The aim: To optimize the treatment of CAP patients with hypertension, complicated by exudative pleuritis. Materials and methods: The study included 43 CAP patients, aged 46 to 65 years, with viral lesions along with hypertension. The average age of the patients was 52.5 ± 4.5 years. Verification of the CAP diagnosis and its formulation was performed in accordance with the order of the Ministry of Health of Ukraine No. 128 of 19.03.2007 “On the approval of clinical protocols of medical care in “Pulmonology” in all patients, pneumonia was complicated by exudative pleuritis, which was confirmed by X-ray examination. Results: The use of combination drugs, namely, torasemide with prolonged effect and tivortin aspartate in the treatment of patients with community-acquired pneumonia, complicated by exudative pleuritis, combined with hypertension led to more significant positive changes in the values of saturation, blood biochemical parameters, as well as accelerated recovery of patients, which was confirmed by the positive dynamics of X-ray examination. Conclusions: As a result of the treatment, all parameters of the quality of life of the patients were greatly improved, which resulted in a significant reduction in functional limitations and high social activity of the patients, which significantly reduced the cost of treatment.

Pneumonia in children remains one of the most serious diseases. Despite intensive study, the diagnosis and treatment still present a variety of problems. The article is aimed to identify these problems and show the ways of their solution.One of the criteria for the diagnosis of pneumonia is pulmonary infiltrate on the X-ray image. Community-acquired pneumonia is often caused by Pneumococci and mycoplasma; respiratory viruses, influenza usually serve as contributing factors for bacterial infection. The high level of hyperand underdiagnosis of community-acquired pneumonia is associated with its similarity with ARVI; the registration of general violations (signs of the severity of the disease) improves the quality of diagnosis. The clinical and radiological picture helps to distinguish pneumococcal pneumonia from pneumonia caused by mycoplasma. To improve the diagnosis of community-acquired pneumonia, it is necessary to study inflammation markers during the first examination of the patient, which is impossible in the conditions of home care. Due to the growth of resistance of pneumococci to macrolides a community-acquired pneumonia of pneumococcal (and unclear) etiology requires the use of amoxicillin in high doses and macrolides (in case of atypical pneumonia). Gentle therapy of destructive pneumonia, steroids for metapneumonic pleurisy allow avoiding invasive interventions and help to repair lung tissue.

Perioperative bleeding occupies a leading place among all surgical complications and, despite the rapid development of surgery, remains relevant to this day. In addition to an increase in mortality, bleeding can cause the development of other postoperative complications, which lead to disability of patients and to a decrease in the quality of life in all age groups. Most perioperative bleeding are caused by technical errors. This article reviews the problem of perioperative bleeding from the point of view of impaired coagulation capabilities of the body. The main etiopathogenetic features of hemostasis during the development of this complication are considered. The analysis of postoperative complications, which were directly or indirectly caused by bleeding during or after surgery, is presented. The prevalence of these complications in various areas of surgery has been demonstrated. More detailed study of the hemostasis system and the identification of predictors of hemostasis difficulties before the surgery may cause an improvement in the results of surgical treatment and reduce the number of postoperative complications and the duration of hospital stay. Accordingly, this will lead to a decrease in the cost of treatment and an increase in patient satisfaction with the medical care. In connection with the above, there is a great interest among surgeons and anesthesiologists in preventing the development of perioperative bleeding.

Introduction: An essential quality of non-ionic contrast agents is that of containing in their chemical composition, elements with high atomic weight, which determine an increasing absorption of the X-ray beam and this will cause intense radiopacity. The possibilities for radiological exploration with non-ionic contrast agents are very wide. Currently the radiological examination with contrast agent no longer constitutes a risky step in medical diagnosis. Aims: To highlight the importance of using the non-ionic contrast agents Optiray 350 and Ultravist 370 in dog’s myelography and to compare the changes induced in cerebrospinal fluid and blood biochemical constituents by the administration of those non-ionic contrast substances. Materials and Methods: To determine the influence of non-ionic preparations Optiray 350 and Ultravist 370 on cerebrospinal fluid and on some haematological parameters were included in the study a total of 10 dogs. Results: Myelographies with Optiray 350 and Ultravist 370 in dogs with severe spinal condition negatively influence biochemical and haematological blood status, being necessary to take preventive measures. Optiray 350 and Ultravist 370 administration cause a slight sensitization of liver with blood biochemical parameters return to normal within 24-48 hours. Conclusion: Non-ionic contrast agents Optiray 350 and Ultravist 370 offer a very good opacification of the subarachnoidian space, but because it causes liver sensitization, we recommend using them with caution and only after a prior check of liver function.

