The Role of Proprioceptive Training as a Method of Functional Rehabilitation of Ankle Sprains in Futsal Players

Each sports branch is characterized by an increasingly fierce competition on the way to performance, the need to overcome the adversary involving from the athletes an intense effort of speed, strength, endurance, ability to concentrate under stress conditions, all these demands increasing the chances of occurrence of the injuries. The study follows the importance of the implementation of proprioceptive training as a component part of the functional rehabilitation program, thus highlighting itsbeneficial effects: diminishing the symptoms, increasing joint mobility and muscle strength, regaining static and dynamic stability, essential parameters of sportsmen’s reintegration. The following tests were applied to the subject: joint assessment, muscle testing, Single Leg Balance Test (SLB), Single Legged Hop Test (SLH), 6 Meter Timed Hop Test, Star Excursion BalanceTest (SEBT) and evaluation of symptomatology and gait using The Foot and Ankle Disability Index (FADI) Score and Sports Module questionnaire. After applying the rehabilitation program, we found that the subject developed a significant improvement of thebalance on unstable surfaces (trampoline, balance board), maintaining the position in unipodal support for a duration of 4 times greater than after the initial evaluation. Also, the speed of execution for the jumps forwards over the distance of 6 meters was improved, the time difference between testing with the healthy limb (4.39 seconds) and that with the affected limb (4.67 seconds) beingbelow 10%. Therefore, we can conclude that the recovery program led to the elimination of joint instability, the results obtained representing essential indices of functional performance

ABSTRACT Introduction: Ankle sprains are recurrent injuries in basketball, hence more and more athletes are taping their ankles to promote joint stability, aiming at improving dynamic balance and, consequently, functional performance. Objective: To verify the effects of elastic and rigid athletic taping on the functional performance and level of comfort of basketball players with chronic ankle instability. Methods: Twenty-one athletes aged between 18 and 30 years (mean age 23.7 ± 3.2) with chronic ankle instability (CAI), verified using the Cumberland Ankle Instability Tool, were selected to take part in this study. The Star Excursion Balance Test (SEBT) and the Figure-of-8 hop test (F8) were applied unilaterally to assess functional performance, considering the ankle of greater instability in three situations: without athletic taping, with rigid athletic taping and with elastic athletic taping. A draw was held to determine the order in which the tests (held over a number of days) would be applied. A questionnaire was conducted to assess comfort on the same day the athletic tapes were applied. Results: There was no significant difference between the tests in any direction of the SEBT, but there was a significant difference in F8 between the rigid athletic taping x control and elastic athletic taping x control situations. In addition, the elastic athletic tape was considered significantly more comfortable than the rigid athletic tape. Conclusion: Athletic taping appears to effectively improve the dynamic balance and functional performance of athletes with CAI only in activities that cause considerable joint stress, as is the case in F8. Elastic athletic tape appears to be just as effective as rigid athletic tape in these situations, in addition to being a significantly more comfortable alternative. Levef of Evidence I; High quality randomized trial with statistically significant difference or no statistically significant difference but narrow confidence intervals.

Ankle sprain is a commonly recurring sports injury. This study aimed to compare the rehabilitation effects of resistance and balance training programs in patients with recurrent ankle sprain. Patients with recurrent lateral ankle sprain completed a home-based rehabilitation program comprising resistance training (RT; n = 27) or balance training (BT; n = 27). RT consisted of exercises using elastic tube bands, and BT consisted mainly of exercises performed using a variety of balance tools. Exercises were performed for 6 weeks, twice a day for 20 min, 5 days per week. Isokinetic eversion strength, Y-Balance test and hop tests, and foot and ankle outcome score (FAOS) were evaluated. Both RT and BT significantly improved strength and dynamic balance (p < 0.05). Compared to RT, BT also significantly improved the outcome of the crossover hop test (p = 0.008). The changes reflected group and time in pain (p = 0.022), sports (p = 0.027), and quality of life (p = 0.033) of FAOS were significantly greater in BT than RT.

