CLINICAL FEATURES AND THE DEFORMITIES OF NASAL SEPTUM IN CHRONIC RHINOSINUSITIS PATIENTS

Objectives: To study clinical features and the deformities of nasal septal in chronic rhinosinusitis patients and the relationship between nasal septal deformities (NSDs) and chronic rhinosinusitis. Patients and method: The research including 94 patients over 16-year-old diagnosed NSDs with nasosinusal syndromes who underwent nasal endoscopic and sinus CT scan from April 2017 to May 2018, by cross sectional descriptive study. Results: Main funtional symtoms are nasal obstruction 96.8%, nasal discharge 89.4%, headache 71.3% and hyposmia 42.6%. The most prevalent morphologies of nasal septal is deviation (51.0%); NSDs in area 4.5 of the nasal cavity according to Cottle are more common than area 1,2,3 (68.1%); The level of nasal septal deviation caused about two-third obstruction of the nasal cavity is most common (55.3%). There was a statistically significant relationship between the level of nasal septal deviation and the severity of chronic rhinosinusitis based on clinical features, CT scan and the sides of sinusitis: the more obstruent NSDs caused in nasal cavity, the heavier gravity of chronic rhinosinusitis had and the rate of bilateral sinusitis more increased. Conclusion: We found the relationship between the morphology of nasal septal deformities and the severity of chronic rhinosinusitis based on clinical features, the relationship between the gravity of nasal septal deviation and the severity of chronic rhinosinusitis based on clinical features, CT scan and the sides of sinusities. <p class="abstract"><strong>Background:</strong> The intention of this study was to evaluate angles of nasal septal deviation by measuring angle of nasal septal deviation on computerized tomography of the para nasal sinuses (CT PNS) and to study the effect of increasing septal angle on chronic sinusitis. No other study in the literature has measured exact angle of septal deviation in terms of degrees.</p><p class="abstract"><strong>Methods:</strong> A prospective cross-sectional observational study was carried out from September 2015 to August 2017 and 57 patients with diagnosis of having chronic sinusitis, were evaluated with CT PNS (coronal and axial views).  </p><p class="abstract"><strong>Results:</strong> Most patients of chronic rhinosinusitis had septal deviation in grade III, followed by grade II, then grade I.</p><p class="abstract"><strong>Conclusions:</strong> This study concludes that higher the angle of septal deviation, higher the incidence of CRS and vice versa.</p><p> </p>

AbstractBackground:The reliability of nasal obstruction measurements could be improved, and several new techniques are being developed. Our objective was to investigate the use of a new software program, Odiosoft-Rhino, in the assessment of nasal obstruction via analysis of the sounds of nasal expiration.Methods:We compared the nasal symptom scores and Odiosoft-Rhino and acoustic rhinometry test results for 61 patients with known nasal septal deviation.Results:We found a significant difference, and a correlation, between Odiosoft-Rhino results at 2000–4000 Hz and 4000–6000 Hz intervals, and the minimal cross-sectional area 2.2 cm from the nostril, in the right nasal cavity in patients with right-sided deviations. Similar results were observed for the left nasal cavity in patients with left-sided deviations.Conclusions:The Odiosoft-Rhino software test is noninvasive, requires minimal cooperation and experience, and provides results that can be saved as digital data. Additionally, data from the Odiosoft-Rhino test are strongly correlated with acoustic rhinometry results and visual analogue scores of nasal obstruction. It seems that sound intensity within the 2000–4000 Hz and 4000–6000 Hz intervals is more sensitive than other sound intensity intervals. Thus, we speculate that Odiosoft-Rhino testing could be used as a new diagnostic method in order to evaluate nasal airflow in clinical practice.

The aim of the present study was to assess the clinical utility of acoustic rhinometry (AR) compared with active anterior rhinomanometry (AAR) in the evaluation of nasal patency in subjects with nasal septal deviation. Fifty patients were divided into three groups based upon the part of the nasal cavity where the septal deviation was situated (anterior: up to 2.5 cm; middle: between 2.5 and 4.5 cm; posterior: between 4.5 and 8 cm measured from the columella). The control group consisted of 15 subjects with no nasal complaints and no history of nasal disease. Inspiratory and expiratory nasal airway resistance (NAR) at 75 Pa and at 150 Pa before and after decongestion were measured by AAR. Minimal Cross-sectional Area (MCA), distance of MCA, and nasal volume (Vol) were measured before and after decongestion by AR (Rhino 2000). Subjective nasal patency was assessed by Visual Analogue Score (VAS). In the statistical analysis the deviated unilateral nasal cavities were compared with the randomly chosen unilateral nasal cavities of normal subjects. Both techniques AR and AAR were sufficiently sensitive to reveal severe deviations in the anterior nasal cavity (MCA, Volant, NAR75, NAR150, p < 0.05). The techniques were less sensitive in cases of middle and posterior deviations (MCA, Volmid, Volpost, MCAmid, MCApost, NAR75, p > 0.05). The nondecongested inspiratory and expiratory NAR at 150 Pa were the only parameter that differed from normal in cases of posterior deviations. The VAS correlated better with NAR than with MCA. MCA correlated more frequently with expiratory than with inspiratory NAR.

