The aim of this study was to explore the perspectives of occupational health practitioners (OHPs) regarding education and training of mineworkers on occupational noise induced hearing loss (ONIHL) and its impact on mineworkers’ health. Qualitative, in-depth telephonic and face-to-face interviews were conducted with 16 OHPs. Purposive and snowball sampling was utilized to recruit participants. Data were analysed using inductive thematic analysis. Three themes emerged from the data: seeing is believing, not my department, barriers and facilitators to raising awareness (with two subthemes -blame it on the language and level of education, and compensation pay-outs). Superficially, OHPs believe that mineworkers are aware of the impact of noise health, however, the OHPs are not knowledgeable on how the mineworkers are educated on ONIHL and its latent consequences. Furthermore, language, low levels of education and literacy, as well as financial constraints are factors found to affect education and training of the mineworkers about the risks of ONIHL. If the mining industry is committed in eliminating ONIHL, they should prioritize health literacy, and mines need to have an effective awareness raising plan in place at every mine. This plan should consider diversity of the workforce, including linguistic as well as educational level diversity.

Qualitative, in-depth telephonic and face-to-face interviews were conducted with 16 OHPs comprising representatives from the state, employer and labour as well as audiologists and occupational health hygienists.  Purposive and snowball sampling were utilized to recruit participants. Data were analysed using inductive thematic analysis. Findings revealed that mineworkers have a superficial awareness and knowledge of the impact of noise on their hearing and health. Moreover, OHPs are not knowledgeable on how mineworkers are educated on ONIHL and its latent consequences. Furthermore, language, low levels of education and literacy as well as the financial constraints were factors that had a negative impact on raising awareness and training mineworkers. There is a need to prioritize health literacy among mineworkers. Additionally, audiologists need to play an active role in educating mineworkers about the effects of excessive exposure to noise. There is a need to take into account literacy levels and language barriers in planning training material for mineworkers.

Background: Hearing conservation programmes (HCPs) are an important aspect of occupational health efforts to prevent occupational noise-induced hearing loss (ONIHL). In low- and middle income (LAMI) countries, where the incidence of ONIHL is significant, it is important to deliberate on the risk or benefit of HCPs.Objectives: This article is an attempt at highlighting important strategic indicators as well as important variables that the occupational health and audiology community need to consider to plan efficacious HCPs within the South African mining context.Method: The current arguments are presented in the form of a viewpoint publication.Results: Occupational audiology vigilance in the form of engagement with HCPs in the mining industry has been limited within the South African research and clinical communities. When occupational audiology occurs, it is conducted by mid-level workers and paraprofessionals; and it is non-systematic, non-comprehensive and non-strategic. This is compounded by the current, unclear externally enforced accountability by several bodies, including the mining industry regulating body, with silent and/or peripheral regulation by the Health Professions Council of South Africa and the Department of Health. The lack of involvement of audiologists in the risk or benefit evaluation of HCPs during their development and monitoring process, as well as their limited involvement in the development of policies and regulations concerning ear health and safety within this population are probable reasons for this.Conclusions: Increased functioning of the regulatory body towards making the employers accountable for the elimination of ONIHL, and a more central and prominent role for audiologists in HCPs, are strongly argued for.

Literature into exposure to occupational noise in developing countries suggests that the prevalence of occupational noise–induced hearing loss is still high. There is also evidence that the mining industry is aware of this epidemic; however, the efforts to curb occupational noise–induced hearing loss are currently unsuccessful. Therefore, the aim of this study was to explore and document current evidence reflecting trends in the management of occupational noise–induced hearing loss in the mining industry in Africa from 1994 to 2016. A systematic literature review was conducted in line with the Cochrane collaboration guidelines and Preferred Reporting Items for Systematic Reviews and Meta-Analysis. Electronic bibliographic databases such as ScienceDirect, PubMed and Scopus MEDLINE were searched. A total of 1212 titles and abstracts were identified; of which only nine papers formed part of this study. The results indicated that there is a dearth of research on the management of occupational noise–induced hearing loss in Africa. The limited research on the management of occupational noise–induced hearing loss focuses on some aspects of the hearing conservation programme pillars and not on all the pillars as suggested by some scholars in the field. Furthermore, these studies had small sample sizes thereby, minimizing their generalization. There is therefore a need for more studies on the management of occupational noise–induced hearing loss in the mining sector, as there is evidence to suggest that occupational noise–induced hearing loss in African countries is still on the rise.