The majority of respiratory diseases have viral etiology, and they do not require antibacterial therapy since it does not affect the course of the disease, does not reduce the incidence of bacterial complications. The causes of tonsillitis, epiglottitis, pneumonia are bacterial pathogens, such as Streptococcus pyogenes (group A beta-hemolytic streptococcus), Streptococcus pneumoniae,Haemophilus influenza, S. aureus and Moraxella catarrhalis. Home treatment of children remains a global challenge, as it may be the cause of an unfavorable outcome in young children, and, therefore, the issues of diagnostics and rational antibiotic therapy are still relevant. Irrational use of antibiotics can promote growth of resistance of bacterial pathogens, increase the frequency of adverse reactions of therapy and increase the cost of treatment

Proposed on the basis of nondestructive testing technology, such as ultrasonic testing and X-ray detecting, image processing of hull welding based on MATLAB is a simple detecting method to detect the quality of welding. By image processing, physical dimensions, such as circumferences and shapes, as well as the intensity and texture features of welds can be concluded. Then, through numerical analysis, quality problems, including porosities, slags, undercuts, overlaps, welding defects and others, if exist, can be determined. This method decreases the cost and reduces the harm to people’s health. Experimental results showed that the detecting system can calculate and determine the major features of the welds effectively and accurately, and possesses good practical value.

The article presents the results of the analysis of judicial practice for five years (2013-2017) in civil cases related to defects of dental care. Studies have shown that the most common reasons for patients to go to court are claims to orthopedic dentists (in 42% of cases) and dental therapists (in 31% of cases). The specialty of the doctor is not of decisive importance for the occurrence of judicial risks, due to the fact that the quality of complex dental care is most often assessed (in 45% of cases). The largest amounts of payments, exceeding 92 times the cost of treatment, are determined by dentists-surgeons. The study substantiates the need to develop new assessments at the stages of treatment, criteria for assessing the achieved results of treatment and the development of the practice of insurance of professional responsibility of dentists.

Three patients with cryoglobulinemia have been treated with automated plasma exchange procedures in which the replacement fluid was autologous plasma, obtained at a previous plasmapheresis and incubated in the cold to precipitate abnormal protein. All three responded with a reduction in cryoglobulin level and an improvement in clinical manifestations of cryoglobulinemia. The cost of treatment was less than that of conventional plasmapheresis, even though the quality of the replacement fluid was superior. The term “cryoglobulinpheresis” is suggested for this treatment process.

 A study of the direct costs of treating pneumonia of children at Bach Mai hospital from October 2018 to March 2019 that the costs of hospital beds accounts shows for the highest proportion, then comes the cost of medicines and diagnostics. The cost of antibiotics is the highest in the cost of drugs, while the cost of diagnosis is the cost of testing. The cost of treatment is significantly related to the number of days of treatment, in addition, the severity or co-morbidity factors are affected only when considered individually. The support of health insurance can be up to more than 70% of the direct cost. The research results show that the cost of treatment in Viet Nam is lower than some countries in the region, the cost distribution is similar to that of some other domestic studies, but the cost of antibiotics has decreased significantly. Cystic fibrosis is a multi-organ genetic disease that reduces life expectancy and requires costly complex therapy. The economic burden of cystic fibrosis is determined by the cost of treatment, the conditions for optimizing medical technologies based on epidemiological knowledge of the disease course nature and the therapy effectiveness. In many countries cystic fibrosis patient registries have been created that allow to assess the quality, efficiency and optimality of medical care. Registries are formed at the regional, national, and international levels, which facilitates the collection of complete and reliable patient information for subsequent analysis, comparison and synthesis of clinical and epidemiological indicators. The review is devoted to the actual principles of organization of domestic and some foreign cystic fibrosis patient registries. The article selectively provides examples of information from registries. The application of positive experience of the work of foreign registries in Russia can improve the quality of medical care for patients with cystic fibrosis.

Mini C-arm image intensifiers are used commonly in surgery of the upper limb. With relatively low doses of emitted ionising radiation, portability and superior quality of image, they are a useful aid to the operating surgeon. However, these benefits are not so often used outside the theatre setting. This paper examines the use of a mini C-arm image intensifier in the out-patient clinic and presents an audit of 100 consecutive out-patients. We reviewed the potential benefits and effects on their care pathway. We also look at the specific radiation protection issues of the mini C-arm image intensifier in the out-patients clinic. We believe use of the mini C-arm image intensifier in the out-patient setting may speed treatment and reduce the cost of treatment.