Introduction: Chronic ankle instability (CAI) is characterised by a subjective feeling of recurrent instability, continuedepisode of giving way and self-reported disability. A wealth of literature is available reporting causes and risk factors related to CAI,still thereis a lack of understanding regarding rehabilitation procedures. Aim: To compare effect of BOSU ball and wobble board along with strength training on muscle strength, dynamic balance, agility and functional performance in recreational runners with CAI. Materials and Methods: It was an experimental study design conducted at Physiotherapy OPD of SGT Hospital, Gurugram. Data collection was done from July 2019 till October 2019. Analysis and report writing took 2 months i.e from November 2019 till December 2019. 60 male runners in the age group of 18-35 years with CAI were randomly divided into two groups i.e. A (Wobble board) and B (BOSU Ball) with 30 players in each group. All subjects were evaluated for ankle muscle strength, agility, dynamic balance and functional performance as measured by Manual muscle tester, T-test, Y balance test and figure of eight hop test respectively on day one and last day of 3rd and 6th week of intervention. SPSS software version 20.0 was used to analyse result. Mean and Standard deviations were calculated. ANOVA and Student T test was used for analysing inter group differences. Results: The statistical findings in this study postulate that both groups had significantly improved fitness and skill related variables but BOSU ball group showed highly significant improvement compared to wobble board group (p<0.05). Conclusion: The current results support that sensorimotor training can be progressed in difficulty by systematically reducing the base of support with help of BOSU ball.

Background: Despite the potential benefits of virtual reality technology in physical rehabilitation, only a few studies have evaluated the efficacy of this type of treatment in patients with neck pain.Objective: The aim of this study was to compare the effects of virtual reality training (VRT) versus conventional proprioceptive training (CPT) in patients with neck pain.Methods: Forty four participants with nonspecific chronic neck pain were randomly assigned to VRT or CPT in this assessor-blinded clinical trial. A novel videogame called Cervigame® was designed for VRT. It comprises of 50 stages divided into unidirectional and two-directional stages ordered from easy to hard. CPT consisted of eye-follow, gaze stability, eye-head coordination and position and movement sense training. Both groups completed 8 training sessions over 4 weeks. Visual analogue scale score, neck disability index and Y-balance test results were recorded at baseline, immediately after and 5 weeks post-intervention. Mixed repeated measure ANOVA was used to analyze differences between mean values for each variable at an alpha level of 0.05.Results: There were significant improvements in all variables in both groups immediately after and 5 weeks after the intervention. Greater improvements were observed in the visual analogue scale and neck disability index scores in VRT group, and the results for all directions in Y-balance test were similar in both groups. No side effects were reported.Conclusion: Improvements in neck pain and disability were greater in VRT than CPT group. Cervigame® is a potentially practical tool for rehabilitation in patients with neck pain.

Abstract Background CAPOS syndrome (cerebellar ataxia, areflexia, pescavus, optic atrophy, and sensorineural hearing loss) is a rare congenital autosomal dominant disorder. The resulting neurological sequelae of impairments are progressive in nature and may interfere with functional independence, performing activities of daily living (ADL’s), and subsequently, affecting the quality of life (QOL). Since it is an extremely rare disorder, there is a severe dearth in the literature about how specific physiotherapy interventions may affect their functional status. Therefore, our objective was to investigate the effects of proprioceptive neuromuscular facilitation (PNF) and Frenkel’s coordination exercises on functional recovery in a patient with CAPOS syndrome. Case presentation We herein present a case of a 25-year-old Indian male with complaints of generalized body weakness, difficulty visualizing distant objects, nystagmus, progressive sensorineural deafness, and ataxia. He was rehabilitated with a structured/customized physiotherapy protocol consisting of PNF approach and coordination exercises for 4 weeks, 6 days/week, 60 min daily. An improvement in overall functional performance of patient as per post-intervention scores of manual muscle testing, trunk control measurement scale, functional independence measure (components of self-care, transfers, and locomotion), and decline in severity of ataxia on scale for assessment and rating of ataxia scale was observed. Conclusion PNF and Frenkel’s exercises resulted in an improvement in overall functional performance of the patient. Improvement was observed in post-test scores of Manual Muscle Testing (MMT), Trunk Control Measurement Scale (TCMS), and Functional Independence Measure (FIM) for the components of self-care, transfers, and locomotion. Additionally, results also showed a decline in severity of ataxia on post-test scores of scale for the assessment and rating of ataxia (SARA) scale (i.e., from severe to moderate).