Background: The aim of this study was to evaluate the effect of nasal septal deviation (NSD) on ethmoid cell volume and to determine whether there was any correlation between NSD grade and ethmoid cell volume.Methods: Forty computerized tomography (CT) scans from patients with rhinosinusitis symptoms with NSD were evaluated. Septal deviations were classified into three groups according to the degree of deviation on CT. Ethmoid cell volumes were measured and the relationship between NSD and ethmoid cell volume was investigated.Results: There was a moderate but significant negative correlation between the septal deviation angle and the percentage of the ethmoid cell volumes (p = 0.001, r = −0.5152, r2 = 0.2654). Total ethmoid cell volume on the ipsilateral side compared with the contralateral side was found to decrease as the degree of NSD increased.Conclusions: Nasal septal deviation affects the total ethmoid cell volume of the nasal cavity. The results of our study underline the role of ethmoid cell volume in the compensation mechanism equalizing the nasal cavity airflow changes due to NSD.

Study Design: A retrospective observational study. Objective: To clarify the association of the paraspinal muscle area and composition with clinical features in patients with cervical ossification of the posterior longitudinal ligament (OPLL). Methods: Consecutive patients with cervical OPLL who underwent cervical magnetic resonance imaging (MRI) before surgery were reviewed. The cross-sectional area (CSA) and fatty infiltration ratio (FI%) of deep posterior cervical paraspinal muscles (multifidus [MF] and semispinalis cervicis [SCer]) were examined. We assessed the association of paraspinal muscle measurements with the clinical characteristics and clinical outcomes, such as Neck Disability Index (NDI) score. Moreover, we divided the patients into 2 groups according to the extent of the ossified lesion (segmental and localized [OPLL-SL] and continuous and mixed [OPLL-CM] groups) and compared these variables between the 2 groups. Results: 49 patients with cervical OPLL were enrolled in this study. The FI% of the paraspinal muscles was significantly associated with the number of vertebrae ( ρ = 0.283, p = 0.049) or maximum occupancy ratio of OPLL ( ρ = 0.397, p = 0.005). The comparative study results indicated that the NDI score was significantly worse (OPLL-SL, 22.9 ± 13.7 vs. OPLL-CM, 34.4 ± 13.7) and FI% of SCer higher (OPLL-SL, 9.1 ± 1.7% vs. OPLL-CM, 11.1 ± 3.7%) in the OPLL-CM group than those in the OPLL-SL group. Conclusions: Our results suggest that OPLL severity may be associated with fatty infiltration of deep posterior cervical paraspinal muscles, which could affect neck disability in patients with cervical OPLL.

The relationship among role strain, coping, and marital satisfaction is examined in a cross-sectional sample of 73 stepparents. New information is discussed pertaining to differential types of role strain identified in stepparent groups. The association between types of role strain and differential types of coping strategies and coping resources is analyzed. Results document the association between coping resources of self-esteem and self-efficacy and certain dimensions of role strain. An inverse relationship between role strain and marital satisfaction is noted. Based on study results, the authors identify vulnerable subgroups and suggest implications for practice with stepfamilies.

Background: Deviation of the nasal septum (DNS) refers to the convexity of the septum to one side disturbing the nasal physiology with obstructed nasal breathing leading to lateral nasal wall abnormalities and paranasal sinuses (PNS) mucosal disease. Knowledge of nasal morphological parameters plays an important role in planning successful nasal surgery. Our aim was to evaluate the angle of septal deviation (ASD) on CT scan and study its influence on the lateral nasal wall abnormalities and PNS mucosal disease.Methods: A prospective cross-sectional observational study was conducted on 130 patients with clinical evidence of DNS and chronic sinusitis. The direction and severity of DNS was recorded on CT scan along with evaluation of lateral nasal wall and sinus mucosal abnormalities.Results: Increasing ASD had statistically significant correlation with the lateral nasal wall abnormalities, most commonly, contralateral middle and inferior turbinate hypertrophy (p-value <0.0001). No significant association was found with the incidence of ipsilateral or contralateral osteomeatal complex (OMC) obstruction and sinus mucosal disease.Conclusions: The direction and severity of septal deviation has significant impact on contralateral middle and inferior turbinate hypertrophy. The analysis of these ancillary pathologies can be of great help to the surgeon in better management of patients with nasal obstruction.

