Objective: The aim of this paper is to describe the progressive changes of chronic cluster headaches (CHs) in a patient who is being treated by a multimodal approach, using pharmacology, neurostimulation and physiotherapy. Subject: A male patient, 42 years of age was diagnosed with left-sided refractory chronic CH by a neurologist in November 2009. In June 2014, the patient underwent a surgical intervention in which a bilateral occipital nerve neurostimulator was implanted as a treatment for headache. Methods: Case report. Results: Primary findings included a decreased frequency of CH which lasted up to 2 months and sometimes even without pain. Besides this, there were decreased levels of anxiety, helplessness (PCS subscale) and a decreased impact of headache (HIT-6 scale). Bilateral pressure pain thresholds (PPTs) were improved along with an increase in strength and motor control of the neck muscles. These improvements were present at the conclusion of the treatment and maintained up to 4 months after the treatment. Conclusions: A multimodal approach, including pharmacology, neurostimulation and physiotherapy may be beneficial for patients with chronic CHs. Further studies such as case series and clinical trials are needed to confirm these results.

Introduction: Breast cancer in men is less common than in women and treatment recommendations are often derived from clinical trials exclusively involving women. Data on efficacy of CDK 4/6 inhibitors, which are the mainstay of treatment for hormone receptor–positive/HER2-negative advanced breast cancer, are lacking in male patients. Case report: We present a clinical case of prolonged benefit from palbociclib in combination with letrozole and LHRH analogue in a man who had previously been treated with six lines of endocrine therapies and chemotherapy regimens but was still in excellent clinical condition. Conclusions: This clinical case demonstrates that male breast cancer stands out as an endocrine-sensitive disease, which could potentially benefit from CDK 4/6 inhibitors in combination with endocrine agents even in very heavily pretreated settings of disease, underscoring both the importance of an accurate selection of patients for later treatment lines, taking into account disease history and previous treatment responses, and the peculiarity of breast cancer in men, which deserves dedicated clinical trials to tailor future recommendations.

The mortality rate from descending necrotising mediastinitis (DNM) has declined since its first description in 1938. The decline in mortality has been attributed to earlier diagnosis by way of contrast-enhanced computed tomographic (CT) scanning and aggressive surgical intervention in the form of transthoracic drainage. We describe a case of DNM with involvement of anterior and posterior mediastinum down to the diaphragm, managed by cervicotomy and transverse cervical drainage with placement of corrugated drains and a pleural chest drain, with a delayed mediastinoscopy and mediastinal drain placement. We advocate a conservative approach with limited debridement and emphasis on drainage of infection in line with published case series.

The article reports on the clinical case of a patient with a diagnosis of "Stafne cyst", who was treated in the oncology department No. 8 of the Pavlov PSPbSMU. The features of differential diagnosis and diagnostic measures necessary to clarify the diagnosis are given. The situations when it is possible to avoid additional traumatic tests, as well as indications for the choice of surgical intervention or the possibility of refusing it, are analyzed. The presented data can be useful when creating algorithms for the diagnosis and treatment of a Staph cyst and making recommendations on the timing of treatment.

AbstractTo determine the effect of COVID-19 convalescent plasma on mortality, we aggregated patient outcome data from randomized clinical trials (RCT), matched-control, case series, and case report studies. Random-effects analyses of RCT data demonstrated that hospitalized COVID-19 patients transfused with convalescent plasma exhibited a lower mortality rate compared to patients receiving standard treatments. These data provide evidence favoring the efficacy of human convalescent plasma as a therapeutic agent in hospitalized COVID-19 patients.

The aim of this paper is to report the clinical case of a 3-year and 7-month-old male child with microcephaly who had multiple eruption cysts, and to discuss the adopted procedures. The diagnosis of eruption cyst was concluded based on the clinical and radiographic characteristics. The treatment proposed was a surgical incision of the eruption cysts with the patient under general anesthesia. However, due to health reasons, the patient could not have the procedure performed, and, after four months, seven of the eight eruption cysts had regressed with teeth eruption; therefore, no surgical intervention was needed. Regarding the remaining eruption cyst, the therapy adopted was his mother monitoring the condition. It is possible to personalize eruption cyst treatment by monitoring the lesion until its eventual spontaneous disappearance, without any type of intervention.

(1) Rationale for this case report: The current treatment modality for oligodendroglioma is surgical intervention and radiation treatment. This case report shows the effective management with Ayurvedic treatment of a case diagnosed as Grade II oligodendroglioma which was posted for resection and chemotherapy. The MRI results after treatment clearly suggest the significant reduction of the Grade II oligodendroglioma. (2) Presenting concerns: A 26-year young lady who was diagnosed with the Grade II oligodendroglioma was admitted in the hospital. She was posted for the excision, resection, chemotherapy and radiotherapy. The features were suggestive of Grade II oligodendroglioma has been diagnostic by MRI. (3) Interventions: Panchakarma procedures including the snehana, svedana, virechana, nasya and kayaseka was started along with internal medications and surgery was deferred because the patient responded positively. (4) Outcomes: The outcome of this case study reveals that there was significant reduction of the Grade II oligodendroglioma. (5) Main lesson(s) from this case report: Non-surgical intervention of Grade II oligodendroglioma demonstrating the reduction. Multiple clinical trials should be conducted to establish this treatment as general treatment for the oligodendroglioma.

The lip brake is a fold in the oral mucosa, originating in the upper lip and insertion in the alveolar mucosa. At birth, the labial frenum may be inserted in the palatal papilla. As the individual grows, he can migrate to a more upper region, featuring a normal lip curl. However, when it remains inserted in the papilla, it is a case of hypertrophic lip curl. When pathological, this structure can promote the formation of interincisal diastema, periodontal pocket, gingival retraction, aesthetic problems, difficulties in phonation and brushing. The objective of this article is to report a clinical case of upper lip frenectomy using the double clamping technique or Archer technique, performed at the Oral Minor Surgery clinic of the Dentistry School of the Federal Fluminense University, in a male patient, leucoderma, 23 years old, indicated by his orthodontist after recurrence of interincisal diastema. From the procedure performed, it can be concluded that the surgical intervention and the applied technique were effective in solving the reported clinical case.
