected uncommon morphology, CBCT was taken to help confirm canals configuration and aid in the treatment of the case. Root canals treatment was carried without complication under dental operating microscope using a rotary system and continuous wave obturation.

ABSTRACTSuccess in root canal treatment is achieved after thought cleaning and shaping followed by the complete obturation of root canal system. Such treatment may be performed in root canal systems that do not comply with the normal anatomical features described in standard textbooks.
