ABSTRACT Objectives: The aim of this study was to compare the canal debridement capabilities of three single file systems, ProTaper, and K-files in oval-shaped canals. Materials and Methods: Seventy-five extracted human mandibular central incisors with oval-shaped root canals were selected. A radiopaque contrast medium (Metapex; Meta Biomed Co. Ltd., Chungcheongbuk-do, Korea) was introduced into the canal systems and the self-adjusting file (SAF), WaveOne, Reciproc, ProTaper, and K-files were used for the instrumentation of the canals. The percentage of removed contrast medium was calculated using pre- and post-operative radiographs. Results: An overall comparison between the groups revealed that the hand file (HF) and SAF groups presented the lowest percentage of removed contrast medium, whereas the WaveOne group showed the highest percentage (P < 0.001). The ProTaper group removed more contrast medium than the SAF and HF groups (P < 0.05). Conclusions: None of the instruments was able to remove the contrast medium completely. WaveOne performed significantly better than other groups.

It is well described that the presence of microbial flora in the pulp space of the tooth, and the inability of the immune system to remove these pathogens, are the major sources of peri-apical and radicular inflam-mation.1 Eradication of these pathogens from the pulp and root canal space by means of cleaning, shaping, disinfecting and complete obturation is necessary to safeguard the health of the periodontal tissues from endodontic infection and subsequent breakdown.2,3 The basic objectives of cleaning and shaping of root canals include: (1) removal of all infected soft and hard tissues; (2) creating space for delivery of disinfectants and medicaments to the apical part of the canal; (3) facilitating three dimensional obturation and (4) preservation of radicular structures.4 Even modern endodontic file systems leave untouched areas on the root canal walls after preparation and show compaction of hard tissue debris.5 This debris consists of pulp tissue remnants, bacteria and dentine chips of which most is found in the apical part of the prepared root canal system.6 Aiming for a centred preparation that corresponds to the original canal anatomy accompanied by the lowest amount of canal transportation, especially in middle and apical parts of curved canals, will result in the most favourable post instrumented canal shape. The four optimal canal shaping objectives are: (1) to have a tapered funnel from orifice to apex, (2) maintenance of original anatomical canal pathway, (3) apical foramen position should remain constant and (4) leaving the apical opening as small as possible.2,7

Aim: T assess the root canal transportation caused by these rotary NiTi reciprocating file systems using 3D cone beam computed tomography Method: This experimental study was conducted on 60 samples of rotary NiTi reciprocating file systems using CBCT. The extent of canal transportation and the centering ratio was analyzed using independent sample t test. The significance level was set at 95%. Results: Mean apical canal transportation was lower in wave one Gold groups as compare to Reciproc (0.8724±0.04SD vs 0.9463±0.02SD, p=0.000). Mean apical canal transportation at 5 mm was lower in wave one gold group as compare to reciproc blue group (0.5610±0.02SD vs 0.7253±0.06SD, p=0.000). Mean apical canal transportation at 8mm was lower in wave one group as compare to reciproc blue (0.358±0.03SD vs 0.5766±0.10SD, p=0.000). Conclusion: Wave one Gold had high centering ability and lower canal transportation as compare to Reciproc Blue single file system. Abstract The aim of this study was to evaluate the WaveOne Gold and One Shape New Generation systems regarding the bacterial removal from root canals infected with Enterococcus faecalis by comparing them to the conventional WaveOne and One Shape systems. Forty-eight distobuccal root canals of maxillary molars sterilized with ethylene oxide were infected with E. faecalis for 21 days, and then root canal initial bacterial sample was collected with paper cones and plated on M-enterococcus agar. The specimens were randomly divided into 4 groups according to the instrumentation: WaveOne Gold, One Shape New Generation, WaveOne and One Shape. After instrumentation, samples were collected with use of scraping and paper cones at immediate and 7 days after instrumentation. The bacterial reduction was calculated and then made intragroup analysis by Friedman test and intergroup analysis by Kruskal-Wallis with Dunn’s post-hoc test, all at 5% significance. All techniques significantly reduced the number of bacteria in the root canal (p<0.05). WaveOne Gold and One Shape New Generation promoted higher bacterial reduction than WaveOne and One Shape systems (p<0.05), but no significant difference was found between WaveOne Gold and One Shape New Generation or between WaveOne and One Shape (p>0.05). Novel single-file systems promote better bacterial removal than the conventional single-file systems.

ABSTRACT Aim This work describes clinical cases treated with a innovative single-use and single-file nickel-titanium (NiTi) system used in continuous rotation. Background Nickel-titanium files are commonly used for root canal treatment but they tend to break because of bending stresses and torsional stresses. Today new instruments used only for one treatment have been introduced. They help the clinician to make the root canal shaping easier and safer because they do not require sterilization and after use have to be discarded. A new sterile instrument is used for each treatment in order to reduce the possibility of fracture inside the canal. The new One Shape NiTi single-file instrument belongs to this group. Case description One Shape is used for complete shaping of root canal after an adequate preflaring. Its protocol is simple and some clinical cases are presented. It is helpful for easy cases and reliable for difficult canals. Conclusion After 2 years of clinical practice, One Shape seems to be helpful for the treatment of most of the root canals, with low risk of separation. After each treatment, the instrument is discarded and not sterilized in autoclave or re-used. Clinical significance This single-use file simplifies the endodontic therapy, because only one instrument is required for canal shaping of many cases.