The main objectives of this study were to evaluate the short-term effects of resisted sprint and plyometric training on sprint performance together with lower limb physiological and functional performance in collegiate football players. Ninety collegiate football players participated in this three-arm, parallel group randomized controlled trial study. Participants were randomly divided into a control group and two experimental groups: resisted sprint training (RST) (n = 30), plyometric training (PT) (n = 30), and a control group (n = 30). Participants received their respective training program for six weeks on alternate days. The primary outcome measures were a knee extensor strength test (measured by an ISOMOVE dynamometer), a sprint test and a single leg triple hop test. Measurements were taken at baseline and after 6 weeks post-training. Participants, caregivers, and those assigning the outcomes were blinded to the group assignment. A mixed design analysis of variance was used to compare between groups, within-group and the interaction between time and group. A within-group analysis revealed a significant difference (p < 0.05) when compared to the baseline with the 6 weeks post-intervention scores for all the outcomes including STN (RST: d = 1.63; PT: d = 2.38; Control: d = 2.26), ST (RST: d = 1.21; PT: d = 1.36; Control: d = 0.38), and SLTHT (RST: d = 0.76; PT: d = 0.61; Control: d = 0.18). A sub-group analysis demonstrated an increase in strength in the plyometric training group (95% CI 14.73 to 15.09, p = 0.00), an increase in the single leg triple hop test in the resisted sprint training group (95% CI 516.41 to 538.4, p = 0.05), and the sprint test was also improved in both experimental groups (95% CI 8.54 to 8.82, p = 0.00). Our findings suggest that, during a short-term training period, RST or PT training are equally capable of enhancing the neuromechanical capacities of collegiate football players. No adverse events were reported by the participants.

Context: Proprioceptive training on compliant surfaces is used to rehabilitate and prevent ankle sprains. The ability to improve proprioceptive function via such training has been questioned. Achilles tendon vibration is used in motor-control research as a form of proprioceptive stimulus. Using measures of postural steadiness with nonlinear measures to elucidate control mechanisms, tendon vibration can be applied to investigate the underlying rationale of proprioceptive training. Objective: To test whether the effect of vibration on young adults' postural control depended on the support surface. Design: Descriptive laboratory study. Setting: Research laboratory. Patients or Other Participants: Thirty healthy adults and 10 adults with chronic ankle instability (CAI; age range = 18−40 years). Intervention(s): With eyes open, participants stood in bilateral stance on a rigid plate (floor), memory foam, and a Both Sides Up (BOSU) ball covering a force platform. We applied bilateral Achilles tendon vibration for the middle 20 seconds in a series of 60-second trials and analyzed participants' responses from previbration to vibration (pre-vib) and from vibration to postvibration (vib-post). Main Outcome Measure(s): We calculated anterior-posterior excursion of the center of pressure and complexity index derived from the area under multiscale entropy curves. Results: The excursion response to vibration differed by surface, as indicated by a significant interaction of P &lt; .001 for the healthy group at both time points and for the CAI group vib-post. Although both groups demonstrated increased excursion from pre-vib and from vib-post, a decrease was observed on the BOSU. The complexity response to vibration differed by surface for the healthy group (pre-vib, P &lt; .001). The pattern for the CAI group was similar but not significant. Complexity changes vib-post were the same on all surfaces for both groups. Conclusions: Participants reacted less to ankle vibration when standing on the BOSU as compared with the floor, suggesting that proprioceptive training may not be occurring. Different balance-training paradigms to target proprioception, including tendon vibration, should be explored.
