Background: We conducted a study of osteoarticular tuberculosis in patients from private and public settings in a disease endemic area. Our objective was to assess the role of mycobacterial culture and polymerase chain reaction (PCR) in the diagnosis of osteoarticular tuberculosis (TB) in settings where only clinical and imaging diagnosis form the basis for treatment. Methodology: Ninety-three consecutive specimens collected from clinically suspected patients of osteoarticular TB were screened for bacterial culture, mycobacterial culture and in-house nested PCR. In addition, specimens were examined by imaging and histopathology. Ten specimens collected from patients suffering from other bone diseases were included as negative controls. Results: Of the 93 clinically suspected TB patients, mycobacterial culture was positive for Mycobacterium tuberculosis (MTB) in 47 (51%) patients who were confirmed as definite TB cases. Of the remaining patients, 16 (17%) were diagnosed as probable, 19 (20%) as possible, and 11 (12%) as only clinically suspected TB cases. In-house nested PCR was positive in 65 (70%) cases. Fifteen patients were resistant to one or more anti-tuberculous drugs; twelve patients were multi-drug resistant, two of whom were extensively drug resistant. Conclusion: Mycobacterial cultures using liquid media with susceptibility should form the backbone of management of osteoarticular TB. Nested PCR enhances the sensitivity if performed in addition to culture.

In this study we aimed to determine the prevalence of Leishmania spp infection and to report the geographical distribution of Leishmania spp infected cats from Thessaly, central Greece, an endemic region for canine and human leishmaniosis. Blood samples were collected from a total of 150 cats (34 stray and 116 owned) from the area of Thessaly, i.e. Larissa (n=6), Volos (n=94), Trikala (n=30) and Karditsa (n=20). Data on signalment and living conditions were registered for each cat. The samples were tested by Internal Transcribed Spacer 1 nested PCR (ITS1 nested PCR). Geographical Information System (GIS) was used to define the geographical distribution of the Leishmania spp infected cats in relation to the land uses and the altitude. In total, 13.3% (95% CI: 8.3-19.8) of the sampled cats were found positive for Leishmania spp DNA. More precisely, the results are 12.8% (95% CI 6.8-21.2) in Volos, 20% (95% CI 6.8-21.2) in Trikala and 10% (95% CI 12-31.7) in Karditsa while none of the six cats examined from the region of Larissa was found PCR positive. The Leishmania spp infected cats were found in artificial surfaces and associated areas and in cultivated and managed areas with a mean altitude of 81,7 m above sea level (range 14 - 225 ± 51.57 SD). No significant correlation was found between Leishmania spp infection and signalment or living conditions, thus suggest ing that the infection is uniformly distributed among the screened feline population. The findings of this study along with the occurrence of competent vectors of Leishmania infantum in Thessaly, support the hypothesis that cats may serve as a reservoir for Leishmania spp in this region and highlight the need for surveillance in feline population and implementation of control measures. However, the epidemiological role of cats in the maintenance and transmission of Leishmania spp has yet to be defined and deserves further investigation.

SUMMARYAsymptomatic dogs can be potential hosts of leptospirosis. However, the extension of this phenomenon in endemic areas has not yet been clearly defined. This study is aimed at evaluating the role of asymptomatic dogs as carriers of Leptospira in an endemic area of Brazil. A total of 131 male dogs without apparent leptospirosis symptoms were included in the study based on clinical and hematologic exams. Serum and urine samples were collected for microscopic agglutination tests (MAT) and polymerase chain reactions (PCR) targeted the LipL32 gene, respectively. Forty-two dogs (32·1%) presented seroreactivity (titres ⩾100). The serogroup Icterohaemorrhagiae was predominant, representing 92·7% of the seropositive samples. Overall, leptospiral DNA was detected on 26 urine samples (19·8%). PCR positivity was more common (28·6%) on seropositive dogs than on seronegative (15·7%) ones. Nevertheless, MAT was not correlated to PCR (P > 0·05). Age was not associated with seroreactivity, but dogs older than 5 years of age had 4·07 more chances (odds ratio) of being carriers (PCR positive) than younger ones. Although the fact of knowing that asymptomatic dogs can act as leptospiral carriers is not new, the extension of this fact is impressive in an endemic region, and its role and impact on public health cannot be neglected.

Clamydophila psittaci (C. psittaci) has been proposed to be an etiologic factor in extranodal marginal zone lymphoma of mucosa-associated lymphoid tissue (MALT lymphoma) in the ocular adnexa. However, the pathogenetical significance of the infection has not been fully elucidated. Many previous studies have shown controversial results regarding C. psittaci detection rates in said patients, ranging from 0 to 87%. We investigated the presence of C. psittaci in a single institutional cohort (n = 150) of ocular adnexal MALT lymphoma (OAML) patients in Korea. We tried to exclude the methodological biases derived from the different primer sets in polymerase chain reaction-based studies. For that reason, we applied five sets of primers, including four previously reported and one newly designed primer set. There was no case of C. psittaci-positive OAML in repeated trials validated with appropriate positive and negative controls. All 150 cases showed negative results with five primer sets. These results suggest that the pathogenetic role of C. psittaci in ocular adnexal MALT lymphoma might have been overestimated to date, at least in the Korean population. Therefore, the molecular diagnosis of C. psittaci is considered a very low priority.

Background and Aims: Rhinosinusitis is an inflammation of the mucous membrane that may be caused by infectious agents such as bacteria, fungi and viruses. Few studies have been carried out on the role of viruses in Rhinosinusitis patients .The aim of this study was the molecular detection of Adenoviruses in sinus tissues by nested polymerase chain reaction (PCR) in Shiraz.Materials and Methods: In the present study, 103 paraffin-embedded biopsy specimens of sinus tissues were subjected to DNA extraction and tested for adenovirus DNA using Nested PCR. The amplification of a β-globin gene by PCR-based method was used to confirm the quality of extracted DNA.Results: A total of 103 samples of sinus tissues were examined. Of these patients, 50 (48.54%) were male and the rest were female (51.46%). The patients’ age ranged between 2 and 82 years and the mean age was 42.15±1.56 years. The adenovirus DNA was detected in 13 of 103 (12.6%) samples. Conclusions: The results of this study showed that Adenoviruses have high prevalence in rhinosinusitis patients. As a results, it is an important to investigate clinical significance of viral infections especially Adeno viruses in these patients.