Background: The South African mining industry is frequently criticized for its poor health and safety record and high numbers of fatalities, thereby prompting researchers to conduct research on challenges faced by this industry. Therefore, this study aimed to investigate the feasibility of conducting audiological research into occupational noise induced hearing loss (ONIHL) within the South African mining sector. Specific objectives involved determining ease of identifying focal persons in charge of hearing conservation programmes (HCP); establishing response time and rate of identified focal person for interviews; and exploring focal person’s willingness to share information regarding HCP. Methods: A qualitative research strategy comprising online desk research and interviews was undertaken through purposive sampling to recruit participants. Data were collected through checklists, logbooks and interviews. Deductive thematic analysis was used to analyse data. Results: Gaining access into the mining sector is negatively impacted by the following factors: firstly, contact details of focal person are not always listed on the websites. Secondly, prolonged response rate between initial contact and the time in which the participants respond. Lastly, unwillingness to share information regarding the management of ONIHL and progress made in HCP at various mines. Conclusions: Findings contribute evidence regarding possible barriers to effective and successful implementation of application of best practice in HCPs; guided by evidence that is contextually relevant. Challenges with accessing mines for research purposes by external and independent researchers arguably impacts on the available evidence produced by mines, due to a possible unavoidable conflict of interest. Success of HCPs depend on objective evidence regardless of whether it paints the mining industry in positive or negative light. It is only when this evidence is critically engaged with, that real and effective solutions can be deliberated and implemented. Clear, transparent, and open lines of communication between various stakeholders are key to achieving this.

Noise-induced hearing loss is 100% preventable if the collaborative stakeholders in the prevention process are fully committed to the process and implement effective measures timely. Audiologists have within their scope of practice the prevention of hearing loss and this needs to be at the forefront of all advocacy campaigns to prevent occupational hearing loss (OHL). In a systematic review by Moroe, Khoza-Shangase, Kanji and Ntlhakana (2018), where literature into the exposure to occupational noise in developing countries suggested that the prevalence of occupational noise-induced hearing loss (ONIHL) is still high, significant gaps in locally relevant and responsive evidence were identified. There is also evidence that the mining industry is aware of this epidemic; however, the efforts to curb ONIHL are currently unsuccessful. These authors explored and documented current evidence reflecting trends in the management of ONIHL in the mining industry in Africa from 1994 to 2016 through the use of the Preferred Reporting Items for Systematic Reviews and Meta-Analysis. Findings from this systematic review indicated that there is a dearth of research on the management of ONIHL in Africa. The limited research on the management of ONIHL focuses on some aspects of the hearing conservation programme pillars and not on all the pillars as suggested by some scholars in the field. Furthermore, they found that published studies had small sample sizes, thereby minimising their generalisation. Exposure to industrial noise and the resulting effect of occupational hearing loss is a common problem in nearly all industries. This chapter describes industrial noise exposure, its assessment, and hearing disorders that result from overexposure to noise. Beginning with the properties of sound, noise-induced hearing loss and other effects of noise exposure are discussed. The impact of hearing disorders and the influence of other factors on hearing loss are described. Typically, noise-induced hearing loss develops slowly, and usually goes unnoticed until a significant impairment has occurred. Fortunately, occupational hearing loss is nearly always preventable. Therefore, this chapter gives particular attention to recommendations for measures to prevent occupational hearing loss such as engineering noise controls and hearing protection devices.

ObjectivesThe aim of this study was to establish the impact of dementia education and training on the knowledge, attitudes and confidence of health and social care staff. The study also aimed to identify the most effective features (content and pedagogical) of dementia education and training.DesignCross-sectional survey study. Data collection occurred in 2017.SettingsHealth and social care staff in the UK including acute care, mental health community care trusts, primary care and care homes.ParticipantsAll health and social care staff who had completed dementia education and training meeting the minimal standards as set by Health Education England, within the past 5 years were invited to participate in an online survey. A total of 668 health and social care staff provided informed consent and completed an online survey, and responses from 553 participants were included in this study. The majority of the respondents were of white British ethnicity (94.4%) and identified as women (88.4%).OutcomesKnowledge, attitude and confidence of health and social care staff.ResultsHierarchical multiple regression analysis was conducted. Staff characteristics, education and training content variables and pedagogical factors were found to account for 29% of variance in staff confidence (F=4.13, p<0.001), 22% of variance in attitude (knowledge) (F=3.80, p<001), 18% of the variance in staff knowledge (F=2.77, p<0.01) and 14% of variance in staff comfort (attitude) (F=2.11, p<0.01).ConclusionThe results suggest that dementia education and training has limited impact on health and social care staff learning outcomes. While training content variables were important when attempting to improve staff knowledge, more consideration should be given to pedagogical factors when training is aiming to improve staff attitude and confidence.
