Dynamics of morbidity and risk factors affecting the quality of life of patients with benign prostatic hyperplasia

The article discusses the dynamics of morbidity and risk factors that influence the occurrence of benign prostatic hyperplasia (BPH). An increase in the number of diseases of the genitourinary system, among which prostate diseases account for about 25%, is an important medical and social problem, as the symptoms of BPH significantly reduce the quality of life of men. The main parameters of the quality of life of patients with BPH include not only the severity of the signs of the disease, but also the mental well-being and self-awareness of physiological and psychological well-being. Early diagnosis and elimination of risk factors for the development of BPH makes it possible to start timely treatment. Among surgical approaches, transurethral methods are currently the most common: transurethral resection, transurethral enucleation with a bipolar loop and bipolar vaporesection of prostate.

The article considers the risk factors affecting the occurrence of benign prostatic hyperplasia (BPH) and the quality of life of patients with this type of pathology. World statistics indicate that BPH is a disease of mainly elderly and senile men. The main reason leading to the development of BPH is a change in the hormonal background of males. BPH is characterized by symptoms that negatively affect the quality of life of men, causing physical and psychological discomfort. This pathology is accompanied by squeezing of the bladder, which contributes to the appearance of frequent urge to empty it, erectile dysfunction, a decrease or complete absence of sexual desire, which often leads to a decrease in self-esteem and depression. Among the main risk factors that cause BPH controllable and uncontrollable ones are distinguished. Elimination of the influence of controllable risk factors that cause BPH leads to a decrease in the rate of progression of the disease, allows timely initiation of treatment, which has a positive effect on the quality of life of men with this diagnosis.

One of the key concepts of modern medicine is the quality of life associated with health, which is influenced by the initial underlying disease, its timely diagnosis and quality treatment, as well as rehabilitation, further prevention and identification of complications that have arisen. The conditions and lifestyle of the patient play an important role, not only his physical health, but also mental health. Despite the fact that benign prostatic hyperplasia (BPH) is not a life-threatening disease, its main clinical signs are symptoms of dysfunction of the lower urinary tract, which to a greater extent cause discomfort to patients and significantly reduce their quality of life. One of the triggering factors for the development of prostate cancer is neglected cases of BPH progression. The occurrence of prostate cancer, as a consequence of BPH, undoubtedly worsens the patient's quality of life and requires timely diagnosis, detection and immediate treatment. Based on the literature data, the article analyzes the risk factors for the occurrence and the main methods of treatment for BPH and prostate cancer, which negatively affect the patient's quality of life.

Objective: In the last years there is a growing interest in nutraceutical substances that seems able to improve clinical symptoms in patients with lower urinary tract symptoms (LUTS) due to benign prostatic hyperplasia (BPH). In this paper, we evaluated both efficacy and safety of a combination of daidzein with isolase and zinc in patients with LUTS due to BPH. Materials and methods: In a phase I-II study clinical trial we enrolled patients with clinical and instrumental diagnosis of LUTS associated to BPH that received a six-month treatment with a combination of daidzein with isolase and zinc (1 tablet/day). Clinical, laboratory and instrumental analyses were carried out at the time of admission (T0) and 6 months after the ending of the treatment (T1). The Italian version of International Prostatic Symptom Score (IPSS), International Index of Erectile Function (IIEF-5) and Quality of Well-Being (QoL) questionnaires were used. The development of adverse drug reactions (ADRs) and drug interactions (DDIs) were recorded using the Naranjo scale and drug interaction probability scale. Student’s t test and Anova test were used for statistical analysis, and the threshold of statistical significance was set at P < 0.05. Results: We enrolled 71 patients, 62 (87.3%) completed the follow-up and we documented a significant differences between T0 and T1 in terms of IPSS [21.5 ± 1.2 vs 16.2 ± 1.5; (-4.8); p < 0.001], Cmax [9.7 ± 3.7 vs 15.3 ± 2.5; (+5.6); p < 0.001] and QoL [0.56 ± 0.15 vs 0.84 ± 0.19; (+0.28); p < 0.001]. In contrast, no significant difference were recorded in terms of IIEF-5 [p = 0.50] and PSA [p = 0.67]. Finally, we did not record any significant ADRs or DDIs during the study. Conclusions: In this study, we documented that a combination of daidzein with isolase and zinc, reduces the clinical symptoms of LUTS and improves the quality of life in patients with BPH, without the development of ADRs or DDIs.

The purpose of this study is to identify factors that affect health-related quality of life (HRQOL) of older patients with Benign Prostatic Hyperplasia (BPH) and suggest ways to improve the same. Through this, we will improve the self-management practice of patients and promote the treatment of BPH in older patients. The 2015 Korea Health Panel Survey data were used in this study. A total of 422 BPH patients aged 65 or older were included. Logistic regression analysis was conducted to identify factors affecting the HRQOL of older patients with BPH. General characteristics of factors affecting older patients with BPH included income level and type of insurance. In addition, among medical-related characteristics and health behavior factors, subjective health status, unmet medical care needs, moderate physical activity, sitting time, and drinking influenced the HRQOL. Therefore, in order to improve the HRQOL of adult patients with BPH, it is necessary to improve medical accessibility by strengthening primary care. In addition, it is necessary to increase the amount of activity in daily life through healthcare medical devices.