Abstract: Increase of interleukin 6 (IL-6) level occurs in the brain after traumatic brain injury (TBI), however, studies about IL-6 as a prodictor based on CT-scan is still limited. This study was aimed to evaluate the relationship between serum IL-6 level and CT Marshall classification in patients with severe TBI. This was an observational study with a cross sectional design. There were 20 patients with severe TBI admitted at the Emergency Surgery Installation of Prof. Dr. R. D. Kandou Hospital Manado in this study. CT-scan was performed on them to determine the CT Marshall classification and to categorize the hemorrhage location (extra-axial, intra-axial, both), hemisphere (midline/diffuse, dextral, sinistral), and area (frontal, parietal, temporal, occipital, multiple). Venous blood sample used for IL-6 examination was drawn less than 24 hours after trauma. The results showed that mean IL-6 level was 22.0060 pg/mL (SD 4.64494 pg/mL). Patients were distributed relatively uniform in 4 detected categories (diffuse injury II, III, V, and VI) of CT Marshall classification. Final regression model consisted of IL-6, age, and temporal injury as predictors. The Spearman coefficient correlation showed rs = -0.005 (P=0.491). Conclusion: There was no significant relationship between serum Il-6 level and CT Marshall classification, albeit, both of them increased consistantly following the severity of TBI and could be potential predictors to determine the prognosis of severe TBI patients.Keywords: IL-6, CT Marshall, severe TBIAbstrak: Pada cedera otak berat akibat trauma (COBT) terjadi peningkatan ekspresi IL-6 di otak namun penelitian mengenai kemampuannya untuk memrediksi hasil berdasarkan klasifikasi CT scan masih terbatas. Penelitian ini bertujuan untuk mengevaluasi hubungan antara kadar IL-6 serum dan klasifikasi CT Marshall pada pasien dengan COBT. Jenis penelitian ialah observasional dengan desain potong lintang. Hasil penelitian mendapatkan 20 pasien yang dirawat dengan COBT di IRDB RSUP Prof. Dr. R. D. Kandou Manado. CT-scan segera digunakan untuk menentukan klasifikasi CT Marshall dan untuk mengategorikan lokasi (ekstra-aksial, intra-aksial, keduanya), belahan (garis tengah/difus, dekstra, sisnitra), dan area otak yang terlibat dalam cedera. Sampel darah vena untuk IL-6 diambil kurang dari 24 jam setelah trauma. Hasil penelitian mendapatkan rerata kadar IL-6 22,0060 pg/mL (SD 4,64494 pg/mL). Pasien didistribusikan relatif seragam dalam empat kategori yang terdeteksi (difus cedera II, III, V, dan VI) dari klasifikasi CT Marshall. Model regresi akhir terdiri dari IL-6, usia, dan cedera pada area temporal sebagai prediktor. Korelasi antara kadar IL-6 serum dan klasifikasi CT Marshall dianalisis dengan koefisien korelasi Spearman dan mendapatkan rs = -0,005 (P=0,491). Simpulan: Walaupun secara statistik tidak terdapat hubungan bermakna antara kadar Il-6 serum dan CT Marshall namun keduanya secara konsisten meningkat mengikuti COBT dan dapat menjadi prediktor potensial untuk menentukan prognosis pada pasien dengan COBT.Kata kunci: IL-6, CT Marshall, COBT

Background: Chronic rhinosinusitis (CRS) is a frequent condition that is treated by endoscopic sinus surgery (ESS) when medical treatment fails. Irritating or sensitizing airborne agents can contribute to uncontrolled CRS. A prior study showed a linear correlation between occupational exposure and the number of ESS. Methods: In this cross-sectional study we tested the hypothesis that occupational exposure is a risk for undergoing ESS. We sent questionnaires enquiring occupational exposure in patients with CRS with nasal polyps (CRSwNP) or CRS without nasal polyps (CRSsNP). An expert assessed blindly the reported work exposures to inhaled agents. The relationship between occupational exposure on undergoing ESS was analysed. Results: Among all patients who underwent ESS (n=343), 30% reported a relevant occupational exposure, which is significantly higher than the 4.8% found among CRS patients that underwent no prior sinus surgery (n=21). Besides occupational exposure, self-reported doctor-diagnosed asthma were independent variables contributing to the chance of undergoing ESS. Conclusion: In our study we confirm occupational exposure as a risk factor for uncontrolled CRS, if defined by undergoing ESS. In CRS patients with uncontrolled symptoms, despite maximal conservative therapy, the clinician should explore the possible contribution of occupational exposure.
